Background: Previous studies on clinical decision support systems (CDSSs) for the management of renal anemia in patients with end-stage kidney disease undergoing hemodialysis have previously focused solely on the effects of the CDSS. However, the role of physician compliance in the efficacy of the CDSS remains ill-defined. Objective: We aimed to investigate whether physician compliance was an intermediate variable between the CDSS and the management outcomes of renal anemia. Methods: We extracted the electronic health records of patients with end-stage kidney disease on hemodialysis at the Far Eastern Memorial Hospital Hemodialysis Center (FEMHHC) from 2016 to 2020. FEMHHC implemented a rule-based CDSS for the management of renal anemia in 2019. We compared the clinical outcomes of renal anemia between the pre- and post-CDSS periods using random intercept models. Hemoglobin levels of 10 to 12 g/dL were defined as the on-target range. Physician compliance was defined as the concordance of adjustments of the erythropoietin-stimulating agent (ESA) between the CDSS recommendations and the actual physician prescriptions. Results: We included 717 eligible patients on hemodialysis (mean age 62.9, SD 11.6 years;male n=430, 59.9%) with a total of 36, 091 hemoglobin measurements (average hemoglobin and on-target rate were 11.1, SD 1.4, g/dL and 59.9%, respectively). The on-target rate decreased from 61.3% (pre-CDSS) to 56.2% (post-CDSS) owing to a high hemoglobin percentage of >12 g/dL (pre: 21.5%;post: 29%). The failure rate (hemoglobin <10 g/dL) decreased from 17.2% (pre-CDSS) to 14.8% (post-CDSS). The average weekly ESA use of 5848 (SD 4211) units per week did not differ between phases. The overall concordance between CDSS recommendations and physician prescriptions was 62.3%. The CDSS concordance increased from 56.2% to 78.6%. Nano-emulsions are alternatives for incorporation of essential oils in food-products. Despite generation of surfactant-free nano-emulsions is well known (e.g., Ouzo effect) its stabilization is a critical issue. This study shows the behavior of surfactant-free nano-emulsions prepared with two lemongrass essential oils based in dynamic light scattering analysis through exposure to a linear ramp of temperature. The gas-chromatography coupled to mass-spectrometry analysis revealed that β-myrcene was found in the Cymbopogon citratus and absent in C. flexuosus. C. citratus nano-emulsion prepared with deionized water (CCNE-DW) presented initial lower size diameter (208.6 ± 1.9 nm) than C. flexuosus nano-emulsion (245.2 ± 3.1 nm) prepared with deionized water (CFNE-DW), probably due to the presence of β-myrcene capable of avoiding a rapid droplet growth before onset of dynamic light scattering measurements. In silico physicochemical properties suggest that β-myrcene could considered decisive for low size of C. citratus nano-emulsions, since reduction of its concentration may induce destabilization of C. citratus diluted nano-emulsions. Similar initial droplet size was observed for C. citratus nano-emulsion (221.0 ± 1.3 nm) prepared with infusion (CCNE-IN) and C. flexuosus nano-emulsion (215.7 ± 1.7 nm) prepared with infusion (CFNE-IN). The highest droplet reduction was observed for CFNE-DW (DR_25–80ºC = 45.9 %;p<0.0001). Distinct phytochemical profile evidenced by ¹H NMR, since C. citratus infusion shows an envelope of signals with more overlapped peaks between the chemical shifts at δ_H 3.5 – 4.0 ppm, may favored solubilization of droplet components by the infusion and responsible by droplet growth of diluted nano-emulsions, in addition to decrease of β-myrcene content. Building on decades of work in characterizing the dozens of RNA modifications in the microbial epitranscriptome, recent advances in analytical technology and genetics have revealed systems-level functions for many tRNA modifications. The tRNA (uracil-5-)-methyltransferase TrmA and its product, 5-methyl uridine (m⁵U) at position 54 in the T-loop, however, has not been linked to a specific phenotype. Here, we defined the functional and biological roles of TrmA in Pseudomonas aeruginosa (PA14), a major multidrug-resistant pathogen. Surprisingly, though TrmA was found to site-specifically catalyze m⁵U54 on all PA14 tRNAs, loss of TrmA had no effect on the levels of any of 36 tRNA modifications except m⁵U and had minimal effects on multiple phenotypic parameters, including growth rate, morphology, motility, and biofilm formation. However, loss of TrmA conferred a striking polymyxin antibiotic resistance. mRNA and tRNA profiling and proteomics analyses revealed that TrmA regulates the expression of codon-biased gene families at the level of translation, including components of a type III secretion system (T3SS). Loss of TrmA upregulated T3SS, leading to increased macrophage IL-1β in bacterial challenge tests. Altogether, these results revealed novel biological functions of TrmA and its roles in modulating gene expression at multiple levels in P. aeruginosa. © 2025 Elsevier Ltd

