Background: Patients with critical illness may present with disease-related malnutrition upon intensive care unit (ICU) admission. They are at risk of development and progression of malnutrition over the disease trajectory because of inflammation, dysregulated metabolism, and challenges with feeding. Methods: The Global Leadership Initiative on Malnutrition (GLIM) convened a panel of 36 clinical nutrition experts to develop consensus-based guidance statements addressing the diagnosis of malnutrition during critical illness, using a modified Delphi approach with a requirement of ≥75% agreement. Results: 1. To identify pre-existing malnutrition, we suggest evaluation within 48 hours of ICU admission when feasible (100% agreement) or within 4 days (94% agreement). 2. To identify the development and progression of malnutrition, we suggest reevaluation of all patients every 7-10 days (97% agreement). 3. To identify progressive loss of muscle mass, we suggest evaluation of muscle mass as soon as feasible (92% agreement), and again after 7-10 days (89% agreement). 4. To identify the development and progression of malnutrition prior to and after ICU discharge, we suggest reevaluating nutritional status prior to ICU discharge and during clinical visits that follow (100% agreement). Conclusion: Research using consistent etiologic and phenotypic variables offers great potential to assess the efficacy of nutrition interventions for critically ill patients with malnutrition. Assessment of these variables at during and beyond the ICU stay will clarify the trajectory of malnutrition and enable exploration of impactful treatment modalities at each juncture. GLIM offers a diagnostic approach that can be used to identify malnutrition in critically ill patients. © 2025 The Authors

Background: Many sepsis survivor patients develop psychological problems and cognitive impairment. We hypothesize that previous stress exacerbate the cognitive impairment and anxiety in sepsis survivors. To test our hyphothesis, experimental animals that were selectively bred for high and low contextual fear conditioning were subjected to sepsis to evaluate long-term behavior changes. Methods: Adult male Albino Wistar rats selectively bred for high (Carioca high-conditioned freezing - CHF) levels or low (Carioca low-conditioned freezing - CLF) levels of emotionality were subject to sepsis by the cecal ligation and perforation (CLP) or sham operated (no-CLP) as a control group. Elevated plus-maze and inhibitory avoidance tasks were performed 10 days after CLP and non-CLP surgeries. Results: The sepsis survivor rats (wild-type, CHF, and CLF groups) and sham (CHF and CLF groups) presented anxious-like behavior evaluated by elevated plus-maze task. However, only sepsis groups presented aversive memory impairment evaluated by inhibitory avoidance task at 10 days after CLP surgery. The CLF or CHF did not interefere in the long-term impairment triggered by sepsis. Conclusion: Our data suggest that previous stress did not interfere in the long-term cognitive impairment and anxious-like behavior observed in septic survivor rats. © 2018 Elsevier GmbH

Background Polymorphous adenocarcinoma (PAC) is a malignant neoplasm of the salivary gland with a low incidence worldwide, characterized by diverse morphological features. Objectives This study aimed to present a large and well-documented case series from a single Brazilian center. Methods We analyzed demographic data, clinical presentations, histopathological features, and immunohistochemical characteristics of 26 cases of PAC. Results Most patients were women (n = 21), with a mean age of 58 years (range = 36 to 84 years). The typical clinical presentation was a fibrous/fibroelastic and firm nodular lesion, with an average diameter of 2 cm, most found on the palate (n = 15). The alveolar ridge, buccal mucous, and superior lip were also affected. Histologically, various patterns were observed, including tubular, solid, cribriform, papillary, and cystic. Glomeruloid slit-like structures (n = 11), clear cells (n = 13), and mucous cells (n = 8) were also identified. Surface papillary epithelial hyperplasia was observed in four cases, and the presence of myxoid and collagenous areas in the stroma in nine. Fusiform areas were noted in 2 cases and areas of squamous differentiation in one. A clear cell predominance was observed in 2 cases. Neural invasion was present in eight cases. All cases were positive for S-100 and Cytokeratin 7 (CK7), and negative for p-40. The Ki-67 index was low, with a mean of 2.5%. Conclusion In this case series, PAC primarily affected minor salivary glands and exhibited some uncommon microscopic features. Oral pathologists should be aware of these findings to prevent misdiagnosis.

This paper presents a methodology based on automatic knowledge discovery that aims to identify and predict the possible causes that makes a patient to be considered of high cost. The experiments were conducted in two directions. The first was the identification of important relationships among variables that describe the health care events using an association rules discovery process. The second was the discovery of precise prediction models of high cost patients, using classification techniques. Results from both methods are discussed to show that the patterns generated could be useful to the development of a high cost patient eligibility protocol, which could contribute to an efficient case management model. © 2009, IFIP Advances in Information and Communication *** right reserved.

The role of genetic diversity in crop germplasm is an important concept within genetic conservation. In this research, 43 accessions were analyzed at the agro-morphological and genetic levels. Clustering based on the agro-morphological resulted in four sub-groups. Analysis at the genetic level was conducted using 22 microsatellites, which revealed a total number of alleles to be 203, with a range per allele between 2 and 17 and an average of 9.2 alleles per locus. The highest and lowest Polymorphic Information Content (PIC) values were found in RM431 and RM11, which were 0.95 and 0.67, respectively. The genetic diversity value ranged from 0.71 to 0.95. The genetic similarity among accessions ranged from 0.00 to 0.90. Clustering based on the genetic relatedness divided the Java rice samples into two major groups. The classification created through this research many inform future breeding programs aimed at improving the quality and quantity of yield production. © 2020, Korean Society of Crop Science (KSCS).

