OBJECTIVE:Although unhealthy diets exacerbate nutritional and metabolic derangements in patients with end-stage kidney disease (ESKD), how therapeutic diets that possess a variety of different dietary strategies acutely modify diverse biochemical parameters related to cardiovascular disease remains underexplored. METHODS:Thirty-three adults with end-stage kidney disease undergoing thrice-weekly hemodialysis participated in a randomized crossover trial comparing a therapeutic diet with their usual diets for 7 days, separated by a 4-week washout period. The therapeutic diet was characterized by adequate calorie and protein amounts, natural food ingredients with a low phosphorus-to-protein ratio, higher portions of plant-based food, and high fiber content. The primary outcome measure was the mean difference in the change-from-baseline intact fibroblast growth factor 23 (FGF23) level between the 2 diets. The other outcomes of interest included changes in mineral parameters, uremic toxins, and high-sensitivity C-reactive protein (hs-CRP) levels. RESULTS:Compared with the usual diet, the therapeutic diet lowered intact FGF23 levels (P = .001), decreased serum phosphate levels (P < .001), reduced intact parathyroid hormone (PTH) levels (P = .003), lowered C-terminal FGF23 levels (P = .03), increased serum calcium levels (P = .01), and tended to lower total indoxyl sulfate levels (P = .07) but had no significant effect on hs-CRP levels. Among these changes, reduction in serum phosphate level achieved in 2 days, modifications of intact PTH and calcium levels in 5 days, and reductions in intact and C-terminal FGF23 levels in 7 days of therapeutic diet intervention. CONCLUSION:Within the 1-week intervention period, the dialysis-specific therapeutic diet rapidly reversed mineral abnormalities and tended to decrease total indoxyl sulfate levels in patients undergoing hemodialysis but had no effect on inflammation. This review presents a comprehensive perspective on the genomic surveillance of SARS-CoV-2 in Taiwan, with a focus on next-generation sequencing and phylogenetic interpretation. This article aimed to explore how Taiwan has utilized genomic sequencing technologies and surveillance to monitor and mitigate the spread of COVID-19. We examined databases and sources of genomic sequences and highlighted the role of data science methodologies in the explanation and analyses of evolutionary data. This review addressed the challenges and limitations inherent in genomic surveillance, such as concerns regarding data quality and the necessity for interdisciplinary expertise for accurate data interpretation. Special attention was given to the unique challenges faced by Taiwan, including its high population density and major transit destination for international travelers. We underscored the far-reaching implications of genomic surveillance data for public health policy, particularly in influencing decisions regarding travel restrictions, vaccine administration, and public health decision-making. Studies were examined to demonstrate the effectiveness of using genomic data to implement public health measures. Future research should prioritize the integration of methodologies and technologies in evolutionary data science, particularly focusing on phylodynamic analytics. This integration is crucial to enhance the precision and applicability of genomic data. Overall, we have provided an overview of the significance of genomic surveillance in tracking SARS-CoV-2 variants globally and the pivotal role of data science methodologies in interpreting these data for effective public health interventions.

