We report the discovery of the most eccentric transiting brown dwarf in the brown dwarf desert, TOI-2490b. The brown dwarf desert is the lack of brown dwarfs around main sequence stars within ∼ 3 AU and is thought to be caused by differences in formation mechanisms between a star and planet. To date, only ∼ 40 transiting brown dwarfs have been confirmed. TOI-2490b is a (Equation presented) brown dwarf orbiting a (Equation presented) sun-like star on a 60.33 d orbit with an eccentricity of 0.77989 ± 0.00049. The discovery was detected within TESS sectors 5 (30 minute cadence) and 32 (2 minute and 20 second cadence). It was then confirmed with 31 radial velocity measurements with FEROS by the WINE collaboration and photometric observations with the Next Generation Transit Survey. Stellar modelling of the host star estimates an age of ∼ 8 Gyr, which is supported by estimations from kinematics likely placing the object within the thin disc. However, this is not consistent with model brown dwarf isochrones for the system age suggesting an inflated radius. Only one other transiting brown dwarf with an eccentricity higher than 0.6 is currently known in the brown dwarf desert. Proposals of artificial intelligence (AI) solutions based on increasingly complex and accurate predictive models are becoming ubiquitous across many disciplines. As the complexity of these models grows, transparency and users' understanding often diminish. This suggests that accurate prediction alone is insufficient for making an AI-based solution truly useful. In the development of healthcare systems, this introduces new issues related to accountability and safety. Understanding how and why an AI system makes a recommendation may require complex explanations of its inner workings and reasoning processes. Although research on explainable AI (XAI) has significantly increased in recent years and there is high demand for XAI in medicine, defining what constitutes a good explanation remains ad hoc, and providing adequate explanations continues to be challenging. To fully realize the potential of AI, it is critical to address two fundamental questions about explanations for safety-critical AI applications, such as health-AI: (1) What is an explanation in health-AI? and (2) What are the attributes of a good explanation in health-AI? In this study, we examined published literature and gathered expert opinions through a two-round Delphi study. The research outputs include (1) a definition of what constitutes an explanation in health-AI and (2) a comprehensive list of attributes that characterize a good explanation in health-AI. © 2024, CC BY.

The translation of AI-generated brain metastases (BM) segmentation into clinical practice relies heavily on diverse, high-quality annotated medical imaging datasets. The BraTS-METS 2023 challenge has gained momentum for testing and benchmarking algorithms using rigorously annotated internationally compiled real-world datasets. This study presents the results of the segmentation challenge and characterizes the challenging cases that impacted the performance of the winning algorithms. Untreated brain metastases on standard anatomic MRI sequences (T1, T2, FLAIR, T1PG) from eight contributed international datasets were annotated in stepwise method: published UNET algorithms, student, neuroradiologist, final approver neuroradiologist. Segmentations were ranked based on lesion-wise Dice and Hausdorff distance (HD95) scores. False positives (FP) and false negatives (FN) were rigorously penalized, receiving a score of 0 for Dice and a fixed penalty of 374 for HD95. The mean scores for the teams were calculated. Eight datasets comprising 1303 studies were annotated, with 402 studies (3076 lesions) released on Synapse as publicly available datasets to challenge competitors. Additionally, 31 studies (139 lesions) were held out for validation, and 59 studies (218 lesions) were used for testing. Segmentation accuracy was measured as rank across subjects, with the winning team achieving a LesionWise mean score of 7.9. The Dice score for the winning team was 0.65 ± 0.25. Common errors among the leading teams included false negatives for small lesions and misregistration of masks in space. The Dice scores and lesion detection rates of all algorithms diminished with decreasing tumor size, particularly for tumors smaller than 100 mm3. In conclusion, algorithms for BM segmentation require further refinement to balance high sensitivity in lesion detection with the minimization of false positives and negatives. Background Systemic lupus erythematosus (SLE) is a rare immunological disorder and genetic factors are considered important in its causation. Monogenic lupus has been associated with around 30 genotypes in humans and 60 in mice, while genome-wide association studies have identified more than 90 risk loci. We aimed to analyse the contribution of rare and predicted pathogenic gene variants in a population of unselected cases of childhood-onset SLE. Methods For this genetic panel analysis we designed a next-generation sequencing panel comprising 147 genes, including all known lupus-causing genes in humans, and potentially lupus-causing genes identified through GWAS and animal models. We screened 117 probands fulfilling American College of Rheumatology (ACR) criteria for SLE, ascertained through British and French cohorts of childhood-onset SLE, and compared these data with those of 791 ethnically matched controls from the 1000 Genomes Project and 574 controls from the FREX Consortium. Findings After filtering, mendelian genotypes were confirmed in eight probands, involving variants in C1QA, C1QC, C2, DNASE1L3 , and IKZF1 . Seven additional patients carried heterozygous variants in complement or type I interferon-associated autosomal recessive genes, with decreased concentrations of the encoded proteins C3 and C9 recorded in two patients. Rare variants that were predicted to be damaging were significantly enriched in the childhood-onset SLE cohort compared with controls; 25% of SLE probands versus 5% of controls were identified to harbour at least one rare, predicted damaging variant (p=2·98 × 10 −11 ). Inborn errors of immunity were estimated to account for 7% of cases of childhood-onset SLE, with defects in innate immunity representing the main monogenic contribution. Interpretation An accumulation of rare variants that are predicted to be damaging in SLE-associated genes might contribute to disease expression and clinical heterogeneity. The first Better evidence and RecommendatIons for the next Generation HealTh—BRIGHT symposium was held in Chongqing, China between June 21 and 23, 2024. The symposium did not only showcase the recent progress made by multidisciplinary teams in generating and translating evidence for children's healthcare, guideline development and evaluation, and the utilization of AI applications in pediatrics but also fostered transnational and transregional cooperation to promote the advancement of high-quality evidence and guidelines in this field. The symposium contributed significantly to the future development and transformation of research endeavors in pediatrics.

Diagnosis of epilepsy after a first unprovoked seizure is possible according to the guidelines by the International League Against Epilepsy, if the risk recurrence of a second unprovoked seizure is exceeding 60%. However, this cutoff constitutes only a proxy depending on the patients’ history, magnetic resonance imaging (MRI), and electroencephalography (EEG) findings but nevertheless also from the treating neurologists’ individual experience. In a Switzerland-wide observational study, we aim to recruit patients that were admitted to the emergency department with the referral diagnosis of a first and unprovoked seizure. We make use of optimized MRI protocols to identify potential structural epileptogenic lesions, introduce new imaging-based markers of epileptogenecity, and use most recent postprocessing methods as automatic morphometry, spike map analysis, and functional connectivity. With these diagnostic tools, we aim to segregate patients that present with epileptic seizures versus mimicks and non-epileptic seizures and stratify for every finding in MRI and EEG its predictive value for a second unprovoked seizure. These findings shall support neurologists to calculate and not only estimate the seizure recurrence rate in future.
