Proposal of nursing diagnosis "adult disuse syndrome": A conceptual derivation and integrative review

PurposeTo develop a nursing diagnosis proposal derived from the concept of "disuse syndrome" based on an integrative review. MethodsThe conceptual derivation technique, proposed by Walker and Avant, was used in the following phases: an integrative review of the literature was carried out, applying the parameters of Whitermore and Knalf, where 43 articles were selected that covered the existence of the concept in the nursing discipline and other health sciences, the concepts were subsequently analyzed, and the new concept was redefined following the middle-range theory of causality and validation of nursing diagnoses proposed by Oliveira-Lopes et al. FindingsThe proposal of the new nursing diagnosis was developed, including the following elements: diagnostic label adult disuse syndrome, the corresponding definition, 25 defining characteristics, eight related factors, eight risk populations, and 14 associated problems. ConclusionsThe concept of the proposed new diagnosis describes adult disuse syndrome as a phenomenon that transcends the physiological component and should be viewed from a multidimensional view, expressing a clinical judgment related to a series of nursing diagnoses that appear together. Implications for nursing practiceThe results of this study will help nursing professionals to identify the phenomenon of adult disuse syndrome in any setting and in turn guide optimal care interventions that decrease the consequences of adult disuse syndrome.

"I Put a Wall Between Me and My Emotions, but That Wall Was Coming Towards Me": A Descriptive Qualitative Study on Determining Emotion Regulation Experiences of Oncology Nurses

Aims: The intense emotional nature of cancer care makes it crucial for oncology nurses to effectively regulate their emotions, as this impacts their mental health, quality of care, empathy challenges, burnout, and workforce retention. The aim of this study is to determine oncology nurses' experiences with emotion regulation during patient care and how these experiences affect their well-being and the quality of patient care. Methods: This descriptive qualitative study used a sample of 28 oncology nurses from various units, collected data through in-depth semistructured interviews, and analyzed it using content analysis. Results: Emotion regulation challenges encompass situations related to perspective and skills, patient-related conditions, and work-related issues. Emotional responses differ between the first and later years in the profession. Methods used include avoidance, alleviating the emotional burden, and cognitive restructuring, while needs involve gaining awareness and skills to regulate emotions and improving working conditions. Conclusions: This study provides insight into how oncology nurses regulate their emotions during patient care and discusses the positive and negative effects of the emotion regulation strategies used on their mental health. Nursing professionals who struggle with emotion regulation during their early years in the profession describe experiencing burnout and compassion fatigue in the later stages of their careers. Beginning nurses are inexperienced with challenging emotions. They should be trained to regulate their emotions. Implications for Practice: Oncology nurses struggle with regulating their emotions during patient care, which leads to mental health issues and burnout over time. Therefore, it is important to develop effective emotion regulation skills. CUL4B-associated epilepsy: Report of a novel truncating variant promoting drug-resistant seizures and systematic review of the literature.

Background: : Cabezas syndrome is a rare X-linked disease caused by mutations in CUL4B and characterized by developmental delay/intellectual disability, somatic dysmorphisms, behavioural disorder, ataxia/tremors. Although seizures have been formerly reported, their clinical semiology, EEG features and long-term outcome are largely unknown. Purpose: : This study aims to expand knowledge on epilepsy associated with Cabezas syndrome and to understand whether different types of variants in the CUL4B gene or brain MRI abnormalities may influence seizure onset and epilepsy course. Methods: : With this in mind, we characterised the epileptic phenotype of a 17-year-old adolescent harbouring a CUL4B novel variant and performed a systematic literature review of CUL4B-associated seizures, analysing mutation types and neuroimaging features as epilepsy predictors. Results: : Our case observation indicates that CUL4B-associated epilepsy may also be drug-resistant and persist beyond infancy. Literature analysis shows that 43% of CUL4B patients develop seizures, with no statistically significant differences in epilepsy development according to mutation type and neuroimaging features. Conclusion: : Our study extends knowledge of CUL4B-associated epilepsy, offering new insights into disease progression.

