The chemical screening of an algae-containing marine sponge identified as Lendenfeldia species has isolated two 24-homoscalarane sesterterpenoids, including a known compound, felixin A (1) and a new analogue, lendenfeldarane V (2). The structure of 1, obtained in a previous study, was cited for the first time in this study via single-crystal X-ray diffraction analysis. The structure of 2 was ascertained via 2D NMR experiments and a literature review. The absolute configurations of 1 and 2 were delineated with DP4+ computation and specific optical rotation. Homoscalarane 1 exhibited cytotoxicity towards MG63 human osteosarcoma cells. © 2024 Phytochemical Society of Europe

Impact of ultra-processed foods on human health: A comprehensive review of genomic instability and molecular mechanisms

In this review the nexus between genomic instability and human health is investigated, emphasizing the rising consumption of ultra-processed foods (UPFs). Introducing the NOVA food classification system, we explore the significant surge in UPF consumption over the past 3 decades and its correlation with heightened mortality rates. This exploration extends to the development of health issues such as obesity, cancer, type 2 diabetes, inflammatory bowel diseases, cardiovascular diseases, and depression. Existing evidence, including studies involving healthy adolescents and older adults, underscores a clear link between increased consumption of UPFs and heightened DNA damage. The primary objective of this review is to offer a comprehensive examination of the repercussions of elevated UPF consumption on human health. With a specific focus on unraveling the intricate relationship between these dietary choices and genomic instability, the review seeks to enhance our understanding. Through a targeted exploration of molecular pathways, the aim is to illuminate how dependence on UPFs may impact physical and mental well-being. © 2025 Elsevier Inc.

Background: Physicians’ values about what constitute their professional identities are integral in understanding how they ascribe meaning to their practice. However, there is no general consensus on the conceptualization and measurement of physicians’ professional identities. This study developed and validated a values-based scale for measuring physicians’ professional identities. Methods: A hybrid research method was used to gather both qualitative and quantitative data. We employed literature review, semi-structured interview, Q-sorting exercise to examine the conceptualization of emergency physicians’ professional identities and to initially develop a 40-item scale. A panel of five experts assessed the scale’s content validity. Using 150 emergency physicians as our sample, we conducted Confirmatory Factor Analyses (CFA) to test the fit of our hypothesised four-factor model based on our preliminary findings. Results: Initial CFA suggested revisions to the model. Following theoretical assumptions and modification indices, the model was revised and adjusted to a four-factor 20 item Emergency Physicians Professional Identities Value Scale (EPPIVS) with acceptable fit statistics χ2 = 389.38, df = 164, Normed χ2 = 2.374, GFI = 0.788, CFI = 0.862, RMSEA = 0.096. The Cronbach’s alpha, McDonald’s Omega reliability and composite reliability of the subscales ranged from α: 0.748 to 0.868, Omega: 0.759 to 0.868 and CR: 0.748 to 0.851, respectively. Conclusion: The results indicate that the EPPIVS is a valid and reliable scale for measuring physicians’ professional identities. Further research on the sensitivity of this instrument to important changes over career progression in emergency medicine is warranted. © 2023, The Author(s).

Heavy metal concentration in pregnant women affects neurocognitive and behavioral development of their infants and children. The majority of existing research focusing on pregnant women's heavy metal concentration has considered individual environmental factor. In this study, we aim to comprehensively consider lifestyle, food, and environmental factors to determine the most influential factor affecting heavy metal concentration in pregnant women. The Ko-CHENS (Korean CHildren health and ENvironmental Study) is a nationwide prospective birth cohort study in South Korea enrolling pregnant women from 2015 to 2020. A total of 5458 eligible pregnant women were included in this study, and 897 variables were included in questionnaire comprising: maternal general information, indoor and living environment, dietary habits, health behavior, exposure to chemicals. Lead, cadmium and mercury concentration on blood were measured in early, late pregnancy and in cord blood at birth. Variables that might be related to heavy metal concentrations were included in machine learning models. Random forest and XGBoost machine learning models were conducted for predictions. Both models had similar but better performance than multiple linear regression. Kimchi (β = 1.55), seaweed (β = 0.40), fatty fish (β = 1.55), intakes respectively affected lead, cadmium, and mercury exposure through early, late pregnancy and cord blood. © 2025

