Millions of people in the United States experience a reduced or distorted ability to smell or taste. Chemosensory disorders such as anosmia (the inability to smell), parosmia (distorted smell), or dysgeusia (altered taste) have major impacts on health and quality of life including difficulty sensing dangers such as fire or spoilage, a diminished palatability of food and drink that can negatively influence diet and nutrition, feelings of social isolation, and an increased incidence of frailty, anxiety, and depression. Smell or taste dysfunction can also be symptoms of other health issues, including sinonasal disease, cancer, or neurodegenerative disease. Aging adults are disproportionately affected. However, smell and taste function are not regularly assessed as a part of routine healthcare despite their prevalence and impact. This is a lost opportunity, as early detection of a chemosensory disorder would enable patients to obtain needed validation, education and support for their health challenge, could direct both patient and provider to treatment options, and may suggest underlying health issues that should be addressed. To better understand the current barriers to including chemosensory testing as a regular component of health care and to identify opportunities to overcome those barriers, the conference "Towards Universal Chemosensory Testing" was convened on November 5-7, 2023, in Philadelphia, PA. This conference brought together scientists, clinicians, patients, and other experts to discuss these issues and identify collective ways to overcome barriers to testing. This white paper - which is focused primarily on the US healthcare system - is the result of those discussions.

With the adoption of reduced-intensity conditioning (RIC) regimen, allogeneic hematopoietic cell transplantation (alloHCT) is being increasingly used in older adults. However, data are sparse on the safety and efficacy of this procedure in patients aged ≥70 years. Currently the majority of these patients receive either non-myeloablative busulfan (Bu; 3.2-6.4mg/kg) or low-dose total body irradiation (TBI; 2-4.5Gy)- based conditioning regimens, which are associated with a relatively high incidence of disease relapse. We report here the transplant outcomes of patients aged ≥70 years with hematological malignancies who underwent alloHCT with melphalan (Mel)-based conditioning regimen (Mel 100-140mg/m2 plus fludarabine or clofarabine) between May 2007 and September 2016 at City of Hope. We evaluated 53 consecutive patients (median: 71, range: 70-78 years, 35 males and18 females) who underwent AlloHCT for acute myeloid leukemia (n=27), myelodysplastic syndrome (n=14), myeloproliferative neoplasm (n=6), acute lymphoblastic leukemia (n=2), non-Hodgkin lymphoma (n=2), blastic plasmacytoid dendritic cell neoplasm (n=1), or acute promyelocytic leukemia (n=1) from HLA-identical sibling (n=11), matched unrelated donor (8/8, n=38) or mismatched unrelated donor (n=4). Fifty-one patients received peripheral blood stem cell grafts while two received bone marrow grafts. Graft versus host disease (GVHD) prophylaxis was tacrolimus/sirolimus-based in all patients. The disease risk index (DRI) score was high-very high in 36% of patient (n=19) and Karnofsky score was ≥70% in all patients. The HCT comorbidity index was 3 or more in 21 patients. Fifty-two of our 53 patients achieved neutrophil engraftment with the median time to engraft of 15 days (range: 11-29 days). Blood or bone marrow chimerism analyses showed >95% of patients achieved >95% donor chimerism within 6 weeks from alloHCT, consistent with “semi-ablative” nature of Mel-based RIC. Gut dysbiosis might underlie the pathogenesis of type 1 diabetes. In mice of the non-obese diabetic (NOD) strain, we found that key features of disease correlated inversely with blood and fecal concentrations of the microbial metabolites acetate and butyrate. We therefore fed NOD mice specialized diets designed to release large amounts of acetate or butyrate after bacterial fermentation in the colon. Each diet provided a high degree of protection from diabetes, even when administered after breakdown of immunotolerance. Feeding mice a combined acetate- and butyrate-yielding diet provided complete protection, which suggested that acetate and butyrate might operate through distinct mechanisms. Acetate markedly decreased the frequency of autoreactive T cells in lymphoid tissues, through effects on B cells and their ability to expand populations of autoreactive T cells. A diet containing butyrate boosted the number and function of regulatory T cells, whereas acetate- and butyrate-yielding diets enhanced gut integrity and decreased serum concentration of diabetogenic cytokines such as IL-21. Medicinal foods or metabolites might represent an effective and natural approach for countering the numerous immunological defects that contribute to T cell-dependent autoimmune diseases.

We report one year follow-up data on safety and efficacy from the first prospective, multi-center, open-label, single-arm phase I/II clinical trial of TCRalpha/beta and CD19-depleted haploidentical stem cells grafts which were generated with the CliniMACS plus System (Miltenyi Biotec, Germany) and transplanted after reduced-intensity conditioning in pediatric patients with various malignant and non-malignant diseases (***; 2011-005562-38). 30 patients were enrolled in the pediatric cohort of the trial and received single agent MMF as short-term GVHD prophylaxis (40mg/kg/day for 30 days). The grafts were manufactured in 4 GMP sites. The kinetics of immune cell reconstitution were measured in two core labs using standardized phenotypic analyses and the MACSQuant flow cytometer (Miltenyi Biotec, Germany). We previously reported the outcome of the first 100 days. This report covers a median follow-up of 367 days. Results: Thirty patients from six hospitals were treated (13 female, 17 male; median age 7 years, range 1 - 17 years). 10 patients had ALL, 8 AML, 6 solid tumors (soft tissue sarcomas and neuroblastomas), 3 MDS/MPS, and 1 each with lysosomal storage disorder, SCID, and Wiskott Aldrich syndrome. Disease status in acute leukemias/MDS were: CR1 (n=4), relapsed/refractory (n=17). 5/6 patients with solid tumors had relapsed metastatic disease. The conditioning regimen consisted of 15 or 30 mg ATG (Fresenius/Grafalon) or 7 Gy total nodal irradiation, 160 mg/m 2 fludarabine, 10 mg/kg thiotepa, and 140 mg/m 2 melphalan. The median numbers of CD34+ cells, TCRab+ cells and CD20+ cells infused per kg bw were 14.6 x 10 6 (range, 4 - 54.9), 14 x 10 3 (range, 0.62 - 40.6) and 0.55 x 10 5 (range, 0.04 - 1.85), respectively. Main cellular components of the graft were NK and TCRgd+ cells (median 6.67 x 10 7 and 1.58 x 10 7 cells per kg bw, respectively) with consistently high log depletions of TCRab+ and CD19+ cells: 4.75 and 3.44, respectively.
