Artificial intelligence (AI) and intelligent personal assistants (IPAs) are becoming more and more important. This is no longer limited to private use but also becoming increasingly important in everyday business life. The identification of optimization potentials through the use of AI and IPAs is therefore relevant from both a theoretical and a practical point of view. This paper, therefore, identifies concrete use cases for relevant processes in small- and medium sized enterprises (SME) in the service industry and enhances them with AI and IPA capabilities. based on a prototype, the use cases were presented to 10 experts who were interviewed regarding their usefulness and influencing factors. Subsequently, the results of the interviews were categorized and validated again by a quantitative survey within the expert panel. As a result, the use cases were evaluated with regard to the specific influencing factors and the potential for optimization was determined. The use cases were evaluated based on this data. It was shown that IPA features in particular are perceived as useful. On average, AI and IPA features have a cost savings potential of over 31%. This shows the importance of these features and the need to consider them when modeling modern business processes.

Background Interest in standardizing descriptions of health outcomes is increasing. In a Health Outcome Descriptor (HOD), outcomes are systematically described covering four domains: Symptoms, Time horizon, Testing and Treatment, and Consequences. Given the lack of HODs for Rheumatoid Arthritis (RA), the aim of this study was to review published RA outcome descriptions and map them to the HOD framework. Methods We conducted a rapid review of patient-decision aids (PtDAs) and patient preference studies to assess how seven RA outcomes have been described in English to patients. These outcomes were selected by author consensus, from a living systematic review of RA drug therapy. After data extraction and a thematic content analysis, a narrative summary for each outcome was provided. Results We included 11 PtDAs and 27 patient preference studies. Overall, the descriptions of the same health outcome varied widely across studies. Adverse events (AEs) were described in most cases ( N = 26/38). For both PtDAs and preference studies, few provided a description for patient-important outcomes like remission ( N = 2/11 and N = 1/27 respectively) and pain ( N = 2/11 and N = 6/27 respectively). From an HOD perspective, the descriptions focused primarily on symptoms patients may experience (94 %), and less on the other domains (18–38 %). Conclusion There is wide variability in the content of the published RA outcome descriptions, as well as a lack of descriptions regarding common patient-important outcomes. As this study provides a detailed overview of existing descriptions, it may inform future development of HODs for RA.

Background: The COVID-19 pandemic highlighted gaps in health surveillance systems, disease prevention, and treatment globally. Among the many factors that might have led to these gaps is the issue of the financing of national health systems, especially in low-income and middle-income countries (LMICs), as well as a robust global system for pandemic preparedness. We aimed to provide a comparative assessment of global health spending at the onset of the pandemic;characterise the amount of development assistance for pandemic preparedness and response disbursed in the first 2 years of the COVID-19 pandemic;and examine expectations for future health spending and put into context the expected need for investment in pandemic preparedness. Methods: In this analysis of global health spending between 1990 and 2021, and prediction from 2021 to 2026, we estimated four sources of health spending: development assistance for health (DAH), government spending, out-of-pocket spending, and prepaid private spending across 204 countries and territories. We used the Organisation for Economic Co-operation and Development (OECD)'s Creditor Reporting System (CRS) and the WHO Global Health Expenditure Database (GHED) to estimate spending. We estimated development assistance for general health, COVID-19 response, and pandemic preparedness and response using a keyword search. Health spending estimates were combined with estimates of resources needed for pandemic prevention and preparedness to analyse future health spending patterns, relative to need. Findings: In 2019, at the onset of the COVID-19 pandemic, US$9·2 trillion (95% uncertainty interval [UI] 9·1–9·3) was spent on health worldwide. We found great disparities in the amount of resources devoted to health, with high-income countries spending $7·3 trillion (95% UI 7·2–7·4) in 2019;293·7 times the $24·8 billion (95% UI 24·3–25·3) spent by low-income countries in 2019. Background: Respiratory tract infections are a common health issue, driving interest in preventive strategies like nutritional supplements, while evidence on their usage and effectiveness remains limited. In this context, social media platforms, particularly X (formerly Twitter), provide a unique opportunity to gather large-scale public health-related data. Objectives: In this study, we aimed to survey participants’ uses and opinions on nutritional supplements in prevention or treatment of respiratory tract infections, by using X. Methods: A survey was conducted between 1st and 15th December 2022. A single open-ended question “Which are the best dietary supplements to counteract respiratory infections?