背景:中风作为一种疾病,中医早在2 000多年前即有诸多经典著作对其进行描述。然而,大部分有关针刺对中风后遗症患者功能恢复的疗效的临床对照试验未能得出针刺具有长期疗效的结论。并且,很多试验以表面肌电图测量患者骨骼肌的电活动以评定针刺刺激的即时疗效的结论也不一致。这些研究结果对针刺在神经肌肉水平的作用提出了质疑。本研究旨在比较徒手针刺影响健康人和中风后肌张力亢进患者的肱二头肌功能及力量的即时效果。方法与设计:本研究拟开展含有4个平行对照组的随机单盲临床试验。分别针刺健康受试者及中风后遗症痉挛性轻瘫患者的曲池(LI11)及天泉(PC2)2个穴位。表面肌电图测量针刺对于肌肉功能的即时疗效及肱二头肌的等长张力作为主要结局指标。次要结局指标包括每组中医证候的出现频率及频率分布。讨论:本研究的临床试验设计在方法学上较之前的类似试验有些改善。预计这一研究能够证实针刺刺激对健康人及中风后遗症患者的神经肌肉的作用。临床试验注册:巴西临床试验注册(***,注册号为RBR-5g7xqh).

The blind sportsmen have recorded significant improvements in their performances since their first participation in the Paralympics Games in Toronto 1976. In athletics, mainly running, they have to be guided by other athletes who have to follow them closely, especially in a competitive run, where the distance between them cannot exceed 50 cm, that is, between the visually impaired and their guide athlete. Although many guides are provided with an excellent preparation, the gradual growth of the blind athletes' performance will lead us to a new problem, which will have to be solved. Who will lead the blind athletes without limiting them? Thinking about offering another possibility that allows the visually impaired athlete to move freely in sports activities, this study aimed to develop the Remote Guide. The Remote Guide is an equipment which uses the principles of model airplane, which is coupled to a headset and put in contact with the surface skin, guides the individual through tactile and auditory signals to the directions left, right and also allow them to stop. For checking the coefficient of validity and objectivity of the Remote Guide, there was a correlation of Person. For this test was used the 30 meters dash. Six men, disabled athletes in the Paralympics level, visually impaired, accompanied by their guide athletes have performed this distance. Concerning the results, there werent statistically significant differences between the averages results from the Remote Guide and the data collected by disabled athletes and their guides. The validity of the device has showed a correlation of r 0.9361. By this way, the Remote Guide has an excellent reliability coefficient, which can be used for giving support to unleash the disabled athletes' performance. © 2011 IEEE.

It was with great excitement that we planned the combined meetings of Northeast Aquaculture Conference and Exposition (NACE) and the International Conference on Shellfish Restoration (ICSR) with the 33rd Milford Aquaculture Seminar (MAS). Bringing these different but complementary audiences together resulted in a meeting that combined many different aspects of fisheries restoration and aquaculture under one *** hundred attendees including government representatives, research scientists, industry, and academia at both the university and vocational high school levels attended this event. The meeting commenced on Wednesday December 12th 2012, with over eighty people attending seven field trips to area aquaculture farms and research facilities. The formal program began on Thursday, December 13th with a plenary session including invited speakers Eric Schwabb, Acting Assistant Secretary for Conservation and Management at NOAA; Sebastian Belle, Executive Director of the Maine Aquaculture Association and Boze Hancock from the Nature Conservancy, who discussed the role of aquaculture in fisheries restoration. John Bullard, the Northeast Regional Administrator of NOAA Fisheries Service, addressed the group during the luncheon the following day. The technical papers and workshops presented were divided into 35 sessions over three days of the meeting and included topics such as the history of aquaculture, aquaculture hatchery innovations, siting and planning issues, risk management, aquaculture business management, ocean acidification, climate change, as well as aquaculture disease issues and potential remedies. Having persons present who have experienced problems and successes, along with those in manufacturing and government responsible for addressing concerns and sharing best practices, was invaluable. Discussions among this cross-section of persons who represent different aspects of aquaculture were as important as the well-presented and interesting formal papers

Epley's maneuver in benign paroxysmal positional vertigo associated with Meniere's disease

