Introduction Parents are facing tremendous stress in relation to the COVID-19 pandemic and the effectiveness of the COVID-19 vaccination program for children. we aimed to investigate parents’ willingness to vaccinate their children against COVID-19 in North Kivu province, (DRC). Methods A cross-sectional survey between 1 December 2021 to 20 January 2022 in six health zones (Goma, Karisimbi, Butembo, Beni, Kamango, and Katwa) was conducted in the province of North Kivu. In each health zone, we selected five clusters (Health area) using the method of probabilistic selection proportional to population size. Results In total, 522 parents participated in our study. Overall, 32.8% of parents intended to vaccinate their children. In the multivariate analysis, a younger age of parents (aOR: 2.40, CI: [1.50–3.83]), a higher level of fear that “a member of my family” could contract COVID-19 (aOR: 2.35, CI: [1.38–4.02]), a higher level of perceived vulnerability to COVID-19 within the family (aOR: 1.70, CI: [1.005–2.2881]), a higher level of perceived susceptibility to COVID-19 within the family (aOR: 3.07, CI: [1.80–5.23]), and a history of vaccination against COVID-19 among parents (aOR: 16.47, CI: [8.39–32.33]), Conclusion Were all significantly associated with the intention of parents to have their children or adolescents vaccinated. According to the different explanatory factors of the will to have their children vaccinated, an emphasis on the health education of parents who are prone to refusal or hesitation of the vaccine, by addressing the common reasons for the refusal of the vaccine and highlight the vaccine's benefits. Planning of polio vaccination campaigns in Nigeria is based on the most granular level of administrative boundaries i.e. settlements and their accessibility in the conflict-ridden state of Borno. This was a challenge due to the different settlement lists being used at various administrative levels and constant migration due to insecurity. The State, supported by the partners of the Polio Eradication Initiative (PEI) in Borno, engaged the community leaders at the local level to create a master list of settlements. Data collection was conducted to ensure all settlements are geolocated for more efficient campaigns. Consequently, planning, delivery, and progress for campaigns have been refocused to be location-based and geospatially evidenced as maps complement the settlement list to provide more reference. Clear communication channels and feedback loops have been established to impart insight into the current accessibility, which is reflected in the master list. The results indicate a significant impact on vaccination reach in Borno in the last two years due to a collaborative effort of all the PEI partners aided by innovative technology. The same procedure is being followed to boost routine immunization in Borno, thereby providing assurance of the sustainability of processes put in place for the Polio program.

Eradication of the poliovirus hinges on successful last-mile vaccination of children under five years. A segment of the population missed out can result in Wild Polio Virus (WPV) transmission to other locations. In Borno in Nigeria, the insurgency has led to mass displacement. Apart from the migrating populace, there are the nomadic settlers who are culturally mobile. The prevailing insecurity makes tracking of these nomads harder, as they change their routes to avoid contact with the insurgents or to fall victim to the rustling of their herds. Thus, it becomes difficult to administer the required doses to the nomadic children, which is imperative for sustained immunity in their community. To address these challenges, a concerted effort was made to understand the current movement patterns by mapping their existing routes. This was done in collaboration with all the key stakeholders, most importantly, the community and nomadic group leaders. The route mapping exercise has aided in the planning and monitoring of polio vaccination activities targeted at the nomadic populace. It is hoped that with innovative strategies to reach and vaccinate the nomads, the transmission of WPV across communities will be significantly diminished.

Nigeria has made tremendous strides in polio vaccination coverage since Geographic Information Systems (GIS) technology was introduced in 2009 to track vaccination teams. This paper takes the case of Borno state in Northeast Nigeria, where a combination of technology, an environment of trust, collaboration, and cohesiveness between all stakeholders, and contextual insight and cooperation of local partners have played a critical role in increasing coverage and accountability and in the reduction of missed settlements. A single source of data, i.e. the master list of settlements (MLoS) maintained by the State, is used to plan all polio campaigns. During campaigns, daily feedback regarding visitation status was given at the Local Government Area (LGA) level, so missed settlements can be planned for on subsequent days. Post-campaign feedback at the end of campaigns allows for more effective micro-planning of the mop-up or subsequent campaigns. Satellite imagery was used to provide additional insight into hard-to-reach areas. Settlement visitation in accessible areas increased from 80.7% in November 2017 to 98.7% in March 2019 and settlements missed went from 10.9% in November 2017 to 0.9% in March 2019. The results indicate that technology integrated with local insight can make a significant contribution towards identifying the underserved and unreached settlements for immunization activities and providing proof of coverage, and, thereby, towards more impactful health interventions.

