Superior vena cava syndrome presenting as chylothorax

Chylothorax caused by superior vena cava (SVC) syndrome is a rare but potentially life-threatening complication requiring a multidisciplinary diagnosis and management approach. We present a case of a 27-year-old female with end-stage renal disease who developed chylothorax secondary to SVC syndrome caused by venous stenosis from a tunneled hemodialysis (HD) catheter. The patient had a history of ongoing hemodialysis through a tunneled catheter placed in the right internal jugular vein approximately seven months before presentation. She presented with dyspnea, chest pain, and a large left-sided pleural effusion. A multidisciplinary diagnostic workup and management included 2 thoracentesis, pleural fluid studies, serial radiological tests, right and left heart catheterizations, and blood serum studies with flow cytometry. They revealed that SVC stenosis around the hemodialysis catheter was causing the patient's pathology. The patient underwent veno-plasty of the right SVC and brachiocephalic veins and replacement of HD catheter leading to the resolution of the chylothorax and significant improvement in respiratory symptoms. This report will highlight the approach to diagnosing and managing chylothorax and a review of existing medical literature. © 2023

Background: High-pressure injection injury is an uncommon but potentially limb-threatening presentation that in many cases should be treated as a surgical emergency. Case Report: We present a 46-year-old patient with a high-pressure injection injury from SCUBA equipment who developed pneumomediastinum. Why Should an Emergency Physician Be Aware of This?: High-pressure injection injuries are rare and may have significant morbidity. Injection injuries from air are uncommon, may be managed differently than injection with other materials, and may be associated with unique complications. © 2022 Elsevier Inc.

When life-saving measures lead to trauma: Subcapsular hepatic hematoma after CPR

Cardiopulmonary resuscitation (CPR) is a critical intervention for cardiac arrest but can result in significant internal injuries due to the force of chest compressions. Among these, subcapsular hepatic hematoma is a rare and serious complication. Here we present a 55-year-old male with a history of alcohol abuse presented with a severe ischemic stroke and subsequently required CPR due to pulseless ventricular tachycardia. Following resuscitation, the patient developed a subcapsular hepatic hematoma, likely caused by the trauma of chest compressions. The patient also had a complex clinical course involving hemorrhagic conversion of the stroke and the need for anticoagulation due to bilateral pulmonary emboli, which further complicated the management of the hepatic hematoma. The identification of the hematoma was achieved through contrast-enhanced CT imaging after the patient exhibited worsening abdominal discomfort and signs of internal bleeding. The management focused on balancing the need for anticoagulation with the risks of further bleeding from the hematoma. A multidisciplinary approach was essential, involving close monitoring, potential surgical intervention, and careful adjustment of anticoagulant therapy. This case emphasizes the importance of recognizing the potential for internal injuries following CPR, especially in patients requiring anticoagulation. Early detection through imaging and a coordinated, multidisciplinary approach are crucial in managing such complications and improving patient outcomes.

A novel multimodal approach to C2 vertebroplasty in the setting of osteolytic metastases: A case report

Image-guided vertebral augmentation effectively treats pathologic vertebral lesions, though managing pathology in the cervical spine presents unique challenges. Posterolateral approaches, while safer, require prone positioning that may not be feasible for all patients. Open anterolateral and transoral approaches increase infection risk and typically require general anesthesia. We present a novel multimodal anterolateral approach using ultrasound, fluoroscopy, and cone-beam CT for percutaneous C2 vertebral augmentation in a patient with metastatic lesions. A 72-year-old man with multiple myeloma presented with a C2 vertebral body lytic lesion at high risk for dislocation. Due to presence of a stabilizing cervical collar and the risk associated with prone positioning, vertebral augmentation access was limited. With the patient in supine position, we utilized cone-beam CT with overlay guidance to plan a right lateral trajectory. Ultrasound identified vital vasculature, enabling targeted hydrodissection to create a safe access path with real-time image-guidance. An 11G trocar needle was advanced under ultrasound guidance, with intermittent cone-beam CT adjustments to ensure precise placement. After confirming access to the right lateral mass of C2 via fluoroscopy, 3.5cc of PMMA cement was successfully injected without extravasation. The patient experienced no complications or neurologic deficits on follow-up and was cleared to remove the cervical collar 10 days postprocedure. This case demonstrates the feasibility and efficacy of a multimodal anterolateral approach for C2 vertebral augmentation when conventional approaches are contraindicated. By integrating ultrasound-guided hydrodissection with advanced imaging, this technique offers a safe alternative for anatomically constrained patients while avoiding the risks associated with traditional approaches.

Complicated diverticulitis: Diagnostic precision and surgical solutions in a patient with chronic kidney disease

Diverticulitis, an inflammation of diverticula in the colon, can lead to severe complications such as perforation and abscess formation. A 42-year-old female with polycystic kidney disease and chronic kidney disease stage III presented with severe abdominal pain, fever, and inability to tolerate oral intake. The patient was a previous smoker who smoked 0.5 packs per day for 25 years. Initial evaluation revealed leukocytosis and elevated creatinine. A CT scan identified pneumoperitoneum and mild sigmoid diverticulitis, suggesting a perforated viscus. She underwent urgent exploratory laparotomy, which confirmed the CT findings and resulted in an appendectomy, sigmoid colon resection, and ostomy creation. Postoperatively, the patient faced complications, including recurrent pneumoperitoneum and subcutaneous emphysema, detected through follow-up CT scans, leading to further surgical interventions. CT imaging was pivotal in diagnosing, monitoring, and guiding treatment, with noncontrast CT being beneficial given her renal impairment. Early diagnosis and CT imaging are crucial in managing complicated diverticulitis. Postoperative monitoring with CT scans is essential for detecting complications. Follow-up care should include regular colonoscopies to assess diverticular disease and dietary modifications to prevent recurrence. Combining clinical, surgical, and radiologic data ensures effective management and improves patient outcomes.

Magnetic resonance (MR)-guided radiation therapy (RT) is enhanc- ing head and neck cancer (HNC) treatment through superior soft tissue con- trast and longitudinal imaging capabilities. However, manual tumor segmentation remains a significant challenge, spurring interest in artificial intelligence (AI)- driven automation. To accelerate innovation in this field, we present the Head and Neck Tumor Segmentation for MR-Guided Applications (HNTS-MRG) 2024 Challenge, a satellite event of the 27th International Conference on Medical Image Computing and Computer Assisted Intervention. This challenge addresses the scarcity of large, publicly available AI-ready adaptive RT datasets in HNC and explores the potential of incorporating multi-timepoint data to enhance RT auto- segmentation performance. Participants tackled two HNC segmentation tasks: automatic delineation of primary gross tumor volume (GTVp) and gross metastatic regional lymph nodes (GTVn) on pre-RT (Task 1) and mid-RT (Task 2) T2- weighted scans. The challenge provided 150 HNC cases for training and 50 for final testing hosted on *** using a Docker submission framework. In total, 19 independent teams from across the world qualified by submitting both their algorithms and corresponding papers, resulting in 18 submissions for Task 1 and 15 submissions for Task 2. Evaluation using the mean aggregated Dice Simi- larity Coefficient showed top-performing AI methods achieved scores of 0.825 in Task 1 and 0.733 in Task 2. These results surpassed clinician interobserver vari- ability benchmarks, marking significant strides in automated tumor segmentation for MR-guided RT applications in HNC. © The Author(s) 2025.
