Regular Consumption of Black Tea Kombucha Modulates the Gut Microbiota in Individuals with and without Obesity

Background: Kombucha, a fermented beverage obtained from a Symbiotic Culture of Bacteria and Yeast, has shown potential in modulating gut microbiota, although no clinical trials have been done. Objectives: We aimed to evaluate the effects of regular black tea kombucha consumption on intestinal health in individuals with and without obesity. Methods: A pre-post clinical intervention study was conducted lasting 8 wk. Forty-six participants were allocated into 2 groups: normal weight + black tea kombucha (n = 23);and obese + black tea kombucha (n = 23). Blood, urine, and stool samples were collected at baseline (T0) and after 8 wk of intervention (T8). Results: A total of 145 phenolic compounds were identified in the kombucha, primarily flavonoids (81%) and phenolic acids (19%). Kombucha favored commensal bacteria such as Bacteroidota and Akkermanciaceae, especially in the obese group. Subdoligranulum, a butyrate producer, also increased in the obese group after kombucha consumption (P = 0.031). Obesity-associated genera Ruminococcus and Dorea were elevated in the obese group at baseline (P < 0.05) and reduced after kombucha consumption, becoming similar to the normal weight group (Ruminococcus: obese T8 × normal weight T8: P = 0.27;Dorea: obese T8 × normal weight T0: P = 0.57;obese T8 × normal weight T8: P = 0.32). Fungal diversity increased, with a greater abundance of Saccharomyces in both groups and reductions in Exophiala and Rhodotorula, particularly in the obese group. Pichia and Dekkera, key microorganisms in kombucha, were identified as biomarkers after the intervention. Conclusions: Regular kombucha consumption positively influenced gut microbiota in both normal and obese groups, with more pronounced effects in the obese group, suggesting that it may be especially beneficial for those individuals. IDENTIFYING POTENTIATORS OF ADVERSE REACTIONS TO ANTIRETROVIRAL DRUGS IN A PRIMARY CARE MODEL

INTRODUCTION Adverse drug reactions (ADRs) in Antiretroviral Treatment (ART) are influenced by multiple potentiators related to the patient, the disease, the drug, the environment and medical treatment, these ADRs are highly prevalent and are identified as an important risk factor that predisposes patients to ADRs. It was considered necessary to determine the demographic, social, and clinical factors associated with ADRs from antiretrovirals in HIV-positive patients, who were treated by the specialized comprehensive care program in a primary health care model. METHODOLOGY Observational, cross-sectional, analytical, and retrospective study with a population of patients on antiretroviral therapy in a primary care program. The outcome evaluated was adverse drug reactions vs. sociodemographic, pharmacological and clinical factors. For the statistical analysis, univariate, bivariate and multivariate analyses were performed, where a multiple binary logistic regression was used for explanatory purposes. RESULTS A total of 5406 records of patients with antiretroviral therapy were analyzed, the prevalence of ADR was 16.68%, the multivariate analysis showed that the variables that increase the probability of ADR are age, education, area of residence, pharmacological group, HDL cholesterol levels, adherence, persistence, change of two or more times of ARV and treatment time. CONCLUSION Antiretrovirals, as well as the risk factors that are mainly associated with the occurrence of ADRs in this study, contribute to health professionals at all levels to anticipate, identify and minimize ADR, as well as to understand the need for close follow-up and monitoring to avoid the occurrence of serious ADRs.

