There has been a growing interest in Health informatics applications, research, and education within the Middle East and North African Region over the past twenty years. People of this region share similar cultural and religious values, primarily speak the Arabic language, and have similar health care related issues, which are in dire need of being addressed. Health informatics efforts, organizations, and initiatives within the region have been largely under-represented within, but not ignored by, the International Medical informatics Association (IMIA). Attempts to create bonds and collaboration between the different organizations of the region have remained scattered, and often, resulted in failure despite the fact that the need for a united health informatics collaborative within the region has never been more crucial than today. During the 2017 MEDINFO, held in Hangzhou, China, a new organization, the Middle East and North African Health informatics Association (MENAHIA) was conceived as a regional non-governmental organization to promote and facilitate health informatics uptake within the region endorsing health informatics research and educational initiatives of the 22 countries represented within the region. This paper provides an overview of the collaboration and efforts to date in forming MENAHIA and displays the variety of initiatives that are already occurring within the MENAHIA region, which MENAHIA will help, endorse, support, share, and improve within the international forum of health informatics.

Sexual health is the state of well-being regarding sexuality. Sexual health is highly valued and associated with overall health. Overall health and well-being are more than the absence of disease or dysfunction. Health care systems adopting whole health models of care need to incorporate a holistic assessment of sexual health. This includes assessing patients' sexual orientation and gender identity (SOGI). If health systems, including but not limited to the Veterans Health Administration (VHA), incorporate sexual health into whole health they could enhance preventive care, promote healthy sexual functioning, and optimize overall health and well-being. Assessing sexual health can give providers important information about a patient's health, well-being, and health goals. Sexual concerns or dysfunction may also signal undiagnosed health conditions. Additionally, collecting SOGI information as part of a sexual health assessment would allow providers to address problems that drive disparities for lesbian, gay, bisexual, transgender, queer, and similar minority (LGBTQ+) populations. Health care providers do not routinely assess sexual health in clinical practice. One barrier is a gap in communication between patients and providers. Providers cite beliefs that patients will bring up sexual concerns themselves or might be offended by discussing sexual health. Patients often report an expectation that providers will bring up sexual health and being comfortable discussing sexual health with their providers. Within the VHA, the lack of a sexual health template within the electronic health record (EHR) adds an additional barrier. The VHA's transition toward whole health and updates to its EHR provide unique opportunities to integrate sexual health assessment into routine care. Background & Aims Magnetic resonance-based steatosis measurement of the liver has become the non-invasive gold standard; however very few diagnostic thresholds have been independently validated and none in children. This study was designed to validate diagnostic thresholds, assess repeatability and the relationship to histology of a proprietary MRI volumetric liver fat fraction (VLFF) method (HepaFat-Scan ® ) and magnetic resonance spectroscopy (MRS) proton density fat fraction (PDFF) method. Methods This cross-sectional study assesses the diagnostic accuracy of MRI-VLFF and MRS-PDFF of 50 children who had a clinically indicated liver biopsy. Twenty-five children were randomly selected for intra-examination repeatability. The positive pixel count (PPC) algorithm was used to quantify the area of fat vesicles in the histology. The area under the receiver operating characteristic curve (AUROC) was used to assess the diagnostic accuracy. Bland-Altman plots were used to compare the methods and examine the repeatability. Results The MRI-VLFF ranged from 0.3% to 35.4% and the MRS-PDFF ranged from 0.5% to 36.9%. Based on histopathological analysis, the diagnostic accuracy of MRI-VLFF for steatosis detection (grade 0 vs 1-3) showed an AUROC of 0.95 (95% confidence interval [CI]: 0.89-1.00), whereas MRI-PDFF classified grade 0 vs 1-3 showed an AUROC of 0.96 (95% CI: 0.91-1.00). The repeatability coefficients were 1.8 (±0.5) % for MRI-VLFF and 2.1 (±0.6) % for MRS-PDFF. There was no bias between MRI-VLFF and PPC steatosis. To allow direct comparison, MRS-PDFF was converted to MRS-VLFF. Small, but significant biases were measured between PPC steatosis and MRS-VLFF (−1.38%) and between MRI-VLFF and MRS-VLFF (−1.39%). Conclusions Both MRI-VLFF and MRS-PDFF accurately estimated hepatic steatosis in children with excellent agreement with the histological assessment.
