Perceived importance of emotional intelligence for clinical pharmacy practice and suggested improvements: a focus group study of postgraduate pharmacists

BackgroundEmotional intelligence (EI) is a critical set of skills that impacts clinical pharmacists' well-being and positively influences high-level patient-centred care. Describing pharmacists' perceptions may support the integration of EI development approaches into their professional development *** aim of this study was to analyse pharmacists' perceptions of the characteristics of emotionally intelligent clinical pharmacists, the importance of EI in clinical practice, and educational models and approaches to enhancing *** qualitative study with a focus group methodology was conducted with pharmacy practitioners using a semi-structured guide grounded in the EI competency framework and existing qualitative research methodology practices. Purposive sampling was conducted until information and meaning saturation occurred. The focus group recordings were transcribed and independently coded by two researchers. The conventional content analysis of qualitative data was applied with the inductive thematic approach at its *** to the 17 focus group participants, emotionally intelligent clinical pharmacists are perceived as self-confident communicators who control and manage emotions, work well under pressure, and handle every situation effectively. Emotional self-control, self-awareness, awareness of others, tolerance, understanding, and empathy have emerged as key EI competencies required for challenges in clinical practice. EI lectures with reflections from clinical applications, behaviour modelling, and behaviour-changing methods were perceived to be of particular importance for pharmacist education and development *** pharmacy practitioners perceived EI competencies as necessary for their professional success and high-quality patient-centred care. They suggested that EI competencies be a focal point in pharmacy professional development programmes.

Music therapy during weaning from mechanical ventilation in the intensive care unit: Study protocol for a randomised control trial

IntroductionPeople admitted to the intensive care unit (ICU) who require mechanical ventilation (MV) must complete the weaning process as soon as possible to reduce associated risk factors. Previous research suggests that music therapy may be beneficial for improving breathing capacity and relaxation, but studies related to weaning are limited. This article presents a music therapy protocol for a randomised controlled trial (RCT).MethodICU patients aged 18-80 who meet the clinical criteria for initiating weaning are eligible for this RCT with two parallel groups. Participants (n = 100) are randomly assigned to a control group (receiving standard treatment during weaning) and an intervention group (receiving daily music therapy intervention combined with standard treatment). The outcome measures are (1) duration of weaning in hours, (2) dosage of sedatives and analgesia, (3) agitation, (4) delirium, (5) perception of pain, and (6) perceived stress at 90 days follow-up. During music therapy, (7) blood pressure, heart rate, respiratory rate and oxygenation level are only measured in the intervention group. The flexible music therapy protocol addresses patient needs from a humanistic biopsychosocial perspective. It includes an assessment of individual music preferences (adapted from the Musical Life Story Questionnaire) and methods such as entrainment, clinical instrumental improvisation, songwriting, and receptive music therapy to reinforce breathing capacity and promote relaxation, stimulation, communication, and emotional *** findings of this study may offer relevant implications for healthcare delivery and the application of music therapy during weaning in the ICU.

Examining the influence of tourism on beer demand in retail markets

Beer is a widely consumed beverage in Spain, particularly among the large numbers of tourists that visit our coastal regions. By taking into consideration the Spanish consumption statistics provided by the Spanish Ministry of Agriculture, Fisheries and Food, it is intended to measure the influence of tourism on beer sales, once having controlled the main factors of demand (prices, income, unemployment) and having avoided the multicollinearity between tourist frequentation and seasonality. It is concluded that tourism has a positive and significant impact on the relevant market of each region and month, as well as on the differences between the income of the local population and the one of tourists.

Combined generalized and focal epilepsy with reflex features in Adaptor protein complex 4-associated hereditary spastic paraplegias: A cohort observational study

