The clinical engineer is the professional inside the hospitals that uses the knowledge of engineering in favor the resolutions of questions from the health environment. In addition to equipment management, clinical engineers have a number of diversified assignments and they need ensure the safety and satisfaction of human beings during them. One way to promote this is through hospital ergonomics, the observation of ergonomic parameters can help identify ergonomic problems that can be solved, promoting health, satisfaction and safety in the health units. Methods: The present study intended to establish a relationship between the attributions of clinical engineering and ergonomics, in order to demonstrate the ability of promoting ergonomics that clinical engineers possess, using bibliographic research as a method to provide information and data for the development of the research. Results: Comparing ergonomics parameters and clinical engineering attributions found in the literature it was possible to identify several convergences between them. Discussion: With the comparison, this relationship was perceptible, demonstrating the opportunity that clinical engineers have in ergonomics of offering better health services. © 2018 IEEE.

This research proposes the walking promotion device using arm swing induced by parametric excitation. Generally, the walking is promoted by amplification of arm swing. The proposed device is wearable on wrist such a watch. It consists of weight, link and motor. Moving the weight in the longitudinal direction of the forearm, it occurs parametric excitation. In order to design the link length and weight, the arm and device are modeled and simulated with their parameters. In the result, the weight of the device is set to 0.10kg, the link length of it is set to 0.02 m, and the target amplification angle is decided 1°. To perform a preliminary evaluation experiment, 2-links pendulum was made with following the simulation conditions. Comparing with the states of disabled and enabled the device, the shoulder angle increased 2.23° in average. In addition, an evaluation experiment with human arm was conducted. As the results, the device can increase the arm swing.

Background: Diabetes mellitus is the most challenging and fastest-growing global public health concern. Approximately 10.5% of the global adult population is affected by diabetes, and almost half of them are undiagnosed. The growing at-risk population exacerbates the shortage of health resources, with an estimated 10.6% and 6.2% of adults worldwide having impaired glucose tolerance and impaired fasting glycemia, respectively. All current diabetes screening methods are invasive and opportunistic and must be conducted in a hospital or laboratory by trained professionals. At-risk participants might remain undetected for years and miss the precious time window for early intervention to prevent or delay the onset of diabetes and its complications. Objective: We aimed to develop an artificial intelligence solution to recognize elevated blood glucose levels (≥7.8 mmol/L) noninvasively and evaluate diabetic risk based on repeated measurements. Methods: This study was conducted at KK Women’s and Children’s Hospital in Singapore, and 500 participants were recruited (mean age 38.73, SD 10.61 years;mean BMI 24.4, SD 5.1 kg/m2). The blood glucose levels for most participants were measured before and after consuming 75 g of sugary drinks using both a conventional glucometer (Accu-Chek Performa) and a wrist-worn wearable. The results obtained from the glucometer were used as ground-truth measurements. We performed extensive feature engineering on photoplethysmography (PPG) sensor data and identified features that were sensitive to glucose changes. These selected features were further analyzed using an explainable artificial intelligence approach to understand their contribution to our predictions. Results: Multiple machine learning models were trained and assessed with 10-fold cross-validation, using participant demographic data and critical features extracted from PPG measurements as predictors. Traditional building design education in the disciplines of building services, civil engineering, mechanical engineering, etc. focusses on quantifiable factors, i.e. factors which can be subject of calculation, simulation and dimensioning. Since non-quantifiable factors are not part of calculation or simulation models, there is a tendency that they are not adequately considered in education and, as a result, in building planning and operation. In the case of human thermal comfort, building professionals are required to understand the mechanisms and impact of non-quantifiable factors related to human thermal comfort and health, most of which are part of the adaptive thermal comfort concept. This paper identifies the challenges in teaching the concept of adaptive thermal comfort through a collection of lived experiences of the authors. The paper then proposes appropriate intended learning outcomes and teaching methods to equip students with the background knowledge, understanding and skills required to consider human adaptation in their designs. © 2020 16th Conference of the International Society of Indoor Air Quality and Climate: Creative and Smart Solutions for Better Built Environments, Indoor Air 2020. The elimination of small and potentially toxic boron species in water is always a significant challenge for conventional membrane technologies. In this study, facile intercalation and functionalization of β-cyclodextrin (CD) onto layered double hydroxides (LDHs) have resulted in the formation of accessible hydroxyl functional groups and anions that exhibit strong sorption with boron. Through meticulous molecular design and optimization, the modified LDH possessing the maximum boron adsorption capacity of 96.1 mg g-1 was employed to fabricate a polyamide-based thin-film nanocomposite (TFN) membrane with a loading of 0.10 wt.%. This resultant membrane displayed a high salt rejection and water permeance of 99.4 % and 2.68 LMH bar-1 for brackish water, respectively, which can be attributed to the increased interlayer spacing of 1.67 nm and the thinner selective layer. The innovative TFN membrane also achieved a high boron rejection of 82.3 % against brackish water containing 2,000 mg L-1 NaCl and 15 mg L-1 B at pH 8. This pioneering study provides valuable insights into the design of brackish water reverse osmosis (BWRO) membranes, through the synergistic use of macrocyclic molecules and inorganic layered nanomaterials, that could potentially revolutionize water reuse and boron removal applications. © 2023, The Authors. An HIV-1 broadly neutralizing antibody overcomes structural and dynamic variation through highly focused epitope targeting