The increasing prevalence of multidrug-resistant Acinetobacter baumannii as an opportunistic pathogen in wound infections raises significant concerns due to its antibiotic resistance and biofilm-mediated antibiotic tolerance. This underscores the urgent need to explore an alternative approach to effectively managing wound infections caused by MDR A. baumannii. In this study, combining subinhibitory concentrations of curcumin (25 µg/ml) or chitosan (156 µg/ml) with photodynamic therapy-PDT (10 J/cm²) resulted in a significant reduction of planktonic viabilities and biofilm biovolume of MDR A. baumannii clinical isolates in vitro. In murine A. baumannii-associated wound infection, a combination of curcumin (25 µg/ml) or chitosan (156 µg/ml) with PDT (10 J/cm²) resulted in complete bacterial eradication with wound healing at Day 15 of post-treatments with these combination therapies. Our study demonstrated that combining curcumin or chitosan with PDT at their subinhibitory concentrations represents a promising alternative strategy as localized therapy for effectively managing A. baumannii-associated wound infection. © 2025 The Authors

Past air sampling detection (ASD) systems had no definitive positioning;most allocation of sampling points was based on experience. This research discovered that sampling points inside the electrical rooms were significantly affected by the ambient airflow, causing the detection system to have difficulties in fire detection. Fire dynamics simulator (FDS) was used to simulate very early smoke detection apparatus (VESDA) effectiveness within the protection area of an electrical room. Results showed that ASD sampling points in electrical rooms should be placed above high-voltage transformers and capacitors, while rearranging the high risk machines together to avoid long detection pipelines, which increases the transportation time of the gas, thus hindering early detection by the ASD system. Field tests using a smoke detector tester yielded transportation times of 33 s, which was in very good agreement with the simulated results of 35 s by ASPIRE, showing the accuracy of using software to simulate transportation times. Therefore, this research combined FDS with the transportation times of different pipeline designs to conduct performance assessment of VESDA. After appropriate rearrangement of machines, VESDA detection times dropped from 106.0-171.0 s during the original setup to 21.0-31.7 s, assuring the facility to have more time for emergency response. However, it was noticed that FDS software did not have the estimation tool for VESDA transportation times;therefore software such as ASPIRE should be used to calculate the gas transportation times inside the detection pipelines, and then compiled with FDS results to avoid underestimation of actuation times. © 2017 The Authors. Microbial growth may occur in carpet dust when exposed to elevated moisture levels, but it is unknown how diurnal variations in equilibrium relative humidity (ERH) affect this growth. Our goal is to apply the "time-of-wetness" (TOW) concept from fungal growth on drywall to model fungal growth in carpet. Used carpet samples were embedded with dust, placed in an experimental chamber and then the ERH within the chambers was varied from 50% to either 85% or 100% for a period of 0, 6, 12, 18, or 24 hours per day over a two-week period. Quantitative polymerase chain reaction (qPCR) measurements of fungi revealed that for the 85% and 100% samples, growth could be modeled with the standard TOW and activation limited growth curve. These measurements demonstrated that the relative growth rate fit the TOW model with the Pearson Correlation Coefficients of 0.958 for 100% TOW and 0.898 for 85% TOW. DNA sequencing revealed increased abundance of species in the genera Aspergillus and Penicillium, among others, at increased moisture. Ultimately, this data can be used to accurately model fungal growth in housing based on moisture and can be utilized in future public health, policy, and epidemiological models. © 2018 15th Conference of the International Society of Indoor Air Quality and Climate, INDOOR AIR 2018. The aim of the study was to investigate whether morphology (i.e. compact/diffuse) of brain arteriovenous malformations (bAVMs) correlates with the incidence of hemorrhagic events in patients receiving Stereotactic Radiosurgery (SRS) for unruptured bAVMs. This retrospective study included 262 adult patients with unruptured bAVMs who underwent upfront SRS. Hemorrhagic events were defined as evidence of blood on CT or MRI. The morphology of bAVMs was evaluated using automated segmentation which calculated the proportion of vessel, brain tissue, and cerebrospinal fluid in bAVMs on T2-weighted MRI. Compactness index, defined as the ratio of vessel to brain tissue, categorized bAVMs into compact and diffuse types based on the optimal cutoff. Cox proportional hazard model was used to identify the independent factors for post-SRS hemorrhage. The median clinical follow-ups was 62.1 months. Post-SRS hemorrhage occurred in 13 (5.0%) patients and one of them had two bleeds, resulting in an annual bleeding rate of 0.8%. Multivariable analysis revealed bAVM morphology (compact versus diffuse), bAVM volume, and prescribed margin dose were significant predictors. The post-SRS hemorrhage rate increased with larger bAVM volume only among the diffuse nidi (1.7 versus 14.9 versus 30.6 hemorrhage per 1000 person-years in bAVM volume < 20 cm³ versus 20–40 cm³ versus > 40 