Chronic care manages long-term, progressive conditions, while acute care addresses short-term conditions. Chronic conditions increasingly strain health systems, which are often unprepared for these demands. This study examines the burden of conditions requiring acute versus chronic care, including sequelae. Conditions and sequelae from the Global Burden of Diseases Study 2019 were classified into acute or chronic care categories. Data were analysed by age, sex, and socio-demographic index, presenting total numbers and contributions to burden metrics such as Disability-Adjusted Life Years (DALYs), Years Lived with Disability (YLD), and Years of Life Lost (YLL). Approximately 68% of DALYs were attributed to chronic care, while 27% were due to acute care. Chronic care needs increased with age, representing 86% of YLDs and 71% of YLLs, and accounting for 93% of YLDs from sequelae. These findings highlight that chronic care needs far exceed acute care needs globally, necessitating health systems to adapt accordingly. © 2025. The Author(s).

Epigenetic modifications are associated with the regulation of co/post-transcriptional processing and differential transcript isoforms are known to be important during cancer progression. It remains unclear how disruptions of chromatin-based modifications contribute to tumorigenesis and how this knowledge can be leveraged to develop more potent treatment strategies that target specific isoforms or other products of the co/post-transcriptional regulation pathway. Rapid developments in all areas of next-generation sequencing (DNA, RNA-seq, ChIP-seq, Methyl-CpG, etc.) have provided new opportunities to develop novel integration and data-mining approaches, and also allows for exciting hypothesis driven bioinformatics and computational studies. Here, we present a program that we developed and summarize the results of applying our methods to analyze datasets from patient matched tumor or normal (T/N) paired samples, as well as cell lines that were either sensitive or resistant (S/R) to treatment with an anti-cancer drug, 5-Azacytidine (http://***/projects/chiprnaseqpro/). We discuss additional options for user-defined approaches and general guidelines for simultaneously analyzing and annotating epigenetic and RNA-seq datasets in order to identify and rank significant regions of epigenetic deregulation associated with aberrant splicing and RNA-editing.

The microstructure of a material intimately affects the performance of a device made from this material. The microstructure, in turn, is affected by the processing pathway used to fabricate the device. This forms the process–structure–property triangle that is central to material science. There has been increasing interest to comprehensively understand and subsequently exploit process–structure–property (PSP) relationships to design processing pathways that result in tailored microstructures exhibiting optimal properties. However, unraveling process–structure–property relationships usually requires systematic and tedious combinatorial search of process and system variables to identify the microstructures that are produced. This is further complicated by the necessity to interrogate the properties of the huge set of corresponding microstructures. Motivated by this challenge, we focus on developing a generic methodology to establish and explore PSP pathways. We leverage recent advances in high performance computing (HPC) and high throughput computing (HTC) with the premise that a domain expert should be able to focus on domain specific PSP problems while the highly specialized HPC/HTC knowledge needed to approach such problems should be hidden from the domain expert. Our hypothesis is that PSP exploration can be naturally formulated in terms of a standard paradigm in cloud computing, namely the MapReduce programming model. We show how reformulating PSP exploration into a MapReduce workflow enables us to take advantage of advances in cloud computing while requiring minimal specialized knowledge of HPC. We illustrate this generic approach by exploring PSP relationships relevant to organic photovoltaics. We focus on identifying microstructural traits that correlate with specific properties of the photovoltaic process: exciton generation, exciton dissociation and charge generation. Donor selection for Hematopoietic Stem Cell Transplant often requires physicians to manually select 3 to 5 donors from a list of 100s of genetically compatible donors as identified by HLA-based matching algorithms. The decision process is complicated by a lack of strict guidelines governing a "secondary" selection process, which is based upon non-HLA donor attributes. Our research is aimed at modeling this "secondary" decision process which can help physicians choose the right donors, based on donor attributes and historical choice behavior. Proposed black box models will help in improving selection consistency.

Brazil is the second country in the world in aesthetic surgical procedures, with 905,000 procedures, being behind only the U.S., according to information corresponding to the year 2011 from the International Society of Aesthetic Plastic Surgery (ISAPS) and the Brazilian Society of Plastic Surgery (BSPS). Given this scenario, we chose to study the adipose cells, which are a organ with intense metabolic and endocrine activity. Accumulated in excess adipocytes causes the disadvantages metabolic and endocrine in the organism, leading to obesity in general and located. This research referred to obesity marked by deposition in the abdominal area, that is denominated central or androide obesity, in the case of a localized adiposity. The objective of the research was to analyze the effects of therapeutic ultrasound and medium frequency current in this particular type of adiposity. The therapeutic protocol consisted of the utilization therapy of therapeutic ultrasound 3 MHz and medium frequency current (1 kHz, with Bursts of 2 ms), performed in ten sessions of 40-minute each. The sample consisted of 16 women, of whom 14 have finalized treatment. The assessment instruments used were: percentage of total body fat by bioimpedance, skinfold measurement in the region where localized adiposity was. We observed the reduction in measures measured in abdominal circumference and percentage decrease in total body fat measured on a digital scale bipolar bioimpedance, as well as the decrease measured by skinfold plicometer in all patients submitted to treatment. The results of submission to the treatment of Combined Therapy can be considered satisfactory, due the record in reducing the parameters analyzed, despite being necessary to a more thorough study on the subject and scientific substantiation in a larger sample. Clinical genetic testing identifies variants causal for hereditary cancer, information that is used for risk assessment and clinical management. Unfortunately, some variants identified are of uncertain clinical significance (VUS), complicating patient management. Case-control data is one evidence type used to classify VUS. As an initiative of the Evidence-based Network for the Interpretation of Germline Mutant Alleles (ENIGMA) Analytical Working Group we analyze germline sequencing data of BRCA1 and BRCA2 from 96,691 female breast cancer cases and 302,116 controls from three studies: the BRIDGES study of the Breast Cancer Association Consortium, the Cancer Risk Estimates Related to Susceptibility consortium, and the UK Biobank. We observe 11,207 BRCA1 and BRCA2 variants, with 6909 being coding, covering 23.4% of BRCA1 and BRCA2 VUS in ClinVar and 19.2% of ClinVar curated (likely) benign or pathogenic variants. Case-control likelihood ratio (ccLR) evidence is highly consistent with ClinVar assertions for (likely) benign or pathogenic variants; exhibiting 99.1% sensitivity and 95.3% specificity for BRCA1 and 93.3% sensitivity and 86.6% specificity for BRCA2. This approach provides case-control evidence for 787 unclassified variants; these include 579 with strong or moderate benign evidence and 10 with strong pathogenic evidence for which ccLR evidence is sufficient to alter clinical classification.
