Dissociative Trait as a Mediator of Problematic Internet Use in Children with attention-deficit/hyperactivity Disorder and Learning Disabilities

Purpose: This study used data from a national birth cohort study to investigate the duration of internet use at the age of 12 years among children diagnosed with attention-deficit/hyperactivity disorder (ADHD), autism spectrum disorder (ASD), intellectual disabilities (IDs) and learning disabilities (LDs) at the age of five and a half years (66 months) to understand whether an ADHD, ASD, ID and LD diagnosis in childhood increases the risk of problematic internet use (PIU) in adolescence. Furthermore, the pathway relationship of dissociative absorptive trait with PIU and these diagnoses was also investigated. Methods: The 5.5- and 12-year-old Taiwan Birth Cohort Study dataset was used (N = 17,694). Results: More boys were diagnosed with LDs, IDs, ADHD and ASD;however, girls were at increased likelihood of PIU. ID and ASD diagnoses were not associated with increasing PIU likelihood. However, children who had been diagnosed with LDs and ADHD, along with higher dissociative absorptive trait, had an indirectly increased likelihood of PIU in adolescence. Conclusions: Dissociative absorption was found to be a mediating factor between childhood diagnosis and PIU and can be used as a screening indicator in prevention programs to reduce the duration and severity of PIU in children diagnosed with ADHD and LDs. Furthermore, with the increased prevalence of smartphone usage in adolescents, education policy-makers should pay greater attention to the issue of PIU in female adolescents. © 2023, The Author(s), under exclusive licence to Springer Science+Business Media, LLC, part of Springer Nature.

Colchicine is one of the oldest known microtubule-targeting agents and also represents a classic example of axial chirality and atropisomerism in medicine. This is because colchicine's axially chiral methoxytropone-trimethoxybenzene (called the AC ring) is directly responsible for tubulin binding and is thermodynamically set into the requisite form by a point chiral acetamido group on its B ring. Indeed, desacetamidocolchicine (DAAC), a colchicine analogue without the acetamido group, racemizes within minutes. Herein, we describe the synthesis as well as physical and biological characterization of a series of AC ring-containing molecules that represent B-ring further deconstructed variants of DAAC. These studies revealed a novel analogue with an AC ring that is highly stable to epimerization based not on thermodynamic stabilization but rather a high rotational barrier energy. Profiling and characterization of the dihedral angles were carried out computationally and experimentally using vibrational circular dichroism, demonstrating that the ground state dihedral angles of the new molecules differ significantly from those of colchicine. However, despite this difference, the molecule retained antiproliferative, tubulin-binding, and tubulin polymerization inhibitory activity.

Examining the Perception of Vocal Deviation by Dysphonic and Nondysphonic Women

Aim: To investigate the auditory perception of vocal deviation by dysphonic and nondysphonic women and to correlate the accuracy rate with acoustic measurements. Methods: In total, 24 dysphonic women allocated to an experimental group (EG) and 10 nondysphonic women allocated to the control group (CG) participated in this study. The volunteers filled in the vocal screening form the vocal symptom scale (VoiSS) self-assessment protocol and made voice recordings during the emission of the vowel /Ɛ/, whose degree of deviation was examined through perceptual-auditory and acoustic evaluation;all the participants underwent audiometry. Three experiments were conducted for the perception tests. The first experiment consisted of randomly presenting 38 stimuli in isolation, 10 of which were healthy voices and 28 with different degrees of deviation, so that each participant could classify them as normal or altered. In the second experiment, the participants heard only breathy and healthy voices with different degrees of deviation and had to discriminate the presence or absence of breathiness. The third experiment was similar to the second experiment but with a rough voice. Results: Women in the EG had a lower hit rate (52.2%) in identifying dysphonic voices than women in the CG (69.6%) (P < 0.001). The women in the EG had a lower accuracy rate in identifying predominantly rough (62.7%) and breathy (62%) voices, compared with the accuracy rate of the women in the CG (73% and 75.6%, respectively) (P value < 0.001). There was a moderate negative correlation between the hit rate of nondysphonic women and the shimmer values. Conclusions: Dysphonia can interfere with the identification of dysphonic voices. Dysphonic women had a lower accuracy rate in identifying dysphonia and roughness and breathiness parameters than nondysphonic women. © 2025 The Voice Foundation

The synergistic interaction between urbanicity and maternal education affecting childcare pressure related to autism spectrum disorder in two national birth cohort studies

