Aims and Objectives: This study aims to analyse the trends in the incidence, prevalence and medical costs of pressure injuries (PIs) among genders in Taiwan. Background: The treatment of PIs is complex and costly, often leading to complications and increased mortality. This issue significantly impacts healthcare quality and incurs substantial medical and social costs, warranting attention. Methods: A retrospective cohort study was conducted using data from Taiwan's National health Insurance Database to obtain and calculate the incidence, prevalence, and medical costs of PIs in the country between 2001 and 2015 as well as to analyse high-risk groups and the medical care utilisation of patients following the STROBE reporting guidelines. Results: Between 2001 and 2015, 15,327 incident case of PIs were diagnosed. During the study period, the prevalence rate of PIs per 100,000 population rose from 26.3 to 189.6, with approximately 11.5%–16.3% of patients undergoing surgical debridement. The PIs prevalence rate increased by 7.2-fold, and hospitalisation costs accounted for 91.7%–96.0% of the total medical costs. Patients with older age, comorbidities, poorer financial status and lower education levels were found to be likely to develop PIs. These predisposing factors differed between males and females. The prevalence of PIs was higher in patients ≥75 years old than in patients from other age groups. Moreover, PI-related medical expenses have been increasing annually. Conclusions: In Taiwan, the rising incidence of PIs is driving up medical costs. Effective care and prevention of PIs necessitate a comprehensive plan from the entire healthcare system. Relevance to Clinical Practice: This research fills a gap in the available data on the incidence, prevalence, and medical costs of PIs in Taiwan and Asia. Patient or Public Contribution: The findings can be used to help develop clinical guidelines for preventive education and treatment of PIs. © 2024 John Wiley & Sons Ltd.

Opisthorchis viverrini infection causes various complications in patients, ranging from asymptomatic to severe chronic disease including cholangiocarcinoma (CCA). O. viverrini is endemic in Southeast Asia and acting as a risk for CCA. Early diagnosis of O. viverrini infection can reduce the number of CCA cases. The routine diagnosis for opisthorchiasis is direct wet smear, sometimes coupled with concentration techniques, which has limitations when investigating light infection or if done by laboratorians with lack of experiences. PCR-based methods have been established for the detection of O. viverrini egg DNA from stool samples, but have never fully succeeded for light infections when compared to wet smear concentration techniques. This study aims to improve the PCR-based method for detection of O. viverrini eggs in stool samples by targeting the genes ITS-2, cox1, and cyb. The results reveal higher sensitivity than conventional concentration techniques, with all newly designed primers. ITS-2 has an overall sensitivity of 76.9% with 66.7% in the samples with < 50 EPG, while cox1 has shown 96.2% overall sensitivity and 94.1% in the same EPG intervals. Interestingly, the new pointing target, cyb, has shown 100% sensitivity in all egg intervals in this study, particularly for light infections (EPG less than 100). No cross-reactivity was found in Taenia spp., Trichuris trichiura, Ascaris lumbricoides, Capillaria philippinensis, and hookworm. The procedure is convenient, with shorter steps compared to previous reports, and it appears appropriate for use in the diagnosis of light infection with O. viverrini. These three genes are good candidates for use in PCR-based detection of the parasite eggs. Further testing with a larger cluster of samples is however necessary. © 2021, Indian Society for Parasitology.

Sudden cardiac arrest (SCA) poses a significant health challenge, necessitating accurate predictions of neurological outcomes in comatose patients, where good outcomes are defined as the recovery of most cognitive functions. Electroencephalogram (EEG) serves as a valuable biomarker for monitoring neurological states due to its rich, time-dependent information. This study aims to predict neurological outcomes using early EEG data by employing the Transformer model, which leverages multi-headed attention to identify patterns in lengthy sequences such as hour-long EEG recordings. Unlike traditional methods that use subsampled EEG epochs, we utilize the entire EEG sequences, subdivided into time steps, allowing our model to capture detailed temporal patterns via the attention mechanism. Moreover, we trained our proposed model using each EEG recording as an individual data sample but evaluated our model through aggregated patient-wise predictions. This approach allows us to boost the data sample size. Our results demonstrate promising predictive performance, achieving an AUROC of 0.82 and AUPRC of 0.90 on the holdout test set and an AUROC of 0.73 and AUPRC of 0.93 on an external test set with patient-wise predictions. This study highlights the potential of utilizing attention mechanisms to capture important time series progressions across EEG sequences for improving SCA prognosis. © 2024 IEEE.