Candidiasis, a fungal infection caused by Candida spp., poses a growing clinical challenge due to the development of antifungal resistance. This study assessed the efficacy of dry crude pomegranate peel extract (DCPPE) as an antifungal agent against Candida infections (C. albicans, C. parapsilosis, C. krusei and C. glabrata) by investigating its effects on fungal growth, biofilm disruption, and fungal cell membrane integrity, as well as evaluating its safety regarding hemolysis at different concentrations. The DCPPE exhibited inhibitory activity against all tested Candida strains, with MIC of 1 % (10 mg/mL). These results may be associated with the phenolic composition of the peel extract, which includes compounds like gallic acid, punicalagin A, punicalagin B, and ellagic acid. Furthermore, the DCPPE disrupted Candida biofilms and demonstrated safety with respect to hemolysis at concentrations up to 60 mg/mL. However, no evidence of a direct interaction with the fungal cell wall or ergosterol in the fungal membrane was found. Thus, our results highlight the potential of DCPPE as a promising alternative for the treatment of candidiasis. Nevertheless, further research is needed to fully clarify the underlying mechanisms and optimize its clinical efficacy. © 2025 Elsevier Ltd

The wide distribution of mosquitoes in the territory of Indonesia with differences in geographical location causes mosquitoes to have the ability to adapt to the environment. The ability to adapt to the environment results in high variations phenotypically and genotypically so that Mutation, recombination or migration of genes is the cause of genetic diversity in a population. The purpose of this research is to study the characteristics and inventory of distribution areas, as well as to analyze genetic variations using morphological and molecular approaches. Genetic typology data through the RAPD approach provides information about the genetic variation of mosquitoes in an effort to control them as disease vectors. Genetic characterization was carried out by PCR-RAPD using three primers, namely OPA 11, OPA 12, OPA 15 and OPL12. Data were analyzed using UPGMA with a similarity coefficient using the Jaccard Coefficient of Similarity presented as a dendrogram. The results showed that the variation of the mosquito polymorphy band from the Kelurahan Kemelak Bindung Langit, Komering Ulu Regency was high. With the DNA band profile of RAPD results as many as 37 bands with a polymorphy DNA band presentation of (86.65%). © 2021 IEEE.

Grading the gradient: Evidence for time-dependent memory reorganization in experimental animals

In humans, damage limited to the hippocampus produces temporally graded retrograde amnesia, with relative sparing of remote compared to recent memory. This observation forms the cornerstone of the idea that as memories mature they are reorganized in a time-dependent manner. In this paper, we evaluate evidence for similar time-dependent reorganization in experimental animals. In the majority of behavioral paradigms examined, these studies provide evidence that memories are gradually reorganized over time, with the hippocampus and cortex playing preferential (although not necessarily mutually exclusive) roles in the expression of recent and remote memory, respectively. This pattern is not observed in all tasks, however. For example, in the water maze hippocampal activity is always necessary for memory expression. Identifying situations when the hippocampus is, and is not, required for remote memory expression will help us to understand hippocampal contributions to memory, and, more generally, whether changes in memory organization lead to qualitative changes in the nature of memory. © Springer Verlag 2007.

Routine serum biomarkers do not always accurately reflect impaired liver function. To overcome this limitation, we synthesized novel bile acid (BA) derivatives (NIRBADs) with near-infrared (NIR) fluorescence that can penetrate the abdominal wall and be detected extracorporeally. NIRBAD dynamics in the liver parenchyma were recorded through intravital imaging in mice and extracorporeally in both rats and mice. NIRBAD metabolism was analyzed using HPLC-MS/MS and fluorimetry. Transport was investigated in cells expressing BA transporters, whose interactions with NIRBADs were assessed through molecular docking and dynamics simulations. The hepatic NIRBAD clearance time (NCT) was evaluated in animal models with impaired secretory function: rats with hepatocellular cholestatic damage induced by phalloidin and mice with obstructive cholestasis caused by bile duct ligation (BDL), as well as with spontaneous development of sclerosing cholangitis (Mdr2^-/-). NIRBADs were taken up by cells expressing NTCP or OATP1B3, but minimally by OATP1B1. These findings were consistent with the NIRBAD dynamics in the liver parenchyma and in silico studies. Following intravenous administration of a non-toxic dose, the time course of NIR fluorescence in the rat liver aligned with biliary output. In mice with BDL, hepatic NIR fluorescence remained stable throughout the experimental period. Phalloidin administration impaired rat bile flow, induced a decrease in biliary NIRBAD-1 output, and caused an increase in NCT. Furthermore, the NCT was significantly longer in Mdr2^-/^- than in wild-type mice. In conclusion, a novel, noninvasive, real-time test based on cholephilic probes with NIR fluorescence detectable extracorporeally serves as a valuable tool for assessing hepatobiliary secretory function. © 2025 The Authors
