AbstractIntroductionThe lipid accumulation product (LAP) and visceral adipose index (VAI) are clinical markers of visceral obesity and were proposed as simple tools to estimate cardiovascular risk and mortality. The objective of this study was to analyze the accuracy of the VAI and LAP for high cardiovascular risk patients. MethodsA cross-sectional observational study of accuracy was carried out in 193 patients of both sexes. In addition to the variables VAI and LAP, presence of comorbidities, education, level of physical activity and anthropometric data were obtained. Cardiovascular risk was determined by the Framingham score. ResultsNo significant difference was observed in the sample in gender distribution (44.6% women; 55.4% men), 24.4% had low cardiovascular risk, 48.7% intermediate risk and 26.9% high cardiovascular risk. Linear regression analysis showed that VAI and LAP explain, respectively, only 2.4% and 5.2% of the variation in cardiovascular risk expressed by the Framingham score. The analysis of areas under the curve (AUC) for receiver operating characteristic (ROC) indicated a significant effect only of LAP to diagnose individuals with high cardiovascular risk, but with low sensitivity and specificity. ConclusionOur results indicate that VAI and LAP explain only a small percentage of the variation in the Framingham cardiovascular risk score. LAP index still deserves more attention in a cohort study, because, even with the limitations of a cross-sectional study, we observed an acceptable sensitivity for it so that the LAP can be used as a screening criterion for requesting more accurate tests.

Long coronavirus disease (COVID) is a multisystem condition that affects a significant proportion of individuals following severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, with persistent symptoms ranging from fatigue and cognitive dysfunction to cardiovascular disorders. It is estimated that 30-60% of infected individuals experience symptoms lasting more than 12 weeks. Despite advances in understanding acute infection, the pathophysiological mechanisms underlying long COVID remain unclear. Current hypotheses suggest that viral persistence, immune dysfunction, and metabolic alterations play central roles. Omics approaches, including metabolomics, proteomics, and lipidomics, have played a crucial role in investigating molecular changes, identifying biomarkers, and refining therapeutic strategies. This review discusses recent advances in understanding long COVID, addressing its mechanisms, risk factors, the impact of viral variants, and the role of vaccination, with an emphasis on the importance of omics technologies in elucidating this condition.

The Lipid Accumulation Product (LAP) is a clinical marker of visceral obesity and has been proposed as a simple, inexpensive, and accurate tool to estimate cardiovascular risk and mortality. The aim of this study was to verify the association of LAP with anthropometric, biochemical, visceral adiposity index and IR in adults and the elderly. This single cross-section center clinical study, with experimental, analytical, primary, and observational design, included 210 participants. Anthropometric (Body Mass Index (BMI), Waist Circumference (WC), and Neck Circumference (NC)), LAP, Visceral Adipose Index (VAI), and biochemical parameters (fasting glycemia, insulinemia (to calculate the Homa-IR index), total cholesterol, LDL-c, HDL-c, and triglycerides) were evaluated. The results showed that by separating the sample into three groups (adequate BMI and WC, adequate BMI and elevated WC, and elevated BMI and WC), the group with high BMI and WC showed a high value of LAP and VAI compared to the other groups, with a significant difference. Still, the data show a positive and significant correlation when relating the LAP with VAI, HOMA-IR, BMI, WC, NC, total cholesterol, triglycerides, and Diastolic Blood Pressure. It also showed an inversely proportional relationship when associating LAP with HDL-c (p < 0.0001). Thus, we show that LAP is closely related to visceral adiposity, IR, altered lipid parameters, and blood pressure, especially diastolic in the patients included in our study. For these reasons, we suggest that LAP is a reliable indicator of promising visceral adiposity for early detection of cardiovascular risk in the adult and senior population.

