Introduction: Immediate postnatal hypothermia is an independent risk factor for death in premature newborns. Three randomized controlled trials (RCTs) and five historical controlled trials show statistically significant differences in admission temperature between infants wrapped in occlusive skin wrap and unwrapped infants. This paper presents a study protocol for The Vermont Oxford network (VON) Heat Loss Prevention (HeLP) Trial, a multicentre RCT of two interventions (standard of care vs. occlusive wrap) that investigates the effect of polyethylene occlusive wrap applied immediately after birth on mortality in infants born 24 + 0/7 to 27 + 6/7 week gestation. Methods: Inclusion criteria include: infants 24 + 0/7 to 27 + 6/7 weeks gestational age and a firm decision prior to birth to provide full resuscitative measures. Exclusion criteria comprise infants born with blistering skin conditions or congenital anomalies that are not covered by skin. The primary outcome measure is all-cause mortality until discharge from the hospital or at six months corrected gestational age. The secondary outcome measures include baseline and post-stabilization axillary temperatures, acidosis, hypotension, hypoglycaemia, seizures in the first 12 h, patent ductus arteriosus, and respiratory distress syndrome. Long-term follow-up at 18 to 24 months corrected age will be assessed with the combined risk of death and major neurosensory disability as the primary outcome. Discussion: Key covariates and protocol deviations are addressed and steps to monitor these are described. Wrapping may prove an inexpensive and easy method to benefit premature newborns in level I and II nurseries, in both developed and developing countries, as well as large tertiary care centres. The purpose of this project is to design, develop, and validate a case-based e-learning module that promotes the development of second-year engineering students' epistemic beliefs as well as the development of their abilities to deal with real-world, ill-defined engineering problems. Thirty-one second-year engineering students vicariously experienced real-world problems through a case-based e-learning module that consists of four major phases: exploring situations, constructing reality, creating solutions, and reflecting on the product and the process. The changes of students' problem-solving skills as well as their epistemic growth were measured and analyzed. The results reveal that students' problem-solving performances were improved when they were guided through the four phases of learning activities in the module. However, their guided problem-solving abilities failed to transfer to solve another problem without guidance. With regard to their epistemic growth, no change was observed after their completion of the learning activities in the module. The implications of designing a case-based e-learning environment to develop students' problem-solving skills as well as to facilitate their epistemic growth are discussed.

Utilizing experience-based co-design to improve the experience of patients accessing emergency departments in New South Wales public hospitals: an evaluation study

Background This paper presents the findings of three multisite evaluations of Experience-Based Co-design (EBCD) programmes conducted in Emergency Departments (EDs) and associated departments in seven public hospitals in New South Wales, Australia. Method Data for the evaluations were derived from: EBCD documentation provided by the participating sites;interviews with 117 key informants;performance data and the policy and academic literature on EBCD. Results Respondents described EBCD as a successful and sustainable method of improving the individual patient experience and the overall quality of a health service. Demonstrated successes were reported to lead to aspects of the EBCD approach spreading within services. However, like any quality improvement activity, EBCD was not without its challenges. The principal challenge particular to the EBCD projects outlined here was their deployment in ED settings. Because of their ambulant patient populations, these settings made sustaining consumer engagement for the duration of the project problematic and required tailoring EBCD to accommodate consumers' involvement preferences and constraints. Conclusion The primary strength of EBCD over and above other service development methodologies was reported to be its ability to bring about improvements simultaneously in both the operational efficiency and the inter-personal dynamics of care. However, careful consideration must be given to the constraints inherent in transient patient specialties and what needs to be done to tailor EBCD to suit the particular setting in which it is deployed.

