The Epstein-Barr virus (EBV) infects over 90 % of the human population, often behaving as a harmless passenger in most hosts. However, since 1997, it has been classified as a group 1 carcinogen by the International Agency for Research on Cancer (IARC) due to its causal association with several malignancies. Most studies on EBV primary infection and EBV-associated lymphomas have been performed in adults from developed countries. The complex interplay between age of acquisition and symptomatic versus asymptomatic infection is related to the subsequent risk of EBV-associated cancers or autoimmune diseases. This review discusses some characteristics of EBV infection and EBV-associated lymphomas in children from low-and middle-income regions, with a focus on the local immune response.

Objective: The OXA-48-like enzymes have the capacity to hydrolyse carbapenems and are members of class D beta-lactamases that are primarily detected in Enterobacterales . The allelic variant blaOXA-163 , which has low hydrolytic activity towards carbapenems, was detected in Argentina in 2011 and has spread successfully since then, giving sporadic origin to novel local variants. The aim of this study was to analyse the phenotypic profile and dissemination strategies of two novel OXA enzymes, blaOXA-438 and blaOXA-567 , located in Escherichia coli M17224 and Klebsiella pneumoniae M21014, respectively, isolated from two paediatric patients. Methods: Minimum inhibitory concentration measurements were performed to determine the phenotypic profile of the clinical isolates, transconjugants and transformant cells. Biparental conjugation, PCR, Sanger and whole-genome sequencing were performed to determine the complete genetic characteristics of the plasmids. Results: Both isolates were found to be resistant to carbapenems and susceptible to ceftriaxone. blaOXA-438 was located on a 69-kb IncN2 plasmid, while blaOXA-567 was found on a 175-Kb IncC2 plasmid, both transferable by biparental conjugation. The close genetic environment of the blaOXA genes suggests a common origin likely involving mobile genetic elements. Finally, the clinical case of M21014 revealed that the patient had previous infections with two genetically related K. pneumoniae ST6838 that carried blaOXA-163 on an IncC2 plasmid with equal size and genetic hallmarks to that of M21014, providing strong evidence for the intra-patient emergence of blaOXA-567. Conclusions: This research underscores the need for ongoing surveillance and integral studies to understand the emergence, biochemistry and dissemination capacity of OXA enzymes with the overarching aim to halt their spread. EPSTEIN-BARR-VIRUS (EBV) LATENT MEMBRANE-PROTEIN (LMP) IN TUMOR-CELLS OF HODGKINS-DISEASE IN PEDIATRIC-PATIENTS

Twenty-nine out of 31 consecutive pediatric patients with Hodgkin's disease treated at our hospital from 1988 to 1992 were studied. The selection criterion was the availablity of sufficient formalin-fixed, paraffin-embedded tissue for analysis. Patient age ranged from 3 to 15 years with a median age of 7 years. Lymph node biopsies were examined for the presence of Epstein-Barr virus (EBV)-encoded latent membrane protein (LMP) in malignant cells by peroxidase immunolabeling. LMP positivity was present in 10/15 (67%) of mixed cellularity, 1/6 (17%) of lymphocyte predominance, 0/7 (0%) of nodular sclerosis, and 1/1 (100%) of lymphocyte depletion. Positive cases by age range were: 10/12 (83%) for 3-6 years and 2/17 (11%) for 7-15 years. The association between EBV and Hodgkin's disease in children appeared to be more frequent in patients with mixed cellularity and those in the 3-6 age range, though examples of EBV-positive tumors were found in other histologic subtypes, stages and ages. Findings indicate that Hodgkin's disease in children is at least as strongly linked to EBV as in adults. Furthermore, we suggest that the EBV is associated with a subgroup of patients which can be defined on the basis of the age at diagnosis. (C) 1995 Wiley-Liss, Inc.

Utility of new mature erythrocyte and reticulocyte indices in screening for iron-deficiency anemia in a pediatric population

Introduction Iron-deficient erythropoiesis attributable to nutritional deficiency is the most common cause of anemia in early childhood. Beckman Coulter has proposed new parameters: red blood cell size factor (RSf) and low hemoglobin density (LHD%). RSf = root MCV x MRV LHD% = 100 root 1 - 1[1/(1 + e (1.8(30-MCHC)))] The aims of the present communication were to establish reference ranges (RR) for RSf and LHD% in a healthy pediatric population;to evaluate clinical utility of these parameters in the diagnosis of iron-deficiency anemia (IDA);and to compare RSf and LHD% with RET-He. Methods Two hundred healthy infants and 51 with IDA were analyzed on Coulter LH750 and Sysmex XT2000i analyzers. Results RR for RSf in children aged 0.5-6years, 82.2-102.0 and 83.7-103.1fl in the group aged 6-18years. RR for LHD% was 1.1-9.0%. ROC analysis for RSf and LHD% in the diagnosis of IDA showed an AUC 0.8460 and 0.8654, respectively. The best RSf value to detect a restricted erythropoiesis was 89fl, sensitivity 92%, specificity 81%. LHD% optimal cutoff point was 6.0%, sensitivity 80%, and specificity 88%. Good correlation was observed between these parameters with Ret-He, r=0.888 and r=-0.790, respectively. Conclusion RSf and LHD% could be useful tools in the screening of IDA. These parameters can be obtained in the course of routine blood counts, with no additional cost or need for more blood sampling.

