Pilot Testing a Series of Value-Based Care Training Courses

Purpose: The US healthcare system currently emphasizes volume of services over value. To facilitate changing to a value-based care model, one managed care organization developed a series of online courses designed to teach clinicians value-based care principles and practices. A pilot test was conducted to obtain feedback regarding course content and design, so the courses could be revised prior to their launch. Patients and Methods: A representative cross section of the courses' target audience (n = 50) was recruited to participate in the pilot test, and data were collected through an online survey. Descriptive statistics were calculated for responses to close-ended survey questions, and affinity analysis was performed on responses to open-ended survey questions. Issues identified were then categorized as urgent/not urgent with respect to course revision. Results: Nearly a quarter (24%) of respondents indicated that the course contained incorrect or misleading information. Other responses noted inconsistencies in course content, eg, misspelled or mispronounced words, slides that were hard to read, etc. Conclusion: This study demonstrates how a pilot test was used as part of a formative assessment to improve course content and design. During a pilot test, attention should be paid to making it easy for participants to provide feedback.

Inclusion of Potentially Inappropriate Medicines for the Older Adults in the Brazilian Consensus in Accordance with International Criteria

Aim: The use of potentially inappropriate medications (PIM) can impair the safety and effectiveness of pharmacotherapy in the older adults. Thus, several countries have lists and criteria to indicate these drugs, in order to promote the safety of prescription and the rational use of drugs in geriatric practice. Objective: This study sought to contribute to the inclusion of PIM for the older adults in the Brazilian criterion (BCPIM/2016) - current list used in Brazil and reference in Latin American countries - through expert approval, comparing convergences with international AGS lists BEERS/2019, STOPP/START/2015, PRISCUS/2010 and EU (7)-PIM List/2015. Methods: This is a critical analysis of potentially inappropriate medications for use in the older adults present in the list of Brazilian criteria, together with their absence of some drugs that are on international lists (BEERS/2019;Priscus/2010;Stopp/Start/2015;EU7-PIM list/2015). This study was subdivided in 6 stages: selection of national criteria, classification of drugs according to Anatomic Therapeutic Chemical, comparison between BCPIM/2016 with international lists, selection of drugs not included in the Brazilian list, selection of experts for evaluation and suggestions about drugs not included in the Brazilian list and the synthesis of the analysis carried out by the specialists. Results: We cataloged 66 drugs marketed in Brazil that are on international lists, but not in the Brazilian consensus, of which 24 were validated by experts as necessary for inclusion in this consensus, considering the risks and benefits in health care for the older adults. However, the lists have divergences and similarities between them. We observed that eight drugs were common to all criteria studied, mainly related to the nervous system. Setting Tiered management Triggers using a Values-based Approach in an Indigenous-led Cumulative Effects management System

Indigenous-led cumulative effects assessment and management (CEM) has emerged in recent years as a proactive and strategic approach for addressing the cumulative impacts of industrial development and other activities. CEM identifies and monitors high-priority values and stressors and develops management strategies to restore and improve the condition of those values. As Indigenous-led CEM evolves, it faces a major challenge in linking cumulative effects assessment and monitoring information to effective management actions. One promising approach to address this challenge is the use of tiered management triggers, which are a series of progressive markers associated with specified degrees of change in the condition of a value, designed to reflect increasing levels of concern about the value. These types of limits or thresholds inform decision-makers that they need to act, or act more intensively, to restore values to an acceptable state. In this paper, we present a novel method for setting tiered management triggers that was developed in an Indigenous-led CEM program. We co-developed this six-step method with the Metlakatla First Nation (located on the northwest coast of British Columbia, Canada) and applied it in a series of community workshops to select triggers for three values in the Metlakatla CEM program: butter clams, housing, and food, social, and ceremonial activity. The method draws from participatory frameworks including structured decision-making and collaborative planning. The workshop results show that management triggers can successfully be established through a values-focused process of social choice, involving community engagement and informed by relevant scientific research and other knowledge.

The non-alcoholic beverage industry stands out as one of the fastest-growing agroindustrial sectors, despite the difficulties generated by the COVID-19 pandemic, with optimistic projections in the coming years. Its importance lies in the economic, productive, and social contribution and the social responsibility programs companies carry out for their interest groups. However, the industry faces challenges such as competition, government restrictions related to the environment, quality and safety of products and processes, and product diversification due to changes in consumer demand for more natural products. Therefore, companies in the non-alcoholic beverage sector must establish and implement appropriate strategies to maintain the quality of their products, optimize costs, and satisfy continuous and changing customer requirements. The present study presents the application of the Lean Manufacturing methodology as a philosophy of continuous improvement aimed at achieving early victories in terms of quality and productivity. It costs through three-phase methods, which consisted of 1) diagnosis of the current state of the production line, 2) identification of the types and sources of waste that affect the efficiency and quality of the process, 3) Action Plan and implementation of appropriate Lean Manufacturing techniques and, 4) Evaluation of the impact of the improvement proposal on time, cost, quality, and customer satisfaction indicators. The case study is presented in the process of changing the flavor of a non-alcoholic beverage company, which is of high value in Latin America and is part of one of the largest companies worldwide. The results evidenced significant improvements in time lost due to flavor change, which went from 234 min to 170 min, decreased costs due to a reduction in time lost in flavor change went from USD 258,750 to USD 203,500, and improvement in a corporate culture focused on quality and continuous improvement, with an impact on reducing non

The global COVID-19 pandemic produced several changes in nearly every aspect of our lives. Ride-sharing platforms such as Uber and Lyft must adapt their strategies and aims to stay afloat. The analysis in this study is based on 216,120 tweets in the U.S. between January 1, 2019, and December 30, 2021, about Uber. It includes four separate analyses: Popularity and Usage Analytics, Sentimental Analytics, Voice Analytics, and Topic Mining Analytics. The result shows that usage and popularity of Uber on Twitter negatively affect Covid and death cases. In contrast, vaccination helps mitigate the shock of Covid. Additionally, ' 'Uber's policy and business model was beneficial in improving its positive image during the pandemic;On the early breakout of Covid in the U.S. Uber had a jump on the positive sentiment, mainly because Uber provided safer service than public transportation.

