Epidemiological Characterization of Patients with Intestinal Stomas

Objective To characterize the sociodemographic and clinical variables of people with intestinal stomas. Materials and Methods We conducted a cross-sectional study with 47 patients of a Specialized Rehabilitation Center (CER II/APAE) in the municipality of Três Lagoas, state of Mato Grosso do Sul, midwestern Brazil, from December 2019 to June 2020. Data was analyzed using inferential descriptive statistics (Anderson-Darling, Chi-squared, and Mann-Whitney normality tests). Results Regarding the patients, 87.23% were from Três Lagoas, 51.06% were female, 40.43% were aged from 60 to 69 years, 59.57% were married, 53.19% were brown, 59.57% were catholic, 36.17% finished elementary school, 46.81% were retired, and 57.45% earned a monthly income below 1 minimum wage. Moreover, 61.70% had undergone terminal colostomy (61.70%), 61.70% had received guidance about its placing, 57.45% had it placed due to situations of urgency, 74.47% had a stoma installed due to a neoplasia, 38.30% were permanent, with 46.81% located in the inferior left quadrant (ILQ), 59.57% presented pasty effluent, 63.83% had a circular diameter, 53.19% had pouches with 2 pieces and 57.45%, with a flexible base, 87.23% had other adjunct equipment, and 95.74% had been trained in self-care. The most common complication was skin/peristomal irritant contact dermatitis (59,57%), and 65,95% of these cases were solved by teaching self-care. The type of stoma was significantly associated with the consistency of the effluent and the size of the protrusion (p > 0.05). Conclusion The results found can support strategies to implement practices to promote health, develop new public policies, to provide training in self-care, and prevent and treat complications. © 2023 Elsevier Editora Ltda. Educational Intervention on Cleaning and Sanitation of Surfaces in a Pediatric Hospitalization Unit

Introduction: Environmental surfaces may serve as a reservoir for various microorganisms and consequently, they represent a potential risk for the spread of healthcare-associated infections. Objective: This study aimed to assess the cleaning and sanitation of surfaces (CSS) before and after implementing a Standardization program for Cleaning and Sanitation of Surfaces (SPCSS). Methods: An analytical, comparative, and intervention study was conducted from 2020 to 2021 in a pediatric hospitalization unit in midwest Brazil. Four frequently touched surfaces were monitored before and after the cleaning and sanitation process using the following methods: Adenosine Triphosphate (ATP) quantification, Colony-Forming Unit (CFU) count, and visual inspection. The study consisted of three stages: stage I (situational diagnosis of the CSS process), stage II (implementation of the SPCSS), and stage III (assessment 60 days after implementing the program). A total of 576 assessments were performed in all three study stages. Results: The CSS process was effective in all three study stages by using the ATP and CFU methods. In stage I, statistically significant results were obtained for four surfaces using the ATP method, and two by the CFU count. In stages II and III, all surfaces presented lower ATP and CFU results (p<0.05). In the visual inspection, only the bathroom door handle (stage I: p=0.041;stage III: p=0.007) and toilet flush handle (stage I: p=0.026;stage III: p=0.007) passed the test. Implications for Nursing: This study presents subsidies to evaluate the cleaning and disinfection process carried out by the nursing and hygiene team. Conclusion: The SPCSS exerted a positive impact on the CSS process. © 2023 The Author(s).

