Background: To our knowledge, no previous study has examined the inter-relationship between frailty, dysglycaemia, and mortality in frail older adults with type 2 diabetes who are on insulin therapy. We used continuous glucose monitors (CGMs) to profile this patient population and determine the prognostic value of CGM metrics. We hypothesised that incremental frailty was associated with increased hypoglycaemia or time below range (TBR). Methods: HARE was a multicentre, prospective, observational cohort study with mortality hazard analysis carried out in four hospitals in Hong Kong. Eligible participants were community-living adults aged 70 years and older;had had type 2 diabetes for 5 years or more;were on insulin therapy;were frail;and were not hospitalised at the time of frailty assessment and CGM recording. Glucose control was characterised according to the Advanced Technologies and Treatments for Diabetes 2019 international consensus clinical targets. Frailty index was computed, and comprehensive frailty assessments and targeted serum metabolic profiling were performed. The Jonckheere-Terpstra test for trend was used to analyse frailty index tertiles and variables. Inter-relationships between CGM metrics and frailty, glycated haemoglobin A_1c (HbA_1c), and serum albumin were characterised using adjusted regression models. Survival analysis and Cox proportional hazard modelling were performed. Findings: Between July 25, 2018, and Sept 27, 2019, 225 participants were recruited, 222 of whom had CGMs fitted and 215 of whom had analysable CGM data (190 were frail, 25 were not frail). Incremental frailty was associated with older age, greater HbA_1c, worse renal function, and history of stroke. Eight of 11 CGM metrics were significantly associated with frailty. Decreased time in range (TIR;glucose concentration 3·9–10·0 mmol/L) and increased time above range (TAR) metrics were strongly correlated with increased frailty and hyperglycaemia,

Despite the crucial role of interfacial perpendicular magnetic anisotropy in a Co(Fe)/MgO-based magnetic tunnel junction, the underlying mechanism is still being debated. Here, we report an anatomical study of the oxygen and Mg effect on Pt/Co bilayers through repeated in situ anomalous Hall effect measurements, controlled oxygen exposure, and Mg deposition in an ultrahigh vacuum system. We found that chemisorbed oxygen not only quenches the effective magnetic moment of the Co surface layer, but also softens its magnetic anisotropy. However, a subsequent Mg dusting on the oxygen preexposed Pt/Co surface can recover the magnetic anisotropy. The ab initio calculations on the exchange splitting and orbital hybridization near the Fermi level give a clear physical explanation of the experimental observations. Our results suggest that the Co(Fe)-O-M bond plays a more important role than the widely perceived Co(Fe)-O bond does in realizing interfacial perpendicular magnetic anisotropy in Co(Fe)/MgO heterostructures.

Metacognitive learning approach for online tool condition monitoring

As manufacturing processes become increasingly automated, so should tool condition monitoring (TCM) as it is impractical to have human workers monitor the state of the tools continuously. Tool condition is crucial to ensure the good quality of products - Worn tools affect not only the surface quality but also the dimensional accuracy, which means higher reject rate of the products. Therefore, there is an urgent need to identify tool failures before it occurs on the fly. While various versions of intelligent tool condition monitoring have been proposed, most of them suffer from a cognitive nature of traditional machine learning algorithms. They focus on the how-to-learn process without paying attention to other two crucial issues - what-to-learn, and when-to-learn. The what-to-learn and the when-to-learn provide self-regulating mechanisms to select the training samples and to determine time instants to train a model. A novel tool condition monitoring approach based on a psychologically plausible concept, namely the metacognitive scaffolding theory, is proposed and built upon a recently published algorithm - recurrent classifier (rClass). The learning process consists of three phases: what-to-learn, how-to-learn, when-to-learn and makes use of a generalized recurrent network structure as a cognitive component. Experimental studies with real-world manufacturing data streams were conducted where rClass demonstrated the highest accuracy while retaining the lowest complexity over its counterparts. Despite substantial declines since 2000, lower respiratory infections (LRIs), diarrhoeal diseases, and malaria remain among the leading causes of nonfatal and fatal disease burden for children under 5 years of age (under 5), primarily in sub-Saharan Africa (SSA). The spatial burden of each of these diseases has been estimated subnationally across SSA, yet no prior analyses have examined the pattern of their combined burden. Here we synthesise subnational estimates of the burden of LRIs, diarrhoea, and malaria in children under-5 from 2000 to 2017 for 43 sub-Saharan countries. Some units faced a relatively equal burden from each of the three diseases, while others had one or two dominant sources of unit-level burden, with no consistent pattern geographically across the entire subcontinent. Using a subnational counterfactual analysis, we show that nearly 300 million DALYs could have been averted since 2000 by raising all units to their national average. Our findings are directly relevant for decision-makers in determining which and targeting where the most appropriate interventions are for increasing child survival. © 2022, The Author(s).
