Symmetry of upper eyelid after unilateral blepharoptosis repair with minimally invasive conjoint fascial sheath suspension technique

AIM:To investigate the symmetry of upper eyelid in patients with unilateral mild and moderate blepharoptosis who underwent unilateral minimally invasive combined fascia sheath(CFS)***:A retrospective study of patients who underwent unilateral minimally invasive CFS suspension surgery between January 2018 and December *** criteria included unilateral mild and moderate ptosis,good levator muscle function(>9 mm)and follow-up of at least ***-and post-operative symmetry was graded subjectively for marginal reflex distance 1(MRD1),tarsal platform show(TPS)and eyebrow fat span(BFS).A t-test was used to evaluate MRD1,TPS and BFS asymmetry by calculating delta *** Bézier curve tool of the Image J software was used to extract the upper eyelid contours,where the symmetry was measured by the percentage of overlapping curvatures(POC).RESULTS:Totally 105 patients(105 eyelids)were included(mild group,n=84;moderate group,n=21).Postoperatively,all patients increased MRD1 and decreased TPS in the ptotic eye while maintaining unchanged *** asymmetric delta value for MRD1 was measured to be 1.48±0.86 preoperatively,and it decreased to 0.58±0.67 postoperatively in all cases(P=0.0004).In patients with mild ptosis,the asymmetry value of TPS fell significantly from 1.15±0.62 to 0.68±0.38(P=0.0187).The symmetry of the upper eyelid contour increased in all subgroups of patients,with a POC of 59.39%±13.45%preoperatively and POC of 78.29%±13.80%***:Minimally invasive CFS suspension is proved to be an effective means of improving the symmetry of unilateral ptosis in terms of MRD1(all subgroups),POC(all subgroups)and TPS(only mild group),whereas BFS is unaffected.

Predicting the Risk of Incident Cardiovascular Disease Using Retinal Vessel Caliber Measured by Artificial Intelligence in a Chinese Community-Based Population

Background: The predictive value of retinal vessel caliber, measured through artificial intelligence (AI), for incident cardiovascular disease (CVD) risk remains to be explored. Our study aimed to investigate the association between AI-measured retinal vessel caliber and the risk of incident CVD in a Chinese community-based population. Methods: We conducted a prospective cohort study on subjects from a Beijing community in China who were free of prevalent stroke or myocardial infarction and underwent retinal photography at baseline. By deep-learning algorithm, retinal vessel caliber measurements, including central retinal arteriolar equivalent (CRAE), central retinal venular equivalent (CRVE), and arteriolar-to-venular ratio (AVR), were obtained from 0.5 to 1 disc diameter from the optic disc margin. The primary endpoint was an incident major adverse cardiovascular event (MACE), defined as a composite of first non-fatal stroke, first non-fatal acute myocardial infarction (AMI), and cardiovascular death. The secondary endpoints included first stroke, first AMI, and cardiovascular death. Findings: A total of 3,585 participants were included. The mean age was 56.49 ± 8.94 years, and 2335 (65.1%) were female. Prevalence of prior hypertension and diabetes was 1594 (44.5%) and 676 (18.9%), respectively. The mean values for CRAE, CRVE, and AVR were 190.83 ± 29.92 μm, 278.26 ± 36.34 μm, and 0.69 ± 0.08, respectively. During the follow-up (median, 7.53 years), there were 293 cases of incident MACE (8.2%), 226 strokes (6.3%), 57 AMIs (1.6%), and 44 cardiovascular deaths (1.2%). The Kaplan–Meier curves showed significant differences in cumulative hazards of the primary endpoint among tertiles of CRAE, CRVE, and AVR. In Cox proportional-hazards models adjusting for covariates, CRAE was negatively associated with the risk of incident MACE (per 10 μm increase, hazard ratio [HR] = 0.95, 95% confidence interval [CI]: 0.92-0.99, P = 0.009) and the risk of incident stroke (per 10 μm

Ethnicity optimized indices enhance the diagnostic efficiency of early Keratoconus: A multicenter validation study

