Maternal Dietary Patterns by Reduced Rank Regression Are Associated With Low Birth Weight Outcomes in Mothers and Children’s Environmental health Cohort

AbstractObjectivesThis study aims to identify the associations between maternal dietary patterns and the risks of low birth weight (≤ 10th percentile). MethodsA total of 1,751 mothers and their newborns recruited for the Mothers and Children’s Environmental health cohort study between 2006 and 2010. A semiquantitative FFQ for nutrient intakes was collected and dietary patterns were derived using Reduced Rank Regression (RRR). A total of 138 food items were categorized into 40 pre-defined food groups. In this study, log transformed maternal intakes of folate, iron, and zinc were selected as the intermediate response variables based on the associations with birth weight. Associations were assessed by logistic regression with adjustment for confounding factors. ResultsAll of energy and nutrient intakes of dietary pattern 1, characterized by high intakes of grain, green/yellow, and light-colored vegetables, legumes, fruits, red meat, poultry, eggs, fishes, seaweeds, tofu/soymilk, yogurt, and nuts significantly increased as the from quartile one to quartile four. Biochemical marker levels such as triglyceride, C-reactive protein, and malondialdehyde levels were significantly decreased from quartile one to quartile four in pattern 1. Pregnant women, who adhered to pattern 1 had a lower risk of low weight at birth in the highest quartile compared to the lowest quartile (adjusted odds ratio 0.35, 95% confidence interval 0.32–0.95). No association was observed for pattern 2(green/yellow vegetables, light-colored vegetables, kimchi, and seaweeds) and 3(grains, milk, and yogurt) with low weight at birth. ConclusionsMothers who practiced good nutrition such as various food groups were likely to have a lower risk of low weight at birth. This study was the first to use a birth cohort to investigate the association between maternal dietary pattern and low weight at birth using RRR method, which highlights the important role of whole foods or quality of nutrients during pregnancy.

Autism Spectrum Disorder (ASD) is an early childhood disorder that affects individual ability to interact and socialize with other people. Children with ASD have problems interacting with their peers and have difficulty to exercise their social skills. In this study, a virtual reality (VR) based assistive tool was modelled and simulated using a gaming software called Dark Basic Professional (DBPro). The assistive tool was developed to support ASD children to interact and apply the social skills. Seven tasks were designed for the VR assistive tool based on three targeted skills including facial expression recognition, reading comprehension, and task delivery. As a preliminary study, only three non-ASD children were participated in the VR assistive tool experiment to analyse the tasks. The experiment results showed that all participants successfully performed five out of seven tasks. However, all participants failed to perform the video prompting task while one participant was not able to recognize the emotion from robotics-based faces in the facial expression. The results from pulse sensor showed that the heart rate was stable during the basic experiment using VR but unstable during the applied experiment and real-world discussion. There is a need to review the overall modelling and simulation technique, and the number of participants among ASD children should be increased for the future study.

BACKGROUND Disease-related single nucleotide polymorphisms(SNPs)based genetic risk score(GRS)has been proven to provide independent inherited risk other than family history in multiple cancer *** To evaluate the potential of GRS in the prediction of pancreatic cancer *** In this case-control study(254 cases and 1200 controls),we aimed to evaluate the association between GRS and pancreatic ductal adenocarcinoma(PDAC)risk in the Chinese *** GRS was calculated based on the genotype information of 18 PDAC-related SNPs for each study subject(personal genotyping information of the SNPs)and was weighted by external odd ratios(ORs).RESULTS GRS was significantly different in cases and controls(1.96±3.84 in PDACs vs 1.09±0.94 in controls,P<0.0001).Logistic regression revealed GRS to be associated with PDAC risk[OR=1.23,95%confidence interval(CI):1.13-1.34,P<0.0001].GRS remained significantly associated with PDAC(OR=1.36,95%CI:1.06-1.74,P=0.015)after adjusting for age and *** analysis revealed an association of increased risk for PDAC with higher *** with low GRS(<1.0),subjects with high GRS(2.0)were 99%more likely to have PDAC(OR:1.99,95%CI:1.30-3.04,P=0.002).Participants with intermediate GRS(1.0-1.9)were 39%more likely to have PDAC(OR:1.39,95%CI:1.03-1.84,P=0.031).A positive trend was observed(P trend=0.0006).CONCLUSION GRS based on PDAC-associated SNPs could provide independent information on PDAC risk and may be used to predict a high risk PDAC population.

