Humans acquire and accumulate knowledge through language usage and eagerly exchange their knowledge for advancement. Although geographical barriers had previously limited communication, the emergence of information technology has opened new avenues for knowledge exchange. However, it is unclear which communication pathway is dominant in the 21st century. Here, we explore the dominant path of knowledge diffusion in the 21st century using Wikipedia, the largest communal dataset. We evaluate the similarity of shared knowledge between population groups, distinguished based on their language usage. When population groups are more engaged with each other, their knowledge structure is more similar, where engagement is indicated by socioeconomic connections, such as cultural, linguistic, and historical features. Moreover, geographical proximity is no longer a critical requirement for knowledge dissemination. Furthermore, we integrate our data into a mechanistic model to better understand the underlying mechanism and suggest that the knowledge "Silk road" of the 21st century is based online. © 2022, CC BY.

Comparison of adherence between fixed and unfixed topical combination glaucoma therapies using Japanese healthcare/pharmacy claims database: a retrospective non-interventional cohort study

BackgroundAdherence to chronic therapies is crucial to prevent the progression of disease, such as glaucoma. However, only a limited number of studies have investigated them using real-world data in Japan. This study aimed to evaluate Japanese patients' adherence to fixed- and unfixed-combination eye drops as a second-line therapy for glaucoma in real-world *** retrospective, non-interventional cohort study utilized a commercially available Japanese healthcare database (MinaCare database). Medical/pharmacy claims data were collected from 2011 to 2016. The primary endpoint was adherence to medications, assessed by proportion of days covered (PdC) with medication during a 12-month post-index period. Meanwhile, the secondary endpoints included the persistence *** total of 738 patients were included in this study: 309 and 329 in the fixed- and unfixed-combination cohorts, respectively. Prostaglandin analog (PG)/beta -blocker (BB) was most commonly claimed in 241/309 (78.0%) patients in the fixed-combination cohort. In the unfixed-combination cohort, PG and BB were claimed in 130/329 (39.5%) patients, whereas PG and alpha 2-agonist were claimed in 87/329 (26.4%) patients. Patients were more adherent to the fixed-combination than the unfixed-combinations (mean PdCs [Sd], 79.1% [32.1] vs. 62.2% [38.0];P<0.0001). The proportion of patients with good adherence (PdCreater than or equal to>80%) was also higher in the fixed-combination cohort (69.6%) than in the unfixed-combination cohort (48.6%) (P<0.0001). during the 12-month post-index period, the persistence rate was higher in the fixed-combination cohort than in the unfixed-combination cohort (47.6% [95% confidence intervals (CI): 41.9-53.0] vs. 24.9% [95% CI: 20.4-29.7], P<0.0001).ConclusionsJapanese patients with glaucoma preferred the fixed-combination therapies over the unfixed-combination therapies. Hence, fixed-combination therapies would contribute to the improvement of adherence.

The Ed has been increasingly recognized as a key setting for suicide prevention. Zero Suicide (ZS) is an aspirational goal to eliminate suicide for all patients within a health care system through utilization of best practices. However, there has been limited exploration of ZS implementation within the Ed. As Ed nurses play an important role in suicide prevention through their close contact with patients at risk for suicide, ZS implementation would benefit from tailored strategies for Ed nurse leadership. We describe the ZS framework and provides strategies for nurse leaders to adapt each ZS component in the adult Ed.

The proceedings from the 30th August 2023 (day 2) of the workshop "Physiologically Based Biopharmaceutics Models (PBBM) Best Practices for drug Product Quality: regulatory and Industry Perspectives" are provided herein. day 2 covered PBBM case studies from six regulatory authorities which provided considerations for model verification, validation, and application based on the context of use (COU) of the model. PBBM case studies to define critical material attribute (CMA) specification settings, such as active pharmaceutical ingredient (API) particle size distributions (PSds) were shared. PBBM case studies to define critical quality attributes (CQAs) such as the dissolution specification setting or to define the bioequivalence safe space were also discussed. Examples of PBBM using the credibility assessment framework, COU and model risk assessment, as well as scientific learnings from PBBM case studies are provided. Breakout session discussions highlighted current trends and barriers to application of PBBMs including: (a) PBBM credibility assessment framework and level of validation, (b) use of disposition parameters in PBBM and points to consider when iv data are not available, (c) conducting virtual bioequivalence trials and dealing with variability, (d) model acceptance criteria, and (e) application of PBBMs for establishing safe space and failure edges.

Background: Information on prescriptions of oral analgesics for the treatment of pain is beneficial. However, there have been few reports on the prescription status of oral analgesics from a nation-wide, large-scale prescription database in Japan. research design and methods: The authors analyzed the prescription data of 2,042,302 patients prescribed oral analgesics in 2017. The numbers/proportions of patients prescribed oral analgesics, adherence with approved doses, co-prescription patterns, dose changes, drug adherence, and treatment-discontinuation rates were evaluated. results: Loxoprofen was prescribed to 32.5% of the patients, followed by celecoxib, prescribed to 16.0% of patients. Acetaminophen and pregabalin were prescribed to 10.5% and 9.4% of patients, respectively. Many analgesics were prescribed at lower doses than the approved doses. The most frequently used concomitant medication was pregabalin. For duloxetine and pregabalin, high proportions of patients were prescribed these drugs for > 90 days. Conclusions: Loxoprofen was the most prescribed of the non-steroidal anti-inflammatory drugs in Japan. The information obtained provides an overview of prescribed oral analgesics in Japan and could be useful for potential research into prescribed oral analgesics in the future.

