Patients diagnosed with co-morbidities are clinically compromised, facing social and psychological challenges. The identification of high risk patients is a key question to maximize their quality of life and the health System's resources. Objective: to determine patterns that identify eligible patients for Case Management programs, with KDD and AI tools. Methodology: analysis, selection and correction, formatting and mining of data, analysis of results, data submission to an AI tool. Results: solid recommendations on benefits that health Systems could obtain by using these tools, regarding the acknowledgement of their users profile and the use of predictive modeling. © 2006 The authors and IOS Press. The goal of this investigation was to develop improved photosensitizers for use as antimicrobial drugs in photodynamic therapy of localized infections. Replacement of the oxygen atom in 5-(ethylamino)-9-diethylaminobenzo[a]phenoxazinium chloride (1) with sulfur and selenium afforded thiazinium and selenazinium analogues 2 and 3, respectively. All three dyes are water soluble, lipophilic, and red light absorbers. The relative photodynamic activities of the chalcogen series were evaluated against a panel of prototypical pathogenic microorganisms: the Gram-positive Enterococcus faecalis, the Gram-negative Escherichia coli, and the fungus Candida albicans. Selenium dye 3 was highly effective as a broad-spectrum antimicrobial photosensitizer with fluences of 4−32 J/cm² killing 2−5 more logs of all cell types than sulfur dye 2, which was slightly more effective than oxygen analogue 1. These data, taken with the findings of uptake and retention studies, suggest that the superior activity of selenium derivative 3 can be attributed to its much higher triplet quantum yield.

We review the evidence from a number of surveys in which our NetLab has been involved about the extent to which the Internet is transforming or enhancing community. The studies show that the Internet is used for connectivity locally as well as globally, although the nature of its use varies in different countries. Internet use is adding on to other forms of communication, rather than replacing them. Internet use is reinforcing the pre-existing turn to societies in the developed world that are organized around networked individualism rather than group or local solidarities. The result has important implications for civic involvement.

Variation in routine psychiatric workload: The role of financing source, managed care participation, and mental health workforce competition

This study was conducted to examine the association between psychiatrists' demographic characteristics, payment source, and managed care participation and psychiatrists' practice workload, and between the supply of other mental health providers in a psychiatrist's county of practice and psychiatrists' practice workload. Data from the 1996 American Psychiatric Association National Survey of Psychiatric Practice were merged with national countywide measures of mental health workforce and environmental data from the 1996 Area Resource File. In comparison to male psychiatrists, female psychiatrists treat fewer patients per week, provide less total hours of weekly patient care, and obtain fewer new monthly referrals. An increase in psychiatrists' managed care participation was associated with only minor increases in the number of patients per week, weekly time spent in clinical care, and number of new monthly referrals. The supply of other mental health providers was not associated with variation in practice workload. Once psychiatrists participate in managed care plans, an increase in their participation rate does not significantly expand clinical practice workload. The supply of other mental health providers was not significantly associated with variation in psychiatrists' workload, which suggests that substitution effects may not be evident with this aspect of psychiatric practice.

Research: The Emergency Contraception Collaborative Prescribing Experience in Washington State

Objective To describe how prescribers and pharmacists view the Emergency Contraceptive Pills (ECP) program, and to evaluate pharmacists’ performance through the use of a consumer survey. Design Self-administered provider satisfaction surveys were mailed 6 months after the program’s inception. Consumer satisfaction surveys were distributed at the point of ECP service for return by mail. Setting The program encouraged pharmacists and prescribers in western Washington to enter into collaborative prescribing agreements, increasing consumers’ access to ECP. Patients or Other Participants Pharmacists who had attended ECP training sessions, prescribers who had authorized pharmacists to prescribe ECP, and women who had been prescribed ECP by pharmacists. Main Outcome Measures Providers’ reasons for participating, attitudes toward the ECP program, and experiences with ECP as a result of the program; feedback from women receiving ECP from pharmacists. Results 309 pharmacist surveys and 55 prescriber surveys were sent, of which 159 (51%) and 27 (49%), respectively, were returned. Meeting patient needs and having a professional responsibility to participate were commonly reported reasons for ECP program involvement. Both pharmacists and prescribers (92%) reported being "satisfied" or "very satisfied" with their prescribing agreements. On the 470 consumer surveys returned out of 7,000 distributed (6.5%), pharmacists were rated highly satisfactory for their interactions with patients and the quality of information about ECP use given, but less satisfactory for information about adverse effects, recognition and follow-up of ECP failure, and regular contraceptive methods. Conclusion All participants expressed satisfaction with the ECP program. This example should support the initiation of similar programs in other states.

Occupational Hazard Exposure and at Risk Drinking

This study examined associations between workers' reported exposure to occupational hazards and at risk drinking. A sample of 15, 907 working adults was drawn from the 1985 National health Interview Survey (NHIS) (weighted sample represented 85, 395, 000 workers). This was the only year the NHIS included questions on both occupational hazard exposure and at risk drinking. Occupational hazard exposures included chemical/biological substances, physical hazards, injury risk, and mental stress. At risk drinking was defined as binge drinking and drinking and driving. Prevalence adjusted odds ratios were estimated. Sixty percent of workers reported exposure to one or more occupational hazards with considerable variation among and within occupations. In all, 31% reported binge drinking and 15% drove after drinking too much. In a multivariate analysis that controlled for background characteristics, workers who reported occupational hazard exposures were 1.2 to 1.4 times more likely to engage in binge drinking than workers without exposures. Similar results were found for drinking/driving. All multivariate results were statistically significant. Findings suggest workers who report occupational hazard exposures are at greater risk of both binge drinking and drinking/driving. Occupational and environmental health nurses can lead workplace initiatives to reduce occupational hazard exposure and, simultaneously, invest in health promotion efforts to curb at risk drinking among workers.

In this paper, we estimate carbon monoxide (CO) concentrations and exposures in a well-mixed, hypothetical village kitchen using measured CO emission factors and published typical values for house, fuel, and activity parameters. The estimate was made to compare CO exposure potentials for a range of no-flue fuel/stove combinations. Based upon this quantitative comparison, recommendations were made on CO exposure reduction by fuel/stove switching. If their house conditions and activity patterns are comparable to those typical values assumed in our estimation, people using biomass and coal cookstoves could have daily CO exposures greater than the exposure equivalents of health-based national standards and WHO guidelines.

Cookstoves in developing countries are individually small, but so numerous that, depending on emission factors, they could be significant influences on global and regional carbon monoxide (CO) inventories. This paper presents a new database of CO emission factors for commonly used cookstoves in developing countries. The emission factors were determined using a carbon balance approach for 56 types of fuel/stove combinations in China and India. These include various stoves (e.g., traditional, improved, mud, brick, and metal, with and without chimney) using animal dung, different species of crop residues and wood, root fuel, charcoal, kerosene, and several types of coals and gases. The chosen fuel/stove combinations represent a large fraction of the total in developing countries. Thus, the database can be used to improve estimates of CO emission inventories. The CO emission factors ranged widely, from 3.0×10-2 g/kg for the coal gas/traditional stove to 2.8×102 g/kg for the charcoal/Angethi stove, nearly 4 orders of magnitude. Since stove efficiencies and fuel energy contents were measured simultaneously, CO emission factors on the basis of a standard cooking task (energy delivered) were also determined and reported in this paper. Task-based emission factors are particularly useful for comparing the air pollution potential of different fuel/stove combinations and assessing the impacts of substitutions.
