The decision to determine whether or not a journal to be published in Majalah Kedokteran Nusantara can be decided by using the Decision Support System with certain methods. One of methods used is fuzzy logic method and is applied to the Microsoft Visual Basic programming language. From the results of the research, the first step that must be done in determining a journal is published was to identify the problem and to determine how to evaluate the quality of the journal which is done by calculating the parameter values of the variables that have been determined by the editorial board. The parameters used in the calculation were title of articles, abstract, background, content, conclusions and bibliography. The conclusions obtained from the research is to building a decision support system to determine a journal published in Majalah Kedokteran Nusantara using fuzzy logic method was not so difficult to do, because it utilized Fis Sugeno Matlab application and by using fuzzy logic methods and utilizing fuzzyfication to get linguistic variables were very helpful in determining how a journal is published. © Published under licence by IOP Publishing Ltd.

PURPOSE:Urothelial carcinoma (UC) is a common disease in developed counties. This study aimed to identify autocrine roles and signaling pathways of gremlin 1, DAN family BMP antagonist (GREM1), which inhibits tumor growth and epithelial-mesenchymal transition (EMT) in UC. METHODS:Systematic in vitro and in vivo studies using genetic engineering, different urinary bladder urothelial carcinoma (UBUC)-derived cell lines, and mouse models were performed, respectively. Further, primary upper tract urothelial carcinoma (UTUC) and UBUC specimens were evaluated by immunohistochemistry. RESULTS:GREM1 protein levels conferred better disease-specific and metastasis-free survival rates and played an independent prognostic factor in UTUC and UBUC. Hypermethylation is the primary cause of low GREM1 levels. In different UBUC-derived cell lines, the autocrine/secreted and glycosylated GREM1 interacted with transforming growth factor beta 1 (TGFB1) and inhibited TGFβ/BMP/SMAD signaling and myosin light chain 9 (MYL9) transactivation, subsequently cell proliferation and epithelial-mesenchymal transition (EMT). Secreted and glycosylated GREM1 also suppressed tumor growth, metastasis, and MYL9 levels in the mouse model. Instead, cytosolic GREM1 promoted cell proliferation and EMT by activating the tumor necrosis factor (TNF)/AKT/nuclear factor kappa B (NFκB) axis. CONCLUSIONS:Clinical associations, animal models, and in vitro indications provided solid evidence to show that the epithelial autocrine GREM1 is a novel tumor suppressor in UCs. The glycosylated-GREM1 hampered cell proliferation, migration, invasion, and in vitro angiogenesis through interaction with TGFB1 to inactivate TGFβ/BMP/SMAD-mediated EMT in an autocrine manner.

Background: Pruritus is the most common initial symptom reported by patients with dermatomyositis (DM) and polymyositis (PM). However, there is limited data regarding the impact of pruritus on cancer and mortality in patients with DM and PM. In this study, we aimed to investigate how pruritus is associated with cancer and mortality in patients with DM and PM. Methods: This nationwide, population-based retrospective cohort study included adult DM and PM patients from Taiwan’s National health Insurance Research Database between 2005 and 2022. Sex- and age-matched pruritic patients, identified by over 6 weeks of antipruritic medication use, and nonpruritic patients were analyzed. The primary outcome was cancer occurrence or all-cause mortality. The association between pruritus and these outcomes was estimated using Cox proportional hazards models. Results: Among 919 matched pairs of pruritic and nonpruritic patients, cancer was observed in 19.96% in the long-term pruritic group (LPG), 14.63% in the short-term pruritic group (SPG), and 10.34% in the non-pruritic group (NPG) ( p < 0.0001). All-cause mortality was documented as 30.37% in the LPG, 29.69% in the SPG, and 37.76% in the NPG ( p < 0.0001). After adjusting for sex, age, and other comorbidities, pruritus was associated with an increased risk of cancer (hazard ratio (HR) 1.708, 95% confidence interval (CI) 1.229–2.374) and a lower risk of all-cause mortality (HR 0.483, 95% CI 0.409–0.569). Conclusion: This population-based study revealed that pruritus appeared to be associated with increased risks of cancer and decreased all-cause mortality. Thus, pruritus may serve as a pragmatic factor for risk stratification and tailored treatment strategies in DM and PM. Cancer screening, particularly for nasopharyngeal and breast cancers in East Asian populations, is recommended for patients with DM or PM, especially those presenting with pruritus. BACKGROUND:This study aimed to identify adverse events following the first three doses of COVID-19 vaccines in hemodialysis (HD) patients. Risk factors associated with postvaccination adverse events were explored. METHODS:Postvaccination adverse events in 438 HD patients who received 3 doses of COVID-19 vaccines were prospectively assessed. The adverse events among three doses were compared using generalized linear mixed models. Factors associated with adverse events were assessed with multivariate analyses. RESULTS:The vast majority of participants received Oxford/AstraZeneca ChAdOx1 as their first two doses and Moderna mRNA-1273 as their third dose. Overall, 79%, 50% and 84% of the participants experienced at least one adverse event after their first, second, and third doses, respectively. These adverse events were mostly minor, short-lived and less than 5% reported daily activities being affected. Compared with the first dose, the second dose caused a lower rate of adverse events. Compared with the first dose, the third dose elicited a higher rate of injection site reactions and a lower rate of systemic reactions. Multivariate analyses showed that every 10-year increase of age (odds ratio 0.67, 95% confidence intervals 0.57-0.79) was associated with decreased risk of adverse events, while female sex (2.82, 1.90-4.18) and arteriovenous fistula (1.73, 1.05-2.84) were associated with increased risk of adverse events. Compared with Oxford/AstraZeneca ChAdOx1, Moderna mRNA-1273 was associated with an increased risk of injection site reactions. CONCLUSIONS:COVID-19 vaccination was well tolerated in HD patients. Age, sex, dialysis vascular access and vaccine types were associated with postvaccination adverse events.

