Background Improving antimicrobial use is a key component of controlling antimicrobial resistance. Multiple factors influence inpatient provider antimicrobial prescribing decisions, making it challenging to develop standard methods to evaluate prescribing quality in hospitals. In 2015, CDC’s Emerging Infections program (EIP) conducted a hospital antimicrobial use prevalence survey and collected data to assess prescribing quality in selected scenarios, including fluoroquinolone (FQ) treatment. Methods EIP sites (CA, CO, CT, GA, MD, MN, NM, NY, OR, TN) each recruited up to 25 hospitals for the survey. Each hospital selected a survey date during May–September 2015. Among randomly selected inpatients on the survey date, EIP staff identified those ≥18 years old who received FQ treatment on the survey date or the day prior and reviewed medical records to gather data on underlying conditions, infections, and diagnostic tests. We used these data to update a previously developed prescribing quality assessment pathway that categorized FQ treatment as supported or unsupported based on medical record documentation. Results Among 12,299 patients in 199 hospitals, 1084 (8.7%) received FQ treatment; 756 (70%) were treated for a single infection type during their hospitalization and were ≥18 years old. The pathway categorized FQ treatment as supported for 646 (85.4%) and unsupported for 110 patients (14.6%) (figure). Almost half of unsupported treatment was due to a lack of compatible signs or symptoms of infection in a patient from whom an organism susceptible or likely susceptible to an FQ was identified from a nonsterile site (49/110 patients, 44.5%), suggesting colonization. Conclusion Utilization of a pathway that incorporates detailed clinical data enabled us to apply a standard approach to assess FQ prescribing quality in hospitals. Background Klebsiella pneumoniae carbapenemase (KPC) and Verona integron-encoded metallo-β-lactamase (VIM) have been the most commonly identified carbapenemases among carbapenemase-producing carbapenem-resistant Enterobacteriaceae (CP-CRE) in Kentucky since 2013. Understanding the frequency and epidemiology of these CP-CRE can help inform prevention strategies. Methods We reviewed reports of KPC- and VIM-producing CRE from January 2013 through December 2017. CRE became reportable in Kentucky in February 2015 and statewide request to laboratories and healthcare facilities for isolate submission for mechanism testing was made in September 2017. Prior to that time, mechanism testing for CRE was conducted at a limited number of laboratories or during outbreak investigations. Demographic data included age, sex, and inpatient or outpatient status. Descriptive analyses were performed. Results As of December 31, 2017, a total of 156 CP-CRE isolates had been identified (124 KPC, 31 VIM, 1 NDM), with an increase from 2013 ( n = 13) to 2017 ( n = 48). KPC was identified in isolates from 124 patients; VIM was identified in isolates from 26 patients, with 4 patients (15%) having multiple organisms with the mechanism. KPC was identified most commonly from Klebsiella pneumoniae (57/124, 46%); VIM was identified most commonly from Enterobacter cloacae (14/31, 45%). KPC was found in 6 different Enterobacteriaceae genera; VIM in 4. KPC-producing CRE were identified in 22 acute-care and long-term acute-care facilities in 14 counties, with nine reporting >2 isolates. Fifteen percent (19/124) of KPC-producing CRE were isolated from outpatients. VIM-producing CRE were identified in two acute-care facilities located in two urban areas; one was from an outpatient. Patients with VIM were younger than those with KPC (43 vs. 60 years, P < 0.001). Conclusion KPC is the predominant carbapenemase in Kentucky and is more widely disseminated than VIM, which has been limited to two facilities. CRE

Background: Rotavirus vaccination has reduced diarrhoeal morbidity and mortality globally. The monovalent rotavirus vaccine was introduced into the public immunization program in South Africa (SA) in 2009 and led to approximately 50% reduction in rotavirus hospitalization in young children. The aim of this study was to investigate the rotavirus genotype distribution in SA before and after vaccine introduction. Materials and methods: In addition to pre-vaccine era surveillance conducted from 2002 to 2008 at Dr George Mukhari Hospital (DGM), rotavirus surveillance among children <5 years hospitalized for acute diarrhoea was established at seven sentinel sites in SA from April 2009 to December 2014. Stool specimens were screened by enzyme immunoassay and rotavirus positive specimens genotyped using standardised methods. Results: At DGM, there was a significant decrease in G1 strains from pre-vaccine introduction (34%;479/1418;2002–2009) compared to post-vaccine introduction (22%;37/170;2010–2014;p for trend <.001). Similarly, there was a significant increase in non-G1P[8] strains at this site (p for trend <.001). In expanded sentinel surveillance, when adjusted for age and site, the odds of rotavirus detection in hospitalized children with diarrhoea declined significantly from 2009 (46%;423/917) to 2014 (22%;205/939;p <.001). The odds of G1 detection declined significantly from 2009 (53%;224/421) to 2010–2011 (26%;183/703;aOR = 0.5;p <.001) and 2012–2014 (9%;80/905;aOR = 0.1;p <.001). Non-G1P[8] strains showed a significant increase from 2009 (33%;139/421) to 2012–2014 (52%;473/905;aOR = 2.5;p <.001). Conclusions: Rotavirus vaccination of children was associated with temporal changes in circulating genotypes. Despite these temporal changes in circulating genotypes, the overall reduction in rotavirus disease in South Africa remains significant. © 2017 Elsevier Ltd