The effects of Epley's maneuver in benign paroxysmal positional vertigo (BPPV) associated with Menière's disease are controversial. Aims: To evaluate the progression of positional vertigo and nystagmus after one or more of Epley's maneuvers in BPPV associated with Menière's disease, and the recurrence of BPPV. Method: a retrospective study of 62 patients with BPPV associated with Menière's disease, that underwent Epley's maneuver, and that were monitored during 12 months after elimination of positional nystagmus. Results: One Epley's maneuver was required to eliminate positional nystagmus in 80.7% of the patients, two in 16.1%, and three in 3.2%;after elimination of nystagmus, positional vertigo was suppressed in 71.0% of the patients, improved in 27.4% and remained unaltered in 1.6%. Four weeks after elimination of positional nystagmus, all patients were asymptomatic. Recurrence of BPPV was seen in 19.4% of the cases, with elimination of the positional vertigo and nystagmus by means of the specific maneuver for the involved canal. Conclusions: In BPPV associated with Menière's disease, vertigo and positioning nystagmus were eliminated with one, two or three Epley maneuvers. BPPV recurrence was resolved by using a specific maneuver for the affected canal. © Revista Brasileira de Otorrinolaringologia. All Rights reserved.

Identifying dead regions in the cochlea through the TEN Test

An audiogram is not sufficient to indicate cochlear dead regions. Aim: To investigate cochlear dead regions in sensorineural hearing loss subjects using the TEN test. Site: CEDALVI/ HRAC-USP-Bauru/Sao Paulo/Brazil, August 2003 to February 2004. Study Design: A contemporary cross-sectional cohort study. Material and Methods: The TEN test was applied in three groups: G1(5 women with pure-tone thresholds within normal limits);G2(4 women and 5 men with moderate sensorineural flat hearing loss);G3(19 women and 24 men with mild to severe sloping sensorineural hearing loss). Results: In the G1 group the TEN value required to eliminate the test tone was, on average, close to the absolute threshold for all frequencies. No dead regions were found in the ears tested in group G2. 76 ears were tested in group G3, and six showed no evidence of dead regions in the cochlea. Conclusions:The TEN test was an effective test to indicate a dead region in the cochlea of subjects with sloping sensorineural hearing loss. There is evidence that pure-tone detection is different for subjects with high frequency sensorineural hearing loss and flat hearing loss;we observed a significant difference between the masked threshold and the absolute threshold only in sloping hearing loss and not for flat hearing loss. Ms. Sibley is currently working as a clinical instructor and practicing nurse-midwifery at the University of Colorado School of Nursing Graduate Program Nurse-Midwifery Tract. Robert O. Ruhling received his Ph.D. in Exercise Physiology from Michigan State University in 1970. He spent two years on NIH Post-Doctoral Research Traineeship (Cardiovascular Physiology) at the Institute of Environmental Stress University of California Santa Barbara. In 1972 he assumed the Directorship of the Human Performance Research Laboratory in the College of Health at the University of Utah. Currently he is Professor of Physical Education and active in both teaching and research. Joyce Cameron-Foster received her B.S. in Nursing from the University of California Los Angeles California in 1958. She received her M.S. in Nursing and Nurse-Midwifery from Yale University in 1961 and her Ph.D. in Health Science from the University of Utah in 1981. Dr. Foster is currently working as an Associate Professor in the Graduate Nurse-Midwifery Program University of Utah. Carol Christensen received her Ph.D. in Physical Education from the University of Utah in 1980. Prior to coming to Utah she earned her M.A. at the University of California Santa Barbara in Ergonomics in 1976. Currently Dr. Christensen is Assistant Professor in the Department of Human Performance San Jose State University. Timothy Bolen received his B.S. degree in Education from Central Michigan University in 1977. He worked for two years with the Central Michigan University Adult Fitness Program as an Exercise Leader. He is completing requirements for his M.S. in Physical Education at the University of Utah. Currently Mr. Bolen is on the staff of the Cardiac Rehabilitation Institute at Borgess Medical Center Kalamazoo Michigan.

The influence of aerobic swin conditioning on levels of maternal physical fitness and the influence of each swim bout on selected maternal and fetal circulatory parameters were investigated. Thirteen women who met criteria for a low-risk homogeneous sample were randomly assigned to either an experimental (n = 7) or a control (n = 6) group. Experimental subjects participated in ten weeks of a swim conditioning program, while control subjects maintained their normal activities (to exclude aerobic conditioning). Fitness levels for all subjects were determined from respiratory gas analysis during graded exercise testing on a treadmill prior to and following the swim conditioning program. Testing indicated that experimental subjects were able to maintain their initial fitness levels over the twelve weeks of increasing gestation, while control subjects were not able to do so. These findings suggest the effectiveness of the swim conditioning program in countering a potential decrease in fitness levels. Importantly, maternal blood pressure, pulse, and fetal heart rate response remained within clinically normal limits during swim conditioning for experimental subjects, and during treadmill testing for all subjects.
