Background: The COVID-19 pandemic highlighted gaps in health surveillance systems, disease prevention, and treatment globally. Among the many factors that might have led to these gaps is the issue of the financing of national health systems, especially in low-income and middle-income countries (LMICs), as well as a robust global system for pandemic preparedness. We aimed to provide a comparative assessment of global health spending at the onset of the pandemic;characterise the amount of development assistance for pandemic preparedness and response disbursed in the first 2 years of the COVID-19 pandemic;and examine expectations for future health spending and put into context the expected need for investment in pandemic preparedness. Methods: In this analysis of global health spending between 1990 and 2021, and prediction from 2021 to 2026, we estimated four sources of health spending: development assistance for health (DAH), government spending, out-of-pocket spending, and prepaid private spending across 204 countries and territories. We used the Organisation for Economic Co-operation and Development (OECD)'s Creditor Reporting System (CRS) and the WHO Global Health Expenditure Database (GHED) to estimate spending. We estimated development assistance for general health, COVID-19 response, and pandemic preparedness and response using a keyword search. Health spending estimates were combined with estimates of resources needed for pandemic prevention and preparedness to analyse future health spending patterns, relative to need. Findings: In 2019, at the onset of the COVID-19 pandemic, US$9·2 trillion (95% uncertainty interval [UI] 9·1–9·3) was spent on health worldwide. We found great disparities in the amount of resources devoted to health, with high-income countries spending $7·3 trillion (95% UI 7·2–7·4) in 2019;293·7 times the $24·8 billion (95% UI 24·3–25·3) spent by low-income countries in 2019. Summary Background Across low-income and middle-income countries (LMICs), one in ten deaths in children younger than 5 years is attributable to diarrhoea. The substantial between-country variation in both diarrhoea incidence and mortality is attributable to interventions that protect children, prevent infection, and treat disease. Identifying subnational regions with the highest burden and mapping associated risk factors can aid in reducing preventable childhood *** We used Bayesian model-based geostatistics and a geolocated dataset comprising 15 072 746 children younger than 5 years from 466 surveys in 94 LMICs, in combination with findings of the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2017, to estimate posterior distributions of diarrhoea prevalence, incidence, and mortality from 2000 to 2017. From these data, we estimated the burden of diarrhoea at varying subnational levels (termed units) by spatially aggregating draws, and we investigated the drivers of subnational patterns by creating aggregated risk factor *** The greatest declines in diarrhoeal mortality were seen in south and southeast Asia and South America, where 54·0% (95% uncertainty interval [UI] 38·1-65·8), 17·4% (7·7-28·4), and 59·5% (34·2-86·9) of units, respectively, recorded decreases in deaths from diarrhoea greater than 10%. Although children in much of Africa remain at high risk of death due to diarrhoea, regions with the most deaths were outside Africa, with the highest mortality units located in Pakistan. Indonesia showed the greatest within-country geographical inequality; some regions had mortality rates nearly four times the average country rate. Reductions in mortality were correlated to improvements in water, sanitation, and hygiene (WASH) or reductions in child growth failure (CGF). We report a characterization of the multi-band flux variability and correlations of the nearby (z=0.031) blazar Markarian 421 (Mrk 421) using data from Metsähovi, Swift, Fermi-LAT, MAGIC, FACT and other collaborations and instruments from November 2014 till June 2016. Mrk 421 did not show any prominent flaring activity, but exhibited periods of historically low activity above 1 TeV (F>1TeV -12 ph cm-2 s-1) and in the 2-10 keV (X-ray) band (F2-10 keV -11 erg cm-2 s-1), during which the Swift-BAT data suggests an additional spectral component beyond the regular synchrotron *** highest flux variability occurs in X-rays and very-high-energy (E>0.1 TeV) γ-rays, which, despite the low activity, show a significant positive correlation with no time lag. The HRkeV and HRTeV show the harder-when-brighter trend observed in many blazars, but the trend flattens at the highest fluxes, which suggests a change in the processes dominating the blazar variability. Enlarging our data set with data from years 2007 to 2014, we measured a positive correlation between the optical and the GeV emission over a range of about 60 days centered at time lag zero, and a positive correlation between the optical/GeV and the radio emission over a range of about 60 days centered at a time lag of 43+-96 *** observation is consistent with the radio-bright zone being located about 0.2 parsec downstream from the optical/GeV emission regions of the jet. The flux distributions are better described with a LogNormal function in most of the energy bands probed, indicating that the variability in Mrk 421 is likely produced by a multiplicative process. Decision support methods aim at assisting in the decision-making process by simplifying the analysis of the problem and justifying the choice of a particular potential action. Recent researches have shown that the hybridization of methods is able to overcome limitations presented by the methods when applied separately: the classification of alternatives before submitting them to an ordination methodology would be an e ective way of filtering the set to be ordered. Specific Practices of Capability Maturity Model Integration were analyzed through a decision making model, assisted by the methods SAC and ZAPROS III-i. The results will be compared to previous studies.

