Primary biliary cirrhosis: From bench to bedside

Primary biliary cirrhosis(PBC) is a chronic non-suppurative destructive intrahepatic cholangitis leading to cirrhosis after a protractive non cirrhotic stage. The etiology and pathogenesis are largely unknown and autoimmne mechanisms have been implicated to explain the pathological lesions. Many epitopes and autoantigens have been reported as crucial in the pathophysiology of the disease and T and B cells abnormalities have been described, the exact pathways leading to the destruction of small intrahepatic ductules are mostly speculative. In this review we examined the various epidemiologal and geoepidemiological data as well as the complex pathogenetic aspects of this disease, focusing on recent in vivo and in vitro studies in this field. Initiation and progression of PBC is believed to be a multifactorial process with strong infuences from the patient's genetic background and by various environmental factors. The role of innate and adaptive immunity, including cytokines, chemokines, macrophages and the involvement of apoptosis and reactive oxygen species are outlined in detailed. The current pathogenetic aspects are presented and a novel pathogenetic theory unifying the accumulated clinical information with in vitro and in vivo data is formulated. A review of clinical manifestations and immunological and pathological diagnosis was presented. Treatment modalities, including the multiple mechanisms of action of ursodeoxycholate were finally discussed.

We derive a posteriori bounds in non-Hilbertian norms for finite element approximations to the Allen-Cahn phase field model. The derivation of the bounds relies crucially on an approximate spectral estimate for the linearised steady part of the partial differential equation, yielding only polynomial dependence on the phase separation length. A second key ingredient in the proof of the a posteriori bounds is the use of the elliptic reconstruction framework, enabling the control of the error in the L∞(Lr)-norms, r âˆˆ [2, ∞ ]. For simplicity of the presentation, only the spatially discrete method is considered here. © 2013 John Wiley & Sons, Ltd.

Graphene based polymer nanocomposites are hybrid materials with a very broad range of technological applications. In this work, we study three hybrid polymer/graphene interfacial systems (polystyrene/graphene, poly(methyl methacrylate)/graphene and polyethylene/graphene) through detailed atomistic molecular dynamics (MD) simulations. Density profiles, structural characteristics and mobility aspects are being examined at the molecular level for all model systems. In addition, we compare the properties of the hybrid systems to the properties of the corresponding bulk ones, as well as to theoretical predictions.

Supernova (SN) 2016bdu is an unusual transient resembling SN 2009ip. SN 2009ip-like events are characterized by a long-lasting phase of erratic variability which ends with two luminous outbursts a few weeks apart. The second outburst is significantly more luminous (about 3 mag) than the first. In the case of SN 2016bdu, the first outburst (Event A) reached an absolute magnitude Mr ≈ −15.3 mag, while the second one (Event B) occurred over one month later and reached Mr ≈ −18 mag. By inspecting archival data, a faint source at the position of SN 2016bdu is detectable several times in the past few years. We interpret these detections as signatures of a phase of erratic variability, similar to that experienced by SN 2009ip between 2008 and mid-2012, and resembling the currently observed variability of the luminous blue variable SN 2000ch in NGC 3432. Spectroscopic monitoring of SN 2016bdu during the second peak initially shows features typical of a SN IIn. One month after the Event B maximum, the spectra develop broad Balmer lines with P Cygni profiles and broad metal features. At these late phases, the spectra resemble those of a typical Type II SN. All members of this SN 2009ip-like group are remarkably similar to the Type IIn SN 2005gl. For this object, the claim of a terminal SN explosion is supported by the disappearance of the progenitor star. The similarity with SN 2005gl suggests that all members of this family may finally explode as genuine SNe, although the unequivocal detection of nucleosynthesised elements in their nebular spectra is still missing. We observe a spontaneous parity breaking bifurcation to a ferromagnetic state in a spatially trapped exciton-polariton condensate. At a critical bifurcation density under nonresonant excitation, the whole condensate spontaneously magnetizes and randomly adopts one of two elliptically polarized (up to 95% circularly polarized) states with opposite handedness of polarization. The magnetized condensate remains stable for many seconds at 5 K, but at higher temperatures, it can flip from one magnetic orientation to another. We optically address these states and demonstrate the inversion of the magnetic state by resonantly injecting 100-fold weaker pulses of opposite spin. Theoretically, these phenomena can be well described as spontaneous symmetry breaking of the spin degree of freedom induced by different loss rates of the linear polarizations.

Introduction: We attempt to investigate if the mechanical properties of an intraluminal thrombus (ILT) could be assessed non-invasively through modern imaging techniques. Thrombus compressibility and deformation are evaluated invivo. Report: Five patients underwent electrocardiographically (ECG) gated computed tomography scan during surveillance of their abdominal aortic aneurysm (AAA). ILT volumes and cross-sectional areas during peak systole and end diastole were recorded and found to remain constant. Discussion: ILT elastic behaviour can be estimated non-invasively. Under physiologic arterial pressure, an ILT is incompressible and does not deform longitudinally. © 2013 European Society for Vascular Surgery.
