Background: The COVID-19 pandemic highlighted gaps in health surveillance systems, disease prevention, and treatment globally. Among the many factors that might have led to these gaps is the issue of the financing of national health systems, especially in low-income and middle-income countries (LMICs), as well as a robust global system for pandemic preparedness. We aimed to provide a comparative assessment of global health spending at the onset of the pandemic;characterise the amount of development assistance for pandemic preparedness and response disbursed in the first 2 years of the COVID-19 pandemic;and examine expectations for future health spending and put into context the expected need for investment in pandemic preparedness. Methods: In this analysis of global health spending between 1990 and 2021, and prediction from 2021 to 2026, we estimated four sources of health spending: development assistance for health (DAH), government spending, out-of-pocket spending, and prepaid private spending across 204 countries and territories. We used the Organisation for Economic Co-operation and Development (OECD)'s Creditor Reporting System (CRS) and the WHO Global Health Expenditure database (GHED) to estimate spending. We estimated development assistance for general health, COVID-19 response, and pandemic preparedness and response using a keyword search. Health spending estimates were combined with estimates of resources needed for pandemic prevention and preparedness to analyse future health spending patterns, relative to need. Findings: In 2019, at the onset of the COVID-19 pandemic, US$9·2 trillion (95% uncertainty interval [UI] 9·1–9·3) was spent on health worldwide. We found great disparities in the amount of resources devoted to health, with high-income countries spending $7·3 trillion (95% UI 7·2–7·4) in 2019;293·7 times the $24·8 billion (95% UI 24·3–25·3) spent by low-income countries in 2019. Background: Neck pain is a highly prevalent condition that leads to considerable pain, disability, and economic cost. We present the most current estimates of neck pain prevalence and years lived with disability (YLDs) from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) by age, sex, and location, with forecasted prevalence to 2050. Methods: Systematic reviews identified population-representative surveys used to estimate the prevalence of and YLDs from neck pain in 204 countries and territories, spanning from 1990 to 2020, with additional data from opportunistic review. Medical claims data from Taiwan (province of China) were also included. Input data were pooled using DisMod-MR 2.1, a Bayesian meta-regression tool. Prevalence was forecast to 2050 using a mixed-effects model using Socio-demographic Index as a predictor and multiplying by projected population estimates. We present 95% UIs for every metric based on the 2·5th and 97·5th percentiles of 100 draws of the posterior distribution. Findings: Globally, in 2020, neck pain affected 203 million (95% uncertainty interval [UI] 163–253) people. The global age-standardised prevalence rate of neck pain was estimated to be 2450 (1960–3040) per 100 000 population and global age-standardised YLD rate was estimated to be 244 (165–346) per 100 000. The age-standardised prevalence rate remained stable between 1990 and 2020 (percentage change 0·2% [–1·3 to 1·7]). Globally, females had a higher age-standardised prevalence rate (2890 [2330–3620] per 100 000) than males (2000 [1600–2480] per 100 000), with the prevalence peaking between 45 years and 74 years in male and female sexes. By 2050, the estimated global number of neck pain cases is projected to be 269 million (219–322), with an increase of 32·5% (23·9–42·3) from 2020 to 2050. Predicting visual features from text for image and video caption retrieval

This paper strives to find amidst a set of sentences the one best describing the content of a given image or video. Different from existing works, which rely on a joint subspace for their image and video caption retrieval, we propose to do so in a visual space exclusively. Apart from this conceptual novelty, we contribute Word2VisualVec, a deep neural network architecture that learns to predict a visual feature representation from textual input. Example captions are encoded into a textual embedding based on multi-scale sentence vectorization and further transferred into a deep visual feature of choice via a simple multi-layer perceptron. We further generalize Word2VisualVec for video caption retrieval, by predicting from text both 3-D convolutional neural network features as well as a visual-audio representation. Experiments on Flickr8k, Flickr30k, the Microsoft Video Description dataset and the very recent NIST TrecVid challenge for video caption retrieval detail Word2VisualVec's properties, its benefit over textual embeddings, the potential for multimodal query composition and its state-of-the-art results. Back pain is a common and debilitating disorder with largely unknown underlying biology. Here we report a genome-wide association study of back pain using diagnoses assigned in clinical practice; dorsalgia (119,100 cases, 909,847 controls) and intervertebral disc disorder (IDD) (58,854 cases, 922,958 controls). We identify 41 variants at 33 loci. The most significant association (ORIDD = 0.92, P = 1.6 × 10−39; ORdorsalgia = 0.92, P = 7.2 × 10−15) is with a 3’UTR variant (rs1871452-T) in CHST3, encoding a sulfotransferase enzyme expressed in intervertebral discs. The largest effects on IDD are conferred by rare (MAF = 0.07 − 0.32%) loss-of-function (LoF) variants in SLC13A1, encoding a sodium-sulfate co-transporter (LoF burden OR = 1.44, P = 3.1 × 10−11); variants that also associate with reduced serum sulfate. Genes implicated by this study are involved in cartilage and bone biology, as well as neurological and inflammatory *** is known about the biology of back pain, a leading cause of disability. Here the authors report 30 new back pain loci, implicating genes involved in cartilage/bone biology, as well as neurological and inflammatory processes.

