Chromatographic investigation on the ethyl acetate extract of the soft coral Lobophytum durum collected from the waters of Guishan Island in northeast Taiwan resulted in the isolation of three sterols 1–3, including one novel sterol, 3β-hydroxy-25-methylene-5-cholesten-7-one (1), and two previously reported sterols, 7-oxogorgosterol (2), and (24S)-ergostane-3β,5α,6β,25-tetraol 25-monoacetate (3). The structures of sterols 1–3 were determined by extensive spectroscopic data analysis and by comparison with those previously reported in the literature. Additionally, the absolute configuration of sterol 3 was identified by a single-crystal X-ray diffraction analysis for the first time in this study. The anti-inflammatory activities of sterols 1–3 were evaluated by examining their inhibitory effects on the generation of superoxide anions and the release of elastase by human neutrophils. © 2023, The Authors. Background: US postsecondary education students have disproportionately high rates of food insecurity and sugar-sweetened beverage consumption, and low rates of fruit and vegetable consumption. No large-scale study has investigated institutional factors predictive of these public health challenges. Objective: To examine whether institutional characteristics are predictive of food insecurity and dietary behaviors, including institutional minority designation, region, size, setting, Carnegie Classification, percentage of students living with family, and percentage of first-generation college students. Design: Data were from the National College Health Assessment-III fall 2021 to fall 2022 cross-sectional surveys. Participants and setting: Students (N = 129 295) from 204 US colleges and universities. Main outcome measures: Self-reported food insecurity and consumption of 0 fruits (vs ≥ 1), 0 vegetables (vs ≥ 1), and ≥1 sugar-sweetened beverages (vs 0) daily. Statistical analysis performed: Random-intercept Poisson multilevel models, adjusted for individual sociodemographic characteristics and institutional factors. Results: The prevalence of food insecurity and of consuming 0 fruits, 0 vegetables, and ≥1 sugar-sweetened beverages daily was 40%, 16%, 11%, and 67%, respectively. Attending an institution that was predominantly White (vs minority serving), in the US South (vs Northeast/Midwest), with 10 percentage-points fewer students living with family, or 10 percentage-points more first-generation college students was associated with a 13% (95% CI 1.05 to 1.20), 9% (95% CI 1.02 to 1.15), 3% (95% CI 1.01 to 1.06), and 15% (95% CI 1.12 to 1.19) higher prevalence of food insecurity, respectively. Performers With History of Voice Injury: A Survey Study of Treatment Outcomes and Vocal Function

Introduction: Past studies show that performers are more susceptible to voice injury, have higher incidence of injury, and experience greater vocal impairment than non-performers. Despite literature demonstrating otherwise, there remains fear and stigma that voice injury is a career-ending circumstance. Much of this is due to a lack of information about post-treatment vocal function. Methods: An anonymous online survey was distributed via email, flyer, and social media to a target audience of performers with a history of voice injury. It inquired about occupation, vocal symptoms, professionals consulted, and treatment adherence. Outcome measures included ability to perform, resolution of symptoms, and attitudes about their voices after voice injury. Findings were analyzed descriptively with statistical analysis to determine factors that may be related to favorable outcomes. Results: The survey was completed by 151 performers representing a range of genres, including musical theatre, classical, and popular genres. The most reported vocal symptoms were decreased range, singing voice quality changes, increased singing effort, and vocal fatigue. Most initially sought care from an otolaryngologist, laryngologist, or voice teacher. Diagnoses and recommendations varied, but those who adhered to treatment were more likely to report resolution of voice symptoms (P = 0.025). Those with symptoms for 2–4 weeks reported greater vocal confidence than those with a longer symptom duration (P = 0.0251). Performers working with a voice teacher were more likely to find treatment helpful (P = 0.0174). Those with neurogenic voice conditions reported less vocal reliability than participants with other pathologies (P = 0.0155). Conclusion: The majority of participants continued to perform, reported resolved or improved voice symptoms after treatment, and reported positive attitudes about their voices, regardless of their injury or current presence or absence of pathology on exam. We previously identified seven distinct multimorbidity clusters associated with major depressive disorder through a comprehensive analysis of 1.2 million individuals of multiple cohorts. These clusters, characterized by unique clinical, genetic, and psychiatric and somatic illness risk profiles, implicate divergent treatment pathways and disease management strategies. This study aims to deepen the understanding of these clusters by analyzing drug prescriptions, evaluating the effectiveness of antidepressant treatment strategies, and identifying potential markers for personalized medicine. Utilizing drug prescription data in the format of ATC codes, we performed epidemiological assessments, including multimorbidity (number of diseases), polypharmacy (number of chemical substances), and drug burden (number of prescriptions) analyses across the clusters. We applied linear regression models to assess strength and predictive capability of cluster membership on various metrics, and logistic regression to explore associations with treatment-resistant depression. We also quantified and visualized common antidepressant treatment sequences within each cluster. Our findings indicate significant variations in polypharmacy and drug burden across clusters, with distinct patterns emerging that correlate with the clusters’ profiles. Clusters liable to multimorbidity have higher drug burden, even after correction for number of diseases. Furthermore, the three clusters with higher risk for MDD showed different antidepressant treatment profiles;two required significantly more antidepressant prescriptions and had a higher risk for TRD. The detailed pharmacological profiling presented in this study not only corroborates the initial cluster definitions but also enhances our predictive capabilities for treatment outcomes in MDD. By linking pharmacological data with comorbidity profiles, we pave the way for targeted therapeutic interventions. © 2025 The Author(s)

