Background and objective: The promising use of artificial intelligence (AI) to emulate human empathy may help a physician engage with a more empathic doctor-patient relationship. This study demonstrates the application of artificial empathy based on facial emotion recognition to evaluate doctor-patient relationships in clinical practice. Methods: A prospective study used recorded video data of doctor-patient clinical encounters in dermatology outpatient clinics, Taipei Municipal Wanfang Hospital, and Taipei Medical University Hospital collected from March to December 2019. Two cameras recorded the facial expressions of four doctors and 348 adult patients during regular clinical practice. Facial emotion recognition was used to analyze the basic emotions of doctors and patients with a temporal resolution of 1 second. In addition, a physician-patient satisfaction questionnaire was administered after each clinical session, and two standard patients gave impartial feedback to avoid bias. OBJECTIVE:Although unhealthy diets exacerbate nutritional and metabolic derangements in patients with end-stage kidney disease (ESKD), how therapeutic diets that possess a variety of different dietary strategies acutely modify diverse biochemical parameters related to cardiovascular disease remains underexplored. METHODS:Thirty-three adults with end-stage kidney disease undergoing thrice-weekly hemodialysis participated in a randomized crossover trial comparing a therapeutic diet with their usual diets for 7 days, separated by a 4-week washout period. The therapeutic diet was characterized by adequate calorie and protein amounts, natural food ingredients with a low phosphorus-to-protein ratio, higher portions of plant-based food, and high fiber content. The primary outcome measure was the mean difference in the change-from-baseline intact fibroblast growth factor 23 (FGF23) level between the 2 diets. The other outcomes of interest included changes in mineral parameters, uremic toxins, and high-sensitivity C-reactive protein (hs-CRP) levels. RESULTS:Compared with the usual diet, the therapeutic diet lowered intact FGF23 levels (P = .001), decreased serum phosphate levels (P < .001), reduced intact parathyroid hormone (PTH) levels (P = .003), lowered C-terminal FGF23 levels (P = .03), increased serum calcium levels (P = .01), and tended to lower total indoxyl sulfate levels (P = .07) but had no significant effect on hs-CRP levels. Among these changes, reduction in serum phosphate level achieved in 2 days, modifications of intact PTH and calcium levels in 5 days, and reductions in intact and C-terminal FGF23 levels in 7 days of therapeutic diet intervention. CONCLUSION:Within the 1-week intervention period, the dialysis-specific therapeutic diet rapidly reversed mineral abnormalities and tended to decrease total indoxyl sulfate levels in patients undergoing hemodialysis but had no effect on inflammation. The development of information and communication technology at this time has touched all sides of life. Almost no side of human life is not touched by this information technology, which is more synonymous with the world of computerization. No exception also with the computerization that is popular in the corporate environment and government agencies. If in ancient times everything was still done manually. However, at this time all leaders and management have realized the importance of this information technology products that can facilitate them in carrying out the day-to-day corporate functions. Decision Making System of goods purchasing is one of the determinants of the accumulation of goods. If the decision is taken right then stockpiling of goods can be avoided. In this thesis the authors designed a web-based system that helps Management in making the decision of the purchase amount of goods. the system was developed by using Microsoft Visual Studio 2010 and using SQL Server database processing applications. The end result of system design is expected to help minimize the accumulation of goods.

Background: Decades of steady improvements in life expectancy in Europe slowed down from around 2011, well before the COVID-19 pandemic, for reasons which remain disputed. We aimed to assess how changes in risk factors and cause-specific death rates in different European countries related to changes in life expectancy in those countries before and during the COVID-19 pandemic. Methods: We used data and methods from the Global Burden of Diseases, Injuries, and Risk Factors Study 2021 to compare changes in life expectancy at birth, causes of death, and population exposure to risk factors in 16 European Economic Area countries (Austria, Belgium, Denmark, Finland, France, Germany, Greece, Iceland, Ireland, Italy, Luxembourg, the Netherlands, Norway, Portugal, Spain, and Sweden) and the four UK nations (England, Northern Ireland, Scotland, and Wales) for three time periods: 1990–2011, 2011–19, and 2019–21. Changes in life expectancy and causes of death were estimated with an established life expectancy cause-specific decomposition method, and compared with summary exposure values of risk factors for the major causes of death influencing life expectancy. Findings: All countries showed mean annual improvements in life expectancy in both 1990–2011 (overall mean 0·23 years [95% uncertainty interval [UI] 0·23 to 0·24]) and 2011–19 (overall mean 0·15 years [0·13 to 0·16]). The rate of improvement was lower in 2011–19 than in 1990–2011 in all countries except for Norway, where the mean annual increase in life expectancy rose from 0·21 years (95% UI 0·20 to 0·22) in 1990–2011 to 0·23 years (0·21 to 0·26) in 2011–19 (difference of 0·03 years). In other countries, the difference in mean annual improvement between these periods ranged from –0·01 years in Iceland (0·19 years [95% UI 0·16 to 0·21] vs 0·18 years [0·09 to 0·26]), to –0·18 years in England (0·25 years [0·24 to 0·25] vs 0·07 years [0·06 to 0·08]). Chronic care manages long-term, progressive conditions, while acute care addresses short-term conditions. Chronic conditions increasingly strain health systems, which are often unprepared for these demands. This study examines the burden of conditions requiring acute versus chronic care, including sequelae. Conditions and sequelae from the Global Burden of Diseases Study 2019 were classified into acute or chronic care categories. Data were analysed by age, sex, and socio-demographic index, presenting total numbers and contributions to burden metrics such as Disability-Adjusted Life Years (DALYs), Years Lived with Disability (YLD), and Years of Life Lost (YLL). Approximately 68% of DALYs were attributed to chronic care, while 27% were due to acute care. Chronic care needs increased with age, representing 86% of YLDs and 71% of YLLs, and accounting for 93% of YLDs from sequelae. These findings highlight that chronic care needs far exceed acute care needs globally, necessitating health systems to adapt accordingly. © 2025. The Author(s).