Impact of routine rotavirus vaccination on all-cause and rotavirus hospitalizations during the first four years following vaccine introduction in Rwanda

Background: Rwanda introduced pentavalent rotavirus vaccine into its national immunization program in 2012. To determine the long-term impact of rotavirus vaccine on disease burden in a high burden setting, we examined trends in rotavirus and all-cause diarrhea hospitalizations in the first four years following rotavirus vaccine introduction. Methods: We used data from an active surveillance system, from a review of pediatric ward registries, and from the Health Management Information System to describe trends in rotavirus and all-cause diarrhea hospitalizations from January 2009 through December 2016. Percent reductions were calculated to compare the number of all-cause and rotavirus diarrhea hospitalizations pre- and post-rotavirus vaccine introduction. Results: The proportion of diarrhea hospitalizations due to rotavirus declined by 25–44% among all children <5 years of age during 2013–2015 with a shift in rotavirus hospitalizations to older age groups. The proportion of total hospitalizations due to diarrhea among children <5 years of age decreased from 19% pre-vaccine introduction to 12–13% post-vaccine introduction. In the national hospital discharge data, substantial decreases were observed in all-cause diarrhea hospitalizations among children <5 years of age in 2013 and 2014 but these gains lessened in 2015–2016. Discussion: Continued monitoring of long-term trends in all-cause diarrhea and rotavirus hospitalizations is important to ensure that the impact of the vaccination program is sustained over time and to better understand the changing age dynamics of diarrhea and rotavirus hospitalizations in the post-vaccine introduction era. © 2018

Incidence of acute disseminated encephalomyelitis in the Jiangsu province of China, 2008–2011

Background: It is important to have an estimate of the incidence of acute disseminated encephalomyelitis (ADEM) because the incidence of ADEM is unknown and the outcomes undefined in China. Objectives: This study attempts to describe ADEM incidence in large Chinese populations located in four geographically different and moderately distant areas of the same province. Methods: A retrospective investigation was conducted with ADEM patients in Nanjing, Nantong, Yancheng and Xuzhou. The survey was carried out in regions that might have received patients meeting the case definition of ADEM provided by the International Pediatric MS Study Group from 2008 to 2011. A total of 125 hospitals were included and 412 patients were identified through the hospital information systems (HIS). Results: The incidence of ADEM was 0.32/100,000/year. There are two peaks on the age-specific ADEM rates curve. One is 0.77/100,000/year among 0- to 9-year-olds, the other is 0.45/100,000/year in those aged 50–59 years. The incidence rate found for ADEM in males was 0.34/100,000/year, and in females was 0.29/100,000/year. The highest incidence rate was in Nanjing (0.40/100,000/year). Conclusions: The average annual incidence of ADEM was 0.32/100,000/year. The peak age of onset was 50–59 years old and 0–9 years old. The incidence among males was insignificantly higher than that among females. There was no significant difference in incidence by seasonal variation. © 2015, © The Author(s) 2015.

Yaws Prevalence, Lessons from the Field and the Way Forward towards Yaws Eradication in Ghana

Despite past WHO/UNICEF led global yaws eradication efforts, the disease seems to persist. The true burden is however not known for comprehensive action. Ghana's data showed significant increase in notified cases since the 1970s. Recognizing limitations in routine data, we carried out a yaws treatment survey in 2008 in three purposively selected districts to establish the prevalence and learn lessons for renewed action. Of 208,413 school children examined, 4,006 were suspected yaws cases (prevalence 1.92 (95% CI: 1.86-1.98) percent). Of 547 schools surveyed, 13% had prevalence between 5% and 10% while 3% had prevalence above 10%. The highest school prevalence was 19.5%. Half of the schools had cases. The large sample allowed aggregating the school results by administrative levels. The lowest aggregated prevalences of 0.23%, 0.40%, and 0.64% were in the urban sub-districts of Asamankese, Oda, and Achiase, respectively, while the highest of 8.61%, 3.69%, and 1.4% were in rural Akroso, Mepom, and Aperade, respectively. In conclusion, the prevalence of yaws is high in some schools in rural, hard-to-reach areas of Ghana. Considering past global eradication efforts, our findings suggest yaws may be resurging for which programmatic action is needed.