Type 2 diabetes (T2D) is a heterogeneous disease that develops through diverse pathophysiological processes and molecular mechanisms that are often specific to cell type. Here, to characterize the genetic contribution to these processes across ancestry groups, we aggregate genome-wide association study data from 2,535,601 individuals (39.7% not of European ancestry), including 428,452 cases of T2D. We identify 1,289 independent association signals at genome-wide significance (P < 5 × 10) that map to 611 loci, of which 145 loci are, to our knowledge, previously unreported. We define eight non-overlapping clusters of T2D signals that are characterized by distinct profiles of cardiometabolic trait associations. These clusters are differentially enriched for cell-type-specific regions of open chromatin, including pancreatic islets, adipocytes, endothelial cells and enteroendocrine cells. We build cluster-specific partitioned polygenic scores in a further 279,552 individuals of diverse ancestry, including 30,288 cases of T2D, and test their association with T2D-related vascular outcomes. Cluster-specific partitioned polygenic scores are associated with coronary artery disease, peripheral artery disease and end-stage diabetic nephropathy across ancestry groups, highlighting the importance of obesity-related processes in the development of vascular outcomes. Our findings show the value of integrating multi-ancestry genome-wide association study data with single-cell epigenomics to disentangle the aetiological heterogeneity that drives the development and progression of T2D. This might offer a route to optimize global access to genetically informed diabetes care.

Background: Physicians invest hours creating patient notes, which are rich in information but difficult for computers to analyze due to their unstructured format. GPT-4 reshaped our ability to process text, yet it is unknown how well this model can handle medical notes. This project aims to compare GPT-4’s ability to annotate medical notes against experienced physicians across three different languages at multiple institutions and countries. Methods: This study included eight sites from four countries - the United States, Colombia, Singapore, and Italy. Each site contributed seven de-identified notes (admission, progress, or consult) from hospitalized patients. GPT-4 assessed each note by answering 14 questions, including demographic information, clinical judgments, data quality, and patients’ eligibility for a hypothetical study enrollment. For validation, two physicians from each site independently evaluated GPT-4's responses. Findings: Overall, 56 medical notes, written in English, Italian, and Spanish, were analyzed. A total of 784 responses from GPT-4 were generated. Both physicians agreed with GPT-4’s response 79% of the time (622/784, 95%CI 76-82%). Only one of the two physicians agreed with GPT-4’s response 10% of the time (82/784, 95%CI 8-13%). Neither physician agreed with GPT-4’s response 10% of the time (80/784, 95%CI 8-13%). Both physicians agreed with GPT-4 more often in notes written in Spanish and Italian than in English, with agreement rates of 88% (86/98, 95%CI 79-93%), 84% (82/98, 95%CI 75-90%), and 77% (454/588, 95%CI 74-80%), respectively. Hallucinations were rare (10/784, 95%CI 0-2%). GPT-4 correctly selected patients for a hypothetical study enrollment based on three criteria 90% of the time (95%CI 81-98%). Interpretation: The findings indicate that GPT-4 annotations demonstrated a high agreement rate with physicians across all languages. Background: Despite major advances in artificial intelligence (AI) research for healthcare, the deployment and adoption of AI technologies remain limited in clinical practice. In recent years, concerns have been raised about the clinical, technical, ethical and legal risks associated with healthcare AI. To increase adoption in the real world, it is essential that AI technologies are trusted and accepted by patients, clinicians, health organisations and authorities. This paper describes the FUTURE-AI framework as the first international consensus guideline for trustworthy AI in healthcare. Methods: The FUTURE-AI consortium was founded in 2021 and now comprises 117 interdisciplinary experts from 50 countries representing all continents, including AI scientists, clinical researchers, biomedical ethicists, and social scientists. Over a two-year period, the consortium established guiding principles and best practices for trustworthy and deployable AI through an iterative process comprising an in-depth literature review, a modified Delphi survey, and online consensus meetings. Findings: The FUTURE-AI framework was established based on six guiding principles for trustworthy AI in healthcare, i.e. Fairness, Universality, Traceability, Usability, Robustness and Explainability. Through consensus, a set of 30 best practices were defined, addressing technical, clinical, socio-ethical and legal dimensions of trustworthy AI. The recommendations cover the entire lifecycle of healthcare AI, from design, development and validation to regulation, deployment, and monitoring. Interpretation: FUTURE-AI is a structured, risk-informed framework which provides guidance for constructing healthcare AI tools that will be trusted, deployed and adopted in real-world clinical practice. Researchers are encouraged to take the recommendations into account in proof-of-concept stages to facilitate future translation towards clinical practice of healthcare AI. OBJECTIVES:To evaluate the effects of the modification of an etch-and-rinse (ER) adhesive system with fibrous particles with or without doxycycline (DOX) as a therapeutic ingredient. METHODS:Polymeric fibers were synthesized via electrospinning using a 10 wt% polymer solution based on poly-(ε-caprolactone) (PCL). The PCL solution was separated into two aliquots: one was kept unchanged, and the other was laden with 25 wt% of DOX. Each electrospun fiber mat was further processed in a cryogenic impact mill to obtain small-sized fibrous particles, which were characterized with morphological, chemical (FTIR), and antibacterial activity (against Streptococcus mutans and Lactobacillus) analyses. The 2-step ER adhesive One-Step (Bisco) was separated into three aliquots: one was kept unmodified to serve as an unfilled control, and the other two aliquots were added with 20 wt% of DOX or PCL fibrous microparticles. The fiber-modified adhesives were tested for degree of conversion (DC), optical properties (color change [ΔE], translucency, and refractive index [RI]), wettability properties, and bonding potential (microtensile bond strength/µTBS) to dentin after immediate testing and one-year of water storage. The data were statistically analyzed using ANOVA and Kruskal-Wallis tests with α= 5 %. The bond strength data was also analyzed using Weibull analysis. RESULTS:The morphology of the PCL and DOX particles demonstrated a nonuniform diameter distribution similar to each other. While the PCL particles did not inhibit the growth of S. mutans and Lactobacillus, the DOX particles resulted in significant inhibition halos greater than chlorhexidine (control). The incorporation of the fibrous particles into the adhesive altered the color appearance of the material (ΔE>1.8 - above the acceptability threshold); translucency was slightly reduced only in the PCL adhesive, and the RI was similar among the tested adhesives. AbstractObjectivesThe aim was to develop a human challenge model in which modulation of immune and inflammatory response by food ingredients can be evaluated. We hypothesized that oral cholera vaccination, in addition to inducing a specific antibody response, induces a significant increase in gut inflammatory response. MethodsTwenty healthy men (age 30.6 ± 1.8 y; BMI 24.9 ± 0.6) were enrolled in the study. Fecal and blood samples were collected at baseline. After a 2-week run-in period (D0-D14), subjects were vaccinated with oral cholera vaccine Dukoral (D15). After a period of 2 weeks, a second vaccination was administered (D29). Fecal samples were collected the day before (D14; D28) and the days (D16/D17; D30/D31) after each vaccination, as well as on D42. Blood samples were collected before vaccination (D15; D29), and on D16 and D17/D31, on D43. Primary outcome was fecal calprotectin concentration, secondary outcomes were serum levels of cholera toxin (CTB)-specific IgA and IgG. Other markers of local and systemic inflammatory response included beta-defensins, IP-10, IL-1 ra and hsCRP. Changes over time were tested by means of a linear mixed model. Outliers were identified with the 1.5xIQR rule and excluded from analysis. ResultsFecal calprotectin did not increase after the first vaccination. After the second vaccination, a significant increase was observed: from 12.8 ± 2.5 μg/g feces (mean ± SEM) on day 29 to 18.0 ± 2.9 μg/g on day 31 ( P = 0.017). Plasma CTB-specific IgA and IgG were strongly increased after the first vaccination, with a further increase after the second vaccination. Plasma CRP slightly decreased on D17, compared to D15 ( P = 0.016). IL-1ra significantly decreased 2 days after the first vaccination ( P = 0.011), whereas no change was observed after the second vaccination. Beta-defensin was significantly increased at D31 compared to D29 (from 42.7 ± 7.0 to 80.7 ± 16.1 ( P = 0.014)). IP-10 did not show any response to vaccination. ConclusionsIn a

