Improving colorectal cancer screening in rural primary care: Preliminary effectiveness and implementation of a collaborative mailed fecal immunochemical test pilot

Background Mailed fecal immunochemical test (FIT) outreach can improve colorectal cancer (CRC) screening rates. We piloted a collaborative mailed FIT program with health plans and rural clinics to evaluate preliminary effectiveness and refine implementation strategies. Methods We conducted a single-arm study using a convergent, parallel mixed-methods design to evaluate the implementation of a collaborative mailed FIT program. Enrollees were identified using health plan claims and confirmed via clinic scrub. The intervention included a vendor-delivered automated phone call (auto-call) prompt, FIT mailing, and reminder auto-call;clinics were encouraged to make live reminder calls. Practice facilitation was the primary implementation strategy. At 12 months post mailing, we assessed the rates of: (1) mailed FIT return and (2) completion of any CRC screening. We took fieldnotes and conducted postintervention key informant interviews to assess implementation outcomes (eg, feasibility, acceptability, and adaptations). Results One hundred and sixty-nine Medicaid or Medicare enrollees were mailed a FIT. Over the 12-month intervention, 62 participants (37%) completed screening of which 21% completed the mailed FIT (most were returned within 3 months), and 15% screened by other methods (FITs distributed in-clinic, colonoscopy). Enrollee demographics and the reminder call may encourage mailed FIT completion. program feasibility and acceptability was high and supported by perceived positive benefit, alignment with existing workflows, adequate staffing, and practice facilitation. Conclusion Collaborative health plan-clinic mailed FIT programs are feasible and acceptable for implementation in rural clinics and support CRC screening completion. Studies that pragmatically test collaborative approaches to mailed FIT and patient navigation follow-up after abnormal FIT and support broad scale-up in rural settings are needed.

This paper introduces a target human following mobile robot system. Especially, the robot follows a smartphone user as the target. The proposed system includes global following based on Wi-Fi-based waypoint maps and local following based on walking state matching. This paper focuses on target human detection for local following based on cross-correlation coefficients of walking motion signals including vertical acceleration data measured by target user's smartphone and horizontal velocities of legs measured by an LRF installed in the robot. Also, this paper provides results of detection experiments. The results show that higher cross-correlation coefficients can be obtained for the target human than the other pedestrians. Next, a method for calculating the cross-correlation coefficients online is introduced, and the results of the experiment are shown. These results show that the following target can be identified in online calculation of cross-correlation coefficients. That contributes for development of actual human following system.

The purpose of this study is development of an autonomous mobile robot navigation system based on deep reinforcement learning with a monocular camera, without 2D-LiDAR. The proposed system is based on DDQN(Double Deep Q-network) as deep reinforcement learning. The system requires the input data as states of DDQN that include the range data around the robot. In this paper, the range data is estimated from a monocular camera instead of 2D-LiDAR. Monocular camera is relatively cheap compared to LiDAR, which can lower the hurdles for spreading robots in the world. The proposed system converts the depth images estimated from monocular camera to 2D range data that is input to the learned model based on 2D plane. The learning on 2D plane is effective to obtain stable models from deep reinforcement learning. Then, we conduct two experiments and evaluate the proposed system. The results show the autonomous navigation was achieved according to camera image-based states.

Innovative safety in the workplace is vital as the high safety risks associated with electrical engineering construction can lead to injuries or even fatalities. Using computer vision technology, we experimented with scenarios such as 'normal operations' and 'unexpected incidents' to enhance safety measures. We integrated an Internet of Things system into the setup, enabling the system to quickly detect and alert to unexpected events in real-time, thereby improving workplace safety. © 2024 IEEE.

With the advent of the post-quantum era, the most worrying issue is information security. Quantum computers have been able to crack common asymmetric cryptography, such as RSA, DSA, ECC, etc, by Shor’s Algorithm. In terms of common symmetric-key algorithm, Grover’s algorithm can reduce the difficulty of brute-force cracking from N attempt to $\sqrt{N}$ [1], which decreases the security. In other words, the symmetric-key algorithm doubling the key size can effectively resist quantum attacks. Hence, AES-256 will be a worthwhile research topic in the future.