GPCR oligomerization across classes: A2AR-mediated regulation of mGlu5R activation

The adenosine A2A receptor (A2AR), a class A GPCR, is a known player in neurological diseases, including Parkinson's disease and Alzheimer's disease, and is also implicated in SARS-CoV-2 infection. Recent studies have revealed its oligomerization with metabotropic glutamate receptor type 5 (mGlu5R), a class C G protein coupled receptor (GPCR) that exists in the homodimeric form. Simultaneous activation of both receptors synergistically enhances mGlu5R-mediated effects in the hippocampus. Despite their importance, the molecular mechanisms governing these interactions remain unclear. In this study, we used molecular modelling techniques, including molecular docking, extensive molecular dynamics (MD) simulations, and detailed analysis, to elucidate the interactions between mGlu5R and A2AR in the inactive and active states. Our findings provide molecular-level insights into the permissive role of A2AR in mGlu5R activation, demonstrating that the inactive A2AR interface within the oligomer blocks the mGlu5R transmembrane helix 6 (TM6), which is crucial for activation. Upon A2AR activation, the oligomer interface undergoes conformational rearrangement, exposing mGlu5R-TM6 and allowing for mGlu5R activation. Furthermore, we identified a pivotal role of the mGlu5R-TM4:A2AR-TM4 interface in facilitating mGlu5R activation. These results highlight the intricate architecture of the mGlu5R:A2AR oligomer, advancing our understanding of GPCR oligomerization and its regulatory mechanisms on receptor activity.

Balancing Technology, Ethics, and Society: A Review of Artificial Intelligence in Embryo Selection

The introduction of artificial intelligence (AI) in embryo selection during in vitro fertilization presents distinct ethical and societal challenges compared to the general implementation of AI in healthcare. This narrative review examines ethical perspectives and potential societal implications of implementing AI-driven embryo selection. The literature reveals that some authors perceive AI as an extension of a technocratic paradigm that commodifies embryos, considering that any embryo selection methods undermine the dignity of human life. Others, instead, contend that prioritizing embryos with the highest viability is morally permissible while cautioning against discarding embryos based solely on unproven AI assessments. The reviewed literature identified further potential ethical concerns associated with this technique, including possible bias in the selection criteria, lack of transparency in black-box algorithms, risks of "machine paternalism" replacing human judgment, privacy issues with sensitive fertility data, equity of access, and challenges in maintaining human-centered care. These findings, along with the results of the only randomized controlled trial available, suggest that the introduction of AI-driven embryo selection in clinical practice is not currently scientifically and ethically justified. Implementing and deploying ethical and responsible AI in embryo selection would be feasible only if the ethical and societal concerns raised are adequately addressed.

Heterogeneous redox responses in NHDF cells primed to enhance mitochondrial bioenergetics

Aging and lifestyle-related diseases, such as cardiovascular diseases, diabetes, cancer, and neurodegenerative disorders, are major global health challenges. These conditions are often linked to redox imbalances, where cells fail to regulate reactive redox species (RRS), leading to oxidative stress and cellular damage. Although antioxidants are known to neutralize harmful RRS, their clinical efficacy remains inconsistent. One reason for this inconsistency is the inadequacy of current in vitro models to accurately mimic in vivo redox conditions. This study addresses the gap in understanding the heterogeneity of redox responses in cells by using metabolically primed human dermal fibroblasts (NHDF), a model relevant for precision mitochondrial medicine. We investigated how metabolic priming, which enhances mitochondrial bioenergetics, influences redox responses to oxidative stress induced by hydrogen peroxide (H2O2) and tert-butyl hydroperoxide (tBHP). Specifically, we explored the impact of cell population density and cell cycle distribution on redox dynamics. Our findings indicate that NHDF cells cultured in oxidative phosphorylation-promoting medium (OXm) exhibit significantly larger variability in oxidative stress responses. This variability suggests that enhanced mitochondrial bioenergetics necessitates a constant regulation of the cellular redox machinery, potentially leading to heterogeneous responses. Additionally, cells grown in OXm showed increased mitochondrial polarization and a lower percentage of cells in the G2/M phase, contributing to the observed heterogeneity. Key factors influencing this variability included cell population density at the time of oxidant exposure and fluctuations in cell cycle distribution. Our results highlight the necessity of employing multiple oxidants in metabolic priming models to achieve a comprehensive understanding of oxidative stress responses and redox regulation mechanisms. A numerical approach for a 1D tumor-angiogenesis simulations model

Angiogenesis, the formation of new blood vessels, is critical in both normal and pathological contexts, especially cancer. This process involves complex interactions among endothelial cells, tumor angiogenic factors, matrix metalloproteinases, angiogenic inhibitors, and neoplastic tissues. Different mathematical and computational models have been proposed for representing the tumor angiogenesis process. Among these, we focus on partial differential equations models which are able to capture the dynamic and spatial complexities in tumor growing. Our starting point is a PDE system which mimics the angiogenesis evolution. The aim of this work is to combine both spatial and time discretization methods for designing a matrix-based model. This approach allows us to observe some error properties of numerical schema proposed, by deducing the cumulative error among space and time. Experimental tests include convergence studies, for validating the reliability of the method. Results confirm our approach is useful for addressing real angiogenesis problem.