AbstractBackgroundHeterozygous germline variants in HRAS can lead to RASopathies, a group of disorders caused by gain-of-function variants in several components and modulators of the Ras-MAPK pathway. Recently, different missense variants, indels and a frameshift variant, have been associated with attenuated or distinctive phenotypes, suggesting a wider clinical spectrum. Case presentationWe report a pair of twins with shared distinctive features, including mild intellectual disability, anxiety, psychosis, dysmorphism, short stature, early hair loss, vitamin D deficiency, osteopenia and hematuria, harboring a novel de novo nonsense variant in HRAS. Targeted RNA-sequencing demonstrates that this variant affects splicing. ConclusionOur report provides evidence that variants affecting transcript processing in HRAS may lead to an attenuated phenotype associated with neuropsychiatric features. Further studies are needed to fully understand the mechanism and phenotypic variability.

Generalized Linear Models for Longitudinal Data with Biased Sampling Designs: A Sequential Offsetted Regressions Approach

Biased sampling designs can be highly efficient when studying rare (binary) or low variability (continuous) endpoints. We consider longitudinal data settings in which the probability of being sampled depends on a repeatedly measured response through an outcome-related, auxiliary variable. Such auxiliary variable- or outcome-dependent sampling improves observed response and possibly exposure variability over random sampling, even though the auxiliary variable is not of scientific interest. For analysis, we propose a generalized linear model based approach using a sequence of two offsetted regressions. The first estimates the relationship of the auxiliary variable to response and covariate data using an offsetted logistic regression model. The offset hinges on the (assumed) known ratio of sampling probabilities for different values of the auxiliary variable. Results from the auxiliary model are used to estimate observation-specific probabilities of being sampled conditional on the response and covariates, and these probabilities are then used to account for bias in the second, target population model. We provide asymptotic standard errors accounting for uncertainty in the estimation of the auxiliary model, and perform simulation studies demonstrating substantial bias reduction, correct coverage probability, and improved design efficiency over simple random sampling designs. Ovarian vein sampling, and serum and urine testosterone monitoring in ovarian Leydig cell tumors: A report of two cases

Background: Ovarian Leydig cell tumors are rare, testosterone-producing tumors that pose diagnostic challenges. Cases: A 36-year-old woman presented with 10 years of amenorrhea, facial hair growth and clitoromegaly. A 59-year-old woman presented after 2 years of voice deepening and terminal hair growth. Testosterone concentrations were elevated for both patients;however, imaging failed to identify ovarian or adrenal pathology. For the first patient, selective ovarian venous sampling was performed with results suggesting right ovarian testosterone production. Right ovarian Leydig cell tumors were found in both patients after salpingo-oophorectomy. Testosterone levels immediately declined following tumor removal. Conclusion: Additional diagnostic modalities, such as ovarian venous sampling, should be considered when the etiology of hyperandrogenism cannot be identified through lab work or imaging. In addition, sequential post-operative testosterone levels in serum or urine can help confirm adequate removal of the ovarian tumor. © 2019 The Authors
