Background Patients with giant cell arteritis (GCA) have an increased risk of developing aortic aneurysms. Retrospective studies have shown that 18 F-fluorodeoxyglucose (FDG) uptake in large vessels at diagnosis increases the risk of developing aortic complications during follow-up. Objectives To prospectively evaluate the association between vascular FDG uptake at diagnosis and the evolution of aortic diameter and volume in patients with GCA. Methods GCA patients who had FDG positron emission tomography (PET) imaging at diagnosis within 3 days of initiation of glucocorticoids and who were prospectively followed for at least 2 years were included. PET scans were visually scored (0-3) at 7 vascular areas and a total vascular score (TVS) was calculated, ranging from 0 to 21. Patients underwent a computed tomography (CT) scan at diagnosis and yearly thereafter for a maximum of 10 years. The association between vascular FDG uptake and aortic diameter and volume was estimated by linear mixed effect models with random intercept and slope adjusted for age, sex, cardiovascular risk factors, CT or PET/CT scan and intravenous contrast. The ascending aorta, aortic arch, and descending aorta were considered aneurysmatic when the diameter was ≥45, ≥40, and ≥35 mm, respectively. Results Hundred patients (mean age 70 years, 68% females) were included, of which 74 patients had FDG uptake ≥2 in any large vessel, 60 in the thoracic aorta and 49 in the abdominal aorta. Median follow-up was 80 months (IQR 47-110). The increase in ascending and descending aortic diameter and in thoracic aortic volume was higher in patients with vascular FDG uptake ≥2 compared to those without ( Table 1 ). This increase was also significantly associated with TVS ( Figure 1 ). FDG uptake was not associated with an increase in abdominal aortic diameter nor volume. Background: Gout, a common crystal arthropathy, is associated with increased risk of cardiovascular disease. We aimed to identify how this risk varies by individual cardiovascular disease across a broad spectrum of conditions. Methods: In this matched case–control study, we used linked primary and secondary electronic health records from the UK Clinical Practice Research Datalink to assemble a cohort of individuals with a first-time diagnosis of gout between Jan 1, 2000 and Dec 31, 2017, who were aged 80 years or younger at diagnosis, and free of cardiovascular diseases up to 12 months after diagnosis. The control cohort comprised up to five control individuals per patient with gout, matched on age, sex, socioeconomic status, geographical region, and calendar time, randomly selected among individuals free of gout at any time before and during the study period. The cohorts were followed up until June 30, 2019. We investigated the incidence of 12 cardiovascular diseases and used Cox proportional hazards models to examine differences in people with and without gout, overall and by subgroups of sex, age, socioeconomic status, and year of study inclusion. We further adjusted models for known cardiovascular risk factors (blood pressure, BMI, smoking status, cholesterol, type 2 diabetes, chronic kidney disease, and history of hypertension). Findings: We identified 152 663 individuals with gout (mean age 56·2 years [SD 13·3];120 324 [78·8%] men and 32 339 [21·2%] women) and 709 981 matched controls (mean age 56·5 years [13·2];561 002 [79·0%] men and 148 979 [21·0%] women). Of these individuals, 31 479 (20·6%) with gout and 106 520 (15·0%) without gout developed cardiovascular disease during a median follow-up of 6·5 years (IQR 3·1–10·5). Patients with gout had higher risk of cardiovascular diseases than matched controls (hazard ratio [HR] 1·58 [95% CI 1·52–1·63]). To guide and assist applicants for the preparation and presentation of genetically modified (GM) plant market authorization applications, the GMO Panel of EFSA has developed guidance for the risk assessment of GM plants and derived food and feed, which describes principles, concepts, data requirements and issues to be considered when performing the risk assessment of GM plants and derived food and feed. The objective of this framework contract is to provide statistical support to the EFSA GMO Panel for the evaluation of GMO application dossiers with respect to the statistical analysis of collected data from field trials carried out for the comparative assessment according to the requirements described in the EFSA guidance on statistical considerations for the safety evaluation of GMOs and EFSA guidance on the risk assessment of food and feed from genetically modified plants. This annual report describes the different tasks performed in 2020 under the framework contract and summarizes their outcome. The evaluations of the statistical analysis of applications during 2020 met to a large extent the requirements reported in the guidance documents. Only a few minor non-compliances were recurrently reported in most of the applications.