The electronic health record (EHR) data, widely used by hospitals and healthcare professionals, contain valuable information about the patient and treatments and has become increasingly relevant to clinical natural language processing (NLP) tasks. Although the growing number of EHR systems, these medical data contain sensitive information and cannot be released publicly without data security measures and privacy protection. The de-identification process is necessary to detect and remove all sensitive information in EHRs. There are several techniques to de-identify textual data, such as rule-based, machine learning and hybrid methods. This work proposes a new method to de-identify Portuguese clinical data based on dictionaries, phonetic normalization, similarity measures and regular expressions. We used actual EHR cardiology data from a Brazilian hospital to develop an algorithm to identify and replace data such as proper names, birth dates, credentials, emails, and phone numbers. Although still under development, our method correctly de-identified 318 documents (93.67% of the evaluated documents). © 2024, The Author(s), under exclusive license to Springer Nature Switzerland AG.

Background and aim: Aloysia gratissima is used in popular medicine as an analgesic and sedative. However, studies on its biological activities are still scarce. Therefore, this work aimed to evaluate the antinociceptive and anti-inflammatory effect of A. gratissima leaves essential oil (OAG) in mice. Experimental procedure: The OAG was obtained by hydrodistillation and its composition was determined by gas chromatography-mass spectrometry. The sesquiterpenes pinocamphone, β-pinene, and guaiol were the major constituents of OAG. The effect of OAG (1, 10, and 30 mg/kg, p.o.) was assessed by pre-treating male mice 1h before the formalin assay, carrageenan-induced peritonitis test, or the paw edema induced by arachidonic acid (AA). The acute toxicity of OAG (2000 mg/kg, p.o.) was also investigated. Results and conclusion: The minimum effective dose of OAG in the formalin test was 1 mg/kg. This dose did not affect the locomotor activity of mice. The OAG decreased the number of leukocytes/mm³ in the peritoneal exudate of mice and reduced paw edema 45 min after the arachidonic acid injection. OAG treatment elicited a reduction in NPSH levels in the paw tissue and increased NPSH levels in mice blood plasma and did not cause mice mortality. The OAG is devoid of acute toxicity and shows anti-inflammatory activity, which can be related to the presence of pinocamphone, guaiol, and β-pinene and is mediated, at least in part, by mechanisms that involve the participation of NPSH. © 2024 Center for Food and Biomolecules, National Taiwan University

Evaluation of age-associated DNA methylation markers in colorectal cancer of Thai population

Age prediction using biomarkers has been extensively studied which can be helpful in forensic and health sciences. DNA methylation has been demonstrated to be the most promising molecular biomarkers for age prediction in forensic sciences, and many prediction models have been developed based on blood samples. However, besides age, DNA methylation has also been affected by various factors, including diseases. To determine the effect of colorectal cancer (CRC) on age prediction, the present study analyzed DNA methylation of age-associated CpGs in the peripheral blood of CRC patients and matched controls aged 50–60 years using bisulfite conversion and pyrosequencing. The analyzed CpGs were in six genes (ASPA, EDARADD, ELOVL2, KLF14, PDE4C and TRIM59) constituted in three evaluated prediction models, namely Bekaert 2015, Thong 2017 and Thong 2021. Following the age prediction using these models, the results showed no significant difference of predicted age in CRC patients compared to the controls. This suggests that CRC may not influence DNA methylation of the selected CpGs and the accuracy of age prediction. The results from this study could provide helpful information for selecting DNA methylation-based biomarkers for age prediction. © 2022 The Authors

Hamstring tendon rupture in elite judo athletes

Objectives To describe the characteristics of hamstring tendon rupture and the rehabilitation process in elite judo athletes who underwent surgical intervention and physical therapy. Design Case series study. Participants Three professional judo athletes. Main outcome measures Characteristics of the injury, surgical repair, rehabilitation process, and return to sport were evaluated. Results The sample consisted of two women and one man (37.3 ± 11.0 years old), elite judo athletes (24–31 years of practice; participation in Olympic Games, Pan American Games, and World Championships). All athletes injured the supporting leg and had the rupture after abruptly extending the knee with the hip in flexion, in a closed kinetic chain. There were between-athletes differences in the physical therapy regimen before and after the surgery, and in the surgical repair techniques. Athletes returned to sport after 3-, 5-, and 7-months post tendon surgical repair. More specifically, one of the athletes returned to Uchi Komi at 3-months and to Randori at 5-months, while the other two returned at 5- and 7-months post-surgery, respectively. There were no cases of injury recurrence. Conclusion Hamstring tendon rupture occurred after abruptly extending the knee with the hip in flexion, in a closed kinetic chain, affecting the supporting leg in judo athletes. Physical therapy seems to be an important resource for recovery, since these athletes’ return to sport was early compared to other modalities reported in the literature.