Background: Patients with genetic deficiency of the adaptor protein complex 4 (AP-4) exhibit earlyonset developmental delay, spastic diplegia, intellectual disability, speech impairment. The phenotype overlaps with other hereditary spastic paraplegias and cerebral palsies. Febrile seizures are common at onset. Epilepsy has been described in more than half of cases, arising in early infancy often with status epilepticus, but no typical seizure semiology or electroencephalographic features have been identified thus far. Purpose: We aimed to specifically investigate the epileptological characteristics of the syndrome to unveil possible biomarkers of seizure development and prognosis in AP-4 deficiency. Methods: Observational cohort study on patients with bi-allelic pathogenic variants in AP-4 subunits and epilepsy. We focused on the seizure semiology, electroencephalographic characteristics and response to antiseizure medications. Results: Patients harboured pathogenic variants in AP4S1 (n = 5) or AP4M1 (n = 1). The phenotype included spastic paraparesis, intellectual disability, speech/language impairment, microcephaly, and MRI evidence of hypoplasia of the corpus callosum. In 66 % of the patients, febrile seizures preceded the onset of epilepsy, which spanned from infancy to adolescence (range=14 months-13 years). Absences (66 %) and focal motor seizures (50 %) were common. No patient met the criteria for drug-resistance. Peculiar electroencephalographic features arose after the epilepsy onset and persisted at long-term follow-up: bilateral and asynchronous focal discharges combined with independent diffuse spike-wave-discharges (100 %) and reflex abnormalities (66 %). Conclusion: In AP-4 complex disease, epilepsy could arise beyond early infancy, until adolescence, with variable combination of generalized and focal seizures. The prognosis was favourable. Integrating source tracking tools for marine shellfish tracing: Metataxonomic and isotopic analyses

Shellfish farming is a significant activity in the Northern Adriatic Sea, particularly in Italy, where it plays a crucial role in the European industry of live bivalve mollusks. This study aimed to exploit different analyses, including metataxonomy and isotopic profiling, to address these issues. Isotopic analysis showed differences between habitats, with lower delta 15N 15 N in sea samples, likely indicating lesser anthropogenic impact compared to lagoon environments. Additionally, seasonal variations in delta 15N, 15 N, delta 13C 13 C and C/N highlighted differences in primary productivity and organic matter sources across seasons. Metataxonomic analysis revealed significant variability in microbial taxa across harvesting habitats and seasons. While no marked differences were observed in alpha and beta diversity based on habitat, distinct seasonal clustering indicated the influence of environmental factors on microbial community structure. Gammaproteobacteria were prevalent across habitats and seasons, with notable fluctuations in other taxa, such as Alphaproteobacteria, Epsilonproteobacteria, Flavobacteria, and Bacteroidia, particularly in lagoon habitats during winter. Overall, this study highlights the potential of microbial and isotopic characterization for tracing the origin of shellfish products. Metataxonomic analysis identified microbial signatures indicative of habitat and season, with implications for ecological dynamics and consumer safety. Isotopic analysis provided information on habitat characteristics and harvesting seasons, enhancing product traceability.

The attrition rate of drugs in clinical trials is generally quite high, with estimates suggesting that approximately 90% of drugs fail to make it through the process. The identification of unexpected toxicity issues during preclinical stages is a significant factor contributing to this high rate of failure. These issues can have a major impact on the success of a drug and must be carefully considered throughout the development process. These late-stage rejections or withdrawals of drug candidates significantly increase the costs associated with drug development, particularly when toxicity is detected during clinical trials or after market release. Understanding drug-biological target interactions is essential for evaluating compound toxicity and safety, as well as predicting therapeutic effects and potential off-target effects that could lead to toxicity. This will enable scientists to predict and assess the safety profiles of drug candidates more accurately. Evaluation of toxicity and safety is a critical aspect of drug development, and biomolecules, particularly proteins, play vital roles in complex biological networks and often serve as targets for various chemicals. Therefore, a better understanding of these interactions is crucial for the advancement of drug development. The development of computational methods for evaluating protein-ligand interactions and predicting toxicity is emerging as a promising approach that adheres to the 3Rs principles (replace, reduce, and refine) and has garnered significant attention in recent years. In this review, we present a thorough examination of the latest breakthroughs in drug toxicity prediction, highlighting the significance of drug-target binding affinity in anticipating and mitigating possible adverse effects. In doing so, we aim to contribute to the development of more effective and secure drugs.

The increase in the available G protein-coupled receptor (GPCR) structures has been pivotal in helping to understand their activation process. However, the role of protonation-conformation coupling in GPCR activation still needs to be clarified. We studied the protonation behavior of the highly conserved Asp(2.50) residue in five different class A GPCRs (active and inactive conformations) using a linear response approximation (LRA) pK( a) calculation protocol. We observed consistent differences (1.3 pK units) for the macroscopic pK( a) values between the inactive and active states of the A2AR and B2AR receptors, indicating the protonation of Asp(2.50) during GPCR activation. This process seems to be specific and not conserved, as no differences were observed in the pK( a) values of the remaining receptors (CB1R, NT1R, and GHSR).