Nontuberculous mycobacteria (NTM) are environmentally acquired opportunistic pathogens that cause chronic lung disease in susceptible individuals. While presumed to be ubiquitous in built and natural environments, NTM environmental studies are limited. While environmental sampling campaigns have been performed in geographic areas of high NTM disease burden, NTM species diversity is less defined among areas of lower disease burden like Colorado. In Colorado, metals such as molybdenum have been correlated with increased risk for NTM infection, yet environmental NTM species diversity has not yet been widely studied. Based on prior regression modeling, three areas of predicted high, moderate, and low NTM risk were identified for environmental sampling in Colorado. Ice, plumbing biofilms, and sink tap water samples were collected from publicly accessible freshwater sources. All samples were microbiologically cultured and NTM were identified using partial rpoB gene sequencing. From these samples, areas of moderate risk were more likely to be NTM positive. NTM recovery from ice was more common than recovery from plumbing biofilms or tap water. Overall, nine different NTM species were identified, including clinically important Mycobacterium chelonae. MinION technology was used to whole genome sequence and compare mutational differences between six M. chelonae genomes, representing three environmental isolates from this study and three other M. chelonae isolates from other sources. Drug resistance genes and prophages were common findings among environmentally derived M. chelonae, promoting the need for expanded environmental sampling campaigns to improve our current understanding of NTM species abundance while opening new avenues for improved targeted drug therapies.

Purpose: Environmental factors such as bathing may play a role in atopic dermatitis (AD) development. This analysis utilized data from the Community Assessment of Skin Care, Allergies, and Eczema (CASCADE) Trial (NCT03409367), a randomized controlled trial of emollient therapy for AD prevention in the general population, to estimate bathing frequency and associated factors within the first 9 weeks of life. Methods: Data were collected from 909 parent/newborn dyads recruited from 25 pediatric and family medicine clinics from the Meta-network Learning and Research Center (Meta-LARC) practice-based research network (PBRN) consortium in Oregon, North Carolina, Colorado, and Wisconsin for the CASCADE trial. Ordinal logistic regression was used to conduct a cross-sectional analysis of the association between bathing frequency (measured in baths per week) and demographic, medical, and lifestyle information about the infant, their family, and their household. Variables were selected using a backwards-stepwise method and estimates from the reduced model are reported in the text. Results: Moisturizer use (OR = 2.03, 95% CI: 1.54-2.68), Hispanic or Latino ethnicity (OR = 1.97, 95% CI: 1.42-2.72), a parental education level lower than a 4-year college degree (OR = 2.48, 95% CI: 1.70-3.62), living in North Carolina or Wisconsin (compared to Oregon;OR = 2.12 and 1.47, 95% CI: 1.53-2.93 and 1.04-2.08, respectively), and increasing child age (in days;OR = 1.02, 95% CI: 1.01-1.02) were significantly associated with more frequent bathing, while pet ownership (OR = 0.67, 95% CI: 0.52-0.87) was significantly associated with less frequent bathing. Conclusions: We found significant ethnic, geographic, and socioeconomic variation in bathing frequency before 9 weeks of age that may be of relevance to AD prevention studies.

Planning clean, livable, and resilient cities is challenging. This becomes even more complex when considering the rapid population growth and urban development. The development of nature-based solutions (NbS) is acknowledged as a viable strategy to address this issue. It has multiple human and environmental health benefits under both normal and extreme climatic scenarios. This study aimed to examine how different conditions and factors are linked to the development of NbS for urban climate change adaptation, with a specific focus on stormwater and flooding management. Through a systematic literature review, this study proposes a taxonomy of NbS to better understand the processes and hierarchies involved in developing NbS. The analysis covered multiple factors related to geographical focus and scale, sector, climatic conditions, adaptation and mitigation measures, ecosystem services, and co-benefits. These factors were linked to ten classified NbS approaches and 29 specific NbS interventions. An analysis of 582 empirical studies showed that Sustainable Urban Drainage Systems (SUDS) are the most dominant approaches of NbS for adaptation, followed by Low-Impact Development (LID), Green Infrastructure (GI), Sponge City (SC), and Blue-Green Infrastructure (BGI). Among the different interventions, green roofs play a significant role in providing multifunctional ecosystem services and adaptation benefits such as stormwater and climate regulation. Policymakers and developers aiming to expand the implementation of NbS can consider the taxonomy proposed in this study to choose the right types of NbS and obtain maximum co-benefits.

We reviewed the application of modern technology for rapid and accurate multi-person real-time pose detection in the hazardous field of electrical engineering. We focused on two leading pose detection technologies: YOLOv8 and OpenPose. To optimize performance, we integrated these two techniques into the LSTM model for training and investigated frame rates and accuracy. © 2024 IEEE.