ANALYSIS OF DYNAMIC BALANCE AND ANKLE MOBILITY IN FUNCTIONAL FITNESS PRACTITIONERS, ACCORDING TO GENDER

BackgroundFunctional Fitness Training (FF), known as Crosstraining, CrossFit®, or high-intensity functional training, comprises a physical conditioning modality characterized by a wide range of movements, including running, rowing, basic and olympic weight lifting, in addition to gymnastic movements. Because it is a relatively new modality, little is known about the specific functional attributes of FF practitioners. ObjectivesTo analyze dynamic balance and ankle mobility of recreational FF practitioners according to gender. MethodsThis is a cross-sectional study and data collection was carried out at a university clinic in Campo Grande/MS. The sample was constituted in a non-probabilistic way, for convenience. Recreational FF practitioners of both genders, aged between 18 and 59 years old, and who had been training under professional supervision for at least one month were included. For dynamic balance analysis, the Lower Quarter Y-Balance Test (YBT-LQ) was used in the anterior, posteromedial, and posterolateral reach directions. The averages of three attempts were recorded for each direction, for each lower limb. After normalizing each reach distance by the length of the lower limb, the composite score and the average between the right and left composite scores were obtained. To assess ankle mobility, the Weight-bearing lunge test was performed, recording the average angle of three attempts for each lower limb. To compare results according to sex, the Student's t-test or Mann-Whitney test was used, taking into account the distribution of data. Conclusions were taken at a significance level of 5%. ResultsThe sample consisted of 75 participants, most female (69.3%; n=52), aged between 18 and 55 years (mean 32.3±7.0), and average time of experience in the modality of 39.8±25.6 months. The review highlights the significance of traditional knowledge in using edible fruits to address health issues, specifically focusing on inflammatory bowel disease (IBD). Edible fruits are rich in beneficial components like dietary fiber, vitamins, minerals, polyphenols, bioactive peptides, and antioxidants which contribute to overall health and show potential in managing gastrointestinal disorders. The objective of the review is to provide strong evidence supporting the effectiveness of edible fruits in preventing and treating IBD. The study conducted an extensive review of research published from 2001 to 2022, gathering data from reputable sources and evaluating the utilization of edible fruits in IBD treatment. This review identified 36 species of edible fruits across 10 families historically used in traditional medicine. Various bioactive molecules like tannins, flavonoids, saponins, terpenoids, alkaloids, and ascorbic acid have been isolated from these fruits. Recent studies have shown that compounds from edible fruits can counteract intestinal inflammation in animal models by influencing key signaling pathways and exhibiting immunomodulatory, anti-inflammatory, and antioxidant properties. The review suggests that extracts from edible fruits could offer potential alternative or supplementary treatments for individuals with IBD. However, the review also acknowledges limitations in some prior studies, including inadequate dosages, insufficient standards, and an incomplete understanding of mechanisms. Further research is needed to fully elucidate the mechanisms underlying the effects of these extracts and compounds. © 2023 The Author(s)

Bredemeyera floribunda Wild (Polygalaceae), also known as pau-de-ovo is a medicinal plant used to treat snakebites, as tonic and fortifiers to treat fatigue and exhaustion. The aim of the present study was to document the knowledge of herbal sellers in the preparation of the so-called “B. floribunda fortifier” and to compare the plant root content with the recommended nutritional values and the permitted limit for medicinal plants. The study revealed that, unlike other regions of Brazil, in the midwest region, the root of the plant is used as food to treat fatigue and exhaustion. The contents in B. floribunda root decreased in the following order: Ca > K > P > Na > Fe > Mg > Si > Mn > Zn > Cu. Daily intake of 30 grams of B. floribunda roots are considered as an excellent source of Cr, Cu, Mn and Se for people 19 to 50 years of age according to the RDA. In addition, Cu and Fe showed content above the established limit for medicinal plants stipulated by other countries. The root of the plant has low K, Mg, P, Na and Zn content. When the contents in roots of the plant are below the oral dose of reference, consumption of this plant can be considered safe and without risk to human health. On the other hand, concentration of elements such as Cu, Fe, Ni, Al, Cr and Se exceeding the allowable limit may cause serious health problems due to prolonged ingestion and large amounts consumed daily. The Brazilian legislation does not establish a permissible limit of macro- and micro-elements in medicinal plants;thus, the results presented in this study will contribute to the Brazilian public health policy on medicinal plants.

