Long-term intraocular pressure fluctuation of primary angle closure disease following laser peripheral iridotomy/iridoplasty

Background Many studies indicated that short-term and long-term intraocular pressure （IOP） fluctuations in primary open angle glaucoma patients might lead to glaucomatous progression. However, seldom study has evaluated the long-term fluctuation of IOP in primary chronic angle closure diseases. The objective of this study was to investigate the long-term IOP fluctuation of primary angle closure diseases and its associations following laser peripheral iridotomy （LPI） with or without laser peripheral *** A total of 158 patients with primary angle closure suspect （PACS, n=21）, primary angle closure （PAC, n=81） and primary angle closure glaucoma （PACG, n=55） had been treated by LPI with or without laser peripheral iridoplasty and followed up for more than 12 months. IOP was measured with Goldman applanation tonometer. Multivariate linear regression with generalized estimating equation （GEE） regression models was used to evaluate the association of long-term IOP fluctuation （maximum IOP minus minimum IOP） with gender, age, baseline IOP, baseline peripheral anterior synechia （PAS）,baseline vertical cup/disc ratio （VCDR）, baseline mean deviation （MD）, need for IOP-lowering *** IOP fluctuation during follow-up in PACS, PAC and PACG groups were （4.83±2.90）, （5.67±3.35）, and （9.40±7.14）mmHg, respectively. IOP fluctuation was strongly correlated with baseline IOP （r=0.356, P 〈0.001）, PAS （r=0.374, P 〈0.001）.IOP fluctuation was higher in patients with higher baseline IOP （0.18 mmHg per unit increase, 95% CI： 0.05-0.31 mmHg）.Conclusions Long-term IOP fluctuation in PACG group was larger than that in PACS or PAC group. Eyes with higher baseline IOP were observed to have larger long-term IOP fluctuation.

A new rat model of glaucoma induced by intracameral injection of silicone oil and electrocoagulation of limbal vessels

Background A satisfied glaucoma model is absent *** aim of this study was to evaluate the effect of a combination of intracameral injection of silicone oil and electrocoagulation of corneal limbal vessels and episcleral veins in the rats to establish glaucoma *** Operation was performed in each of the left eyes of 90 adult male *** eyes were used as *** of intraocular pressure （IOP） was performed with an applanation tonometer （Tong-Pen）.Retinal ganglion cells （RGCs） were retrogradely labeled by applying FluoroGold onto the bilateral superior *** During the follow-up （24 weeks）,the lOP of the study eyes was significantly higher （P〈0.05） than the control eyes （at final examination,IOP of control eyes was （13.4±1.0） mmHg and lOP of study eyes was （16.1±1.8） mmHg）.Correspondingly,at 24 weeks after operation,the RGCs density of the study eyes （2286.11±290.45/mm^2） was significantly lower than the control eyes （2626.46±164.85/mm^2,P〈0.01）.In the operated eyes,histological examination showed excavation of optic disc and increased neuroglial cells in the optic nerve,reduced thickness of retina and diminution of retinal ganglion cells,and atrophy of ciliary body and ***,the anterior chamber angle of the operated eye remained *** A combination of intracameral injection of silicone oil and electrocoagulation of comeal limbal vessels and episcleral veins may establish a reliable glaucoma model for further research.

Loratadine-associated cystoid macular edema: A case report

Purpose: To report the first known case of bilateral cystoid macular edema in a patient undergoing long-term loratadine treatment. Observations: A 49-year-old Chinese woman who had been undergoing treatment with loratadine for the past 6 years presented with decreased visual acuity and bilateral cystoid macular edema (CME). Upon cessation of loratadine, macular edema partially resolved, and visual acuity markedly improved. Fundus autofluorescence (FAF), optical coherence tomography (OCT), and fluorescence fundus angiography (FFA) were used to document the severity of CME and its subsequent resolution after cessation of loratadine therapy. Conclusions and Importance: Long-term use of loratadine might cause CME that partially resolves with discontinuation of the drug. The pathophysiology of drug-induced CME without leakage remains unclear. Dysfunction of histamine receptor1-expressed retinal neurons and the associated signal transduction, toxicity to Müller cells or RPE cells with subsequent intracellular fluid accumulation, and subclinical damage to the blood-retina barrier leading to leakage of extracellular fluid, have been proposed. © 2022 The Authors

Multi-scale Spatio-temporal Transformer-based Imbalanced Longitudinal Learning for Glaucoma Forecasting from Irregular Time Series Images

Glaucoma is one of the major eye diseases that leads to progressive optic nerve fiber damage and irreversible blindness, afflicting millions of individuals. Glaucoma forecast is a good solution to early screening and intervention of potential patients, which is helpful to prevent further deterioration of the disease. It leverages a series of historical fundus images of an eye and forecasts the likelihood of glaucoma occurrence in the future. However, the irregular sampling nature and the imbalanced class distribution are two challenges in the development of disease forecasting approaches. To this end, we introduce the Multi-scale Spatio-temporal Transformer Network (MST-former) based on the transformer architecture tailored for sequential image inputs, which can effectively learn representative semantic information from sequential images on both temporal and spatial dimensions. Specifically, we employ a multi-scale structure to extract features at various resolutions, which can largely exploit rich spatial information encoded in each image. Besides, we design a time distance matrix to scale time attention in a non-linear manner, which could effectively deal with the irregularly sampled data. Furthermore, we introduce a temperature-controlled Balanced Softmax Cross-entropy loss to address the class imbalance issue. Extensive experiments on the Sequential fundus Images for Glaucoma Forecast (SIGF) dataset demonstrate the superiority of the proposed MST-former method, achieving an AUC of 98.6% for glaucoma forecasting. Besides, our method shows excellent generalization capability on the Alzheimer’s Disease Neuroimaging Initiative (ADNI) MRI dataset, with an accuracy of 90.3% for mild cognitive impairment and Alzheimer’s disease prediction, outperforming the compared method by a large margin. Pathologic myopia (PM) is a common blinding retinal degeneration suffered by highly myopic population. Early screening of this condition can reduce the damage caused by the associated fundus lesions and therefore prevent vision loss. Automated diagnostic tools based on artificial intelligence methods can benefit this process by aiding clinicians to identify disease signs or to screen mass populations using color fundus photographs as inputs. This paper provides insights about PALM, our open fundus imaging dataset for pathological myopia recognition and anatomical structure annotation. Our databases comprises 1200 images with associated labels for the pathologic myopia category and manual annotations of the optic disc, the position of the fovea and delineations of lesions such as patchy retinal atrophy (including peripapillary atrophy) and retinal detachment. In addition, this paper elaborates on other details such as the labeling process used to construct the database, the quality and characteristics of the samples and provides other relevant usage notes. © 2023, CC BY-NC-ND.