“ was asked. One week after the start of the survey, a poll was posted to get more relevant information and boost the survey's reach. Total endorsements were calculated for each tweet posted as the total sum of replies, retweets, and likes. Results: The open-ended question received a total of 118 retweets, 39 quotes, and 371 likes, while the poll received 56 retweets, 13 quotes, and 67 likes. A total of 495 replies, 2,251 retweets, 5,118 likes, and 148 quotes were received for the question and its related tweets. Vitamin D (1,607 endorsements), zinc (1,347 endorsements), vitamin C (803 endorsements), magnesium (694 endorsements), and honey (661 endorsements) were the nutritional supplements that received most endorsements. Conclusion: Various foods, drinks, and natural ingredients have been suggested as potentially helpful for counteracting respiratory infections. Approximately half of respondents indicated using such supplements for themselves. The result of this study supports the idea that the X platform can be used as an effective survey tool to study global health-related behaviours and trends. © 2025 The Authors

Background: The causes for immune-mediated inflammatory diseases (IMIDs) are diverse and the incidence trends of IMIDs from specific causes are rarely studied. The study aims to investigate the pattern and trend of IMIDs from 1990 to 2019. Methods: We collected detailed information on six major causes of IMIDs, including asthma, inflammatory bowel disease, multiple sclerosis, rheumatoid arthritis, psoriasis, and atopic dermatitis, between 1990 and 2019, derived from the Global Burden of Disease study in 2019. The average annual percent change (AAPC) in number of incidents and age standardized incidence rate (ASR) on IMIDs, by sex, age, region, and causes, were calculated to quantify the temporal trends. Findings: In 2019, rheumatoid arthritis, atopic dermatitis, asthma, multiple sclerosis, psoriasis, inflammatory bowel disease accounted 1.59%, 36.17%, 54.71%, 0.09%, 6.84%, 0.60% of overall new IMIDs cases, respectively. The ASR of IMIDs showed substantial regional and global variation with the highest in High SDI region, High-income North America, and United States of America. Throughout human lifespan, the age distribution of incident cases from six IMIDs was quite different. Globally, incident cases of IMIDs increased with an AAPC of 0.68 and the ASR decreased with an AAPC of −0.34 from 1990 to 2019. The incident cases increased across six IMIDs, the ASR of rheumatoid arthritis increased (0.21, 95% CI 0.18, 0.25), while the ASR of asthma (AAPC = −0.41), inflammatory bowel disease (AAPC = −0.72), multiple sclerosis (AAPC = −0.26), psoriasis (AAPC = −0.77), and atopic dermatitis (AAPC = −0.15) decreased. The ASR of overall and six individual IMID increased with SDI at regional and global level. Countries with higher ASR in 1990 experienced a more rapid decrease in ASR. Interpretation: The incidence patterns of IMIDs varied considerably across the world. Background: High-resolution estimates of Hiv burden across space and time provide an important tool for tracking and monitoring the progress of prevention and control efforts and assist with improving the precision and efficiency of targeting efforts. We aimed to assess Hiv incidence and Hiv mortality for all second-level administrative units across sub-Saharan Africa. Methods: In this modelling study, we developed a framework that used the geographically specific Hiv prevalence data collected in seroprevalence surveys and antenatal care clinics to train a model that estimates Hiv incidence and mortality among individuals aged 15–49 years. We used a model-based geostatistical framework to estimate Hiv prevalence at the second administrative level in 44 countries in sub-Saharan Africa for 2000–18 and sought data on the number of individuals on antiretroviral therapy (ART) by second-level administrative unit. We then modified the Estimation and Projection Package (EPP) to use these Hiv prevalence and treatment estimates to estimate Hiv incidence and mortality by second-level administrative unit. Findings: The estimates suggest substantial variation in Hiv incidence and mortality rates both between and within countries in sub-Saharan Africa, with 15 countries having a ten-times or greater difference in estimated Hiv incidence between the second-level administrative units with the lowest and highest estimated incidence levels. Across all 44 countries in 2018, Hiv incidence ranged from 2·8 (95% uncertainty interval 2·1–3·8) in Mauritania to 1585·9 (1369·4–1824·8) cases per 100 000 people in Lesotho and Hiv mortality ranged from 0·8 (0·7–0·9) in Mauritania to 676·5 (513·6–888·0) deaths per 100 000 people in Lesotho. Background: The sustainable development goals (SDGs) aim to end Hiv/AIDS as a public health threat by 2030. Understanding the current state of the Hiv epidemic and its change over time is essential to this effort. This study assesses the current sex-specific Hiv burden in 204 countries and territories and measures progress in the control of the epidemic. Methods: To estimate