An Analysis of Factors Contributing to Cost Overruns in the Global Construction Industry

The construction industry builds infrastructure and strengthens the global economy, but it struggles with cost overruns. This systematic study used scientometrics and Social Network Analysis to examine the multifaceted factors causing construction project cost escalations. After reviewing 405 scholarly works, the research mapped and analyzed 66 interconnected cost overrun factors in 69 high-impact studies between 2000 and 2024. To uncover the patterns, the study used two main research methods. First, this study applied a scientometric analysis that reviewed trends and gaps from the previous studies. Second, this study used Social Network Analysis (SNA) to examine how different factors were connected and which factors had the strongest influence on cost overruns. The methodology comprised a systematic literature search, document selection, scientometric analysis, factor standardization, and an SNA application. Seven critical drivers with high network centrality were identified: planning and scheduling issues, project estimation inaccuracies, design inefficiencies, negative weather conditions, scope definition challenges, contractual ambiguities, and unforeseeable site conditions. By applying the SNA degree centrality (DC), the analysis quantified the significance of each factor within the network. With the use of this dual analysis, a novel mapping of the main causes of cost overruns was produced, leading to the discovery of seven core factors that significantly affected project outcomes, including planning and scheduling issues, project estimation problems, and design inefficiencies. The findings advance the knowledge of the dynamics of cost overruns and offer practical insights for enhancing cost management practices in the construction industry.

The sustainability of building infrastructure has been a growing critical global concern. However, stakeholders often overlook the sustainability aspect of systems in their everyday lives. This paper presents an exploration of the potential of augmented reality (AR) technology as a tool for enhancing public engagement around sustainable systems in buildings. We introduce an innovative approach to address the challenge of gap between sustainable infrastructure and user awareness. Through data collection such as access counts, usage duration, and user feedback, we can evaluate the effectiveness of AR applications in improving awareness and engagement in sustainability. This study illustrates how AR technology can facilitate a deeper connection between the public and sustainable systems, thus paving the way for a more environmentally friendly society. © 2024 ISARC. Introduction: The Food and Drug Administration (FDA) audits and validates devices before mass production to ensure high standards, safety, and quality of medical devices being marketed. Despite those measures in place, consumers' trusts in medical devices are still dwindling based on safety and privacy risks that eventually influence the health of patients. Methods: The method employed in this study is conceptual and includes a selection of a company that develops medical devices to use as an example organization to apply the hybrid risk management framework, defined herein in the results and discussion section. Results: The results include a hybrid risk management approach including activities and tools and techniques by risk management phases. Discussion: The discussion includes how to apply the hybrid risk management framework using Abbott Laboratories as an example. Conclusion: To mitigate the chances that risks (adverse events) occur during the manufacturing and use of medical devices, this study has focused on providing a hybrid risk management approach for organizations noting the use of ISO 14971 activities as well as the PMBOK activities.

The limitations of complexity theorizing in project studies are traced back to simplistic and reductionist theorizing strategies. This article offers pragmatist recommendations to develop strong theorizing strategies organized in a triad: orders of theorizing (degree of recursiveness of the theorizing process), levels of theorizing (interactions between micro, meso, and macro loci of analysis), and the integration between orders and levels brought together in a recursive relationship of co-construction. We offer four main contributions to complexity theorizing in project studies: pragmatism is useful, deeper attention should be paid to theorizing across levels, third-order theorizing is needed, and complexifying terminology is required.

In a context marked by an increasingly open information technology environment, with a significant flow of data, the exponential growth of cyber threats and attacks, the embedded security risk in every IT asset, and the challenges related to ensuring the confidentiality, integrity, and availability of information. Adopting an Information Security management System has become an essential requirement for organizations to meet their goals, and safeguard the interests of their key stakeholders, reputation, brand, and value-creating activities. This paper provides an overview of the challenges related to adopting the ISO/IEC 27001 standards by Moroccan firms. The paper analyzes the outcome of the questionnaire with security engineers: Chief Information Security Officer 'CISO', Security Operations Center 'SOC' responsible, incident handling and response team, as well as IT engineers to evaluate the readiness of 62 enterprises to implement an ISMS based on The ISO/IEC 27001 standards. With an emphasis on ISO/IEC 27001 requirements and their significance, related trends, and key issues. In addition, we examine the correlation between ISO/IEC 27001 certification process and the Moroccan regulation. Our findings underscore the limitations related to establishing robust information security strategies within Moroccan enterprises and provide insights into the challenges encountered during the implementation of ISO/IEC 27001. Furthermore, the study emphasizes the importance of awareness, policies and procedures to manage risks effectively including threads related to Artificial Intelligence and Cloud environment. As a result, this paper provides new insight and recommendations that should be considered in the Moroccan context, to assist companies in implementing ISMS and achieving the ISO/IEC 27001 certification. © 2024 IEEE.