Effect of the anti-Alzheimer drug GSK-3β antagonist on numerical modeling of the energy dissipation through the resin-dentin interface

Objectives: The aim of this study was to determine the viscoelastic performance and energy dissipation of conditioned dentin infiltrated with polymeric nanoparticles (NPs) doped with tideglusib (TDg) (TDg-NPs). Methods: Dentin conditioned surfaces were infiltrated with NPs and TDg-NPs. Bonded interfaces were created, stored for 24 h and submitted to mechanical and thermal challenging. Resin-dentin interfaces were evaluated through nano-DMA/complex-loss-storage moduli-tan delta assessment and atomic force microscopy (AFM) analysis. Results: Dentin infiltrated with NPs and load cycled attained the highest complex modulus at hybrid layer and bottom of hybrid layer. Intertubular dentin treated with undoped NPs showed higher complex modulus than peritubular dentin, after load cycling, provoking energy concentration and breakdown at the interface. After infiltrating with TDg-NPs, complex modulus was similar between peri-intertubular dentin and energy dissipated homogeneously. Tan delta at intertubular dentin was higher than at peritubular dentin, after using TDg-NPs and load cycling. This generated the widest bandwidth of the collagen fibrils and bridge-like mineral structures that, as sight of energy dissipation, fastened active dentin remodeling. TDg-NPs inducted scarce mineralization after thermo-cycling, but these bridging processes limited breakdown zones at the interface. Significance: TDg-based NPs are then proposed for effective dentin remineralization and tubular seal, from a viscoelastic approach.

Brain computer interfaces (BCIs) enable users to interact with computers via the decoding of their neural activity. In this work, we seek to show the efficacy of "Inner Speech" as an additional communication paradigm. In BCIs, Electroencephalography (EEG) signals are the most regularly used due to their non-invasive nature of collection. However, a frequent problem plaguing EEG-based systems is the high noise-to-signal ratio which often results in poorly performing decoding models. This is further compounded by both intra- and inter-subject variations with their brain signal domain. In this work, we propose a novel Siamese variational autoencoder (VAE) network which allows for unsupervised representation learning to be performed on EEG data. We further implement a selective framework whereby a contrastive loss can be used to selectively reject training data which may not match the target subject's domain. Finally, by leveraging the lossy compression of the VAE network, the model may be used as a signal pre-processing step towards domain generalisation of the training data. Our results obtained classification accuracy significantly above previous benchmarks while reducing the amount of training time needed through selective learning.

SGLT1 contributes to glucose-mediated exacerbation of ischemia-reperfusion injury in ex vivo rat heart

Hyperglycaemia is common during acute coronary syndromes (ACS) irrespective of diabetic status and portends excess infarct size and mortality, but the mechanisms underlying this effect are poorly understood. We hypothesized that sodium/glucose linked transporter-1 (SGLT1) might contribute to the effect of high-glucose during ACS and examined this using an ex-vivo rodent heart model of ischaemia-reperfusion injury. Langendorff-perfused rat hearts were subjected to 35 min ischemia and 2 h reperfusion, with variable glucose and reciprocal mannitol given during reperfusion in the presence of pharmacological inhibitors of SGLT1. Myocardial SGLT1 expression was determined in rat by rtPCR, RNAscope and immunohistochemistry, as well as in human by single-cell transcriptomic analysis. High glucose in non-diabetic rat heart exacerbated reperfusion injury, significantly increasing infarct size from 45 +/- 3 to 65 +/- 4% at 11-22 mmol/L glucose, respectively (p < 0.01), an association absent in diabetic heart (32 +/- 1-37 +/- 5%, p = NS). Rat heart expressed SGLT1 RNA and protein in vascular endothelium and cardiomyocytes, with similar expression found in human myocardium by single-nucleus RNA-sequencing. Rat SGLT1 expression was significantly reduced in diabetic versus non-diabetic heart (0.608 +/- 0.08 compared with 1.116 +/- 0.13 probe/nuclei, p < 0.01). Pharmacological inhibitors phlorizin, canagliflozin or mizagliflozoin in non-diabetic heart revealed that blockade of SGLT1 but not SGLT2, abrogated glucose-mediated excess reperfusion injury. Elevated glucose is injurious to the rat heart during reperfusion, exacerbating myocardial infarction in non-diabetic heart, whereas the diabetic heart is resistant to raised glucose, a finding which may be explained by lower myocardial SGLT1 expression.
