Corneal Sensory Changes and Nerve Plexus Abnormalities in Chronic Neuropathic Ocular Pain and Dry Eye Postrefractive Surgery

Purpose: Chronic neuropathic ocular pain (NOP) can develop alongside chronic dry eye (DE) post–laser-assisted in-situ keratomileusis (LASIK), yet its specific characteristics remain poorly understood. This study aims to compare the clinical characteristics of patients who developed both DE and NOP after LASIK to those with only DE and to asymptomatic LASIK patients, to facilitate the diagnosis of NOP. Methods: Prospective, cross-sectional “case-control” comparison study. An 89-subject post-LASIK study comprised 3 groups: 34 patients developing NOP and DE (NOP-DE group), 25 patients developing only DE (DE group), and 30 asymptomatic subjects (control group). Assessments included clinical history and symptom questionnaires (OSDI, mSIDEQ, NRS, WFPRS), anxiety and depression evaluation (HADS), tear film stability (osmolarity and TBUT) and production (Schirmer), and ocular surface integrity. Corneal mechanical and thermal sensitivity thresholds were measured using Belmonte's noncontact esthesiometer, whereas tactile sensitivity threshold was assessed pre-/post-topical anesthesia using the Cochet-Bonnet esthesiometer. In vivo confocal microscopy (IVCM) was used to evaluate the sub-basal nerve plexus characteristics and dendritic cell density in the central cornea. Group comparisons and correlations were conducted. Results: Compared with DE group, patients in the NOP-DE group exhibited significantly more DE symptoms with mSIDEQ (P = .019) higher level of pain with NRS and WFPRS, increased use of ocular lubrication (P = .003), greater frequency of patients with pathological results on anxiety and depression questionnaires (P < .001), and a higher prevalence of central sensitization syndromes (P < .001). Additionally, NOP-DE patients demonstrated higher tactile corneal sensitivity post-topical anesthesia (P = .002). IVCM revealed lower nerve density (P = .049) and higher microneuroma density (P = .008) in the sub-basal nerve plexus of NOP-DE patients compared to DE patients

Detection of septic metastases in catheter-related Staphylococcus aureus bacteremia using 18^FFDG-PET/CT: a before-and-after study

Purpose: To evaluate the detection rate of septic metastases in catheter-related S. aureus bacteremia (CR-SAB) episodes by using [18F]FDG-PET/CT. Methods: We conducted a retrospective, before-and-after, single-center study of a prospectively identified catheter-related SAB (CR-SAB) cohort at Hospital Clínic Barcelona. All adult patients hospitalized from January 2006 to December 2022 were included. Primary outcome was the detection of septic metastases before and after integrating [18F]FDG-PET/CT into the diagnostic workflow of CR-SAB in January 2020. Secondary outcomes included 30-day mortality, length of stay, and treatment duration. Results: A total of 598 episodes of CR-SAB were included, 100 in the post-intervention period (2020–2022) and 498 in the pre-intervention period (2006–2019). [18F]FDG-PET/CT scan was performed in 28/100 episodes (28.0%) in post-intervention period, versus 9/498 in pre-intervention period (1.8%). Septic metastases detection rate was higher after [18F]FDG-PET/CT implementation (22/100, 22% vs. 56/498, 11.2% p.004), mainly due to pulmonary septic emboli (13/100, 13.0% vs. 12/498, 2.4% p <.001) and osteoarticular seeding (7/100, 7.0% vs. 11/498, 2.2% p.019). Neither pulmonary septic emboli nor osteoarticular metastases increased 30-day mortality (3/25, 12.0% vs. 57/573, 10.0%, p.732;and 2/18, 11.1% vs. 58/580 10.0%, p.702, respectively). Patients with septic metastases had longer treatment [25.0 (16.0–37.0) vs. 15.0 (13.0–19.0) days, p <.001]. Conclusions: [18F]FDG-PET/CT use in patients with CR-SAB was associated with a higher rate of septic metastases diagnosis, mainly pulmonary and osteoarticular, resulting in longer treatment, but no differences in clinical outcomes were observed. © The Author(s) 2025.

Human neural stem cells（h NSCs） derived from the ventral mesencephalon are powerful research tools and candidates for cell therapies in Parkinson＇s disease. However, their clinical translation has not been fully realized due, in part, to the limited ability to track stem cell regional localization and survival over long periods of time after in vivo transplantation. Magnetic resonance imaging provides an excellent non-invasive method to study the fate of transplanted cells in vivo. For magnetic resonance imaging cell tracking, cells need to be labeled with a contrast agent, such as magnetic nanoparticles, at a concentration high enough to be easily detected by magnetic resonance imaging. Grafting of human neural stem cells labeled with magnetic nanoparticles allows cell tracking by magnetic resonance imaging without impairment of cell survival, proliferation, self-renewal, and multipotency. However, the results reviewed here suggest that in long term grafting, activated microglia and macrophages could contribute to magnetic resonance imaging signal by engulfing dead labeled cells or iron nanoparticles dispersed freely in the brain parenchyma over time.

