Current deep learning models are mostly task specific and lack a user-friendly interface to operate. We present Meta-EyeFM, a multi-function foundation model that integrates a large language model (LLM) with vision foundation models (VFMs) for ocular disease assessment. Meta-EyeFM leverages a routing mechanism to enable accurate task-specific analysis based on text queries. Using Low Rank Adaptation, we fine-tuned our VFMs to detect ocular and systemic diseases, differentiate ocular disease severity, and identify common ocular signs. The model achieved 100% accuracy in routing fundus images to appropriate VFMs, which achieved ≥82.2% accuracy in disease detection, ≥89% in severity differentiation, ≥76% in sign identification. Meta-EyeFM was 11% to 43% more accurate than Gemini-1.5-flash and ChatGPT-4o LMMs in detecting various eye diseases and comparable to an ophthalmologist. This system offers enhanced usability and diagnostic performance, making it a valuable decision support tool for primary eye care or an online LLM for fundus evaluation. © 2025, CC BY-NC-ND.

ABSTRACTABSTRACTPurpose: The Collaborative Ocular Tuberculosis Study (COTS) Group sought to address the diagnostic uncertainty through retrospective cohort analysis of treatment regimens and therapeutic outcomes for patients with tubercular Anterior Uveitis (TAU) across international ***: Multicentre retrospective analysis of patients diagnosed with TAU between January 2004 to December 2014 that had a minimum follow-up of 1 ***: One hundred and sixty-five patients were included. One hundred and seven subjects received antitubercular therapy (ATT) (n = 107/165; 64.9%) with all the patients receiving topical steroid therapy. Treatment failure was noted in 17 patients (n = 17/165; 10.3%), more frequently described in patients that received ATT (n = 13/107, 12.2%), than those that did not receive ATT (n = 4/58, 6.9%).Conclusion: In this retrospective study, addition of ATT did not have any statistically significant impact on outcome in patients with TAU.

Submacular haemorrhage(SMH)is a sight threatening complication that can occur in exudative age related macular degeneration(AMD),but has been described to occur more frequently in eyes with polypoidal choroidal vasculopathy(PCV).Left untreated,SMH carries a grave visual ***,expedient diagnosis and effective management of this complication is of paramount *** treatment strategies for SMH include(I)displacement of blood from the fovea,usually by injection of an expansile gas;(II)pharmacologic clot lysis such as with recombinant tissue plasminogen activator(rtPA);and(III)treatment of the underlying choroidal neovascularization(CNV)or PCV,such as with anti-vascular endothelial growth factor(anti-VEGF)*** three strategies have been employed in isolation or in combination,some concurrently and others in *** has demonstrable effect on the liquefaction of submacular clots but there are remaining uncertainties with regards to the dose,safety and the timing of initial and repeat *** side effects of rtPA include retinal pigment epithelial toxicity,increased risk of breakthrough vitreous haemorrhage and systemic *** cases presenting early,pneumatic displacement alone with anti-VEGF may be ***-VEGF monotherapy is a viable treatment option particularly in patients with thinner SMH and those who are unable to posture post pneumatic displacement.

Background: The application of deep learning to retinal photographs has yielded promising results in predicting age, sex, blood pressure, and haematological parameters. However, the broader applicability of retinal photograph-based deep learning for predicting other systemic biomarkers and the generalisability of this approach to various populations remains unexplored. Methods: With use of 236 257 retinal photographs from seven diverse Asian and European cohorts (two health screening centres in South Korea, the Beijing Eye Study, three cohorts in the Singapore Epidemiology of Eye Diseases study, and the UK Biobank), we evaluated the capacities of 47 deep-learning algorithms to predict 47 systemic biomarkers as outcome variables, including demographic factors (age and sex);body composition measurements;blood pressure;haematological parameters;lipid profiles;biochemical measures;biomarkers related to liver function, thyroid function, kidney function, and inflammation;and diabetes. The standard neural network architecture of VGG16 was adopted for model development. Findings: In addition to previously reported systemic biomarkers, we showed quantification of body composition indices (muscle mass, height, and bodyweight) and creatinine from retinal photographs. Body muscle mass could be predicted with an R² of 0·52 (95% CI 0·51–0·53) in the internal test set, and of 0·33 (0·30–0·35) in one external test set with muscle mass measurement available. The R² value for the prediction of height was 0·42 (0·40–0·43), of bodyweight was 0·36 (0·34–0·37), and of creatinine was 0·38 (0·37–0·40) in the internal test set. However, the performances were poorer in external test sets (with the lowest performance in the European cohort), with R² values ranging between 0·08 and 0·28 for height, 0·04 and 0·19 for bodyweight, and 0·01 and 0·26 for creatinine. OBJECTIVE:To assess the knowledge, attitudes, and practices (KAP) of medical stakeholders regarding the use of generative artificial intelligence (GAI) tools. METHODS:A cross-sectional survey was conducted among stakeholders in medicine. Participants included researchers, clinicians, and medical journal editors with varying degrees of familiarity with GAI tools. The survey questionnaire comprised 40 questions covering four main dimensions: basic information, knowledge, attitudes, and practices related to GAI tools. Descriptive analysis, Pearson's correlation, and multivariable regression were used to analyze the data. RESULTS:The overall awareness rate of GAI tools was 93.3%. Participants demonstrated moderate knowledge (mean score 17.71 ± 5.56), positive attitudes (mean score 73.32 ± 15.83), and reasonable practices (mean score 40.70 ± 12.86). Factors influencing knowledge included education level, geographic region, and attitudes (p < 0.05). Attitudes were influenced by work experience and knowledge (p < 0.05), while practices were driven by both knowledge and attitudes (p < 0.001). Participants from outside China scored higher in all dimensions compared to those from China (p < 0.001). Additionally, 74.0% of participants emphasized the importance of reporting GAI usage in research, and 73.9% advocated for naming the specific tool used. CONCLUSION:The findings highlight a growing awareness and generally positive attitude toward GAI tools among medical stakeholders, alongside the recognition of their ethical implications and the necessity for standardized reporting practices. Targeted training and the development of clear reporting guidelines are recommended to enhance the effective use of GAI tools in medical research and practice.