Context. The recurrent nova RS Ophiuchi (RS Oph) underwent a thermonuclear eruption in August 2021. In this event, RS Oph was detected by the High Energy Stereoscopic System (H.E.S.S.), the Major Atmospheric Gamma Imaging Cherenkov (MAGIC), and the first Large-Sized Telescope (LST-1) of the future Cherenkov Telescope Array Observatory (CTAO) at very-high gamma-ray energies above 100 GeV. This means that novae are a new class of very-high-energy (VHE) gamma-ray emitters. Aims. We report the analysis of the RS Oph observations with LST-1. We constrain the particle population that causes the observed emission in hadronic and leptonic scenarios. Additionally, we study the prospects of detecting further novae using LST-1 and the upcoming LST array of CTAO-North. Methods. We conducted target-of-opportunity observations with LST-1 from the first day of this nova event. The data were analysed in the framework of cta-lstchain and Gammapy, the official CTAO-LST reconstruction and analysis packages. One-zone hadronic and leptonic models were considered to model the gamma-ray emission of RS Oph using the spectral information from Fermi-LAT and LST-1, together with public data from the MAGIC and H.E.S.S. telescopes. Results. RS Oph was detected at 6.6σ with LST-1 in the first 6.35 hours of observations following the eruption. The hadronic scenario is preferred over the leptonic scenario considering a proton energy spectrum with a power-law model with an exponential cutoff whose position increases from (0.26 ± 0.08) TeV on day 1 up to (1.6 ± 0.6) TeV on day 4 after the eruption. The deep sensitivity and low energy threshold of the LST-1/LST array will allow us to detect faint novae and increase their discovery rate. © 2025, CC BY.

With the COVID-19 pandemic, behavioural scientists aimed to illuminate reasons why people comply with (or not) large-scale cooperative activities. Here we investigated the motives that underlie support for COVID-19 preventive behaviours in a sample of 12,758 individuals from 34 countries. We hypothesized that the associations of empathic prosocial concern and fear of disease with support towards preventive COVID-19 behaviours would be moderated by trust in the government. Results suggest that the association between fear of disease and support for COVID-19 preventive behaviours was strongest when trust in the government was weak (both at individual- and country-level). Conversely, the association with empathic prosocial concern was strongest when trust in the government was high, but this moderation was only found at individual-level scores of governmental trust. We discuss how motivations may be shaped by socio-cultural context, and outline how findings may contribute to a better understanding of collective action during global crises.

Background: Experimental and epidemiological studies indicate an association between exposure to particulate matter (PM) air pollution and increased risk of type 2 diabetes. In view of the high and increasing prevalence of diabetes, we aimed to quantify the burden of type 2 diabetes attributable to PM_2·5 originating from ambient and household air pollution. Methods: We systematically compiled all relevant cohort and case-control studies assessing the effect of exposure to household and ambient fine particulate matter (PM_2·5) air pollution on type 2 diabetes incidence and mortality. We derived an exposure–response curve from the extracted relative risk estimates using the MR-BRT (meta-regression—Bayesian, regularised, trimmed) tool. The estimated curve was linked to ambient and household PM_2·5 exposures from the Global Burden of Diseases, Injuries, and Risk Factors Study 2019, and estimates of the attributable burden (population attributable fractions and rates per 100 000 population of deaths and disability-adjusted life-years) for 204 countries from 1990 to 2019 were calculated. We also assessed the role of changes in exposure, population size, age, and type 2 diabetes incidence in the observed trend in PM_2·5-attributable type 2 diabetes burden. All estimates are presented with 95% uncertainty intervals. Findings: In 2019, approximately a fifth of the global burden of type 2 diabetes was attributable to PM_2·5 exposure, with an estimated 3·78 (95% uncertainty interval 2·68–4·83) deaths per 100 000 population and 167 (117–223) disability-adjusted life-years (DALYs) per 100 000 population. Approximately 13·4% (9·49–17·5) of deaths and 13·6% (9·73–17·9) of DALYs due to type 2 diabetes were contributed by ambient PM2·5, and 6·50% (4·22–9·53) of deaths and 5·92% (3·81–8·64) of DALYs by household air pollution.
