Evaluating the accuracy of impaired skin integrity in critically ill patients: Key characteristics and clinical implications

Objective: To evaluate the accuracy of the defining characteristics of the nursing diagnosis Impaired skin integrity (00046) in patients admitted to intensive care units (ICUs). Methods: A cross-sectional diagnostic accuracy study was conducted with 105 adult patients admitted to an ICU. A latent class model with random effects was used to test the sensitivity and specificity of the defining characteristics investigated. The diagnosis Impaired skin integrity (00046) was the dependent variable, whereas sociodemographic and clinical data were the independent variables. Results: Impaired skin integrity was present in 3.75% of the sample. The defining characteristic with the best accuracy for the diagnosis was dry skin, with high sensitivity (0.9994) and specificity (0.9106). Other characteristics stood out in terms of sensitivity measures: altered skin color (0.9994) and foreign matter piercing skin (0.9994). In terms of specificity, the following stood out: desquamation (1.000), localized area hot to touch (0.9901), pruritus (0.9897), bleeding (0.9802), and hematoma (0.9208). Conclusion: The defining characteristics that helped infer the diagnosis Impaired skin integrity (00046) with greater certainty were dry skin, altered skin color, foreign matter piercing skin, desquamation, localized area hot to touch, pruritus, bleeding, and hematoma. Implications for nursing practice: Identifying defining characteristics with high diagnostic accuracy for Impaired skin integrity (00046) enables nurses to expand their clinical perspective on this dysfunction, which can affect the skin of critically ill patients, and to develop individualized care plans. © 2025 NANDA International, Inc.

With the advancement of technology, people constantly use IT through the Internet. In this modern living environment, many individuals exploit this technological landscape for illegal gains and criminal activities. Judicial investigation units gather relevant evidence from the crime scene and process it when a crime occurs. When the medium used in criminal activity involves information technology, it is referred to as digital evidence. Unlike traditional evidence, which leaves traces if moved or altered, digital evidence can be copied without any detectable changes, posing challenges to maintaining its integrity and accuracy. This article explores how digital identification can ensure that the content of a report matches the evidence seized at the scene. It also addresses how the validity of digital evidence, as presented by the investigating authority, can be upheld in court. Additionally, it discusses the use of digital evidence and electronic handwritten signatures to enhance evidence preservation measures. © 2024 IEEE.

The S-locus lectin receptor kinases (G-LecRKs) have been suggested as receptors for microbe/damage-associated molecular patterns (MAMPs/DAMPs) and to be involved in the pathogen defense responses, but the functions of most G-LecRKs in biotic stress response have not been characterized. Here, we identified a member of this family, G-LecRK-I.2, that positively regulates flg22- and Pseudomonas syringae pv. tomato (Pst) DC3000-induced stomatal closure. G-LecRK-I.2 was rapidly phosphorylated under flg22 treatment and could interact with the FLS2/BAK1 complex. Two T-DNA insertion lines, glecrk-i.2–1 and glecrk-i.2–2, had lower levels of reactive oxygen species (ROS) and nitric oxide (NO) production in guard cells, as compared with the wild-type Col-0, under Pst DC3000 infection. Also, the immunity marker genes CBP60g and PR1 were induced at lower levels under Pst DC3000 hrcC^- infection in glecrk-i.2–1 and glecrk-i.2–2. The GUS reporter system also revealed that G-LecRK-I.2 was expressed only in guard cells. We also found that G-LecRK-I.2 could interact H⁺-ATPase AHA1 to regulate H⁺-ATPase activity in the guard cells. Taken together, our results show that G-LecRK-I.2 plays an important role in regulating stomatal closure under flg22 and Pst DC3000 treatments and in ROS and NO signaling specifically in guard cells. © 2024 Elsevier B.V.

