Introduction: Alzheimer's disease (AD) reports heterogeneity of neuropsychological symptoms misleading the differential diagnosis with other forms of dementia, such as dementia with Lewy bodies (DLB). About 50% of DLB patients are misdiagnosed as AD cases. Likewise, the diagnosis of both diseases is mainly based on clinical characteristics. However, differentiating AD of those with DLB based on neuropsychological symptoms and anatomical and functional brain changes remains challenging. Aim: To establish the main neuropsychological, anatomical, and functional similarities and differences in patients with AD and DLB. Methods: The present study followed the PRISMA guidelines and included studies from the PubMed, Scopus, and Web of Sciences databases, published between January 2000 and July 2022. Results: Forty-one articles were included in this systematic review for critical analysis. Our results suggest that the cognitive key domains to consider in the differential diagnosis are memory, executive function, attention, visuospatial/visuoconstructive skills, and verbal fluency (both semantic and phonological). The stage and severity of both diseases would be essential for differential diagnosis. On the other hand, the anatomical and functional changes suggest a similar atrophy pattern between AD and DLB in the frontal, parietal, temporal, hippocampal, and precuneus regions. Conclusion: The differential diagnosis between AD and DLB is challenging in clinical practice. Brain age estimation by structural MRI in superagers: a BrainAGE approach in elderly people with youthful episodic memory

Background Age-related episodic memory decline is not the only trajectory of healthy ageing. Superagers are the living proof that an 80yo can have the memory of a 50-60yo. We aim to estimate the biological brain age of superagers using the BrainAGE approach to understand the underlying ageing process. In the context of normal ageing, resistance refers to the avoidance of ageing effects and will translate into a deceleration of the ageing process, whereas resilience is the successful coping with age-related changes and will translate into a similar estimated brain age and chronological age. We tested these hypotheses in this study. Method A sample of 63 superagers (mean age 81.8 years ± 1.8 std, 60.3% women) and 55 age-matched controls (mean age 82.4 years ± 1.9 std, 63.6% women) from the Vallecas Project cohort in Madrid, Spain, were selected according to their episodic memory score in the Free and Cued Selective Reminding Test. Superagers scored at or above the mean value of a 50-56yo person, whereas controls scored within one standard deviation for their age. BrainAGE (“Brain Age Gap Estimation”) aggregates to a single value the complex multidimensional aging pattern within the whole brain by applying established kernel regression methods to anatomical brain magnetic resonance images (Franke and Gaser 2019). The BrainAGE score was calculated for both groups as the difference between chronological age and estimated brain age. Positive values indicate an acceleration of the ageing process and negative values indicate a deceleration of the ageing process. Result The BrainAGE score of superagers was -0.5 years, showing that their estimated brain age is lower than their chronological age whereas the estimated brain age of the control group is similar to their chronological age. The difference in BrainAGE between the superager and the control group is significant ( p = 1x10 -6 ). This result indicates a deceleration in the brain ageing process of superagers compared to