Any reliable biomarker has to be specific, generalizable, and reproducible across individuals and contexts. The exact values of such a biomarker must represent similar health states in different individuals and at different times within the same individual to result in the minimum possible false-positive and false-negative rates. The application of standard cut-off points and risk scores across populations hinges upon the assumption of such generalizability. Such generalizability, in turn, hinges upon this condition that the phenomenon investigated by current statistical methods is ergodic, i.e., its statistical measures converge over individuals and time within the finite limit of observations. However, emerging evidence indicates that biological processes abound with non-ergodicity, threatening this generalizability. Here, we present a solution for how to make generalizable inferences by deriving ergodic descriptions of non-ergodic phenomena. For this aim, we proposed capturing the origin of ergodicity-breaking in many biological processes: cascade dynamics. To assess our hypotheses, we embraced the challenge of identifying reliable biomarkers for heart disease and stroke, which, despite being the leading cause of death worldwide and decades of research, lacks reliable biomarkers and risk stratification tools. We showed that raw R-R interval data and its common descriptors based on mean and variance are non-ergodic and non-specific. On the other hand, the cascade-dynamical descriptors, the Hurst exponent encoding linear temporal correlations, and multifractal nonlinearity encoding nonlinear interactions across scales described the non-ergodic heart rate variability ergodically and were specific. This study inaugurates applying the critical concept of ergodicity in discovering and applying digital biomarkers of health and disease. © 2023, CC BY-NC-ND.

Background: Diabetes is one of the leading causes of death and disability worldwide, and affects people regardless of country, age group, or sex. Using the most recent evidentiary and analytical framework from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD), we produced location-specific, age-specific, and sex-specific estimates of diabetes prevalence and burden from 1990 to 2021, the proportion of type 1 and type 2 diabetes in 2021, the proportion of the type 2 diabetes burden attributable to selected risk factors, and projections of diabetes prevalence through 2050. Methods: Estimates of diabetes prevalence and burden were computed in 204 countries and territories, across 25 age groups, for males and females separately and combined;these estimates comprised lost years of healthy life, measured in disability-adjusted life-years (DALYs;defined as the sum of years of life lost [YLLs] and years lived with disability [YLDs]). We used the Cause of Death Ensemble model (CODEm) approach to estimate deaths due to diabetes, incorporating 25 666 location-years of data from vital registration and verbal autopsy reports in separate total (including both type 1 and type 2 diabetes) and type-specific models. Other forms of diabetes, including gestational and monogenic diabetes, were not explicitly modelled. Total and type 1 diabetes prevalence was estimated by use of a Bayesian meta-regression modelling tool, DisMod-MR 2.1, to analyse 1527 location-years of data from the scientific literature, survey microdata, and insurance claims;type 2 diabetes estimates were computed by subtracting type 1 diabetes from total estimates. Mortality and prevalence estimates, along with standard life expectancy and disability weights, were used to calculate YLLs, YLDs, and DALYs. Ultrasound kidney image segmentation presents significant challenges due to missing or ambiguous boundaries. In this study, we introduce a coarse-to-refinement approach incorporating four novel aspects. Firstly, we leverage the properties of a principal curve (PC) to automatically fine-tune the curve shape and employ a neural network's learning ability to reduce model error. Secondly, a deep fusion learning network is utilized for the coarse segmentation step, incorporating a parallel architecture to enhance deep-learning performance. Thirdly, addressing the limitation of standard PC-based methods in determining the number of vertices automatically, we propose an automatic searching polygon tracking method using a mean shift clustering-based approach to replace the projection and vertex extension step in standard PC-based methods. Lastly, we develop an explainable mathematical map function for the kidney contour, as denoted by the neural network output (i.e., optimized vertices), which aligns well with the ground truth contour. We conducted various experiments to evaluate our method's performance, demonstrating its effectiveness in ultrasound kidney image segmentation.