Background The 5- to 7-year shift is a critical period for cognitive development and is particularly sensitive to lifestyle factors such as sedentary behavior (SB). Screen-based activities can be classified into two types: mentally active SB requires higher cognitive engagement (e.g., computer use, electronic gaming), and mentally passive SB involves less cognitive engagement (e.g., TV-watching). Although there has been a growing interest in understanding the role of such forms of SB on cognitive development in recent years, is associated with cognitive development trajectories, considering the moderating role of the SB duration, SB type, and sex. Methods Data from 12,257 children (6178 boys; 6079 girls) in the UK Millennium Cohort Study (MCS), a longitudinal and nationally representative birth cohort study in the UK, were analyzed. The MCS includes children born between September 2000 and January 2002. For this study, we used data from wave 3 (at the age of 5 years) to assess the association between computer use/e-gaming and TV-watching time with cognitive performance assessed in wave 4 (at the age of 7 years). Cognitive performance was assessed using the British Ability Scales II (BAS II), including word reading and pattern construction tests, and an adapted National Foundation for Educational Research (NFER) Progress in Maths test. Statistical analyses were performed via a Generalized Linear Model (GLM) adjusted for confounders and stratified by sex. Results For boys, computer use/e-gaming at the age of 5 years was associated with better word reading performance at the age of 7 years for those who engaged in it for less than 1 h ( β = 0.148, 95% CI: 0.081 to 0.215, p < 0.001) and 1–3 h ( β = 0.209, 95% CI: 0.128 to 0.290, p < 0.001) per day. Regarding maths performance, computer use/e-gaming for less than 1 h ( β = 0.179, 95% CI: 0.120 to 0.237, p < 0.001) and 1–3 h ( β = 0.181, 95% CI: 0.101 to 0.261, p < 0.001) per day at the age of 5 years were associated

Bladder cancer stands as a prevailing neoplasm among men globally, distinguished for its pronounced malignancy attributed to invasiveness and metastatic proclivity. Tannic acid (TA), an organic compound in many plants, has garnered recent attention for its discernible anti-mutagenic attributes. This investigation endeavored to scrutinize the repercussions of TA on grade II bladder cancer, with a concerted focus on unraveling its anti-cancer mechanisms. The cytotoxic effects of TA on grade II bladder cancer cells were investigated using multiple techniques, including MTT assay, flow cytometry, TUNEL assay, and western blot. Our findings revealed that elevated concentrations of TA induced cytotoxic effects in grade II bladder cancer cells. Both flow cytometry and the TUNEL assay substantiated the dose-dependent capacity of TA to prompt apoptosis. Western blot analysis corroborated that TA treatment in bladder cancer cells resulted in the upregulation of cleaved caspase-3 expression and PARP. Furthermore, heightened TA dosage elicited an augmentation in the expression of pro-apoptotic proteins, namely Bax and Bak, alongside a reduction in the expression of the anti-apoptotic protein Bcl-2 within bladder cancer cells. This study confirms TA as a potential anticancer agent, demonstrably diminishing the viability of bladder cancer cells. TA exerts cytotoxicity through the activation of mitochondrial apoptotic pathways. Specifically, TA initiates the cleavage of PARP and caspase-3, concurrently augmenting the expression of pro-apoptotic proteins to facilitate apoptosis. Collectively, the present study indicates that TA effectively impedes the proliferation of bladder cancer cells by instigating apoptosis through the intrinsic mitochondrial pathway.