Epstein-Barr virus (EBV) association and latency profile in pediatric Burkitt's lymphoma: Experience of a single institution in Argentina

The aim of this study is to characterize EBV expression and latency pattern in pediatric Burkitt's lymphoma in a single institution in Argentina. EBV-encoded RNA or protein was analyzed in 27 patients. EBERs was expressed in 37% of patients (29% of immunocompetent and 100% of immunosuppressed patients). EBV-positive cases were observed exclusively in patients younger than 5 years old. EBV association with immunocompetent patients exhibits the sporadic pattern in region under study, while its presence in patients infected with HIV was higher than described previously. EBV latency I profile was present in most of the patients, except for two immunosuppressed patients who displayed LMP1 expression. J. Med. Virol. 86:845-850, 2014. (c) 2013 Wiley Periodicals, Inc.

Clinical, functional, and radiological outcome in children with pleural empyema

Introduction Few studies have prospectively evaluated recovery process and long-term consequences of pleural space infections. Objective To evaluate clinical, pulmonary, and diaphragmatic function and radiological outcome in patients hospitalized with pleural empyema. Material and Methods Previously healthy patients from 6 to 16 years were enrolled. Demographic, clinical, and treatment data were registered. At hospital discharge, and every 30 days or until normalization, patients underwent a clinical evaluation, diaphragmatic ultrasound, and lung function testing. Chest radiographs were performed at subsequent visits only if abnormalities persisted. Results Thirty patients were included. Nineteen (63%) were male, with an age of (mean +/- SD) 9.7 +/- 3.2 years, and body mass index (mean +/- SD) 18.6 +/- 3. Twelve patients (40%) were treated with chest tube drainage only, 12 (40%) exclusively with surgery, and 6 (20%) completed treatment with surgery due to an ineffective chest tube drainage. At hospital discharge, 26 (87%) of patients had abnormal breath sounds at the site of infection, 28 (93%) had a spirometric restrictive pattern, 19 (63%) diaphragmatic motion impairment, and 29 (97%) presented radiological involvement of pleural space, mainly pleural thickening. All patients had recovered diaphragmatic motion and were asymptomatic at 90- and 120-day follow-up control, respectively. Then, with a great individual variability, radiological findings, and lung function returned to normal at 60 days (range 30-180) and 90 days (range 30-180) after hospital discharge, respectively. Conclusion Patients with pleural empyema had a complete and progressive recovery, with initial clinical and diaphragmatic motion normalization followed by radiological and lung function recovery.

Swallowing and Respiratory Distress in hospitalized Patients with Bronchiolitis

The aim of this study was to evaluate the risk of airway and/or pulmonary food or saliva aspiration in infants with moderate respiratory distress who are hospitalized with respiratory syncytial virus (RSV) bronchiolitis. This prospective, descriptive study was conducted during two epidemic RSV seasons at the ricardo Guti,rrez Children's hospital in Buenos Aires, Argentina. Included were otherwise healthy infants in their first wheezing episode with a modified Tal clinical score between 5 and 9. Swallowing was evaluated using a dynamic technetium-99 m colloid radionuclide salivagram using a gamma camera, followed by video fluoroscopy using nonionic and ionic contrast material. Fifteen patients (7 boys) were included. Age at evaluation (mean +/- A SD) was 4.3 +/- A 1.5 months;clinical score was 7.5 +/- A 1.4. Patients required (mean +/- A SD) supplemental oxygen and hospitalization 7.5 +/- A 3.7 and 8.8 +/- A 4.3 days, respectively. All technetium-99 m salivagram (10/10, as the gamma camera equipment was out of service during part of the study) and video fluoroscopy (15/15) studies were normal. No episodes of aspiration or laryngeal penetration were detected in any patient. Our study found that infants hospitalized with moderate respiratory distress due to RSV bronchiolitis did not show aspiration.

Epstein Barr virus in relation to apoptosis markers and patients' outcome in pediatric B-cell Non-Hodgkin lymphoma

In this study, we investigated Epstein Barr virus (EBV) presence, associated to proliferation and apoptosis proteins in pediatric B-cell Non-Hodgkin lymphoma (B-NHL). EBERs, Ki67, active caspase 3, Bax and Bcl2 were analyzed on B-NHL tissue from 40 patients. Forty percent showed EBV expression, significantly higher among patients <= 10 years (P = 0.027), and associated with immunosuppression (P = 0.020), but not associated apotosis markers. However, EBV was associated with a worse event-free survival (P = 0.016), particularly under immunosuppression. Even though EBV did not seem to alter apoptotic pathways, it exhibited survival disadvantage and could be an important cofactor in B-cell lymphoma-genesis in younger children. Aortopulmonary Fistula After an Arterial Switch Operation

Aortopulmonary fistula has been documented in adult patients with acute aortic dissection, atherosclerotic disease, and other comorbidities. However, until now we believe this was not previously reported as a complication after the arterial switch operation in a patient with transposition of the great arteries. We report the case of a 3-month-old boy who underwent an arterial switch operation as a neonate. After balloon dilation to relieve a postoperative supravalvular pulmonary stenosis, he presented with severe congestive heart failure due to an undiagnosed but previously present fistula between the ascending aorta and the main pulmonary artery. Acrocomia Aculeata as an Unreported Cause of Tricuspid Regurgitation

Penetrating chest trauma can produce a wide range of lesions to cardiac structures. Some patients develop signs and symptoms of residual sequelae. We describe the case of an 11-year-old boy with severe tricuspid regurgitation caused by the thorn of a palm tree. A De Vega annuloplasty of the tricuspid valve and a bidirectional Glenn procedure were successfully performed 7 years after the episode. This case illustrates the importance of a thorough investigation of possible valvular heart disease in patients who suffered from chest trauma.