Purpose: This study aimed to evaluate the effectiveness of an ethnically-optimized corneal tomography and biomechanics index using artificial intelligence (AI) techniques for diagnosing early ectasia in Chinese populations and determine the diagnostic indices’ sensitivity, specificity, and cutoff value for clinical practice. Methods: This multicenter case-control retrospective study included 1,012 eyes from three centers in China (Tianjin, Shaanxi, and Shandong). The groups included very asymmetric ectasia with normal topography (VAE-NT, n = 146), contralateral ectasia (VAE-E, n = 127), bilateral keratoconus (KC, n = 247), and normal eyes (NL, n = 492). The diagnostic efficiency of the Corvis Biomechanical Index (CBI), CBI for Chinese populations (cCBI), Tomographic and Biomechanical Index version 1 (TBIv1), TBI version 2 (TBIv2), and TBI for Chinese populations (cTBI) was assessed using receiver operating characteristic (ROC) curves. The diagnostic efficiency was compared using DeLong's test. Results: cTBI had the highest diagnostic accuracy for distinguishing NL from early ectasia (VAE-NT), with an area under the ROC (AUROC) of 0.93 at the cutoff value of 0.41, a sensitivity of 84.39 % and specificity of 92.67 %. cTBI diagnostic efficacy in early ectasia was better than TBIv2, TBIv1, cCBI, and CBI (Delong test, P < 0.01). It also had the best efficacy in distinguishing NL from “disease” (VAE-NT + VAE-E + KC), with an AUROC of 0.98 at the cutoff value of 0.66, sensitivity of 91.05 %, and specificity of 99.39 %. cTBI achieved 100 % sensitivity and specificity for distinguishing NL from clinical ectasia (VAE-E + KC) at a cutoff of 0.81. Conclusions: The ethnicity optimized cTBI index had good diagnostic efficiency for early ectasia in the Chinese population, highlighting the benefit of ethnic-specific parameter optimization. © 2025 British Contact Lens Association

Surgery at early versus late for intermittent exotropia:a Meta-analysis and systematic review

AIM:To compare the outcomes between early surgery and late surgery for intermittent exotropia(IXT)with a ***:Scientific databases including PubMed,Embase,Web of science,Cochrane and China National Knowledge Infrastructure were searched prior to December 16,*** this broad database search,we performed some Meta-analysis including eleven independent studies,to further evaluate the outcome(s)when comparing early versus late surgery for *** boundaries between early and late surgery and the surgery methods were not inconsistent,so subgroup analyses were conducted by different boundaries of age at surgery and different surgical *** pooled odds ratios(ORs)and the 95%confidence interval(CI)were estimated according to the random-effects model for high heterogeneity between ***:Eleven retrospective studies were included in this *** significant difference between early and late surgery was observed for IXT patients(OR_(First follow-up)=0.88,95%CI 0.53-1.44,P=0.61;OR_(Final follow-up)=1.48,95%CI 0.94-2.31,P=0.09).However,sensitivity analysis performed by sequentially omitting individual studies showed that the final follow-up result was not *** analyses revealed that an earlier surgical procedure could lead to a better outcome in the 4-year boundary subgroup as well as the bilateral lateral rectus recession(BLR)subgroup for the final follow-up(OR_(4y)=2.64,95%CI 1.57-4.44,P=0.00;OR_(BLR)=2.25,95%CI 1.36-3.74,P=0.00).CONCLUSION:Early surgery for IXT provides a better long term follow-up outcome when patients are younger than 4 years old or choose the BLR surgical method.

Long-term results of clear lens extraction combined with piggyback intraocular lens implantation to correct high hyperopia

AIM:To assess the refractive outcome of clear lensectomy combined with piggyback intraocular lens implantation in highly hyperopic ***:This case review included 19 eyes of 10 patients with high hyperopia and axial length less than *** lens power was calculated for emmetropia using the Holladay II formula in 17 eyes,and SRK/T formula in 2 eyes following clear lens extraction and piggyback intraocular lens *** were examined periodically over 24 months for visual acuity and spherical equivalent(SE).RESULTS:The mean postoperative SE at 24 months was 0.20±1.39D(range,-3.00 to 2.50D),better than preoperative 9.81±2.62D(range,+6.00 to +14.50D)(P<0.001).Five eyes had SE within±0.5D of emmetropia and 11 eyes within±1.00D at postoperative 24 *** mean postoperative uncorrected visual acuity(UCVA) at 24 months was 0.60±0.36,significantly improved compared to preoperative 1.39±0.33(P<0.001).The mean best-corrected visual acuity(BCVA) at 24 months was 0.49±0.35,not statistically different compared to preoperative 0.38±0.30(P=0.34).Twelve eyes maintained and 1 gained 1 or more Snellen line of BCVA,4 eyes lost 1 line,and 2 eyes lost 2 lines at 24 postoperative *** eyes best-corrected near visual acuity (BCNVA) achieved J1 at postoperative 24 months compared to preoperative 7 eyes and the other 7 eyes better than ***:Clearlensextractioncombinedpiggyback intraocular lens implantation appears to be an effective procedure to correct high hyperopia but mild overcorrection and intralenticular opacification may require secondary procedure.