Kriging is an established methodology for predicting spatial data in geostatistics. Current kriging techniques can handle linear dependencies on spatially referenced covariates. Although splines have shown promise in capturing nonlinear dependencies of covariates, their combination with kriging, especially in handling count data, remains underexplored. This paper proposes a new Bayesian approach to the low-rank representation of geoadditive models, which integrates splines and kriging to account for both spatial correlations and nonlinear dependencies of covariates. The proposed method accommodates Gaussian and count data inherent in many geospatial datasets. Additionally, Laplace approximations to selected posterior distributions enhances computational efficiency, resulting in faster computation times compared to Markov chain Monte Carlo techniques commonly used for Bayesian inference. Method performance is assessed through a simulation study, demonstrating the effectiveness of the proposed approach. The methodology is applied to the analysis of heavy metal concentrations in the Meuse river and vulnerability to the coronavirus disease 2019 (COVID-19) in Belgium.

Deep reinforcement learning for large-scale epidemic control

Epidemics of infectious diseases are an important threat to public health and global economies. Yet, the development of prevention strategies remains a challenging process, as epidemics are non-linear and complex processes. For this reason, we investigate a deep reinforcement learning approach to automatically learn prevention strategies in the context of pandemic influenza. Firstly, we construct a new epidemiological meta-population model, with 379 patches (one for each administrative district in Great Britain), that adequately captures the infection process of pandemic influenza. Our model balances complexity and computational efficiency such that the use of reinforcement learning techniques becomes attainable. Secondly, we set up a ground truth such that we can evaluate the performance of the "Proximal Policy Optimization" algorithm to learn in a single district of this epidemiological model. Finally, we consider a large-scale problem, by conducting an experiment where we aim to learn a joint policy to control the districts in a community of 11 tightly coupled districts, for which no ground truth can be established. This experiment shows that deep reinforcement learning can be used to learn mitigation policies in complex epidemiological models with a large state space. Moreover, through this experiment, we demonstrate that there can be an advantage to consider collaboration between districts when designing prevention strategies. Copyright © 2020, The Authors. The Gut Microbiome Causally Contributes to Interspecific Differences in Pesticide Sensitivity

Explaining interspecific differences in pesticide sensitivity is key to increasing the predictive power of ecotoxicology. Besides species traits, the gut microbiome may provide an untested additional predictive factor since it often plays a role in host defense against stressors. Therefore, we investigated the gut microbiome's causal role in shaping differences in pesticide sensitivity between two congeneric damselfly species. After an antibiotic treatment, reciprocal gut microbiome transplants were performed between pesticide-sensitive and more tolerant larvae, with donor larvae first preexposed to either chlorpyrifos or a solvent control to match the subsequent pesticide treatments of the recipients. The gut microbiome, determined by 16S rRNA gene amplicon sequencing, of both species included pesticide-degrading bacteria, but also showed shared and species-specific responses to the pesticide. Notably, the most pesticide-sensitive combination, with the highest pesticide-induced mortality, consisted of larvae receiving donor gut microbiota, whereas the least sensitive combination consisted of larvae receiving donor gut microbiota, whereby the pesticide did not increase larval mortality. The two mixed donor-recipient gut microbiome combinations resulted in an intermediate sensitivity. Remarkably, recipient larvae experienced a lower chlorpyrifos-induced mortality when they received an donor gut microbiome than when they received their conspecific donor gut microbiome. Our results provide, to our knowledge, the first proof-of-evidence that the gut microbiome causally contributes to species differences in pesticide sensitivity.
