Introduction Myelodysplastic neoplasms (MDS) are a diverse group of clonal diseases affecting hematopoietic stem cells (HSCs). They are characterized by ineffective hematopoiesis, cytopenias, dysplasias, and the potential to progress to acute myeloid leukemia. Diagnosis of MDS involves assessing clinical, morphological, and genetic factors. Cytogenetic analysis plays a crucial role in identifying clonality in suspected MDS patients. Approximately 50-60% of patients exhibit cytogenetic abnormalities, with the most common anomalies being del(5q), monosomy or deletion of 7q, trisomy 8, and del(20q). Objective To show clinical manifestations of a MDS patient with high risk IPSS-R and trisomy 8, mutation of different prognosis. Case report 60-year-old female, reporting no toxic exposure, no elitism or smoking, monitored on the hematology clinic was diagnosed with MDS-IB1 (MDS with increased blasts 1) at the age of 59. The patient has pre-existing conditions including systemic arterial hypertension (SAH) and type 2 diabetes mellitus (DM II) which are treated with losartan, metformin, ferrous sulfate and folic acid. Presents with pancytopenia, including macrocytic anemia (hemoglobin of 6,9 g/dL and VCM of 127 fL), low white cells count (Neutrophils of 1769/mm 3 ), and thrombocytopenia (96,7 x 10 3 /mm 3 ). Bone marrow examination revealed mild hypocellularity as well as dyserythropoiesis, dysgranulopoiesis and 5% blasts. In March 2022, a karyotype examination showed, out of the 26 analyzed metaphases, trisomy 8 in 10 metaphases and trisomy 20 in 10 other metaphases. In February 2023, a follow-up karyotype scan revealed trisomy 8 in 11 out of the 21 metaphases analyzed. Introduction Sirtuins (SIRTs) are a group of proteins that play a crucial role in various cellular processes, including DNA repair, metabolism, and aging. They have been the subject of significant research interest, particularly in relation to cancer. SIRTs have been implicated in promoting cancer progression by inhibiting cell death pathways and facilitating cancer cell survival. Additionally, some studies have linked increased SIRTs family ( SIRT1 to SIRT7 ) gene expression activity to developing drug resistance in cancer cells, making treatment more challenging. Recently, in a systematic review, our group highlighted the scarcity of studies that establish the role of SIRTs genes in the pathobiology of Myelodysplastic neoplasm (MDS). Objective To assess the gene expression profile of SIRTs ( SIRT1, SIRT2, SIRT3, SIRT4, SIRT5, SIRT6 , and SIRT7 ) in relation to the pathogenesis and prognostic progression of MDS. Methodology This analysis included a cohort of 80 elderly patients diagnosed with MDS and bone marrow samples from 10 healthy individuals. MDS patients were diagnosed based on the World Health Organization (WHO) criteria and stratified according to the prognostic criteria established by the International Prognostic Scoring System-Revised (IPSS-R). We assessed the gene expression levels of SIRTs using the Real-Time PCR Gene Expression (RT-qPCR) method. Results We observed low expression of SIRT2 (p = 0.009), SIRT3 (p = 0.048), SIRT4 (p = 0.049), SIRT5 (p = 0.046), SIRT6 (p = 0.043), and SIRT7 (p = 0.047) in MDS patients compared to the control group. Additionally, we found increased expression of SIRT4 (p = 0.029) in patients aged 60 or above. Furthermore, we identified increased expression of SIRT2 (p = 0.016) and SIRT3 (p = 0.036) in patients with hemoglobin levels below 8 g/dL, increased SIRT4 (p = 0.036) in patients with dyserythropoiesis, decreased expression of SIRT2 (p = 0.035), SIRT4 (p = 0.035), and SIRT7 (p = 0.037) in patients with dysgranulopoies