The sudden outbreak of the Coronavirus disease (COVID-19) swept across the world in early 2020, triggering the lockdowns of several billion people across many countries, including China, Spain, India, the U.K., Italy, France, Germany, and most states of the U.S. The transmission of the virus accelerated rapidly with the most confirmed cases in the U.S., and New York City became an epicenter of the pandemic by the end of March. In response to this national and global emergency, the NSF Spatiotemporal Innovation Center brought together a taskforce of international researchers and assembled implemented strategies to rapidly respond to this crisis, for supporting research, saving lives, and protecting the health of global citizens. This perspective paper presents our collective view on the global health emergency and our effort in collecting, analyzing, and sharing relevant data on global policy and government responses, geospatial indicators of the outbreak and evolving forecasts;in developing research capabilities and mitigation measures with global scientists, promoting collaborative research on outbreak dynamics, and reflecting on the dynamic responses from human societies. Chronic care manages long-term, progressive conditions, while acute care addresses short-term conditions. Chronic conditions increasingly strain health systems, which are often unprepared for these demands. This study examines the burden of conditions requiring acute versus chronic care, including sequelae. Conditions and sequelae from the Global Burden of Diseases Study 2019 were classified into acute or chronic care categories. data were analysed by age, sex, and socio-demographic index, presenting total numbers and contributions to burden metrics such as Disability-Adjusted Life Years (DALYs), Years Lived with Disability (YLD), and Years of Life Lost (YLL). Approximately 68% of DALYs were attributed to chronic care, while 27% were due to acute care. Chronic care needs increased with age, representing 86% of YLDs and 71% of YLLs, and accounting for 93% of YLDs from sequelae. These findings highlight that chronic care needs far exceed acute care needs globally, necessitating health systems to adapt accordingly. © 2025. The Author(s).

IT product trial is an important method to promote consumers' attitude toward IT product. Affect is one of the most essential factors and components in determining consumers' attitude. However, previous studies pay more attention to the affective response to trial, which measure the holistic feeling about product trial experience. Affect in product trial is complex and multipartite that different parts may have different influence on consumers' attitude. Therefore, we build the Affective Response Framework to divide consumers' affect into affective response to product, affective response to behaviour, and affective response to environment. And then we analyse the relationship between three components of affective response and consumers' attitude. In order to test the hypothesis, we launch a field study of new IT product trial (an electroencephalogram product) among 205 college students. Results reveal that affective response to product and behaviour have positive influence on attitude toward product;affective response to behaviour and environment have positive influence on attitude toward trial.

Background: In an era of shifting global agendas and expanded emphasis on non-communicable diseases and injuries along with communicable diseases, sound evidence on trends by cause at the national level is essential. The Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) provides a systematic scientific assessment of published, publicly available, and contributed data on incidence, prevalence, and mortality for a mutually exclusive and collectively exhaustive list of diseases and injuries. Methods: GBD estimates incidence, prevalence, mortality, years of life lost (YLLs), years lived with disability (YLDs), and disability-adjusted life-years (DALYs) due to 369 diseases and injuries, for two sexes, and for 204 countries and territories. Input data were extracted from censuses, household surveys, civil registration and vital statistics, disease registries, health service use, air pollution monitors, satellite imaging, disease notifications, and other sources. Cause-specific death rates and cause fractions were calculated using the Cause of Death Ensemble model and spatiotemporal Gaussian process regression. Cause-specific deaths were adjusted to match the total all-cause deaths calculated as part of the GBD population, fertility, and mortality estimates. Deaths were multiplied by standard life expectancy at each age to calculate YLLs. A Bayesian meta-regression modelling tool, DisMod-MR 2.1, was used to ensure consistency between incidence, prevalence, remission, excess mortality, and cause-specific mortality for most causes. Prevalence estimates were multiplied by disability weights for mutually exclusive sequelae of diseases and injuries to calculate YLDs. We considered results in the context of the Socio-demographic Index (SDI), a composite indicator of income per capita, years of schooling, and fertility rate in females younger than 25 years. Many companies are confronted with an increasingly large number of business rules. Consequently, means for efficient and effective management of these business rules are necessary. However, business Rule Management Systems (BRMS) frequently provide only simple business rule organization techniques such as collecting business rules into rule sets according to a single criterion. Many fields organize information by contexts that may be hierarchically structured. Similarly, we propose the use of contexts to manage business rules and associated business vocabulary. We survey context literature and identify dimensions and possible choices for each dimension to compare and relate different context models. For each dimension we propose the appropriate choice for a context model of business rules and business vocabulary. Based on these choices we present a generic context model for business rules and business vocabulary that can be instantiated multi-level: for a specific domain and for a concrete application. We demonstrate the usefulness of the generic context model for business rules and vocabulary by relating it to a use case in the aeronautical domain, the semantic filtering of digital Notices to Airmen. This real world use case addressed in the SemNOTAM project involves thousands of business rules and extensive business vocabularies. These need to be organized along multiple hierarchical context parameters (e.g. aircraft type, mode of operation) such that they become manageable with regard to definition, maintenance, extension, and determining the business rules and business vocabulary relevant for a specific business case (e.g. flight plan of a pilot, responsibility area of a controller).