Adverse events following the first, second and third doses of a COVID-19 vaccine in hemodialysis patients

BACKGROUND:This study aimed to identify adverse events following the first three doses of COVID-19 vaccines in hemodialysis (HD) patients. Risk factors associated with postvaccination adverse events were explored. METHODS:Postvaccination adverse events in 438 HD patients who received 3 doses of COVID-19 vaccines were prospectively assessed. The adverse events among three doses were compared using generalized linear mixed models. Factors associated with adverse events were assessed with multivariate analyses. RESULTS:The vast majority of participants received Oxford/AstraZeneca ChAdOx1 as their first two doses and Moderna mRNA-1273 as their third dose. Overall, 79%, 50% and 84% of the participants experienced at least one adverse event after their first, second, and third doses, respectively. These adverse events were mostly minor, short-lived and less than 5% reported daily activities being affected. Compared with the first dose, the second dose caused a lower rate of adverse events. Compared with the first dose, the third dose elicited a higher rate of injection site reactions and a lower rate of systemic reactions. Multivariate analyses showed that every 10-year increase of age (odds ratio 0.67, 95% confidence intervals 0.57-0.79) was associated with decreased risk of adverse events, while female sex (2.82, 1.90-4.18) and arteriovenous fistula (1.73, 1.05-2.84) were associated with increased risk of adverse events. Compared with Oxford/AstraZeneca ChAdOx1, Moderna mRNA-1273 was associated with an increased risk of injection site reactions. CONCLUSIONS:COVID-19 vaccination was well tolerated in HD patients. Age, sex, dialysis vascular access and vaccine types were associated with postvaccination adverse events.

We examine the robustness of the color-color selection of quiescent galaxies (QGs) against contamination of dusty star-forming galaxies using latest submillimeter data. We selected 18,304 QG candidates out to z ∼ 3 using the commonly adopted NUV-r-J selection based on the high-quality multi-wavelength COSMOS2015 catalog. Using extremely deep 450 and 850 µm catalogs from the latest JCMT SCUBA-2 Large programs, S2COSMOS and STUDIES, as well as ALMA submillimeter, VLA 3 GHz, and Spitzer MIPS 24 µm catalogs, we identified luminous dusty star-forming galaxies among the QG candidates. We also conducted stacking analyses in the SCUBA-2 450 and 850 µm images to look for less-luminous dusty galaxies among the QG candidates. By cross-matching to the 24 µm and 3 GHz data, we were able to identify a sub-group of "IR-radio-bright" QGs who possess a strong 450 and 850 µm stacking signal. The potential contamination of these luminous and less-luminous dusty galaxies account for approximately 10% of the color-selected QG candidates. In addition, there exists a spatial correlation between the luminous star-forming galaxies and the QGs at a. 60 kpc scale. We present confusion-limited SCUBA-2 450-µm observations in the COSMOS-CANDELS region as part of the JCMT Large program, SCUBA-2 Ultra Deep Imaging EAO Survey (STUDIES). Our maps at 450 and 850 µm cover an area of 450 arcmin2. We achieved instrumental noise levels of σ450 = 0.59 mJy beam−1 and σ850 = 0.09 mJy beam−1 in the deepest area of each map. The corresponding confusion noise levels are estimated to be 0.65 and 0.36 mJy beam−1. Above the 4 (3.5) σ threshold, we detected 360 (479) sources at 450 µm and 237 (314) sources at 850 µm. We derive the deepest blank-field number counts at 450 µm, covering the flux-density range of 2 to 43 mJy. These are in agreement with other SCUBA-2 blank-field and lensing-cluster observations, but are lower than various model counts. We compare the counts with those in other fields and find that the field-to-field variance observed at 450 µm at the R = 6′ scale is consistent with Poisson noise, so there is no evidence of strong 2-D clustering at this scale. Additionally, we derive the integrated surface brightness at 450 µm down to 2.1 mJy to be (Equation presented) Jy deg−2, contributing to (41±4)% of the 450-µm extragalactic background light (EBL) measured by COBE and Planck. Our results suggest that the 450-µm EBL may be fully resolved at (Equation presented) mJy, which extremely deep lensing-cluster observations and next-generation submillimeter instruments with large aperture sizes may be able to achieve. © 2024, CC BY.