BACKGROUND: Incidence of premenopausal breast cancer (BC) has risen in recent years, though most existing BC prediction models are not generalizable to young women due to underrepresentation of this age group in model development. METHODS: Using questionnaire-based data from 19 prospective studies harmonized within the Premenopausal Breast Cancer Collaborative Group (PBCCG), representing 783,830 women, we developed a premenopausal BC risk prediction model. The data were split into training (2/3) and validation (1/3) datasets with equal distribution of cohorts in each. In the training dataset variables were chosen from known and hypothesized risk factors: age, age at menarche, age at first birth, parity, breastfeeding, height, BMI, young adulthood BMI, recent weight change, alcohol consumption, first-degree family history of BC, and personal history of benign breast disease (BBD). Hazard ratios (HR) and 95% confidence intervals (CI) were estimated by Cox proportional hazards regression using age as time scale, stratified by cohort. Given that complete information on all risk factors was not available in all cohorts, coefficients were estimated separately in groups of cohorts with the same available covariate information, adjusted to account for the correlation between missing and non-missing variables and meta-analyzed. Absolute risk of BC (in situ or invasive) within 5 years, was determined using country-, age-, and birth cohort-specific incidence rates. Discrimination (area under the curve, AUC) and calibration (Expected/Observed, E/O) were evaluated in the validation dataset. We compared our model with a literature-based model for women < 50 years (iCARE-Lit). RESULTS: Selected model risk factors were age at menarche, parity, height, current and young adulthood BMI, family history of BC, and personal BBD history. BACKGROUND:Colorectal cancer (CRC) is the leading cause of cancer-related death in Puerto Rico, posing significant challenges for patients with multiple chronic conditions (MCC). This qualitative study aimed to explore oncologists' perspectives regarding the care of patients with CRC and MCC in Puerto Rico. METHODS:We conducted semi-structured interviews in Spanish with nine oncologists providing care for patients with CRC in Puerto Rico. We reached data saturation. We performed thematic analysis to identify key patterns and themes within the interview data. The coding scheme evolved through team discussions, with discrepancies addressed for consistency. Quotes were translated from Spanish to English. RESULTS:Five key themes were: (1) social determinants of health, (2) diagnosis pathways, (3) factors influencing treatment decisions, (4) survivorship and end-of-life care, and (5) care coordination and communication. Oncologists treating patients with CRC and MCC identified the lack of a social support network as a notable care coordination challenge. The health insurance system's pre-authorization requirements for procedures and treatments further complicated care delivery, particularly for older adults, who faced challenges navigating these administrative processes without sufficient support. A lack of transportation and local specialized care services was a noted barrier to comprehensive patient care. Communication between patients, physician and caregivers proved challenging when multiple physicians and procedures were involved with patient's care, often requiring patients to schedule appointments with different specialists themselves. Inter-provider communication primarily relied on phone calls or notes sent with the patient. CONCLUSIONS:Oncologists caring for Hispanic older adults with CRC and MCC encounter complex challenges influenced by unmet social needs and the presence of comorbidities. Socioeconomic Position (SEP) is a multidimensional construct encompassing education, income, occupation, and neighborhood distress, influencing chronic pain severity, interference, and duration. However, its impact on placebo analgesia, where reduced pain perception occurs due to treatment belief, remains understudied. Using a quasi-experimental approach, we investigated SEP's influence on placebo analgesia in 401 participants with temporomandibular disorder (TMD) and 400 pain-free individuals. Using latent class analysis, we grouped participants into two SEP groups based on self-reported education, income, occupation, and neighborhood distress indices, including the area deprivation and distressed community indexes. Ancestry Informative Markers (AIMs) and self-reported race were included to account for genetic and demographic influences. Placebo analgesia was elicited using verbal suggestion and classical conditioning. Linear mixed models were employed to analyze SEP's impact, while multiple regression and ANCOVA assessed AIMs’ and race's effects. Comparable placebo effects were observed between participants with TMD and pain-free individuals (F(1,4765.73) = 0.49, p = 0.48). A trend was noted in the main effect of SEP (F(1,4764.5) = 3.64, p = 0.056). Among TMD participants, those with distressed SEP exhibited lower placebo analgesia (F(1,4765.73) = 7.9, p = 0.005), while placebo response did not differ by SEP in pain-free participants (F(1,4765.73) = 0.27, p = 0.59). East Asian ancestry (β = 5.71, 95% CI [1.50, 9.92]) and self-reported Asian (mean = 24.20, sem = 1.52, p = 0.020) were associated with greater placebo analgesia. This study highlights the interplay of SEP, AIMs, and race in placebo analgesia and calls for tailored pain management interventions. Perspective: SEP significantly contributes to pain disparities. Biomedical and Health Services Research in Canada: A Review