Purpose: Remarkable progress in healthcare technology has recently been made alongside changes in concepts related to drugs and medical devices. It is speculated that this progress benefits not only patients but also healthcare professionals, such as medical doctors. We performed a systematic review of the characteristics of current data assessment tools to measure medical doctors’ work-related quality of life (QOL). Methods: A literature search was conducted through PubMed and Ichushi-Web in 2020. The related search terms used were ‘medical doctor,’ ‘quality of work life,’ and ‘questionnaire/interview.’ Two reviewers independently screened the studies, and the characteristics of the QOL assessment tools used in the identified studies were qualitatively reviewed and summarized. Results: In total, 5,443 and 760 articles were retrieved from PubMed and Ichushi-Web, respectively, of which 82 studies were included in this review. Sixty-five (79%) studies used structured questionnaires, and 17 (21%) studies used semistructured questionnaires. In terms of the study purpose, the identified studies mainly included four: mental health, the work or labor situation, satisfaction, and QOL. Components used to measure work-related QOL included satisfaction, burnout, QOL, the work environment, stress, mental health, work-life balance, and others. None of the studies used an originally developed QOL questionnaire to assess the work-related benefits of medical doctors. Conclusion: This systematic review found that there is a lack of studies directly assessing the work-related QOL of medical doctors and a lack of effective data collection tools to assess all work-related QOL components. © 2023 I work for Otsuka Holdings Co., Ltd and the joint research is the university of Tokyo. Breast cancer is a perilous disease, causing a high mortality rate and posing a major public health challenge worldwide. Traditional clinical diagnosis of breast cancer typically relies on tissue biopsy, which is invasive, time-consuming, costly and skill dependent. In contrast, liquid biopsy is minimally invasive by utilizing body fluid samples to detect biomarkers, such as microRNAs (miRNAs). This study explores a novel electrochemical biosensor based on a disposable screen-printed electrode comprising reduced graphene oxide (rGO) and molybdenum disulfide (MoS₂) for detecting miRNA-34a, a key biomarker of breast cancer. The sensor utilizes the advantageous properties of two-dimensional nanomaterials, including high surface area and conductivity, to enhance sensitivity and performance. Additionally, the presence of sulfur in the composite is beneficial for anchoring metal nanoparticles. Consequently, the screen-printed reduced graphene oxide and molybdenum disulfide composite electrodes (SPrGO-MoS₂Es) are effective for functionalization with gold nanoparticles (AuNPs), which further improve conductivity and enable specific probe immobilization through covalent binding. The biosensor detects miRNA-34a by measuring the current response of ferrocyanide as a redox probe using differential pulse voltammetry. The developed SPrGO/MoS₂E biosensor has a detection range of 0.1 nM to 1000 nM and a detection limit of 66 pM with excellent specificity. Moreover, the biosensor demonstrated high accuracy, precision, and stability for detecting miRNA-34a in serum samples with low, medium, and high concentrations. Therefore, it can be an easy-to-use, cost-effective tool with potential applications in point-of-care testing for breast cancer. © 2025

Aim: To develop and test a mobile application that supports the disease self-management of adolescents with type 1 diabetes during their transition to early adulthood. Design: A sequential mixed-methods design was employed. Methods: The application content was designed according to previously identified care needs and expectations, followed by application development on the Android operating system. From the outpatient clinic of the Department of Paediatric Endocrinology and Metabolism at a medical centre in northern Taiwan, 35 individuals aged between 16–25 years participated in application testing. Results: The overall median score of the QUIS was 4–5, most of the 25% quartile was 4–5, and all of the 75% quartile was 5, indicating adequate user interaction satisfaction. © 2022 The Authors. The foramen magnum (FM) occupies a well-protected position, making it particularly intriguing for forensic research. The necessity for methods to estimate sex from cranial fragments becomes evident when only a partial skull is available for identification. The objective of this study was to assess sex differences in anthropometric measurements, including the length of the foramen magnum (FML), width of the foramen magnum (FMW), foramen magnum area (FMA), and circumference, with the aim of developing a binary logistic regression model in Thai cadaveric donors. Sixty adult dry skulls underwent both intraclass and interclass correlation analyses. Subsequently, 240 adult dry skulls of known sex (120 males and 120 females) were meticulously measured and analyzed to determine the optimal threshold for sex determination. Morphometric analysis of the FM was carried out using vernier calipers, and the area of the FM was precisely calculated. The FML, FMW, FMA, and circumference were all found to be significantly larger in males than in females. Furthermore, FM circumference exhibited an AUC (Area Under the Curve) value of 0.908, signifying it as an excellent parameter for sex discrimination. Lastly, binary logistic regression was executed. Significant sex differences in FM circumference were consistently observed, demonstrating an accuracy of 81.62% when validated in 120 dry skulls. © 2024 The Authors

Human movement analysis is crucial in health and sports biomechanics for understanding physical performance, guiding rehabilitation, and preventing injuries. However, existing tools are often proprietary, expensive, and function as "black boxes", limiting user control and customization. This paper introduces vailá—Versatile Anarcho Integrated Liberation Ánalysis in Multimodal Toolbox—an open-source, Python-based platform designed to enhance human movement analysis by integrating data from multiple biomechanical systems. vailá supports data from diverse sources, including retroreflective motion capture systems, inertial measurement units (IMUs), markerless video capture technology, electromyography (EMG), force plates, and GPS/GNSS systems, enabling comprehensive analysis of movement patterns. Developed entirely in Python 3.11.9, which offers improved efficiency and long-term support, and featuring a straightforward installation process, vailá is accessible to users without extensive programming experience. In this paper, we also present several workflow examples that demonstrate how vailá allows the rapid processing of large batches of data, independent of the type of collection method. This flexibility is especially valuable in research scenarios where unexpected data collection challenges arise, ensuring no valuable data point is lost. We demonstrate the application of vailá in analyzing sit-to-stand movements in pediatric disability, showcasing its capability to provide deeper insights even with unexpected movement patterns. By fostering a collaborative and open environment, vailá encourages users to innovate, customize, and freely explore their analysis needs, potentially contributing to the advancement of rehabilitation strategies and performance optimization. Objective: Glucose homeostasis is the only way to manage diabetic progression as all medications used do not cure diabetes. This study was aimed at verifying the feasibility of lowering glucose with non-invasive ultrasonic stimulation. Methods: The ultrasonic device was homemade and controlled via a mobile application on the smartphone. Diabetes was induced in Sprague–Dawley rats through high-fat diets followed by streptozotocin injection. The treated acupoint CV12 was at the middle of the xiphoid and umbilicus of the diabetic rats.
