Background: The sustainable development goals (SDGs) aim to end HIV/AIDS as a public health threat by 2030. Understanding the current state of the HIV epidemic and its change over time is essential to this effort. This study assesses the current sex-specific HIV burden in 204 countries and territories and measures progress in the control of the epidemic. Methods: To estimate age-specific and sex-specific trends in 48 of 204 countries, we extended the Estimation and Projection Package Age-Sex Model to also implement the spectrum paediatric model. We used this model in cases where age and sex specific HIV-seroprevalence surveys and antenatal care-clinic sentinel surveillance data were available. For the remaining 156 of 204 locations, we developed a cohort-incidence bias adjustment to derive incidence as a function of cause-of-death data from vital registration systems. The incidence was input to a custom Spectrum model. To assess progress, we measured the percentage change in incident cases and deaths between 2010 and 2019 (threshold >75% decline), the ratio of incident cases to number of people living with HIV (incidence-to-prevalence ratio threshold <0·03), and the ratio of incident cases to deaths (incidence-to-mortality ratio threshold <1·0). Findings: In 2019, there were 36·8 million (95% uncertainty interval [UI] 35·1–38·9) people living with HIV worldwide. There were 0·84 males (95% UI 0·78–0·91) per female living with HIV in 2019, 0·99 male infections (0·91–1·10) for every female infection, and 1·02 male deaths (0·95–1·10) per female death. Global progress in incident cases and deaths between 2010 and 2019 was driven by sub-Saharan Africa (with a 28·52% decrease in incident cases, 95% UI 19·58–35·43, and a 39·66% decrease in deaths, 36·49–42·36). Elsewhere, the incidence remained stable or increased, whereas deaths generally decreased. Summary Background Across low-income and middle-income countries (LMICs), one in ten deaths in children younger than 5 years is attributable to diarrhoea. The substantial between-country variation in both diarrhoea incidence and mortality is attributable to interventions that protect children, prevent infection, and treat disease. Identifying subnational regions with the highest burden and mapping associated risk factors can aid in reducing preventable childhood *** We used Bayesian model-based geostatistics and a geolocated dataset comprising 15 072 746 children younger than 5 years from 466 surveys in 94 LMICs, in combination with findings of the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2017, to estimate posterior distributions of diarrhoea prevalence, incidence, and mortality from 2000 to 2017. From these data, we estimated the burden of diarrhoea at varying subnational levels (termed units) by spatially aggregating draws, and we investigated the drivers of subnational patterns by creating aggregated risk factor *** The greatest declines in diarrhoeal mortality were seen in south and southeast Asia and South America, where 54·0% (95% uncertainty interval [UI] 38·1-65·8), 17·4% (7·7-28·4), and 59·5% (34·2-86·9) of units, respectively, recorded decreases in deaths from diarrhoea greater than 10%. Although children in much of Africa remain at high risk of death due to diarrhoea, regions with the most deaths were outside Africa, with the highest mortality units located in Pakistan. Indonesia showed the greatest within-country geographical inequality; some regions had mortality rates nearly four times the average country rate. Reductions in mortality were correlated to improvements in water, sanitation, and hygiene (WASH) or reductions in child growth failure (CGF). Chronic care manages long-term, progressive conditions, while acute care addresses short-term conditions. Chronic conditions increasingly strain health systems, which are often unprepared for these demands. This study examines the burden of conditions requiring acute versus chronic care, including sequelae. Conditions and sequelae from the Global Burden of Diseases Study 2019 were classified into acute or chronic care categories. Data were analysed by age, sex, and socio-demographic index, presenting total numbers and contributions to burden metrics such as Disability-Adjusted Life Years (DALYs), Years Lived with Disability (YLD), and Years of Life Lost (YLL). Approximately 68% of DALYs were attributed to chronic care, while 27% were due to acute care. Chronic care needs increased with age, representing 86% of YLDs and 71% of YLLs, and accounting for 93% of YLDs from sequelae. These findings highlight that chronic care needs far exceed acute care needs globally, necessitating health systems to adapt accordingly. © 2025. The Author(s).

This article's objective is to analyze the influence innovative organizational culture has on marketing strategy formulation and results, and how this relationship affects organizational performance. The theoretical basis lies in Menon et al.'s (1999) model. The methodology included a survey with medium to large Brazilian companies in different sectors. The final sample consisted of 153 questionnaires that were valid for analysis. Techniques for statistical analysis including singular and multivariate methods were used, from which the relationships between constructs were established by structural equation modeling. Results show a positive relationship between innovative organizational culture, the marketing strategy process, and organizational performance. The principles of agile development emerge as potential solutions to numerous design problems of software development. In this context, this paper aims to identify and analyze the profile of Brazilian software producers regarding the use of agile principles and their potential to achieve success in producing software. This is a quantitative study made with professionals from the software industry. Data were collected through structured questionnaires and analyzed using technical descriptive analysis, factor analysis and cluster analysis. Based on the categorization of factors, three software organizations profiles have been obtained regarding the use of agile principles and the scope of success in producing software. The implications of this study are that organizations with the highest success rates in software are the same as those that have higher rates for capacity in terms of team, culture, communication with the client, environmental configuration and relationships with external partners. Organizations that used agile software development principles achieved success in software development, but it cannot be said that the use of agile principles alone can guarantee the achievement of such success in software development. Additionally, relationships with external partners, although not contemplated in the related studies investigated, was shown to be a critical factor for success in software development and, therefore, relevant to the agile software development field. In conclusion, this study is relevant because it helps to improve our understanding of the use of agile principles in the development of software and whether these principles are associated with obtaining success in software production; therefore, it explores a new factor that has not been observed previously.

