The workshop “Engaging Older Adults in Cancer Clinical Trials Conducted in the NCI Clinical Trials Network: Challenges and Opportunities” included a Patient Stakeholder Workgroup that explored the needs and concerns of older adults with cancer regarding clinical trials. To accomplish this, the workgroup conducted patient focus groups in which participants were interviewed, recorded conversations were analyzed and coded, and salient themes were identified. The focus groups identified general barriers to accrual such as complex consent forms, general communication, restrictive eligibility, nonreferrals, patient costs, cultural insensitivity, limited accessibility in community settings, and transportation issues. They also identified the influence of knowledgeable information presenters, improved care, family or caregiver support, and the desire to help others as drivers or reasons to participate in clinical trials. The workshop concluded that multi-level interventions could be used to increase the accrual of older adults to National Cancer Institute clinical trials as well as others. © The Author(s) 2022. The process of using ICT to provide services to the public is known as the Indonesian e-Government system, or Sistem Pemerintahan Berbasis Elektronik (SPBE). The e-Government initiative in Jakarta Provincial health Office involves enhancing collaboration among public health entities for efficient data exchange and streamlined processes, especially between the Provincial and District health Offices, public hospitals, government clinics, and primary health care centers (Puskesmas). Achieving interoperability requires standardized protocols and a well-defined architectural model to integrate data seamlessly. This study presents a provincial-level architectural model focused on improving electronic health records interoperability, aiming to promote the adoption of the national Fast healthcare Interoperability Resources (FHIR) health information exchange platform and enhance the integrity of health data in Jakarta. The study methodology involves conducting literature reviews, observations, and discussions with representatives from healthcare facilities to develop the e-Government architecture model and prototype of the infrastructure layer aiming to facilitate the interoperability of Electronic health Records (EHRs) across 93 healthcare facilities, all of which are part of the SPBE users.

Meditation is a family of ancient and contemporary contemplative mind-body practices that can modulate psychological processes, awareness, and mental states. Over the last 40 years, clinical science has manualized meditation practices and designed various meditation interventions that have shown therapeutic efficacy for disorders including depression, pain, addiction, and anxiety. Over the past decade, neuroimaging has been used to examine the neuroscientific basis of meditation practices, effects, states, and outcomes for clinical and nonclinical populations. However, the generalizability and replicability of current neuroscientific models of meditation have not yet been established, because they are largely based on small datasets entrenched with heterogeneity along several domains of meditation (e.g., practice types, meditation experience, clinical disorder targeted), experimental design, and neuroimaging methods (e.g., preprocessing, analysis, task-based, resting-state, structural magnetic resonance imaging). These limitations have precluded a nuanced and rigorous neuroscientific phenotyping of meditation practices and their potential benefits. Here, we present ENIGMA (Enhancing Neuro Imaging Genetics through Meta Analysis)–Meditation, the first worldwide collaborative consortium for neuroscientific investigations of meditation practices. ENIGMA-Meditation will enable systematic meta- and mega-analyses of globally distributed neuroimaging datasets of meditation using shared, standardized neuroimaging methods and tools to improve statistical power and generalizability. Through this powerful collaborative framework, existing neuroscientific accounts of meditation practices can be extended to generate novel and rigorous neuroscientific insights that account for multidomain heterogeneity. This paper presents the development of an object detection system based on the deep learning approach of computer vision to support the laparoscopic surgical robotic position control system. The system comprises two main phases, the training phase, and the real-time operating phase. In the training phase, a modified YOLOv4 algorithm is proposed to be adopted within the Darknet framework for training the weights of detection models. The best detection model is then selected to be assigned to the object detector in the real-time operating phase. The validation result of the detection model across the experiment datasets is 90% accuracy on average during the training phase. To validate the viability of real-time object detection, the proposed system was implemented in two test-cases of gynecologic surgery with the soft-body cadavers. © 2022 IEEE.

