Aims After transcatheter aortic valve replacement (TAVR), cardiovascular and non-cardiovascular comorbidities may offset the survival benefit from the procedure. We aimed to describe the relationships between that benefit and patient comorbidities. Methods and results The study pooled two European cohorts of patients with severe aortic stenosis (AS-pooled): one with patients who underwent (cohort of AS patients treated by TAVR, N=233) and another with patients who did not undergo TAVR (cohort of AS patients treated medically;N=291). The investigators collected the following: calcification prognostic impact (CAPRI) and Charlson scores for cardiovascular and non-cardiovascular comorbidities, activities of daily living (ADL)/instrumental activities of daily living (IADL) scores for frailty as well as routine Society of Thoracic Surgeons (STS) score and Logistic Euroscore. Unlike ADL/IADL scores, CAPRI and Charlson scores were found to be independent predictors of 1-year all-cause death in the AS-pooled cohort, with and without adjustment for STS score or Logistic Euroscore;they were thus retained to define a three-level prognostic scale (good, intermediate, and poor). The survival benefit from TAVR-vs. no TAVR-was stratified according to these three prognosis categories. The beneficial effect of TAVR on 1-year all-cause death was significant in patients with good and intermediate prognosis, hazard ratio (95% confidence interval): 0.36 (0.18;0.72) and 0.32 (0.15;0.67). That effect was reduced and not statistically significant in patient with poor prognosis [0.65 (0.22;1.88)]. Conclusion The study showed that, beyond a given comorbidity burden (as assessed by CAPRI and Charlson scores), the probability of death within a year was high and poorly reduced by TAVR. This indicates the futility of TAVR in patients in the poor prognosis category. © The Author(s) 2022.

Background Rognosis of heart failure with reduced ejection fraction (HFREF) is improved by tailored drugs optimization but poor is known concerning such optimization in HFREF patients implanted by implantable cardiac defibrillator (ICD) or cardiac resynchronization therapy (CRT). We conduct an evaluation of European guidelines implement before device implantation. Methods Between January 2009 and December 2014, data were extracted from the échantillon généraliste des bénéficiaires (EGB) database, a representative 1/97 random sample of the population covered by the French national health insurance system. We included HFREF patients implanted by ICD or CRT. Survival and treatments were analyzed at baseline, 3 months and until January 2016. Results We included a sample of 378 patients (135 CRT, 243 ICD) with a mean age of 68 ± 13 years old. Mean follow-up was 23 months (11–42). At baseline, 36% of patients had only one or no HF drug among beta-blocker (BB), angiotensin-converting enzyme inhibitor (ACE-I) or angiotensin receptor blockers (ARB) and mineralocorticoid receptor antagonist (MRA). Twenty-six percent had an optimal treatment (all the 3 classes). At 3 months, prescription rate was higher compared to baseline but not at the end of follow-up. HF hospitalization rate was higher in non-optimized patient group 28% vs. 14% ( P = 0.001). HFrEF optimal treatment was associated with a better survival (HR = 0.27 [0.15–0.46], P < 0.001) ( Fig. 1 ). Conclusion HFREF drugs are under prescribed before CRT and ICD implantations despite we demonstrated that treatment optimization reduced HF hospitalization.

In practice, multiple objects in images are located by consecutively applying one detector for each class and taking the best confident score. In this work, we propose to show the advantage of grouping similar object classes into a hierarchical structure. While this approach has found interest in image classification, it is not analyzed for the object detection task. Each node in the hierarchy represents one decision line. All the decision lines are learned jointly using a novel problem formulation. Based on experiments using PASCAL VOC 2007 dataset, we show that our approach improves detection performance compared to a baseline approach. We propose a new framework for developing statistical shape models of endometrial tissues from real clinical data. Endometrial tissues naturally form cylindrical surfaces, and thus, we adopt, with modification, a recent Riemannian framework for statistical shape analysis of parameterized surfaces. This methodology is based on a representation of surfaces termed square-root normal fields (SRNFs), which enables invariance to all shape preserving transformations including translation, scale, rotation, and re-parameterization. We extend this framework by computing parametrization-invariant statistical summaries of endometrial tissue shapes, and random sampling from learned generative models. Such models are very useful for medical practitioners during different tasks such as diagnosing or monitoring endometriosis. Furthermore, real data in medical applications in general (and in particular in this application) is often scarce, and thus the generated random samples are a key step for evaluating segmentation and registration approaches. Moreover, this study allows us to efficiently construct a large set of realistic samples that can open new avenues for diagnosing and monitoring complex diseases when using automatic techniques from computer vision, machine learning, etc. Copyright © 2014 SCITEPRESS.

Nous rapportons le cas d’un patient de 24 ans admis pour syndrome coronarien aigu avec sus décalage permanent du segment ST dans le territoire antérieur compliqué de fibrillation ventriculaire. L’angiographie réalisée en urgence révèle une subocclusion de l’inter ventriculaire antérieure proximale. Il est réalisé une désobstruction par thromboaspiration manuelle sous perfusion d’inhibiteurs des glycoprotéines IIbIIIa, permettant de restaurer un flux TIMI 3 et de normaliser l’ECG. Etant donné la charge thrombotique élevée, il n’est pas réalisé de stenting en phase aiguë. Après 48 h de traitement anti thrombotique optimal, un contrôle coronarographique complété par une imagerie endocoronaire de haute résolution (tomographie par cohérence optique : OCT) est réalisé. L’OCT met en évidence une forte diminution de la charge thrombotique et l’absence de lésion organique sous-jacente importante (pas de sténose organique ou de macro rupture de plaque). À la lumière de ces données, il est décidé de poursuivre un traitement médical seul sans recours à l’implantation d’un stent. Le contrôle OCT systématique à 6 mois retrouve un athérome non sténosant sans rupture de plaque ou thrombus. Ce jeune patient est depuis asymptomatique avec 2 ans de suivi et a récupéré une fonction ventriculaire gauche normale. We report the case of a 24-year-old patient admitted for anterior ST segment elevation myocardial complicated by ventricular fibrillation and revealing thrombotic sub occlusion of the left anterior descending coronary artery. Revascularization is achieved by manual thrombectomy and use of Glycoprotein GPIIbIIIa inhibitors and permits to restore TIMI 3 flow. Given the large thrombotic burden, the patient is initially treated medically (optimal anti thrombotic therapy without stenting) and benefits from angiographic control 48 hours later with imaging by Optical Coherence Tomography (OCT). The quality of a mosaic depends on the projective alignment of the images involved. After point-correspondences between the images have been established, bundle adjustment finds an alignment considered optimal under certain hypotheses. This procedure minimizes a nonlinear cost and has to be initialized with care. It is very common to compose inter-frame homographies which have been computed with standard methods in order to get an initial global alignment. This technique is suboptimal if there is noise or missing ho-mographies as it typically uses a small part of the available data. We propose four new closed-form solutions. They all provide non-heuristic initial alignments using all the known inter-frame homographies. Our methods are tested with synthetic and real data and are compared to the standard method. These experiments reveal that our methods are more accurate, taking advantage of the redundant information available in the set of inter-frame homographies. © 2010 IEEE.
