Background: In sub-Saharan Africa, the disease burden from skin diseases, including skin-related neglected tropical diseases (skin NTDs), is extremely high. These diseases often are overlooked due to limited access to health care stemming from, for example, remote geographical locations and a lack of experts. To address these gaps, we developed a mobile health app, eSkinHealth, which is a field-adapted platform to serve as a portable electronic patient chart and for teledermatology. Objective: The purpose of the study is to evaluate the usability and effectiveness of the app in rural Côte d’Ivoire for diagnosing and managing skin NTDs and other skin diseases. Methods: A 2-arm trial with local health care providers and patients with skin diseases was implemented over a 3-month period. The providers were assigned to an intervention receiving the eSkinHealth app or control with usual care. Four nurses and 8 community health care workers participated in each arm. The training was provided on the use of the app to the intervention arm only, while both arms were trained on skin diseases. For the usability study, we evaluated our approach with the System Usability Scale (SUS) and in-depth interviews. For the effectiveness study, our primary outcome was to evaluate the detection and management of 5 skin NTDs as our targeted diseases, namely, Buruli ulcer, leprosy, lymphatic filariasis, scabies, and yaws, using the eSkinHealth app. Procedures of our methods were reviewed and approved by the institutional review board of the Ministry of Health and by Tulane University. Results: The mean age of our participants (providers) was 40.5 and 42.5 years for the intervention and control arms, respectively, and all were male (n=24). The average SUS scores taken from the intervention arm at baseline, the midpoint (6 weeks), and the end of study (12 weeks) were 72.3 (SD 11.5), 72.3 (SD 12.4), and 86.3 (SD 10.8), respectively. Personnel and education data very important for organizations, including the military. All data related to military personnel will give effect positions and careers in the military field. The integration of personnel and education data can assist in processing and decision making. An analysis is needed so that the developed system can be useful for the long term. Analysis with IT balanced scorecard and COBIT ® 2019, will be able to help and fill the gaps in the existing system based on the objectives of the military organization itself. It is shown that the domains BAI05 (Managed Organizational Change) and DSS02 (Managed Service Requests and Incidents) are still not being implemented well. The system developed will have the features and capabilities to support the organization in achieving its goals, including in the field of staffing and education. Besides, it provides convenience in making decisions in terms of positions related to personnel careers.

SG technology is becoming a reality, with several deployments appearing around the world. Despite bringing improvements, 5G technology may not adequately support many envisaged applications that will demand large amounts of network resources. Thus, studies on the next generation, the 6G, are highly desirable. Observing this landscape, in this paper, we present a seminal study about the potential usage of 6G to support such highly demanding applications. Towards this study, we model a 6G system at the simulation level and conduct a case study exploring an application of high-definition video monitoring, applying the concepts of drone-swarm based surveillance. This scenario demands the transmission of large amounts of video data through the network. The acquired results present evidence that 6G can address these high network traffic demands.

Multimodal MRI and tau PET findings from the Autosomal Dominant Alzheimer’s disease cohort caused by the A431E PSEN1 Jalisco mutation