Nanostructured Lipid Carrier for Intracellular Delivery of a Bis(pyridine-2-carboxamidine) DNA Minor Groove Binder Active against Leishmania

Two types of nanostructured lipid carrier (NLC) formulations of the antileishmanial DNA minor groove binding compound 4-(picolinimidamido)-N-(4-(picolinimidamido)phenyl)benzamide (1) were synthesized by the microemulsion method to evaluate their potential as a delivery system to intracellular forms of the Leishmania parasite. Compound 1-loaded NLC formulations, functionalized with folic acid (FA) or not, showed good colloidal stability both during storage and after dilution in biologically relevant media. The drug release, which was not pH dependent, occurred efficiently at 37 degrees C. Compound 1 and its NLC formulations displayed a good selectivity index (SI > 40) and were active in the submicromolar range against promastigotes and metacyclic promastigotes of Leishmania major and in the low micromolar range (similar to 2 mu M) against intramacrophage amastigotes. However, they were inactive against Leishmania mexicana. This proof-of-concept study showed that compound 1 could be loaded effectively in NLC and FA-coated NLC whose size and anionic nature (i.e., FA-NLC) are favorable for uptake into macrophages. NLC functionalization with FA had a beneficial effect on the antileishmanial activity of 1 compared with noncoated NLC formulations, which resulted in an increase in selectivity toward the parasite.

Online simulation versus traditional classroom learnings in clinical pharmacy education: effect on students' knowledge, satisfaction and self-confidence

Background Over the course of the past few years, the area of medical education has experienced a substantial movement towards the establishment of online learning platforms and resources. This study aimed to to evaluate the efficacy of an online simulation learning intervention, MyDispense (R), compared to traditional classroom learning in terms of enhancing knowledge, satisfaction, and self-confidence among participants. Methods A multicentre randomized controlled study was conducted among pharmacy students who were assigned either intervention MyDispense (R) or control traditional classroom learning groups. They were eligible if they previously had experience with online simulation learning. A previously validated questioner were used to measure the outcome of knowledge, satisfaction and self-confidence. Results Both the intervention and control groups revealed significant improvement in knowledge, the P value for pre-post knowledge scores for each group was < 0,001. Despite these internal improvements, this study's findings showed no statistically significant differences (p > 0.05) between the intervention and control groups on knowledge gain, satisfaction, or self-confidence. This represents comparable outcomes irrespective of the group's exposure to intervention. Conclusion The study evaluated the efficacy of online simulation learning intervention MyDispense (R) in comparison to traditional classroom learning. While both strategies effectively improved knowledge, satisfaction, and self-confidence, the findings demonstrated that the online simulation yielded equivalent learning benefits. MyDispense (R) could be an alternative to traditional education in situations where face to face learning is not feasible, with comparable learning outcomes. Clinical trial number not applicable.

The domestication of smart artifacts has transformed our homes into hybrid environments of physical and digital worlds. It also has been changing our mindsets, behaviors, meaning attributions to, expectations from, frustrations about, and interactions with smart artifacts. By extension, the smartness definition is reconstructed by users who are the subject of smart artifact experiences. The current study is aimed at uncovering the user experience of smart artifacts with a focus on cognitive and emotional aspects to better understand what users expect when an artifact is identified as "smart." Therefore, an online research study is conducted to gain insight into the user experience of smart artifacts from content-rich reviews on e-commerce websites. Robot vacuum cleaners, smartwatches, and smart speakers were chosen as exemplary smart artifacts of the study. Because they offer different types of interaction with distinct aspects, our findings indicate that smartness is associated with trust in expertise, emotional engagement, exaggerated evaluation, and intriguing existence concepts about Emotional UX. In Cognitive UX, smartness relates to reducing mental workload, gratifying experience, perceived phenotype, reciprocal acquaintance, trust-building experience, tailored situatedness, shaping sociality, physical competency, and dual enhancement concepts. These findings demonstrate the potential of conceptualization in the early stages of smart artifact design processes.