Background: High-resolution estimates of HIV burden across space and time provide an important tool for tracking and monitoring the progress of prevention and control efforts and assist with improving the precision and efficiency of targeting efforts. We aimed to assess HIV incidence and HIV mortality for all second-level administrative units across sub-Saharan Africa. Methods: In this modelling study, we developed a framework that used the geographically specific HIV prevalence data collected in seroprevalence surveys and antenatal care clinics to train a model that estimates HIV incidence and mortality among individuals aged 15–49 years. We used a model-based geostatistical framework to estimate HIV prevalence at the second administrative level in 44 countries in sub-Saharan Africa for 2000–18 and sought data on the number of individuals on antiretroviral therapy (ART) by second-level administrative unit. We then modified the Estimation and Projection Package (EPP) to use these HIV prevalence and treatment estimates to estimate HIV incidence and mortality by second-level administrative unit. Findings: The estimates suggest substantial variation in HIV incidence and mortality rates both between and within countries in sub-Saharan Africa, with 15 countries having a ten-times or greater difference in estimated HIV incidence between the second-level administrative units with the lowest and highest estimated incidence levels. Across all 44 countries in 2018, HIV incidence ranged from 2·8 (95% uncertainty interval 2·1–3·8) in Mauritania to 1585·9 (1369·4–1824·8) cases per 100 000 people in Lesotho and HIV mortality ranged from 0·8 (0·7–0·9) in Mauritania to 676·5 (513·6–888·0) deaths per 100 000 people in Lesotho. The necessity to efficiently balance and re-allocate system resources among hospitals in a hospital network is paramount, especially as health systems experience increasing demand and costs for health services. In this paper, we proffer a resource allocation-based optimization model that adjusts resources (system inputs) automatically, which provides decision makers (such as health care managers and policy-makers) with a decision-support tool for re-allocating resources in large health systems that are centrally controlled and funded, such as the Military Health System. In these systems, inputs are fixed at certain levels and may only be adjusted within medical treatment facilities, while outputs must be maintained. We provide a mathematical formulation and example solutions from a case study using real-world data from sixteen U.S. Army hospitals. We also find utility in the use of multi-start evolutionary algorithms to store multiple optimal solutions for consideration by decision makers. © 2014 "IIE".

In recent years, organisations within the public sector have experienced the need for transformations seeking effectiveness. In this sense, the aim of the present study was to examine an effort to implement changes in the people management model of a prestigious Brazilian public university. For this study, we adopted the method that comprises qualitative data triangulation, interviews and conclusions according to Becker et al. (2001). The results brought to evidence the pitfalls in implementing the change that ended up failing to materialise. They also brought to light the clash between two management models: The attempted one, which values competency in contrast with the earliest, which operates under the organisational culture of the public sector in Brazil, which is based on seniority and the wage curve flattening. © 2013 Inderscience Enterprises Ltd.

The International Organization for Standardization (ISO) published the standard of ISO 14001 environment management system in September of 1996. After the publication of ISO 14001, many environmental accreditation systems were developed. Many famous brands developed their environment system, for example, the system of restricted substance limit of Nike. This study focused on the supply chain of shoes industry to research that how the environmental management system were maintained and what were their approaches to saved energies, reduced the production cost and passed the environment accreditation. In this study, we interviewed one shoe factory, five leather tanneries, and also finished a survey of 15 leather tanneries, as the result, they all passed the ISO 14001 or the Leather World Group accreditation which are required by many worldwide athlete brands. This study found that the environmental accreditation became a basic element to become more competitive, however it is not the guarantee for new order. However in the process to have the environment system, the leather tannery not only fulfilled the management system to achieve the synergy of quality, reduction of the material loss and power energy, and water resource saving. In this fashion, they are not only enhanced the satisfaction of customer but also reduced the cost of production and strengthen their competitiveness.

Purpose: The incidence of anal cancer is increasing, particularly among HIV and men who have sex with men (MSM) groups. The vast majority of cases are associated with human papillomavirus (HPV), the most common sexually transmitted infection. Epidemiological studies have also documented low survival, which might be linked to lack of appropriate screening, access, and utilization of pertinent health care services. Our objective was to assess the relative survival (1 and 3 years) of anal cancer in Puerto Rico for men and women during the period from 2000-2007. Methods: All histological types of cancer of anus, anal canal, and anorectum (ICD-O-3 codes C210-C218), except for sarcomas, were included. Relative survival was estimated with the use of life tables from the population of Puerto Rico. In addition, the excess survival was compared by age at diagnosis, histology, and stage (defined as local, regional, or distant), using the Poisson regression model. Results: The overall 3-year relative survival in Puerto Rico was the same (53 %) for men and women. Conclusions: Our findings establish baseline survival data for anal cancer in Hispanics from Puerto Rico. Since now, the national guidelines for anal cancer screening and treatment are on their way to be determined;baseline information about survival will allow monitoring the efficacy that standardized screening programs may eventually have in increasing anal cancer survival in this population. © 2014 Springer Science+Business Media.