Background The A431E mutation in PSEN1 is a cause of Autosomal Dominant Alzheimer’s disease (ADAD) that represents a founder effect, apparently arising from the state of Jalisco, Mexico, with over 100 superficially distinct families being affected. The median age of symptom onset is 41 years, with about 40% of cases having an atypical presentation of substantial spastic paraparesis. This study aimed to identify the neuroimaging features associated with the A431E PSEN1 Jalisco mutation to elucidate its pathophysiology and to inform future clinical studies. Method Thirty-one participants with or known to be at-risk for the A431E PSEN1 mutation were enrolled in a comprehensive genetic, clinical, and imaging protocol including MRI and flortaucipir PET scans. Diffusion MRI and flortaucipir PET studies were conducted on sub-samples of the cohort along with appropriate control participants. Result Peak flortaucipir standard uptake value ratio (SUVR) among A431E mutation carriers was predominantly in the lateral parietal cortices relative to the medial temporal lobes with disproportionate signal in motor and sensory cortex relative to persons with late onset AD. Flortaucipir SUVR in 6 brain regions (i.e., medial orbitofrontal, caudal and rostral anterior cingulate, parahippocampal, precuneus, and lateral occipital) were associated with neuropsychiatric symptoms in persons with or at risk for the A431E PSEN1 mutation. Among 21 A431E PSEN1 mutation carriers with available diffusion MRI images, the Ashworth score of spastic paraparesis correlated with mean diffusivity in several brain regions, particularly left inferior parietal, superior parietal, and paracentral lobules (r’s>0.8, p’s< 0.001). In a subset of 10 clinically impaired A431E PSEN1 mutation carriers, mean diffusivity in white matter tracts underlying multiple cortical areas was higher relative to 8 impaired persons with late onset AD or ADAD mutations that do not cause spastic paraparesis. BACKGROUND:Splicing variants are a major class of pathogenic mutations, with their severity equivalent to nonsense mutations. However, redundant and degenerate splicing signals hinder functional assessments of sequence variations within introns, particularly at branch sites. We have established a massively parallel splicing assay to assess the impact on splicing of 11,191 disease-relevant variants. Based on the experimental results, we then applied regression-based methods to identify factors determining splicing decisions and their respective weights. RESULTS:Our statistical modeling is highly sensitive, accurately annotating the splicing defects of near-exon intronic variants, outperforming state-of-the-art predictive tools. We have incorporated the algorithm and branchpoint information into a web-based tool, SpliceAPP, to provide an interactive application. This user-friendly website allows users to upload any genetic variants with genome coordinates (e.g., chr15 74,687,208 A G), and the tool will output predictions for splicing error scores and evaluate the impact on nearby splice sites. Additionally, users can query branch site information within the region of interest. CONCLUSIONS:In summary, SpliceAPP represents a pioneering approach to screening pathogenic intronic variants, contributing to the development of precision medicine. There have been many studies conducted related to Smart City, IT Governance and Big Data. In this study aims to find out how the relationship between the three and how to form a framework to explain it. The methodology used is qualitative by looking for some literature on smart city framework, IT Governance framework, and a Big Data framework. From these results, an overall picture of the relationship between the three is concluded, where Big Data has a role in IT Governance. and also the relationship of IT Governance to the realization of Smart City. The Coupled Model Inter-comparison Project Phase 5 (CMIP5) is the output of many coupled atmosphere-ocean of global climate models (GCMs) and widely used for climate research, especially for driving regional climate model. There are more than 40 CMIP5 GCMs data available, but no single model can be considered as the best for every region. The use of CMIP5 GCMs data for rainfall projection in Indonesia is important to improve the accuracy of the monthly and seasonal rainfall forecast. Then, this study evaluates the capability of the CMIP5 GCMs data for Indonesia region by quantitatively comparing the spatial pattern of the precipitation mean and standard deviation of the CMIP5 data against GPCP, GPCC, and CRU data in the period 1980-2005. Furthermore, the composite analysis is conducted to observe the model performance in reproducing the precipitation characteristic over some areas in Indonesia. In conclusion, the models NorESM1-M, NorESM1-ME, GFDL-ESM2M, CSIRO-MK3-6-0 perform the rainfall mean better than others, while the standard deviation of the rainfall show that the models NorESM1-M, BNU-ESM, CMCC-CMS are superior in which NorESM1-M gives the best performance. The annual precipitation pattern of the model NorESM1-M over various areas in Indonesia is also highly correlated with the observations. Thus, the most suitable model for Indonesia region is NorESM1-M. © Published under licence by IOP Publishing Ltd.

Organizations make large investments in IT, so the value added to the business needs to be understood in order to justify such investments. The IT business value concept is complex, multifaceted, with a certain level of abstraction and intangibility, and with effects that may only be observed after a long period of the technology deployment Although the IT business value theme is yet indubitably relevant, as our literature review has shown, there is still a gap in the existent models, i.e., there is no proposition that addresses all tangible and intangible IT business value facets, able to estimate it. Therefore, we propose a comprehensive qualitative model, whose goal is to represent and organize all value characteristics that an IT initiative may create in an organization, helping to justify investments and even making it possible to compare the aggregated value of different technology options in a selection process. This model organizes the characteristics that contribute to the IT business value, evidencing their relationships and offering high-level prescriptions on how organizations can implement these characteristics in their context when adopting information technologies. We used the idea of Catalogues from Software Engineering to represent this model, and built it based on 84 research papers, providing 58 IT business value characteristics. We claim that the proposed Catalogue contributes to the IT Business Value research, as well as being itself a technological contribution. The Catalogue can support the organizations to better understand the impact of their IT initiatives and justify IT investments.
