BACKGROUND: Sepsis is a life-threatening inflammatory condition in which the invading pathogen avoids the host's defense mechanisms and continuously stimulates and damages host cells. Consequently, many immune responses initially triggered for protection become harmful because of the failure to restore homeostasis, resulting in ongoing hyperinflammation and immunosuppression. METHODS: A literature review was conducted to address bacterial sepsis, describe advances in understanding complex immunological reactions, critically assess diagnostic approaches, and emphasize the importance of studying bacterial bottlenecks in the detection and treatment of ***: Diagnosing sepsis via a single laboratory test is not feasible;therefore, multiple key biomarkers are typically monitored, with a focus on trends rather than absolute values. The immediate interpretation of sepsis-associated clinical signs and symptoms, along with the use of specific and sensitive laboratory tests, is crucial for the survival of patients in the early stages. However, long-term mortality associated with sepsis is now recognized, and alongside the progression of this condition, there is an in vivo selection of adapted ***: Bacterial sepsis remains a significant cause of mortality across all ages and societies. While substantial progress has been made in understanding the immunological mechanisms underlying the inflammatory response, there is growing recognition that the ongoing host-pathogen interactions, including the emergence of adapted virulent strains, shape both the acute and long-term outcomes in sepsis. This underscores the urgent need for novel high-throughput diagnostic methods and a shift toward more pre-emptive, rather than reactive, treatment strategies in sepsis care.

Introduction: Peptic ulcer disease's (PUD) global significance, emphasizing its health burdens and associations with factors like Helicobacter pylori infection and lifestyle habits. The need for comprehensive approaches to prevention and management. Despite global research, there's a gap in understanding PUD in Jeddah, Saudi Arabia, necessitating this study to inform healthcare policies and interventions in the region. The study aims to investigate PUD prevalence, associated factors, healthcare-seeking behavior, and knowledge levels in Jeddah's population to provide insights for public health strategies and clinical practices. Methodology: This cross-sectional study investigated the prevalence of peptic ulcer disease (PUD) and its associated factors in Jeddah, Saudi Arabia, using a validated questionnaire covered demographic details, knowledge, psychosocial aspects, symptoms, healthcare-seeking behavior, and medical history. It was translated into Arabic, culturally adapted, and validated through expert review and pilot testing. A sample size of 561 participants was determined using an online sample calculator, targeting a 95 % confidence level. Ethical approval was obtained from the Institutional Research Review Board (IRRB-02-03,092,023). Data was collected through an online survey using convenience sampling and analyzed with SPSS and Excel, employing both descriptive and inferential statistical methods. Result: The survey in Jeddah, Saudi Arabia, examined peptic ulcer disease (PUD) prevalence and associated factors. Demographic analysis showed 561 participants, almost evenly split by gender (269 males, 292 females), with varying ages and educational backgrounds. Symptoms like abdominal pain were prevalent (398 respondents), as were lifestyle factors such as spicy food consumption (456 respondents) and stress (496 respondents). Background: Noncoding RNAs have recently proven their contributing role in the pathogenesis of numerous diseases. The etiology of psoriasis is still not well defined, and genetic and environmental factors are blamed. We meant to explore the association of circRNAs/microRNAs in psoriasis and inspect their correlation with the clinical features of the disease. Design and Methods: This study included 166 subjects: 83 cases with plaque psoriasis and 83 age- and sex-matched healthy persons as the control group. The expression levels of circRNA000102, circRNA0045272, circRNA0084764, and microRNAs -93,181, 16a, and 21 were estimated by quantitative real-time PCR. Results: CircRNA0045272 was ten-fold up-regulated, whereas circRNA0084764 and circRNA000102 were about two-fold downregulated in patients vs. control. MicroRNAs −93 and 181 were significantly downregulated in patients vs. control, whereas microRNA −16a and 21 were up-regulated in patients vs. control. CircRNA0045272 was positively correlated with microRNA 181, and circRNA0084764 was positively correlated with microRNA 93 and microRNA 181. CircRNA0045272 had the highest sensitivity (98.8%) and specificity (93.98%), and microRNA-16a had sensitivity (98.80%) with specificity (93.98%). Conclusion: We assume that circRNAs can predict psoriasis occurrence and progression by sponging a set of microRNAs and can modulate several genes that disturb immune cell balance and inflammatory elements in psoriasis. © 2024 Taylor & Francis Group, LLC.

Comparative Analysis and Integration of Host Defence Peptides with Conventional Therapies: A systematic review

Host defense peptides (HDPs) have emerged as promising candidates with multifaceted roles in enhancing host immunity against a variety of microbial threats. This review aims to examine the immunomodulatory potential and inducibility of HDPs, providing insights into their ability to enhance host immunity and address the challenge of multidrug-resistant bacteria. Data were extracted from a total of 31 publications. A total of nine (9) Host Defense Peptides (HDPs) were identified from various microorganisms, exhibiting diverse antimicrobial activities. Additionally, eight (8) HDPs were sourced from plants, while ten (10) experimental animal-derived HDPs were also documented. Data also showed that synergistic combinations of HDPs with antibiotics or antivirals enhanced efficacy, including a 50% reduction in viral load with Human Beta-Defensin 2 (hBD-2) and acyclovir and a 60% decrease in viral replication with Defensin 1 and ribavirin. The promising nature of these peptides as alternatives for combating infections is evident, holding significant potential to mitigate the emergence of antimicrobial resistance (AMR).