A workforce that is skilled in informatics and data science is critical to achieving the objectives of the data Modernization Initiative (DMI) in public health. The Minnesota Department of health conducted an informatics workforce assessment in December 2022. The survey captured information in 5 domain areas and across 6 proficiency levels (no knowledge to expert) with an estimated 15-20 minutes to complete. In all of the 5 domains (performing assessments and leading advancements of health information systems; data exchanges and systems interoperability; data and IT governance; data analytics, visualization and reporting; implementation, development and management of systems), the staff who identified themselves at expert proficiency was <3% and as advanced was approximately 10%. Staff with awareness/basic/intermediate proficiency were interested in trainings. University course work (51.8%) followed by online modules (33.4%) were common modes of prior trainings. The staff also identified desired trainings that spanned across various tools/skills (e.g., SQL, R, Tableau, HL7, Rhapsody, REDCap) along with informatics and interoperability overview. These findings will be utilized to offer trainings to interested staff and collaborations with university have been initiated.

Traffic fatalities, with and from increased risky behaviors (reduced seat belt use, increased impairment from licit and illicit substances), have been increasing, especially during the COVID-19 pandemic. Death certificates are a major source of epidemiologic data in the United States, but have known underreporting of drug and alcohol presence. The Fatality Analysis Reporting System (FARS) is one major source of data on fatal crashes with intoxication. This study links FARS data for three counties in Oregon (2019–2021) with local medical examiner and death certificate data (FARS source data) and compares their concordance with blood alcohol concentration and toxicology for three major drug classes by year. For drivers only, our study finds good concordance between FARS and its source data in 2019 but poor concordance in 2020. This discordance may impact future analysis of impaired crash deaths, and we list some suggestions for amelioration. © 2024 The Authors

The research herein proposes a position control system, designed for a single-port surgical robot, based on the lockup table method. The programmable logic controller (PLC) was implemented to move the single- port surgical robot and handle laparoscope and surgical instruments with feedback control. The output responses of the desired angle were investigated under several conditions. The rotating angle allows movement of the laparoscope in the range of -10o to +10o. The error of the desired position of the designed system was less than approximately ± 0.1o. Moreover, the output data from several experiments may be employed to improve the open-loop control systems. This system is also capable of providing computer vision task implementation, enabling semi- automatic or automatic surgery in the future. © 2022 IEEE.

A knowledge graph-based approach for exploring the U.S. opioid epidemic

The United States is in the midst of an opioid epidemic with recent estimates indicating that more than 130 people die every day due to drug overdose. The over-prescription and addiction to opioid painkillers, heroin, and synthetic opioids, has led to a public health crisis and created a huge social and economic burden. Statistical learning methods that use data from multiple clinical centers across the US to detect opioid over-prescribing trends and predict possible opioid misuse are required. However, the semantic heterogeneity in the representation of clinical data across different centers makes the development and evaluation of such methods difficult and non-trivial. We create the Opioid Drug Knowledge Graph (ODKG) - a network of opioid-related drugs, active ingredients, formulations, combinations, and brand names. We use the ODKG to normalize drug strings in a clinical data warehouse consisting of patient data from over 400 healthcare facilities in 42 different states. We showcase the use of ODKG to generate summary statistics of opioid prescription trends across US regions. These methods and resources can aid the development of advanced and scalable models to monitor the opioid epidemic and to detect illicit opioid misuse behavior. Our work is relevant to policymakers and pain researchers who wish to systematically assess factors that contribute to opioid over-prescribing and iatrogenic opioid addiction in the US. Biomedical networks (or graphs) are universal descriptors for systems of interacting elements, from molecular interactions and disease co-morbidity to healthcare systems and scientific knowledge. Advances in artificial intelligence, specifically deep learning, have enabled us to model, analyze, and learn with such networked data. In this review, we put forward an observation that long-standing principles of systems biology and medicine-while often unspoken in machine learning research-provide the conceptual grounding for representation learning on graphs, explain its current successes and limitations, and even inform future advancements. We synthesize a spectrum of algorithmic approaches that, at their core, leverage graph topology to embed networks into compact vector spaces. We also capture the breadth of ways in which representation learning has dramatically improved the state-of-the-art in biomedical machine learning. Exemplary domains covered include identifying variants underlying complex traits, disentangling behaviors of single cells and their effects on health, assisting in diagnosis and treatment of patients, and developing safe and effective medicines. © 2021, CC BY.