Myelodysplastic neoplasms (MDS) are myeloid neoplasms, with bone marrow dysplasia, cytopenias, and increased risk of Acute Myeloid Leukemia (AML). Genotoxic agents and chronic inflammation can promote mutations, with clonal hematopoiesis and genetic instability. Chronic autoimmune and inflammatory disorders increase the risk of MDS. Several molecular and genetic mechanisms are involved in chronic inflammatory signaling of MDS, especially the activation of Toll-Like Receptors (TLRs), whose main intracellular adapter protein, MYD88, combines with IRAK kinases family (IRAK1, IRAK2, and IRAK4) in a complex called Myddosome, promoting TRAF6 phosphorylation and activation of NFKB and MAPK pathways and cytokine secretion. This study evaluated TRAF6, NFKB, and myddosome genes (MYD88, IRAK1, IRAK2, and IRAK4) expression in patients with MDS and AML Myelodysplasia Related (AML-MR), and its association with clinical variables and karyotype. Eighty-two patients, (76 with MDS and 6 with AML-MR) and 10 healthy controls were included. The cytogenetic study was carried out using G-Band and real-time PCR (RT-qPCR) methodology to analyze gene expression. IRAK4 and NFKB expression were higher in MDS with increased blasts (MDS-IB) compared to MDS with low blasts and ring sideroblasts (p < 0.001 and p = 0.026, respectively). MYD88 expression was lower in AML-MR patients compared to controls (p = 0.041), lower in patients with hemoglobin levels below 8 g/dL compared with hemoglobin levels above 10 g/dL (p = 0.036), lower in patients with bone marrow blast percentage > 10% compared to ≤ 10% (p = 0.027) and lower in patients with complex karyotype compared to normal karyotype (p = 0.005). IRAK1 expression was lower in patients with bone marrow blast percentage > 10% compared to ≤ 10% (p < 0.001) and lower in patients with abnormal karyotype compared to normal karyotype (p = 0.031). Diet, sedentary lifestyle, and overall lifestyle are among the primary risk factors for the onset of cancer and have been a challenge for scientific research for decades. Adverse events in oncological treatments are related to their nonspecificity to tumor cells and the cytotoxic effects on normal cells. Therefore, searching for therapies that promote greater efficacy and reduce treatment side effects is continuous. In this search, the Fasting Mimicking Diet (FMD) protocol has gained prominence as it presents a low-calorie, low-protein, and moderate to high unsaturated fat composition. This study evaluated the applicability, feasibility, and safety profile of the nutritional intervention, FMD diet combined with Azacitidine in treating patients with Myelodysplastic neoplasms (MDS). We conducted non-randomized experimental clinical research. The dietary intervention was assessed using anthropometric data, toxicity, and laboratory tests in patients diagnosed with MDS. Twenty-one patients were selected at the first cycle, with an average age of 68 (± 14) years. The FMD diet was used for 12 patients. Nine cases were excluded due to death or inability to maintain the diet protocol. The average weight of the patients was 61.24 kg, and the average BMI was 23.83. In the second treatment cycle, the average weight and BMI were 58.91 kg and 22.71, respectively. Both the 2.33 kg weight loss and the reduction in average BMI were significant (p = 0.020 and p = 0.040, respectively). In the third cycle, the average weight was 57.28 kg, and the BMI was 22.57, with another significant weight reduction (p = 0.013). During the application of the FMD, hematological markers (hemoglobin, mean red blood cell volume, leukocytes, segmented neutrophils, and platelets) were evaluated before and after chemotherapy, showing no change in the hematological profile of patients after chemotherapy with the FMD diet (p > 0.05 for all values). The aim of this study was verify the biological efficacy of the use of a xenograft for bone loss therapy. Blood clot, particulate autogenous bone or anorganic bovine xenograft filled critical size defects (CSD) in rat calvaria (8mm diameter). After 0, 7, 30 and 90 days the animals were killed and macroscopic, radiographic and histopathological analysis were conducted. Although no treatment promoted the total closure of bone defect, autogenous bone group had better bone repair after 90 days, followed by xenograft group that exhibited direct bone neoformation onto, and around, the particles confirming its osteoconductivity. In conclusion, the xenograft tested in vivo showed biocompatibility, biodegradability and osteoconductive properties in rat calvaria CSD.