Using published evidence of research grants, the historical development of funding sources for biomedical and health services research in Canada is discussed. The imbalance between health services research, which includes health services administration and biomedical research, is examined. It is argued that the current funding arrangement should be restructured to accommodate field-based, application research through an independent agency. Needs-focused and integrated research activities that are policy relevant and which involve practitioners are required if current limitations in the system are to be addressed. Thus, health administration as a discipline will receive the necessary technological assistance needed to improve management practices. Grâce aux données publiées sur les subventions à la recherche, on discute du développement historique des sources de financement pour la recherche biomédicale et la recherche sur les services de santé au Canada. Le déséquilibre entre la recherche sur les services de santé, qui comprend l'administration des services de santé et la recherche biomédicale, fait l'objet d'un examen. On prétend que la structure actuelle de financement devrait être remaniée pour tenir compte de la recherche appliquée et pratique au moyen d'un organisme indépendant. Pour repousser les limites actuelles du système, il faut des activités de recherche intégrées et axées sur les besoins qui sont pertinentes à la politique et qui nécessitent la participation des praticiens. HOSPITAL STAY AND MORTALITY ATTRIBUTED TO NOSOCOMIAL ENTEROCOCCAL BACTEREMIA - A CONTROLLED-STUDY

A retrospective cohort study of 97 patients identified by prospective hospital-wide surveillance was conducted to determine the length of hospital stay and mortality attributed to hospital-acquired enterococcal bacteremia. The mean duration of hospitalization for cases was 83 days compared with 44 days for matched controls (p = 0.0001). The mortality rate during the study period was 43% among cases and 12% in matched controls (p < 0.001). Thus the mortality rate attributable to enterococcal bacteremia was 31% and the risk ratio was 4.75. Stepwise discriminant function analysis indicated that the use of vascular catheters and renal dialysis and the presence of immune deficiency were predictors of fatal outcome in cases. Enterococcal bacteremia has become a prominent nosocomial pathogen and is associated with mortality rate well above those expected from the underlying disease.

Understanding wild avian mortality is crucial for effective population management and ex-situ care improvement. Despite Colombia’s status as a global hotspot for bird biodiversity, epidemiological data on mortality causes and trends remain limited. This study aimed to evaluate avian mortality causes, trends, and forecast future deaths of birds admitted to the Avian Pathology Laboratory (LPA) at the Universidad Nacional de Colombia. A 19-year dataset (2004–2023) from LPA was analyzed to identify significant mortality causes and trends based on disease categories, temporal dynamics, sex, and age. Forecasting analyses were conducted. A total of 726 wild birds from 100 genera, including 20 orders, were analyzed, with Psittaciformes (27.5%) and Strigiformes (19.8%) being the most frequent. Adults were more affected (70.6%, P = 0.03), with 1.87-times fold greater odds of death than among young birds (P = 0.006;IC = 1.87;1.19–2.92). Overall, 63.2% of birds were diagnosed with infectious diseases. Time-series analysis revealed increased mortalities in February, March (P < 0.05), and September (P < 0.001), with variations by disease category, age, sex, and taxonomic group. From 2016 to 2023, deaths showed an increasing trend (APC = 26.8% per year;95% CI = 3.62 to 55.23;P < 0.05). The SARIMA model predicted a rise in overall mortality in March, July, and November of 2024, with increased mortality due to infectious diseases in July and among males in September. The study’s findings underscore the predominance of seasonal infectious diseases and highlight the need for further surveillance and analysis of temporal variations, contributing to improved wildlife disease management and forecasting. © The Author(s) 2025.

This article highlights research supporting the concept that increased physical activity and cardiorespiratory fitness attenuate risk of cardiovascular disease, type 2 diabetes, and the metabolic syndrome. Increased activity and fitness also attenuate risk of developing cardiovascular disease in persons who have type 2 diabetes or the metabolic syndrome. Although controversial, relationships between physical activity/physical fitness and type 2 diabetes/metabolic syndrome are largely independent of body weight. Thus, physical inactivity and poor cardiorespiratory fitness are not only important determinants of cardiovascular and metabolic diseases, but they can also be considered common features of these conditions, much like traditional risk factors such as obesity and insulin resistance.