Two new non-withanolidal steroids with a cross-conjugated dienone structural unit in ring A, capnesterones A (1) and B (2), were isolated from the soft coral Capnella imbricata in the southeast waters of Taiwan. The compounds' structures were elucidated through a comprehensive analysis of spectroscopic data, including IR, ESIMS, ¹H NMR, and ¹³C NMR, and through comparison with previous literature. At a concentration of 10 μM, steroids 1 and 2 exhibited reductions in iNOS levels (60 % and 82 %) compared to LPS-stimulated control cells, along with increased release (134 % and 110 %) against COX-2 protein expressions. © 2024 Phytochemical Society of Europe

With the recent rapid development of the Internet of Vehicles and autonomous driving technology, the demand for digital vehicle identification has also increased. Coupled with the gradual maturity of quantum technology, the Internet of Vehicles, which mainly uses wireless communication technology for data transmission, will face the threat of quantum computing and privacy issues. This research proposes the Vehicle Forensics Cloud System (VFCS), a framework combining post-quantum cryptography and blockchain technology to address these issues and provide a comprehensive solution for vehicle digital forensics. © 2024 IEEE.

Two new 16-hydroxybriaranes, 11-dehydroxybriavioid B (1) and 12-epi-briastecholide L (2), have been isolated from an algal-containing octocoral identified as Briareum stechei. The structures of metabolites 1 and 2 were corroborated by extensive 2D-NMR studies and comparison with literature data. Briarane 2 was found to reduce the release of inducible nitric oxide synthase (iNOS) to 87.92 %, at a concentration of 10 μM. © 2023 Phytochemical Society of Europe

The structures of sterols 1–3 were determined by extensive spectroscopic data analysis and by comparison with literatures. Additionally, the 24R-configuration of sterol 3 was identified by a single-crystal X-ray diffraction analysis and 13C chemical shifts for first time. The anti-inflammatory activities of sterols 1–3 were evaluated by examining their inhibitory effects on the generation of superoxide anions and the release of elastase against activated human neutrophils. © 2023

Chemical investigations on the ethyl acetate extract of the octocoral Capnella imbricata collected from the waters of Taiwan, which led to the isolation of three capnellenes (1–3), including one new metabolite, 9(12)-capnellene-8β,10α,15-triol (1), along with two known metabolites, 9(12)-capnellene-5α,8β,10α-triol (2), and 9(12)-capnellene-8β,10α-diol (3). The structures of the isolated metabolites were elucidated through extensive spectroscopic analyses, while the relative stereochemistry of 3 was confirmed by a single-crystal X-ray diffraction analysis for the first time in this study. Compounds 1–3 were evaluated in terms of their anti-inflammatory activity by examining their inhibitory effects on the generation of superoxide anions and the release of elastase by human neutrophils. © 2022 Elsevier B.V.

BACKGROUND AND PURPOSE:Acute respiratory distress syndrome (ARDS) is a catastrophic pulmonary inflammatory dysfunction with a high mortality rate. An overwhelming immune response by neutrophils is a key feature in infective or sterile ARDS. The formyl peptide receptor 1 (FPR1) is a crucial damage-sensing receptor for inflammatory reactions in the initiation and progression of neutrophil-mediated ARDS. However, effective targets for controlling dysregulated neutrophilic inflammatory injuries in ARDS are limited. EXPERIMENTAL APPROACH:Human neutrophils were used to explore the anti-inflammatory effects of cyclic lipopeptide anteiso-C13-surfactin (IA-1) from marine Bacillus amyloliquefaciens. The lipopolysaccharide-induced model of ARDS in mice was used to determine the therapeutic potential of IA-1 in ARDS. Lung tissues were harvested for histology analyses. KEY RESULTS:The lipopeptide IA-1 inhibited immune responses of neutrophils, including respiratory burst, degranulation, and expression of adhesion molecules. IA-1 inhibited the binding of N-formyl peptides to FPR1 in human neutrophils and in hFPR1-transfected HEK293 cells. We identified IA-1 as a competitive FPR1 antagonist, thus diminishing the downstream signalling pathways involving calcium, mitogen-activated protein kinases and Akt. Furthermore, IA-1 ameliorated the inflammatory damage to lung tissue, by decreasing neutrophil infiltration, reducing elastase release and oxidative stress in endotoxemic mice. CONCLUSION AND IMPLICATIONS:The lipopeptide IA-1 could serve as a therapeutic option for ARDS by inhibiting FPR1-mediated neutrophilic injury.