Sudden unexpected death of an individual with epilepsy (SUDEP) can pose a challenge to death investigators, as most deaths are unwitnessed and the individual is commonly found dead in bed. Anatomic findings (e.g., tongue/lip bite) are commonly absent and of varying specificity, limiting the evidence to implicate epilepsy as a cause of or contributor to death. Thus, it is likely that death certificates significantly underrepresent the true number of deaths in which epilepsy was a factor. To address this, members of the National Association of Medical Examiners, North American SUDEP Registry, Epilepsy Foundation SUDEP Institute, American Epilepsy Society, and the Centers for Disease control and prevention convened an expert panel to generate evidence-based recommendations for the practice of death investigation and autopsy, toxicological analysis, interpretation of autopsy and toxicology findings, and death certification to improve the precision of death certificate data available for public health surveillance of epilepsy-related deaths. The recommendations provided in this paper are intended to assist medical examiners, coroners, and death investigators when a sudden, unexpected death in a person with epilepsy is encountered. © 2018 Academic Forensic Pathology International.

Use and Predictors of Expectant Management of Low and Intermediate Risk Localized Prostate Cancer in Elderly Men

Introduction Expectant management reduces overtreatment in low risk but not intermediate risk localized prostate cancer. We assessed the use and predictors of expectant management to understand its uptake in practice in the United States. Methods Using the SEER (Surveillance, Epidemiology and End Results)-Medicare database, we conducted a retrospective cohort study of men 66 years or older diagnosed with low (25,506 patients) or intermediate risk (25,597) localized prostate cancer between 2004 and 2011 and followed through December 31, 2012. We defined expectant management as 1) no definitive therapy and at least 1 prostate specific antigen test or repeat biopsy 4 to 12 months after diagnosis or 2) receiving definitive therapy after prostate specific antigen testing or repeat biopsy 7 to 12 months after diagnosis. We performed separate analyses for low and intermediate risk groups using multiple logistic regressions. Results For men diagnosed with prostate cancer in 2004 to 2011 expectant management increased from 22% to 43% in the low risk group and from 15% to 18% in the intermediate risk group. In the low risk group expectant management increased with age (adjusted OR 1.26 for age 71 to 75 years, 2.21 for 76 to 80 years, 6.33 for greater than 80 years;all p <0.0001 compared to age 66 to 70 years). Expectant management uptake was higher among men with comorbidities (OR 1.29) and those residing in the Pacific region (adjusted OR 0.56 compared to East Coast). Conclusions In United States practice utilization of expectant management has steadily increased in low risk prostate cancer and remained low in intermediate risk prostate cancer through time. While men with comorbidities or advanced age were more likely to undergo expectant management, its use varied substantially by geographic region. Our findings bring attention to the presence of multiple barriers to expectant management implementation. © 2017 American Urological Association Education and Research, Inc.

Introduction Inflammation is related to the development of many cancers. Diet plays an important role in the regulation of inflammation. The dietary inflammatory index (DII ® ) is a literature-derived dietary index designed to assess the impact of diet on individual inflammation status. However, most validation studies of DII conducted to date have been done in Western countries, indicating the need for evaluation in Asian populations. The aim of this study was to validate DII scores derived from a food frequency questionnaire (FFQ) with high-sensitive C-reactive protein (hs-CRP) levels in participants undergoing cancer screening in Japan. Methods In cancer screenings conducted from 2009 to 2013 in the National Cancer Center in Japan, 7919 participants aged 40–70 years answered the FFQ and received blood analysis for hs-CRP. Of these, we excluded participants with a body mass index (BMI) of < 14 kg/m 2 or > 40 kg/m 2 ( n = 13); missing data on smoking status ( n = 1) or metabolic equivalents (Mets, n = 1); history of cancer, stroke, or myocardial infarction ( n = 877); extreme energy intake (upper and lower 2.5 percentiles, n = 349); missing data on alcohol consumption ( n = 120); and missing data of hs-CRP, or hs-CRP > 1.0 mg/dL ( n = 84). Finally, 6474 participants (3825 for men and 2649 for women) were included in this analysis. DII was calculated from crude intake from the FFQ and adjusted for energy using the density method. Adjusted geometric mean levels of hs-CRP were calculated using a multivariable linear regression model by quartiles of DII score. P for trend across quartiles of DII score were calculated using DII quartiles as continuous variables. Further, logistic regression analysis was also applied to estimate odds ratio (OR) [95% confidence interval (95% CI)] of hs-CRP > 0.3 mg/dL across quartiles of DII score.