The existence of broadly cross-reactive antibodies that can neutralize diverse HIV-1 isolates (bnAbs) has been appreciated for more than a decade. Many high-resolution structures of bnAbs, typically with one or two well-characterized HIV-1 Env glycoprotein trimers, have been reported. However, an understanding of how such antibodies grapple with variability in their antigenic targets across diverse viral isolates has remained elusive. To achieve such an understanding requires first characterizing the extent of structural and antigenic variation embodied in Env, and then identifying how a bnAb overcomes that variation at a structural level. Here, using hydrogen/deuterium-exchange mass spectrometry (HDX-MS) and quantitative measurements of antibody binding kinetics, we show that variation in structural ordering in the V1/V2 apex of Env across a globally representative panel of HIV-1 isolates has a marked effect on antibody association rates and affinities. We also report cryo-EM reconstructions of the apex-targeting PGT145 bnAb bound to two divergent Env that exhibit different degrees of structural dynamics throughout the trimer structures. Parallel HDX-MS experiments demonstrate that PGT145 bnAb has an exquisitely focused footprint at the trimer apex where binding did not yield allosteric changes throughout the rest of the structure. These results demonstrate that structural dynamics are a cryptic determinant of antigenicity, and mature antibodies that have achieved breadth and potency in some cases are able to achieve their broad cross-reactivity by "threading the needle" and binding in a highly focused fashion, thus evading and overcoming the variable properties found in Env from divergent isolates.

Objectives: To evaluate changes in the age at menarche in Asian populations. Study design: Retrospective cohort study. Methods: We included 548,830 women from six countries in Asia. The data were sourced from 20 cohorts participating in the Asia Cohort Consortium (ACC) and two additional cohort studies: Japan Multi-institutional Collaborative Cohorts (J-MICC), and Japan Nurse Health Study (JNHS) with data on age at menarche. Joinpoint regression was used to evaluate changes in age at menarche by birth year and by country. Results: The study includes data from cohorts in six Asian countries namely, China, Iran, Japan, Korea, Malaysia and Singapore. Birth cohorts ranged from 1873 to 1995. The mean age of menarche was 14.0 years with a standard deviation (SD) of 1.4 years, ranged from 12.6 to 15.5 years. Over 100 years age at menarche showed an overall decrease in all six countries. China showed a mixed pattern of decrease, increase, and subsequent decrease from 1926 to 1960. Iran and Malaysia experienced a sharp decline between about 1985 and 1990, with APC values of −4.48 and −1.24, respectively, while Japan, South Korea, and Singapore exhibited a nearly linear decline since the 1980s, notably with an APC of −3.41 in Singapore from 1993 to 1995. Conclusions: Overall, we observed a declining age at menarche, while the pace of the change differed by country. Additional long-term observation is needed to examine the contributing factors of differences in trend across Asian countries. The study could serve as a tool to strengthen global health campaigns. © 2024 The Royal Society for Public Health

Wayang Kulit which is made of cowhide and controlled by bamboo sticks is the most traditional folk custom in the Malaysian shadow play in the area. It's similar to Chinese traditional shadow play. The content of the main performance of the Malaysia Wayang Kulit is based on the works of Indian literature in order to understand the world and themselves. However, the dilemma which we encountered is that the performance forms are more traditional and conservative. The main storybooks are too old to attract young people. Now we are making progress on the technology and interactive media and also creating a new performance model. So The Fusion Wayang Kulit founder - Tintoy Chuo uses the innovative elements of Star Wars to deduce new forms of performance content. Therefore, in this study I want to use Leap Motion sensor to combined with Unity 3D program, and refer the traditional Wayang Kulit to design innovative characters. The interactive system is based on experiential learning theory to construct learning content and simulate interactive game mode. This study will summarize the research results to provide the basis and reference for the future related traditional shadow puppet and Technology-assisted teaching.