Innate immune memory (IIM) is the process by which, upon a primary challenge, innate immune cells alter their epigenetic, transcriptional, and immunometabolic profiles, resulting in modified secondary responses. Unlike infections or other immune-system-related diseases, the role of IIM in nonpathogenic contexts is less understood. An increasing body of research has shown that normal microbiota members or their metabolic byproducts induce alternative memory phenotypes, suggesting that memory cells contribute to homeostasis in mucosal areas. In this review, we discuss the newest insights in the emerging field of IIM to the microbiota and the potential of manipulating these long-term responses to promote better mucosal health. © 2024 The Author(s)

Identification of the methionine transporter MetQ in Streptococcus suis and its contribution to virulence and biofilm formation

Streptococcus suis is a Gram-positive bacterium responsible for various infections in both pigs and humans. This study investigates the role of methionine acquisition in the growth and virulence of S. suis. The putative methionine transport system is organised as an operon comprising the metQ gene and genes encoding a transposase and an ATPase, forming a typical tripartite ABC transporter. This operon is conserved across multiple streptococcal species, including both animal and human pathogens. We examined whether MetQ functions as a methionine-binding protein and its role in bacterial infection. Using Western blotting and flow cytometry with a specific antiserum, we demonstrated that MetQ is produced in vitro by the S. suis reference strain P1/7 under methionine-limited conditions and is located on the bacterial cell surface. Growth assays in chemically defined media revealed that a metQ deletion mutant (P1/7∆metQ) exhibited impaired growth under methionine-restricted conditions but grew normally in a nutrient-rich medium, suggesting that MetQ primarily transports methionine. Isothermal Titration Calorimetry demonstrated that MetQ binds L-methionine with a dissociation constant (KD) of 7.1 µM. In a murine infection model, the metQ mutant showed reduced dissemination to internal organs compared to the wild type. Furthermore, the mutant showed decreased intracellular survival in murine macrophages and increased sensitivity to oxidative stress, while exhibited enhanced biofilm formation compared to the wild type. Our findings indicate that MetQ is essential for methionine uptake under methionine-restricted conditions, which is critical for bacterial nutrition, immune evasion, and pathogenicity during infection.

Colonization of the intestinal lumen precedes invasive infection for a wide range of enteropathogenic and opportunistic pathogenic bacteria. We show that combining oral vaccination with engineered or selected niche-competitor strains permits pathogen exclusion and strain replacement in the mouse gut lumen. This approach can be applied either prophylactically to prevent invasion of nontyphoidal Salmonella strains, or therapeutically to displace an established Escherichia coli. Both intact adaptive immunity and metabolic niche competition are necessary for efficient vaccine-enhanced competition. Our findings imply that mucosal antibodies have evolved to work in the context of gut microbial ecology by influencing the outcome of competition. This has broad implications for the elimination of pathogenic and antibiotic-resistant bacterial reservoirs and for rational microbiota engineering.

Investigating the abundance, stability and activity of specialized metabolites in the roots vs leaves of a medicinal tree Oroxylum indicum

The present study examined the differential abundance and stability of bioactive flavonoids in various tissues of the medicinally important rare tree Oroxylum indicum. LC-HR-MS/MS analysis of the methanolic extracts of the old root, old leaves, and young leaves revealed distinct metabolite profiles, with genistein and isorhamnetin dominating in the old leaves, whereas hyperoside and baicalein-7-O-glucuronide were elevated in roots. Metabolite stability of young and old, fresh and stored roots, and leaves was also variable, with old root extracts displaying better retention of metabolite levels upon storage. Molecular docking of 59 specialized flavonoids with human inflammatory enzymes identified oroxin A as a significant anti-inflammatory metabolite that is abundant in old roots. The xanthine oxidase inhibition assay confirmed high anti-inflammatory activity in root methanolic extracts, with 81% inhibition, although storage of extracts reduced inhibitory activity, prominently in leaves. Cytotoxicity assays on MCF-7 cells demonstrated higher cell toxicity by root extracts than by leaf extracts, which decreased upon storage. We conclude that O. indicum tissues have diverse bioactive metabolites with strong anti-inflammatory and anti-cancer potential and are better preserved in old root extracts.

Over the past billion years, the fungal kingdom has diversified to more than two million species, with over 95% still undescribed. Beyond the well-known macroscopic mushrooms and microscopic yeast, fungi are heterotrophs that feed on almost any organic carbon, recycling nutrients through the decay of dead plants and animals and sequestering carbon into Earth’s ecosystems. Human-directed applications of fungi extend from leavened bread, alcoholic beverages and biofuels to pharmaceuticals, including antibiotics and psychoactive compounds. Conversely, fungal infections pose risks to ecosystems ranging from crops to wildlife to humans;these risks are driven, in part, by human and animal movement, and might be accelerating with climate change. Genomic surveys are expanding our knowledge of the true biodiversity of the fungal kingdom, and genome-editing tools make it possible to imagine harnessing these organisms to fuel the bioeconomy.