Retinitis pigmentosa is a genetically heterogeneous retinal degeneration process. There is hardly any treatment available. It is associated with extensive chronic inflammation and the release of proinflammatory cytokines such as TNFα. The blockade of TNFα through systemic or intraocular routes slows retinal degeneration. They are invasive routes with possible side effects. Herein, we propose a noninvasive approach to address the inflammatory component of retinitis pigmentosa. This approach is based on the development of eye drops of nanostructured lipid carriers (NLCs) loaded with the monoclonal antibody against TNFα, adalimumab (ADA). We physicochemically characterized NLC–ADA. We evaluated retinal and corneal toxicity;corneal permeation;diffusion to the retina;and effects on retinal dysfunction, degeneration and inflammation. These results prove that NLC–ADA eye drops exhibit excellent corneal permeation, no toxicity and high retinal distribution in mice. These compounds improve retinal function, reduce retinal degeneration and ameliorate the inflammatory process. In particular, NLC–ADA eye drops reduce M1 microglial activation, macrophage infiltration and the levels of some components of the NLRP3 inflammasome in rd10 mice, a model of retinitis pigmentosa. This strategy offers a noninvasive route that circumvents the blood[sbnd]retinal barrier in a safe and efficient manner. Hence, this approach could offer a promising therapeutic option for treating retinitis pigmentosa regardless of genetic defects. This approach could be useful for other inflammation-related retinal diseases. © 2025 The Authors

This paper presents the AMELIE Authoring Tool for e-health applications. AMELIE provides the means for creating video-based contents with a focus on e-learning and telerehabilitation processes. The main core of AMELIE lies in the efficient exploitation of raw multimedia resources, which may be already available at clinical centers or recorded ad hoc for learning purposes by health professionals. Three real use cases scenarios involving different target users are presented: (1) cognitive skills' training of surgeons in minimally invasive surgery (medical professionals), (2) training of informal carers for elderly home assistance and (3) cognitive rehabilitation of patients with acquired brain injury. Preliminary validation in the field of surgery hints at the potential of AMELIE;and its versatility in different medical applications is patent from the use cases described. Regardless, new validation studies are planned in the three main application areas identified in this work. © Springer International Publishing Sw itzerland 2014.

Continuous cardiorespiratory monitoring is crucial for understanding physiological conditions, particularly respiratory and cardiac diseases. Wearable devices offer an attractive approach for this goal, allowing unobtrusive data collection. This study evaluates two ECG-derived respiration (EDR) algorithms using non-standard electrocardiogram (ECG) leads from a wearable device, and as well as bioimpedance signal for extracting breathing information. The performance is compared against respiratory airflow. 12 healthy volunteers followed a respiratory protocol involving free and paced breathing while ECG, bioimpedance and respiratory airflow were acquired. ECG and bioimpedance were measured using a wearable device, whereas, airflow was recorded using a standard system. Strong linear relationships (Pearson coefficients > 0.90) were observed between EDR signals and respiratory volume, outperforming bioimpedance. The R-wave amplitude algorithm exhibited superior accuracy and lower errors ( © 2023 CinC.

The mission of the biomedical research networking center in bioengineering, Biomaterials and Nanomedicine(CIBER-BBN) can be defined through two strategic lines:(1) To enhance the scientific and technological quality level of each one of its groups and(2) To promote the creation of multidisciplinary teams strengthening alliances combining basic and applied research and innovation among the CIBER-BBN's research groups. Out of these strategic lines, two main objectives have been also defined:(1) To boost clinical translation inducing improvements in patient's health system,and(2) To boost technology transfer of CIBER-BBN's research results through contracts with biomedical industries, the creation of technology-based companies, the enhancement of its patent's portfolio and by offering research services and consultancy on technological research and innovation issues. The main achievements accomplished during its first 8 years will be presented and discussed. The future challenges mainly concerning internationalization and joint innovation will be also presented and discussed.

The modeling and simulation of the respiratory control system allows a better understanding of the physiological system to different stimuli and pathologies. Although different models have been validated under different situations, it is unusual to perform a fitting of their parameters based on environmental characteristics such as altitude, which can significantly affect the accuracy of the simulation results. The purpose of this work is to fit the respiratory control model proposed by Fincham and Tehrani with experimental data obtained under exercise. The experimental test include carbon dioxide production between 0.4 and 2.5 l / min at an altitude of 1538 m above sea level. The variables compared between simulation without fitting and with fitting regarding experimental data were total minute ventilation, arterial partial pressure of oxygen, arterial partial pressure of carbon dioxide and Frequency of breathing. The results evidence the altitude as a stimulus that should be considered in simulations of respiratory models in order to obtain more accurate results. © Springer Nature Singapore Pte Ltd. 2017.