cm³;p = 0.022). The significantly higher post-SRS hemorrhage rate of Spetzler-Martin grade IV–V compared with grade I–III bAVMs (20.0 versus 3.3 hemorrhages per 1000 person-years;p = 0.001) mainly originated from the diffuse bAVMs rather than the compact subgroup (30.9 versus 4.8 hemorrhages per 1000 person-years;p = 0.035). Compact and smaller bAVMs, with higher prescribed margin dose harbor lower risks of post-SRS hemorrhage. Liver modeling in disease via advanced in vitro physiological tissues remains a challenging issue, yet crucial for the future development of relevant tools for drug screening. In that regard, advanced technics such as human induced Pluripotent Stem Cells (hiPSCs) and organ-on-chip are promising technologies for the relevant reproduction of the in vivo micro physiology in in vitro culture conditions. In the present work, the maturation of carboxypeptidase M positive (CPM+) Hepatocyte-Like-Cells (HLCs) in microfluidic culture conditions was investigated. hiPSCs were differentiated in culture dishes until sorting and further amplified until seeding in biochips. After 2 weeks of maturation in biochips, evaluation of the cell metabolism was performed, and samples were collected for characterization via RNA sequencing, proteomics and metabolomics. The omics profile of biochips loaded with CPM+ HLCs was then compared with the one of previously cultured biochips loaded with unsorted HLCs. CPM+ HLCs presented advanced liver characteristics in terms of a higher activity of important Transcriptions Factors (TFs) such as HNF1, HNF4A, and CEBP/A, of the upregulation of steroids/corticoids-related nuclear receptors (FXR, NR3C2, NR4A1 genes), of phase I, and phase II metabolism genes (CYP1A1, CYP1B1, CYP2C18, CYP27A1, several UGT, SULT and GST genes). Further differences in the cellular reorganization, in the lipids and steroids metabolisms, in the OXPHOS respiration and in the expression of TGFβ signaling were also observed. The present study introduces a novel protocol using advances hiPSCs differentiation and sorting methods as well as the organ-on-chip technology to highlight important in vitro liver regeneration and hepatic maturation processes. © 2022 Elsevier B.V.

Background: Hearing loss affects access to spoken language, which can affect cognition and development, and can negatively affect social wellbeing. We present updated estimates from the Global Burden of Disease (GBD) study on the prevalence of hearing loss in 2019, as well as the condition's associated disability. Methods: We did systematic reviews of population-representative surveys on hearing loss prevalence from 1990 to 2019. We fitted nested meta-regression models for severity-specific prevalence, accounting for hearing aid coverage, cause, and the presence of tinnitus. We also forecasted the prevalence of hearing loss until 2050. Findings: An estimated 1·57 billion (95% uncertainty interval 1·51–1·64) people globally had hearing loss in 2019, accounting for one in five people (20·3% [19·5–21·1]). Of these, 403·3 million (357·3–449·5) people had hearing loss that was moderate or higher in severity after adjusting for hearing aid use, and 430·4 million (381·7–479·6) without adjustment. The largest number of people with moderate-to-complete hearing loss resided in the Western Pacific region (127·1 million people [112·3–142·6]). Of all people with a hearing impairment, 62·1% (60·2–63·9) were older than 50 years. The healthcare Access and Quality (HAQ) Index explained 65·8% of the variation in national age-standardised rates of years lived with disability, because countries with a low HAQ Index had higher rates of years lived with disability. By 2050, a projected 2·45 billion (2·35–2·56) people will have hearing loss, a 56·1% (47·3–65·2) increase from 2019, despite stable age-standardised prevalence. Interpretation: As populations age, the number of people with hearing loss will increase. Interventions such as childhood screening, hearing aids, effective management of otitis media and meningitis, and cochlear implants have the potential to ameliorate this burden. Post-pandemic green recovery is pivotal in achieving global sustainable development goals by simultaneously revitalizing economies and reducing greenhouse gas emissions, air pollution and improving public welfare. However, subnational and city-level understanding of green recovery, its efficacy and its alignment with public health is poorly understood. Here we focus on post-COVID-19 low-carbon recovery—economic growth combined with reduced carbon emissions—and explore health co-benefits in Chinese cities. A novel near-real-time daily carbon emission dataset of 48 cities in China is developed, coupled with detailed health and economic municipal statistics and models. We find that, on average, six low-carbon-recovery cities, mainly megacities, saved 1.2 times as many lives per 100,000 population compared with the 42 other cities, and their annual monetary avoided premature deaths per 100,000 population was 1.5 times more than the 42 other cities. The accumulated monetary health co-benefits for low-carbon-recovery cities were US$ 4.2 billion (95% confidence interval, 2.1–6.3) during the post-COVID-19 period. We show that government spending on electric vehicles increases the likelihood of achieving low-carbon recovery in Chinese cities. Our results underscore the significant health co-benefits of low-carbon recovery, pointing to synergies between advancing local welfare and global environmental objectives.