Cordyceps militaris (C. militaris) is a unique medicinal fungus of the genus Cordyceps. It has high economic value due to various biological functions from the extracellular polysaccharides (EPSs) it produces. The aim of this study was to establish a mycelium-immobilization system for EPS production in a bubble column system. First, EPS production was optimized by 90.2% (from 0.72 to 1.37 g/L) using response surface methodology under an alginate-immobilized mycelium system with 6 days of cultivation. The highest EPS production was obtained (5.3 g/L) in a flask system through elongation to 15 days of cultivation. EPS production subsequently further increased to 10.42 g/L utilizing a bubble column reactor. Furthermore, the system can be reused for at least three rounds. In the EPS characteristics analysis, the produced EPSs were mainly composed of glucose and galactose. They presented strong antibacterial activity against Staphylococcus aureus. In terms of antioxidant activity, respective the EPS concentration that caused 50% scavenging of DPPH and ABTS free radicals values (IC50) were 4.65 and 7.92 mg/mL. In summary, these findings should be helpful for establishing a suitable industrial fermentation system for C. militaris and can be studied in the food and medical industries in future work. © 2024 The Authors

Evapotranspiration is an important parameter to evaluate soil water deficit and water use efficiency, especially at places with irregularly distributed precipitation. The aim of this study was to assess the daily actual evapotranspiration (ETa) estimated by the Thornthwaite and Mather soil water balance method adapted for crops (ThM) and by the dual Kc approach with the crop coefficients optimized from inversing modeling and by the adjustment procedure suggested in FAO-56. The models comparison and optimization were performed with actual evapotranspiration determined by the Bowen ratio – energy balance method (ETβ) for sugarcane at full canopy closure grown in Alagoas State, Northeastern Brazil. The objective function of the inverse problem was defined in terms of ETβ and ETa estimated by the ThM and dual Kc method by optimizing single crop coefficient (Kc) and the basal coefficient Kcb, respectively. Both optimized Kc and Kcb were lower than the adjusted Kc FAO56, with optimized Kc only 3% less than the Kc obtained experimentally. ETa estimated by ThM and dual Kc models with optimized crop coefficients (Kc = 1.05 or Kcb = 1.00) had similar high precision (r2 > 0.79) and accuracy (dm > 0.93 and RMSE -1), whereas using the coefficients derived from FAO 56 overestimated ETa in both models. © 2021, CC BY.