IntroductionInteracting with a continuously changing environment may necessitate cancelling a programmed movement, such as a step, after an environmental change to avoid errors and possible falls. This cancellation is successful if it occurs during the few milliseconds from the Anticipatory Postural Adjustments (APA) that precede the step onset, a period in which the center of mass (CoM) and the center of pressure (CoP) can still be aligned. Here we studied the inhibition of steps by the means of Stop Signal Task (SST), a behavioral task matched with a theoretical model developed to study the behavioral and physiological correlates of simple response inhibition [1], e.g., pressing a button. Aim of the present work was investigating if and how the inhibition of a complex multi-effectors movement as in step, accomplishes the assumptions of the model developed for simpler movements and if it is they are reflected in the time evolution of CoM and CoP. Methods12 healthy subjects (age: 33.3±7.1 y; weight: 69.3±17 kg; height: 169.7±8.2 cm) were required to initiate 300 walks from a stationary position (Go trials; GT) in response to the onset of an ahead pointing arrow (Go signal) on a pc monitor, and to interrupt the movement when in 30% of the trials (Stop trials; ST), a road stop signal (Stop signal) replaced the arrow after a variable time, determined using a behavioral tracking staircase algorithm [2]. A stereo-photogrammetric motion analysis system (SMART-DX 6000: BTS) and eight dynamometer platforms (Kistler 9286B; Kistler) were used to detect the kinematic and dynamic parameters of gait initiation. Ad-hoc analysis algorithms were developed by the MATLAB software (R2019b 9.7; MathWorks) to track the time evolution of CoM and CoP trajectories and capture the related reaction times (RT) for GT, error stop trials (eST), and correct stop trials (cST), and to conduct the statistical analysis. Background: Childhood trauma (CT) exposure is associated with a more pernicious course in bipolar disorder (BD). However, few studies have reported differences between BD I and BD II regarding CT exposure. We explore the differences in the CT trajectories in bipolar disorders. Methods: A retrospective cohort study of individuals with BD (BD I = 73 vs BD II = 73) was carried out. Early age at onset (EAO) and suicide ideation/behavior were used as severity outcomes. Timespan between EAO and treatment was documented and the associations between CT and comorbid alcohol used disorder (AUD), anxiety disorders (AD), and post-traumatic stress disorder (PTSD) were also described. Univariate, bivariate analyses, and a Poisson regression model with bootstrap resampling were used. Results: Higher scores of CT, physical abuse (PA), and sexual abuse (SA) were statistically significant for BD II than BD I (p < 0.001, p = 0.048, p < 0.001, respectively). Early age at onset, suicide ideation/behavior and treatment delay were associated with CT in both BD I and BD II. However, AUD and PTSD showed association with CT only for BD I. Limitations: Sample size, non-comparison control group, and recall bias. Conclusions: There are differences in CT subtype exposure between BD I and BD II with regards to early age onset, suicide ideation/behavior, delayed time to treatment, and comorbid mental disorders. These results claim for early access to strategies such as CT exposure screening in individuals with BD to detect possible pernicious course and improve the quality of life and clinical outcomes. © 2024

Carbonic anhydrase inhibitors: a novel pharmacological strategy to rescue Aβ-induced neurovascular pathology and gliosisin vivo

Background Cerebrovascular dysfunction (CVD) and neuroinflammation are associated and contribute to Alzheimer's disease (AD) progression. Most AD cases also present cerebral amyloid angiopathy (CAA), characterized by amyloid beta (Aβ) deposits at the brain vasculature, which trigger CVD and dysregulated brain clearance. In addition, CVD and altered clearance worsen vascular Aβ deposition, inducing cellular stress, neuroinflammation, metabolic waste accumulation, vascular permeability, infiltration of blood-borne cells, and eventually leading to neurodegenerative processes and cognitive impairment. Endothelial cells (ECs) and glial cells are major players in mediating vascular fitness and brain clearance, promoting the removal of toxic material from the brain, including Aβ. Our previous findings demonstrated that Aβ elicits mitochondrial dysregulation and caspase-mediated apoptosis in ECs and glial cells. Moreover, we showed that acetazolamide (ATZ) and methazolamide (MTZ), FDA-approved carbonic anhydrase inhibitors (CAIs), employed in non-AD related conditions, hinder these pathological events. Methods TgSwDI mice, expressing human Amyloid-β Precursor Protein, APP, carrying the Swedish, Dutch and Iowa mutations, were employed as AD/CAA model. Tg mice, which develop, starting at 6 months, fibrillar Aβ burden, primarily in the cerebral microvasculature, and gliosis, were fed a CAI-diet (from 8 to 16 months of age), following which we tested cognitive function, and harvested the brains for biochemical and immunohistochemical analyses. Results ATZ- and MTZ-diet improve behavioral performances, and decrease total brain Aβ burden and caspase-3 activation. Specifically, CAIs reduce Aβ accumulation in ECs, astrocytes and microglia, and the cell-specific Aβ-initiated caspase activity, promoting vascular health, glial anti-inflammatory/pro-healing phenotype and perivascular Aβ phagocytosis, which may underlie cerebral Aβ reduction.