To gain a holistic approach for parental help-seeking behavior and ASD diagnosis, this study aimed to use two national birth cohort study datasets to investigate the pathway relationship between personal and social/environmental factors, including maternal perceived childcare pressure, maternal level of education, urbanization at age 3, and the chance of receiving a autism spectrum disorder (ASD) diagnosis at when children are aged 5.5 years (2.5 years later). The 2003 Taiwan Birth Cohort Pilot Study (TBCS-p; N = 1618) and 2005 Taiwan Birth Cohort Study (TBCS; N = 19,183) datasets were used. The TBCS-p and TBCS both showed similar prevalence of ASD (0.3-0.4%). Children with ASD characteristics, as measured using the Modified Checklist of Autism in Toddlers, had 8.27-18.20 times increased likelihood of receiving ASD diagnosis [TBCS confidence interval (CF) of 5.32-12.86; TBCS-p CF of 2.03-163.46]. Pathway analysis showed that, although having a mother with higher education or who lived in a city decreased the chance for ASD diagnosis, the interactive effect of a maternal higher level of education and living in a city led to an increased likelihood for ASD diagnosis. Additionally, mothers who perceived a higher level of childcare pressure were also more likely to seek medical assistance and diagnosis. Access to healthcare (living in the city), maternal level of awareness (level of education), and perceived childcare pressure are all factors that influence help-seeking behavior and diagnosis. Therefore, medical professionals should pay special attention to the developmental condition of children in underserved backgrounds and rural areas to prevent delayed diagnosis.

Ultrasound kidney image segmentation presents significant challenges due to missing or ambiguous boundaries. In this study, we introduce a coarse-to-refinement approach incorporating four novel aspects. Firstly, we leverage the properties of a principal curve (PC) to automatically fine-tune the curve shape and employ a neural network's learning ability to reduce model error. Secondly, a deep fusion learning network is utilized for the coarse segmentation step, incorporating a parallel architecture to enhance deep-learning performance. Thirdly, addressing the limitation of standard PC-based methods in determining the number of vertices automatically, we propose an automatic searching polygon tracking method using a mean shift clustering-based approach to replace the projection and vertex extension step in standard PC-based methods. Lastly, we develop an explainable mathematical map function for the kidney contour, as denoted by the neural network output (i.e., optimized vertices), which aligns well with the ground truth contour. We conducted various experiments to evaluate our method's performance, demonstrating its effectiveness in ultrasound kidney image segmentation. © 2023 IEEE.

Evidence-based medical procedures to optimise caesarean outcomes: an overview of systematic reviews

Background: The use of caesarean sections (CS) is increasing to unprecedented levels worldwide. As with any surgery, it has risks, and understanding the evidence base for interventions involved in a CS is essential to optimise outcomes and inform recommendations. We conducted an overview of systematic reviews (SRs) of randomised controlled trials (RCTs) to summarise the evidence on medical procedures used in CS. Methods: Searches were conducted in Cochrane Database of Systematic Reviews, PubMed, EMBASE, LILACSs and CINAHL without date or language restrictions from database inception to January 31, 2024, with an updated search performed on January 24, 2025. We included SRs of RCTs that examined the effectiveness and safety of medical procedures used in CS. AMSTAR 2 and GRADE were used to assess the methodological quality of the SRs and the certainty of evidence at outcome level, respectively. We classified each procedure-outcome pair into one of eight categories according to effect estimates and certainty of evidence. The overview was registered at PROSPERO (CRD 42023208306). Findings: We identified 29 SRs (15 Cochrane and 14 non-Cochrane) published between 2002 and 2024 involving 408 unique RCTs including over 116,000 participants. Most reviews included trials from low- and middle-income countries (n = 21, 72.4%), combined both elective and emergency CS (n = 19, 65.5%), and were of high quality (n = 18, 62%), while 24.3% (n = 7) were of low and 13.7% (n = 4) were of critically low quality. The SRs presented 512 procedure-outcome comparisons (271 procedure versus procedure, 241 procedure versus no treatment/placebo). There was insufficient or inconclusive evidence for 350 comparisons (68.4%), clear evidence of benefit for 97 (18.9%), possible benefit for 48 (9.3%), clear or possible no difference of effect for 9 (1.8%), clear evidence of harm for 4 (0.8%) and possible harm for 4 (0.8%). Two new 16-hydroxybriaranes, 11-dehydroxybriavioid B (1) and 12-epi-briastecholide L (2), have been isolated from an algal-containing octocoral identified as Briareum stechei. The structures of metabolites 1 and 2 were corroborated by extensive 2D-NMR studies and comparison with literature data. Briarane 2 was found to reduce the release of inducible nitric oxide synthase (iNOS) to 87.92 %, at a concentration of 10 μM. © 2023 Phytochemical Society of Europe