Background: Soccer related injuries are often reported in studies, but epidemiological research on this theme is rare in Brazil, Furthermore, the conditions in which athletes have returned to sports practice, namely, either symptomatic or asymptomatic, have been neglected in research. Objectives: The aim of this study was to describe the epidemiological features of injuries among professional Brazilian soccer players in relation to location, type, mechanism, severity, recurrence, treatment and, lastly, symptoms in return to sport. Study design: Descriptive and cross-sectional, observational study. Study center: School of Physical Therapy of the University of Mato Grosso do Sul, Brazil. Methods: 116 male professional athletes of teams from a Brazilian state championship were interviewed and information about injuries was recorded using a retrospective reported morbidity questionnaire. Data were analyzed in mean ± SD for physical characteristics and sports practice history in absolute and relative frequencies (chi-square test with Bonferroni's correction) for characterization of soccer injuries in terms of type, location, severity, recurrence and symptoms in return to sport. Results: The numbers of injuries per athlete and per injured athlete were 0.92 and 1.43 respectively. The injuries of muscle-tendon unit and the joint types localized on lower limbs constituted the most important clinical occurrences with significant difference both in relation to other types (p < 0.05). Moderate and severe injuries were the most frequent occurrences. In relation to mechanisms for each type of injury, body contact was at least three times more responsible for injury cases. This type of mechanism was associated with a significantly greater impairment of joint structures. Introduction. Some studies have reported the occurrence of microorganisms isolated from water. Considering these microorganisms, fungi are known to occur ubiquitously in the environment, including water, and some are pathogenic and may cause health problems, especially in immunocompromised individuals. The aim of this study was to identify fungi in hospital water samples and to correlate their presence with the concentration of free residual chlorine. Methods. Water samples (100 mL) were collected from taps (n = 74) and water purifiers (n = 14) in different locations in a university hospital. Samples were filtered through a nitrocellulose membrane and placed on Sabouraud dextrose agar and incubated for 24 hours at 30°C. Fungi were identified according to established methods based on macroscopic and microscopic characteristics (filamentous) and physiological tests (yeasts). Free chlorine residual content was measured at the time of sample collection. Results. Seventy species of fungi were identified in the water samples and about 56% of the water samples contained culturable fungi. Cladosporium oxysporum, Penicillium spinulosum, and Aspergillus fumigatus were the most common filamentous fungi. Aureobasidium pullulans and Candida parapsilosis were the most common yeasts. Chemical analyses revealed that free residual chlorine was present in 81.8% of the samples within recommended concentrations. Among samples from water purifiers, 92.9% showed low levels of free residual chlorine (<0.2 mg/L). There was no significant association between chlorine concentrations (either within or outside the recommended range) and the presence of filamentous fungi and yeasts. Conclusions. This study showed that hospital water can be a reservoir for fungi, some of which are potentially harmful to immunocompromised patients. Background: The causes for immune-mediated inflammatory diseases (IMIDs) are diverse and the incidence trends of IMIDs from specific causes are rarely studied. Methods: We collected detailed information on six major causes of IMIDs, including asthma, inflammatory bowel disease, multiple sclerosis, rheumatoid arthritis, psoriasis, and atopic dermatitis, between 1990 and 2019, derived from the Global Burden of Disease study in 2019. The average annual percent change (AAPC) in number of incidents and age standardized incidence rate (ASR) on IMIDs, by sex, age, region, and causes, were calculated to quantify the temporal trends. Findings: In 2019, rheumatoid arthritis, atopic dermatitis, asthma, multiple sclerosis, psoriasis, inflammatory bowel disease accounted 1.59%, 36.17%, 54.71%, 0.09%, 6.84%, 0.60% of overall new IMIDs cases, respectively. The ASR of IMIDs showed substantial regional and global variation with the highest in High SDI region, High-income North America, and United States of America. Throughout human lifespan, the age distribution of incident cases from six IMIDs was quite different. Globally, incident cases of IMIDs increased with an AAPC of 0.68 and the ASR decreased with an AAPC of −0.34 from 1990 to 2019. The incident cases increased across six IMIDs, the ASR of rheumatoid arthritis increased (0.21, 95% CI 0.18, 0.25), while the ASR of asthma (AAPC = −0.41), inflammatory bowel disease (AAPC = −0.72), multiple sclerosis (AAPC = −0.26), psoriasis (AAPC = −0.77), and atopic dermatitis (AAPC = −0.15) decreased. The ASR of overall and six individual IMID increased with SDI at regional and global level. Countries with higher ASR in 1990 experienced a more rapid decrease in ASR. Interpretation: The incidence patterns of IMIDs varied considerably across the world. Displacement of the non-reduction disk with limitation of opening (DDwLO) can be treatment of the non-surgical form. However, it required scientific and clinic knowledge of diagnostic area of the disorders temporomandibulares. This study was performed with the patient CN, 18 years, white, female, who went to the orofacial pain ambulatory with complaints of extreme pain on the right temporomandibular joint (TMJ) and limitation of maximum opening of 29 mm with shift to right, which she has been suffering for at least three years. After a criterion anamnese the diagnosis hypothesis found was a displacement of the non-reduction disk with limitation of opening. The treatment was based on infiltration in the right TMJ with anesthesic followed by mandibular manipulation. After the therapeutic conduct the patient presented clinical signals of normality with absence of trismus and mandibular movement without shift to right. The patient was followed up during five years not presenting return of the pathology.
