Microsatellite instability (MSI) is a pivotal genetic marker influencing the efficacy of immunotherapy in colorectal cancer. Traditional MSI examination often requires additional genetic or immunohistochemical tests, whereas histology images, widely available in colorectal cancer diagnosis, offer a valuable alternative for MSI prediction. Although Transformer-based models have demonstrated promising outcomes in predicting MSI from histology images, they are hampered by traditional local attention mechanisms that struggle to capture long-range interdependencies and establish a comprehensive global receptive field. In this study, we introduce DiNAT-MSI, a novel framework for histology-based MSI prediction that incorporates the Dilated Neighborhood Attention Transformer (DiNAT). This model enhances global context recognition and substantially expands receptive fields, all without additional computational burden. Our results demonstrate that DiNAT-MSI achieves a superior patientwise AUROC compared to ResNet18 and Swin Transformer, along with commendable explainability. Our work not only illustrates a more accessible diagnostic tool for leveraging histological data but also underscores the potential of Transformerbased models with sophisticated attention designs in advancing precision medicine for colorectal cancer patients. © 2024 IEEE.

Treatments of disuse-induced muscle atrophy entail unmet clinical needs due to the lack of medical devices capable of mimicking physicians manual therapies. Therefore, in this paper we develop and model a wearable soft pneumatic elastomeric actuator to perform deep cyclic compression stimuli on human soft tissues for muscle rehabilitation by mechanotherapy. Static and dynamic characterization of the prototype demonstrate a 2.5 mm active deformation at 100 kPa with 600 mm3/s and a 5 Hz bandwidth. We estimate the transfer function of the experimentally acquired pressure, flow and deformation signals, processed by a Gaussian kernel-based approach. Our mathematical model accurately describes the actuator behavior and enables to extract its mechanical parameters. Then, through computational simulations, we illustrate its efficacy in emulating multiple complex bio-inspired movements. Our proposed methodology aims to improve the control efficiency in wearable soft robotics for muscle atrophy treatment. © 2024 IEEE.

Social network analysis has created a productive framework for the analysis of the histories of patient-physician interactions and physician collaboration. Notable is the construction of networks based on the data of "referral paths"- sequences of patient-specific temporally linked physician visits - in this case, culled from a large set of Medicare claims data in the United States. Network constructions depend on a range of choices regarding the underlying data. In this paper we introduce the use of a five-factor experiment that produces 80 distinct projections of the bipartite patient-physician mixing matrix to a unipartite physician network derived from the referral path data, which is further analyzed at the level of the 2,219 hospitals in the final analytic sample. We summarize the networks of physicians within a given hospital using a range of directed and undirected network features (quantities that summarize structural properties of the network such as its size, density, and reciprocity). The different projections and their underlying factors are evaluated in terms of the heterogeneity of the network features across the hospitals. We also evaluate the projections relative to their ability to improve the predictive accuracy of a model estimating a hospital's adoption of implantable cardiac defibrillators, a novel cardiac intervention. Because it optimizes the knowledge learned about the overall and interactive effects of the factors, we anticipate that the factorial design setting for network analysis may be useful more generally as a methodological advance in network analysis. © 2023 The Author(s).

The process of using ICT to provide services to the public is known as the Indonesian e-Government system, or Sistem Pemerintahan Berbasis Elektronik (SPBE). The e-Government initiative in Jakarta Provincial health Office involves enhancing collaboration among public health entities for efficient data exchange and streamlined processes, especially between the Provincial and District health Offices, public hospitals, government clinics, and primary health care centers (Puskesmas). Achieving interoperability requires standardized protocols and a well-defined architectural model to integrate data seamlessly. This study presents a provincial-level architectural model focused on improving electronic health records interoperability, aiming to promote the adoption of the national Fast healthcare Interoperability Resources (FHIR) health information exchange platform and enhance the integrity of health data in Jakarta. The study methodology involves conducting literature reviews, observations, and discussions with representatives from healthcare facilities to develop the e-Government architecture model and prototype of the infrastructure layer aiming to facilitate the interoperability of Electronic health Records (EHRs) across 93 healthcare facilities, all of which are part of the SPBE users.

Background and hypothesis Previous studies show that people with severe mental illness (SMI) are at higher risk of COVID-19 mortality, however limited evidence exists regarding risk post-vaccination. We investigated COVID-19 mortality among people with schizophrenia and other SMIs before, during and after the UK vaccine roll-out. Study Design Using the Greater Manchester (GM) Care Record to access routinely collected health data linked with death records, we plotted COVID-19 mortality rates over time in GM residents with schizophrenia/psychosis, bipolar disorder (BD) and/or recurrent major depressive disorder (MDD) from February 2020 to September 2021. Multivariable logistic regression was used to compare mortality risk (risk ratios;RRs) between people with SMI (N=190,188) and age-sex matched controls (N=760,752), adjusted for sociodemographic factors, pre-existing comorbidities and vaccination *** Results Mortality risks were significantly higher among people with SMI compared with matched controls, particularly among people with schizophrenia/psychosis (RR 3.14, CI 2.66-3.71) and/or BD (RR 3.17, CI 2.15-4.67). In adjusted models, the relative risk of COVID-19 mortality decreased, though remained significantly higher than matched controls for people with schizophrenia (RR 1.53, CI 1.24-1.88) and BD (RR 2.28, CI 1.49-3.49), but not recurrent MDD (RR 0.92, CI 0.78-1.09). People with SMI continued to show higher mortality rate ratios relative to controls throughout 2021, during vaccination roll-out. Conclusions People with SMI, notably schizophrenia and bipolar disorder, were at greater risk of COVID-19 mortality compared to matched controls. Despite population vaccination efforts that have prioritised people with SMI, disparities still remain in COVID-19 mortality for people with SMI.