In March 2020, the World Health Organization declared a pandemic state in response to the number of infected and killed by COVID-19. Gestational diabetes (GD) is characterized by physiological changes making pregnant women even more vulnerable to viral respiratory infections, such as SARS-CoV-2. For these reasons and due to the restriction of data related to DMG and the involvement by COVID-19, the importance of raising information about these two conditions is evident. Therefore, this study aimed to perform a literature review about the complications of COVID-19 in patients with GD. MEDLINE-PubMed, EMBASE, and Scielo databases were searched and followed the PRISMA guidelines. Only four studies were included in the systematic review. The complications and mortality are more significant in pregnant women with diabetes since hyperglycemia promotes immune changes in the inflammatory state and worsens insulin resistance. The imbalance in the immune response favors the development of infections, the most likely being viral respiratory infections. On the other hand, other studies show insufficient recent evidence to indicate that pregnant women are more susceptible to infection with severe acute coronavirus respiratory syndrome. There are divergences between studies concerning complications related to COVID-19 in patients with GD. In view of this scenario and from the information obtained, it is noted that the literature is scarce regarding the worsening of COVID-19 in patients with GD. Therefore, it is necessary to continue research on these mechanisms that may justify their correlation.

BACKGROUND:Cardiovascular diseases (CVDs) are the leading causes of death worldwide. The main risk factors are hypertension, diabetes, obesity, and increased serum lipids. The peanut (Arachis hypogaea L.), also known as the groundnut, goober, pindar, or monkey nut, belongs to the Fabaceae family and is the fourth most cultivated oilseed in the world. The seeds and skin of peanuts possess a rich phytochemical profile composed of antioxidants, such as phenolic acids, stilbenes, flavonoids, and phytosterols. Peanut consumption can provide numerous health benefits, such as anti-obesity, antidiabetic, antihypertensive, and hypolipidemic effects. Accordingly, peanuts have the potential to treat CVD and counteract its risk factors.
PURPOSE:This study aims to critically evaluate the effects of peanuts on metabolic syndrome (MetS) and CVD risk factors based on clinical studies.
METHOD:This review includes studies indexed in MEDLINE-PubMed, COCHRANE, and EMBASE, and the Preferred Reporting Items for a Systematic Review and Meta-Analysis guidelines were adhered to.
RESULTS:Nineteen studies were included and indicated that the consumption of raw peanuts or differing forms of processed foods containing peanut products and phytochemicals could improve metabolic parameters, such as glycemia, insulinemia, glycated hemoglobin, lipids, body mass index, waist circumference, atherogenic indices, and endothelial function.
CONCLUSION:We propose that this legume and its products be used as a sustainable and low-cost alternative for the prevention and treatment of MetS and CVD. However, further research with larger sample sizes, longer intervention durations, and more diverse populations is needed to understand the full benefit of peanut consumption in MetS and CVD.

Background: The causes for immune-mediated inflammatory diseases (IMIDs) are diverse and the incidence trends of IMIDs from specific causes are rarely studied. The study aims to investigate the pattern and trend of IMIDs from 1990 to 2019. Methods: We collected detailed information on six major causes of IMIDs, including asthma, inflammatory bowel disease, multiple sclerosis, rheumatoid arthritis, psoriasis, and atopic dermatitis, between 1990 and 2019, derived from the Global Burden of Disease study in 2019. The average annual percent change (AAPC) in number of incidents and age standardized incidence rate (ASR) on IMIDs, by sex, age, region, and causes, were calculated to quantify the temporal trends. Findings: In 2019, rheumatoid arthritis, atopic dermatitis, asthma, multiple sclerosis, psoriasis, inflammatory bowel disease accounted 1.59%, 36.17%, 54.71%, 0.09%, 6.84%, 0.60% of overall new IMIDs cases, respectively. The ASR of IMIDs showed substantial regional and global variation with the highest in High SDI region, High-income North America, and United States of America. Throughout human lifespan, the age distribution of incident cases from six IMIDs was quite different. Globally, incident cases of IMIDs increased with an AAPC of 0.68 and the ASR decreased with an AAPC of −0.34 from 1990 to 2019. The incident cases increased across six IMIDs, the ASR of rheumatoid arthritis increased (0.21, 95% CI 0.18, 0.25), while the ASR of asthma (AAPC = −0.41), inflammatory bowel disease (AAPC = −0.72), multiple sclerosis (AAPC = −0.26), psoriasis (AAPC = −0.77), and atopic dermatitis (AAPC = −0.15) decreased. The ASR of overall and six individual IMID increased with SDI at regional and global level. Countries with higher ASR in 1990 experienced a more rapid decrease in ASR. Interpretation: The incidence patterns of IMIDs varied considerably across the world.