age-specific and sex-specific trends in 48 of 204 countries, we extended the Estimation and Projection Package Age-Sex Model to also implement the spectrum paediatric model. We used this model in cases where age and sex specific Hiv-seroprevalence surveys and antenatal care-clinic sentinel surveillance data were available. For the remaining 156 of 204 locations, we developed a cohort-incidence bias adjustment to derive incidence as a function of cause-of-death data from vital registration systems. The incidence was input to a custom Spectrum model. To assess progress, we measured the percentage change in incident cases and deaths between 2010 and 2019 (threshold >75% decline), the ratio of incident cases to number of people living with Hiv (incidence-to-prevalence ratio threshold <0·03), and the ratio of incident cases to deaths (incidence-to-mortality ratio threshold <1·0). Findings: In 2019, there were 36·8 million (95% uncertainty interval [UI] 35·1–38·9) people living with Hiv worldwide. There were 0·84 males (95% UI 0·78–0·91) per female living with Hiv in 2019, 0·99 male infections (0·91–1·10) for every female infection, and 1·02 male deaths (0·95–1·10) per female death. Global progress in incident cases and deaths between 2010 and 2019 was driven by sub-Saharan Africa (with a 28·52% decrease in incident cases, 95% UI 19·58–35·43, and a 39·66% decrease in deaths, 36·49–42·36). Chronic care manages long-term, progressive conditions, while acute care addresses short-term conditions. Chronic conditions increasingly strain health systems, which are often unprepared for these demands. This study examines the burden of conditions requiring acute versus chronic care, including sequelae. Conditions and sequelae from the Global Burden of Diseases Study 2019 were classified into acute or chronic care categories. Data were analysed by age, sex, and socio-demographic index, presenting total numbers and contributions to burden metrics such as Disability-Adjusted Life Years (DALYs), Years Lived with Disability (YLD), and Years of Life Lost (YLL). Approximately 68% of DALYs were attributed to chronic care, while 27% were due to acute care. Chronic care needs increased with age, representing 86% of YLDs and 71% of YLLs, and accounting for 93% of YLDs from sequelae. These findings highlight that chronic care needs far exceed acute care needs globally, necessitating health systems to adapt accordingly. © 2025. The Author(s).

Uncertainty analysis in integrated wastewater treatment modelling is still in its infancy, although techniques from different fields are increasingly used in research for design and control of wastewater treatment plants (WWTPs). However, results should be interpreted with care. This paper shows explicitly the influence of different methodologies and subjective choices on prediction uncertainty for a simple environmental modelling case: A respirometric experiment for acetate degradation without storage. This case uses experimental data and can be used to estimate kinetic parameters of activated sludge in full-scale wastewater treatment modelling. These are subsequently used in more complex activated sludge models (ASMs) for dynamic modelling studies, in order to reduce the number of parameters to calibrate in these overparameterised models. Three uncertainty analysis methodologies are compared: (1) classical parameter estimation (Fisher Information Matrix or FIM-based), which has been performed extensively for respirometric models. The derived parameter confidence intervals are then propagated to the output through Monte Carlo (MC) simulations with and without correlation based sampling;(2) MC simulations from expert-based probability density functions;(3) the generalised likelihood uncertainty estimation (GLUE) method, a well-known uncertainty analysis method applied in hydrological modelling, recently also applied on full-scale WWTPs. The output uncertainty boundaries on the model output are very specific to the method used and to subjective choices like probability density functions in the expert-based method and threshold values in the GLUE method. Besides, classical parameter estimation and GLUE are useful to observe and easily handle correlated parameters, which is very important when using these methods on full-scale models. The main focus of this paper is an evaluation of advanced knowledge representation methods (KRMs) for deriving new knowledge from product use information (PUI) gathered from an industrial product to enhance the development of the next product generation. Inherently manufactured goods possess a product lifecycle from development, production, usage to recycling. The key aspect in this regard is to choose the most appropriate KRM. This paper shows an evaluation of different KRMs and compiles the results. The evaluation covers the well established methods: Classical Logic, Frames, Case based Reasoning, Semantic Network, Ontology, Topic Map, Fuzzy Logic, Rule based System, Decision Tree, Bayesian Network, Neural Network and Support Vector Machine as a related form of a Neural Network. The evaluation results indicate that the methods Rule based System and Bayesian Network meet the requirements best. Both methods are used for the implementation of an exemplary application scenario based on an injection molding machine.