High Performance Computing (HPC) applications' performance and behavior rely on software and hardWare environments Which are often highly configurable. Finding their optimal parametrization is a very complex task. The size of the parametric space and the non-linear relationship between the parameters and the delivered performance make hand-tuning, theoretical modeling or exhaustive sampling unsuitable for most cases. In this paper, We propose an auto-tuning loop that uses black-box optimization to Find the optimal parametrization of IO accelerators for a given HPC application in a limited number of iterations, Without making any assumption on the performance function. After a literature review of the selected methods for tuning the accelerators, We describe their implementation and experimentation in our HPC context using two IO accelerators developed by Atos. We also define several metrics to evaluate the quality of our optimization, as our criteria of success go further than finding the optimal parameters. The obtained results show that this framework successfully improves the execution time of two applications used conjointly With a pure software accelerator and a mixed hardWare-software one. We indeed observe possible time gains of respectively 38% and 20% for each accelerator compared to launching the same application accelerated With the default parameters.

Introduction Most patient-reported outcome (PrO) instruments that measure atopic dermatitis (Ad) symptoms do not have sufficient documented evidence of content validity to satisfy regulatory agency guidance for inclusion in product-labelling claims in the USA or Europe. The objective of this study was to develop a PrO instrument in accordance with regulatory agency guidance to assess daily Ad symptoms during the course of therapy and to establish its content validity and psychometric properties. Methods The Pruritus and Symptoms Assessment for Atopic dermatitis (PSAAd) daily diary was developed based on qualitative interviews with US adolescents and adults with mild-to-severe Ad. Content validity, test-retest reliability, internal consistency reliability, clinically important difference, clinically important responder, convergent validity, and known-group validity were evaluated using correlational and regression methods from phase 2b data from US adults with moderate-to-severe Ad who were treated with abrocitinib. results Patient interviews conducted with US adolescents and adults with mild-to-severe Ad identified 11 relevant symptoms (itch, dryness, redness, flaking, discolouration, pain, bleeding, cracking, bumps, swelling, and weeping/oozing) for inclusion in the PSAAd instrument. All PSAAd psychometric parameters were acceptable based on phase 2b data from US adults with moderate-to-severe Ad. Convergent validity and known-group validity were confirmed by significant correlations between PSAAd and six other PrO measures (r = 0.24-0.91, all p <= 0.01) and dermatology Life Quality Index category (p <= 0.0001), respectively. Conclusions Evidence supports the PSAAd instrument validity, reliability, responsiveness and definitions of clinically important changes/differences for adults with moderate-to-severe Ad.

Owing to the ever-growing impetus towards the development of eco-friendly and low carbon footprint energy solutions, biodiesel production and usage have been the subject of tremendous research efforts. The biodiesel production process is driven by several process parameters, which must be maintained at optimum levels to ensure high productivity. Since biodiesel productivity and quality are also dependent on the various raw materials involved in transesterification, physical experiments are necessary to make any estimation regarding them. However, a brute force approach of carrying out physical experiments until the optimal process parameters have been achieved will not succeed, due to a large number of process parameters and the underlying non-linear relation between the process parameters and responses. In this regard, a machine learning-based prediction approach is used in this paper to quantify the response features of the biodiesel production process as a function of the process parameters. Three powerful machine learning algorithms-linear regression, random forest regression and AdaBoost regression are comprehensively studied in this work. Furthermore, two separate examples-one involving biodiesel yield, the other regarding biodiesel free fatty acid conversion percentage-are illustrated. It is seen that both random forest regression and AdaBoost regression can achieve high accuracy in predictive modelling of biodiesel yield and free fatty acid conversion percentage. However, AdaBoost may be a more suitable approach for biodiesel production modelling, as it achieves the best accuracy amongst the tested algorithms. Moreover, AdaBoost can be more quickly deployed, as it was seen to be insensitive to number of regressors used.

This study analyzes a large-scale navigation dataset that captures travel activities of domestic inbound visitors in Jeju, Korea in the first nine months of 2020. A collection of regression models are introduced to quantify the dynamic effects of local and national COVId-19 indicators on their travel behavior. results suggest that behavior of inbound travelers was jointly affected by pandemic severity locally and remotely. The daily number of new cases in Jeju has a greater impact on reducing travel activities than the national-level daily new cases of COVId19. The impacts of the pandemic did not diminish over time but produced heterogeneous effects on travels with different trip purposes. Our findings reveal the persistence of COVId-19's effects on travel behavior and the variability in travelers' responses across tourism activities with different levels of perceived health risks. The implications for crisis management and recovery strategies are also discussed.