Foodborne illnesses pose a threat to public health, leading to morbidity, mortality, and economic burden annually. Social media, while providing a rich timely source for training AI models for surveillance, requires effective tools for annotation. While Large Language Models (LLMs) have shown promise for generating simple labels, here hierarchical labels composed of entity types like food type and symptom (at individual word level) and the foodborne illness event (at complete post level) are required. For this, we introduce ICL2FID, the first LLM-based hierarchical labeling framework designed to annotate social media posts for foodborne illness detection at two levels using only a few demonstration examples. To utilize the interconnection between post and word levels, ICL2FID instructs the LLM to leverage information from one level when predicting the other level. To combat model hallucination and cyclic dependencies, a verification step improves evidence propagation between interconnected word and post-level labeling tasks. Strategies for custom selection of demonstration examples are designed reducing biases and increasing representation. We compare ICL2FID against traditional supervised learning and other LLM methods, demonstrating that it not only achieves superior accuracy but does so at a fraction of the cost and time. These findings highlight ICL2FID’s potential as a viable alternative for hierarchical label generation in scenarios with limited resources and huge data sets. A machine learning and neural network approach for classifying multidrug-resistant bacterial infections

Antimicrobial resistance (AMR) represents a major public health challenge, significantly complicating infection prevention and treatment. This study employs machine learning and neural network techniques to classify multidrug-resistant Gram-negative bacterial (MDR-GNB) infections using electronic health records from 624 patients at Thatphanom Crown Prince Hospital in Thailand. We compared several algorithms, including Logistic Regression, Random Forest, Support Vector Machine (SVM), Extreme Gradient Boosting (XGBoost), K-Nearest Neighbors (KNN), Multilayer Perceptron (MLP), and Light Gradient Boosting Machine (LightGBM), with the MLP model exhibiting the highest accuracy and specificity. Performance was further enhanced by integrating feature selection methods such as Sequential Forward Selection (SFS), Recursive Feature Elimination with Cross-Validation (RFE-CV), and SelectKBest with data augmentation techniques, including ADASYN and SMOTE variants. Utilizing SHapley Additive exPlanations (SHAP) provided valuable insights into the most influential predictors for MDR-GNB. Notably, the MLP model achieved an AUC of 0.70, surpassing prior studies and highlighting its potential to advance clinical decision-making in managing MDR-GNB infections. © 2025 The Authors

Many patients with leukoaraiosis (LA) exhibit mild and difficult-to-detect symptoms in the early stages, and due to the lack of effective detection methods, the optimal timing for treatment is often missed. This not only makes later treatment more difficult but, in severe cases, can even lead to mild to severe dementia. Therefore, in addition to relying on radiologists to manually interpret magnetic resonance imaging (MRI) scans, finding more effective detection methods has become extremely urgent. This study aimed to combine image processing and deep learning technologies to develop a model capable of accurately identifying and segmenting white matter hyperintensities (WMHs) to assist doctors in the early diagnosis of LA. We first performed efficient preprocessing on MR image slices from two different series and used the ResNet18 model to identify key slices containing WMHs, ultimately achieving precise segmentation of WMH areas with the U-Net model. This research focused on developing innovative preprocessing techniques, improving the accuracy by 12.3% with a limited dataset through preprocessing. Ultimately, the ResNet18 model achieved an F1 score of 0.829, while the U-Net model’s intersection over union (IoU) and Dice coefficient reached 0.750 and 0.857, respectively, showing the potential of this architecture. We propose improvements in the final chapter to construct a more comprehensive model. © 2024 Copyright ACM.

In recent years, many fields have expanded their research methods through the integration of artificial intelligence. In the current medical field, it is widely used in image recognition to diagnose patient symptoms, train clinical prediction models, and improve clinical workflow processes to avoid missing treatment opportunities. These methods have all achieved good results so far. Now, generative AI is also impacting the development of the medical field, not only generating prescriptions that match patient conditions but also being applied in generating medical images for data augmentation and simulating various pathological conditions, thereby aiding in training deep learning models. Stroke is a leading cause of death or disability, ranking as the second leading cause of death worldwide, next to cancer. If cancers were categorized individually, stroke would surpass all single diseases, making it a primary focus of research in brain disease treatment. There are already studies where generative AI has been used to create medical images, which are then manually evaluated by doctors to determine if they were generated by AI. In this project, we plan to generate CT images of strokes using generative AI and use a trained model (to diagnose stroke) to improve accuracy. Through this project, we hope to solve the problem of scarce stroke CT images while also providing support for training stroke-related models. © 2024 Copyright ACM.

Alzheimer’s disease (AD) often presents only mild symptoms in its early stages, and as there is no direct diagnostic method currently available, many patients are diagnosed only after the condition has worsened. Considering that magnetic resonance imaging (MRI) can provide key information in the diagnosis of AD, but the condition may go undiagnosed under manual interpretation due to the subtleness of symptoms and the reliance on radiologists to interpret MRI images, this study aimed to develop an automated method for interpreting brain MR images, hoping to screen for potential AD patients. By combining image processing, the U-Net model, YOLOv8 model, various CNN model architectures, and ensemble data statistics, our method was able to identify features related to AD and predict whether a patient has the disease. Our method can process a complete set of MR images and automatically extract important slices for prediction, making it practical for medical applications. Furthermore, we tested the accuracy and reliability of the method with additional datasets. © 2024 Copyright ACM.