Performances of Different Global Positioning System Devices for Time-Location Tracking in Air Pollution Epidemiological Studies

Background: People's time-location patterns are important in air pollution exposure assessment because pollution levels may vary considerably by location. A growing number of studies are using global positioning systems (GPS) to track people's time-location patterns. Many portable GPS units that archive location are commercially available at a cost that makes their use feasible for epidemiological studies. Methods: We evaluated the performance of five portable GPS data loggers and two GPS cell phones by examining positional accuracy in typical locations (indoor, outdoor, in-vehicle) and factors that influence satellite reception (building material, building type), acquisition time (cold and warm start), battery life, and adequacy of memory for data storage. We examined stationary locations (eg, indoor, outdoor) and mobile environments (eg, walking, traveling by vehicle or bus) and compared GPS locations to highly-resolved US Geological Survey (USGS) and Digital Orthophoto Quarter Quadrangle (DOQQ) maps. Results: The battery life of our tested instruments ranged from < 9 hours to 48 hours. The acquisition of location time after startup ranged from a few seconds to > 20 minutes and varied significantly by building structure type and by cold or warm start. No GPS device was found to have consistently superior performance with regard to spatial accuracy and signal loss. At fixed outdoor locations, 65%-95% of GPS points fell within 20-m of the corresponding DOQQ locations for all the devices. At fixed indoor locations, 50%-80% of GPS points fell within 20-m of the corresponding DOQQ locations for all the devices except one. Most of the GPS devices performed well during commuting on a freeway, with > 80% of points within 10-m of the DOQQ route, but the performance was significantly impacted by surrounding structures on surface streets in highly urbanized areas. An approach for the numerical treatment of delamination in laminated composite components is presented. A first ply failure criterion is employed to predict delamination initiation, while delamination propagation is analyzed using linear elastic fracture mechanics. The combination of initiation and propagation criteria yields a conservative estimation of the load earring capacity of a structure. Furthermore, the growth stability, the sensitivity of the results with respect to a change in the interface properties. and the non-linear structural response caused by the delamination growth process are determined. Two structures are investigated, which show the capability of the proposed approach, a curved laminate and a double lap shear test specimen. (C) 2008 Elsevier Ltd. All rights reserved.

Background and purpose: Carbonic anhydrase (CA) IX is suggested to be an endogenous market of hypoxia. Fluorescent sulfonamides with a high affinity for CA IX (CAI) have been developed and shown to bind to cells only when CA IX protein was expressed and while cells were hypoxic. The aim of this study was to investigate the in vivo CAI binding properties in a xenograft tumour model using fluorescent imaging. Materials and methods: NMRI-nu mice subcutaneously transplanted with HT-29 colorectal tumours were treated with 7% oxygen or with nicotinamide and carbogen and were compared with control animals. CAI accumulation was monitored by non-invasive fluorescent imaging. Results: Specific CAI accumulation could be observed in delineated turnout areas as compared with a non-sulfonamide analogue (P < 0.01). Administration of nicotinamide and carbogen, decreasing acute and chronic hypoxia, respectively, prevented CAI accumulation (P < 0.05). When treated with 7% oxygen breathing, a 3-fold higher CAI accumulation (P < 0.01) was observed. Furthermore, the bound CAI fraction was rapidly reduced upon turnout reoxygenation (P < 0.01). Conclusions: Our in vivo imaging results confirm previous in vitro data demonstrating that CAI binding and retention require exposure to hypoxia. Fluorescent labelled sulfonamides provide a powerful tool to visualize hypoxia response. An important step is made towards clinical applicability, indicating the potential of patient selection for CA IX-directed therapies. (C) 2009 Elsevier Ireland Ltd. All rights reserved. A carbon fiber-based radiation sensor for dosimetric measurement in radiotherapy

A prototype sensor was designed and fabricated to explore the application of carbon fibers for absorbed dose measurements in the ionizing radiation field used for therapeutic radiology. The carbon fiber-based sensor has a 1.8 x 1.8 cm(2) sensing area. Both 6 and 15 MV X-ray beams produced by a Varian Clinac 21 EX medical linear accelerator were used to test the sensor. Radiation dosimetric measurements were performed by varying the dose rates, total dose, and field sizes to characterize various properties of the sensor. For the given dose range from 0 to 600 monitor units, the sensor displayed an excellent linear response. This sensor can be used for dosimetric measurements and real-time dose rate monitoring. Compared to existing MV X-ray measurement technologies in radiotherapy, this prototype sensor provides many advantages over commercially available dosimeters: (i) ease of operation;(ii) low noise at room temperature;(iii) light in mass;(iv) low voltage requirements;(v) potential for miniaturization;and (vi) real-time signal measurement. A higher spatial resolution can be achieved by miniaturizing the physical size of the current sensor design. (C) 2008 Elsevier Ltd. All rights reserved.
