BACKGROUND:Sleep disturbances are among the most common non-motor symptoms of Parkinson's disease (PD), greatly interfering with daily activities and diminishing life quality. Pharmacological treatments have not been satisfactory because of side effects and interactions with anti-parkinsonian drugs. While studies have shown that regular exercise improves sleep quality in normal aging, there is no definitive evidence in PD.
METHODS:In a retrospective study, we determined whether an intense physical and multidisciplinary exercise program improves sleep quality in a large group of patients with PD. We analyzed the scores of PD Sleep Scale (PDSS), which was administered twice, 28 days apart, to two groups of patients with PD of comparable age, gender, disease duration and pharmacological treatment. The control group (49 patients) did not receive rehabilitation, The treated group (89 patients) underwent a 28-day multidisciplinary intensive rehabilitation program (three one-hour daily sessions comprising cardiovascular warm-up, relaxation, muscle-stretching, balance and gait training, occupational therapy to improve daily living activities).
RESULTS:At enrolment, control and treated groups had similar UPDRS and PDSS scores. At re-test, 28 days later, UPDRS and total PDSS scores improved in the treated (p < 0.0001) but not in the control group. In particular, the treated group showed significant improvement in PDSS scores for sleep quality, motor symptoms and daytime somnolence. The control group did not show improvement for any item.
CONCLUSIONS:These results suggest that multidisciplinary intensive rehabilitation treatment may have a positive impact on many aspects of sleep in PD.

There is reasonable doubt that current arrangements for clinical training will sustain projected increased requirements for health education. It is evident that Simulation Education provides a solution to this sustainability problem, and the increasing sophistication of Virtual world technologies create opportunities to migrate elements from traditional to online models, thus addressing the issues of cost of access to traditional Simulation Centres. However, creating exciting online environments in isolation is insufficient in addressing the complex requirements of undergraduate health care training. The authors propose that by balancing the focus between cognitive reasoning structure and the need to establish an emotional connection, they have delivered a high quality, learner-centric online world that increases the "real world readiness" for undergraduates. Furthermore, by delivering the tool through a supportive online community of practice, the learner has a sustainable, continuous opportunity to practice essential skills in a safe environment before entering the clinical environment.

Landfill (TPA) Sukawinatan Palembang is an open dumping system which covers an area of 25 hectares. This system may bring an environmental damage to the surrounding area because it does not provide leachate treatment. Leachate is the landfill waste that dissolves many compounds that contain pollutants from both organic substances and heavy metal origin. This paper presents the results of laboratory analysis on samples of leachate as well as shallow groundwater from the surrounding area. The results were compared to established quality standards to evaluate whether the leachate has influenced the quality of the shallow groundwater in the surrounding area. The results show that there are some indications that the quality of groundwater has been polluted by the leachate of both organic substances and heavy metals produced by the Sukawinatan landfill.

Genome-wide association studies (GWAS) of lung cancer in Asian never-smoking women have previously identified six susceptibility loci associated with lung cancer risk. To further discover new susceptibility loci, we imputed data from four GWAS of Asian non-smoking female lung cancer (6877 cases and 6277 controls) using the 1000 Genomes Project (Phase 1 Release 3) data as the reference and genotyped additional samples (5878 cases and 7046 controls) for possible replication. In our meta-analysis, three new loci achieved genome-wide significance, marked by single nucleotide polymorphism (SNP) rs7741164 at 6p21.1 (per-allele odds ratio (OR) = 1.17; P = 5.8 × 10(-13)), rs72658409 at 9p21.3 (per-allele OR = 0.77; P = 1.41 × 10(-10)) and rs11610143 at 12q13.13 (per-allele OR = 0.89; P = 4.96 × 10(-9)). These findings identified new genetic susceptibility alleles for lung cancer in never-smoking women in Asia and merit follow-up to understand their biological underpinnings.

The fault detection and isolation problem is studied for a class of nonlinear systems. Under structural conditions, a bank of High-Order Sliding-Mode observers is proposed. The value of the equivalent output injection is used for detecting and isolating particular faults in the system. The proposed method provides finite-time isolation of actuator and plant faults. Simulation results support the proposed approach.
